Preliminary Clinical Outcomes after Implantation of Newer-Generation Biodegradable Polymer-Coated Everolimus-Eluting Stent in “Real-World” Patients with Coronary Artery Disease

Background and Objective: In the contemporary practice, the use of drug-eluting stents is still associated with low mortality benefits, restenosis and stent thrombosis. To address these issues, newer generation, thin-strut, biodegradable polymer coated stents has been designed. Thus, the aim of the study is to assess the safety and clinical performance of the Everoflex (Sahajanand Medical Technologies Pvt. Ltd., Surat, India), a newer generation biodegradable polymer coated everolimus-eluting stent, in unselected “real-world” patients with coronary artery disease. Methods: It is a multicentre, retrospective, non-randomized, single-arm study enrolling all the consecutive patients who underwent implantation with the Everoflex for coronary artery disease from April 2014 to March 2016. The primary end-point of the study is 30-day major adverse cardiovascular events (MACE) rate, which consists of cardiac death, myocardial infarction, target lesion revascularization and target vessel revascularization. Stent thrombosis was also analyzed and reported. Results: A total of 340 patients were intervened successfully with 395 everolimus eluting stents (1.3 ± 0.6 stents per patient). Out of total patients (58.7 ± 10.5 years), 77.9% were male and comorbidities like diabetes and hypertension were observed in 31.2% and 35.3% patients, respectively. According to ACC/AHA classification, there were 34.4% type B lesions and 53.2% type C lesions, indicating a higher proportion of complexity involved. Moreover, 57.9% patients had multivessel disease and there were 15.4% total occlusions. At 30 days, follow-up was completed in 100% patients. The MACE was found to be 1.5%, which is a composite of 1.2% cardiac death and 0.3% target lesion revascularization. Stent thrombosis at 30 days was found to be 0.3%. THD reduction with reactive power compensation for fuzzy logic DVR based solar PV grid connected system

Dynamic voltage restorer （DVR） is used to protect sensitive loads from voltage disturbances of the distribution generation （DG） system. In this paper, a new control approach for the 200 kW solar photovoltaic grid connected system with perturb and observe maximum power point tracking （MPPT） technique is implemented. Power quality improvement with comparison is conducted during fault with proportional integral （PI） and artificial intelligence-based fuzzy logic controlled DVR. MPPT tracks the actual variable DC link voltage while deriving the maximum power from a photovoltaic array and maintains DC link voltage constant by changing modula- tion index of the converter. Simulation results during fault show that the fuzzy logic based DVR scheme demonstrates simultaneous exchange of active and reactive power with less total harmonic distortion （THD） present in voltage source converter （VSC） current and grid current with fast tracking of optimum operating point at unity power factor. Standards （IEEE-519/1547）, stipulates that the current with THD greater than 5% cannot be injected into the grid by any distributed generation source. Simulation results and validations of MPPT technique and operation of fuzzy logic controlled DVR demonstrate the effectiveness of the proposed control schemes.

Power quality investigation of a solar PV transformer-less grid-connected system fed DVR

This paper presents a single stage transformerless grid-connected solar photovoltaic （PV） system with an active and reactive power control. In the absence of active input power, the grid-tied voltage source converter （VSC） is operated in a reactive power generation mode, which powers the control circuitry, and maintains a regulated DC voltage to the VSC. A data-based maximum power point tracking （MPPT） control scheme which performs power quality control at a maximum power by reducing the total harmonic distortion （THD） in grid injected current as per IEEE-519/1547 standards is implemented. A proportionalintegral （P1） controller based dynamic voltage restorer （DVR） control scheme is implemented which controls the grid side converter during single-phase to ground fault. The analysis includes the grid current THD along with the corresponding variation of the active and reactive power during the fault condition. The MPPT tracks the actual variable DC link voltage while deriving the maximum power from the solar PV array, and maintains the DC link voltage constant by changing the modulation index of the VSC. Simulation results using Matlab/Simulink are presented to demonstrate the feasibility and validations of the proposed novel MPPT and DVR control systems under different environmental conditions.

Load shedding scheme for the two-area system with linear quadratic regulator

The power system is prone to many emergency conditions which may lead to emergency state of operation with decay in the system frequency. The dramatic change in the frequency can result in cascaded failure of the system. In order to avoid power system collapse, load shedding （LS） schemes are adopted with the optimal amount of load shed. This paper proposed a methodology in a two-area thermal-thermal system for finding the required amount of load to be shed for setting the frequency of the system within minimum allowable limits. The LS steps have been obtained based on the rate of change of frequency with the increase in load in steps. A systematic study has been conducted for three scenarios： the scheme with a conventional integral controller; the scheme with a linear quadratic regulator （LQR）; and the scheme with an LQR and superconducting magnetic energy storage devices （SMES）. A comparison of the results has been presented on the two-area system.

The frequency of based on the load pattern the power system varies of the consumers. With continuous increase in the load, the frequency of the system keeps decreasing and may reach its minimum allowable limits. Further increase in the load will result in more frequency drop leading to the need of load shedding, if excess generation is not available to cater the need. This paper proposed a methodology in a hybrid thermal-hydro system for finding the required amount of load to be shed for setting the frequency of the system within its minimum allowable limits. The load shedding steps were obtained based on the rate of change of frequency with the increase in the load in both areas. The impact of superconducting magnetic energy storage （SMES） was obtained on load shedding scheme. The comparison of the results was presented on the two-area system.

Pars Plana Vitrectomy versus Combined Scleral Buckling—Pars Plana Vitrectomy for Phakic Rhegmatogenous Retinal Detachment with Inferior Breaks

Aims: To compare the results of pars plana vitrectomy (PPV) and combined scleral buckling—PPV (SB/PPV) in phakic rhegmatogenous retinal detachments with inferior breaks. Methods: Randomized, prospective, clinical controlled trial of forty consecutive phakic eyes with primary rhegmatogenous retinal detachment, associated with inferior breaks and not complicated by proliferative vitreoretinopathy ≥grade C, to either PPV (group 1) or combined SB/PPV (group 2). Results: At 6 months follow up the primary reattachment rate was 100% (20/20 cases) in group 2 and 70% (14/20 cases) in the group 1, the difference being statistically significant (p = 0.027). The best corrected visual acuity improved significantly from a preoperative mean of 1.65 ± 1.13 (Range: 0.6 to 3) to a mean of 0.45 ± 0.11 (Range: 0.3 to 0.6) in the group 2 and in the group 1 improved from a preoperative mean of 2.34 ± 0.92 (Range: 0.48 to 3) to a mean of 0.668 ± 0.20 (Range: 0.48 to 1), the difference between the two groups being statistically significant (p = 0.001). Conclusion: Anatomical and functional success rates are significantly better with the use of a scleral explant during PPV for uncomplicated forms of phakic rhegmatogenous retinal detachments with inferior breaks.

Biomarkers in triple negative breast cancer:A review

Breast cancer is an intrinsically heterogeneous disease. In the world about 1 million cases of breast cancer are diagnosed annually and more than 170000 are triplenegative. Characteristic feature of triple negative breast cancer(TNBC) is that it lacks expression of oestrogen,progesterone and human epidermal growth factor receptor-2/neu receptors. They comprise 15%-20% of all breast cancers. We did a systematic review of Pub Med and conference databases to identify studies published on biomarkers in TNBC. We included studies with biomarkers including: Epidermal growth factor receptor,vascular endothelial growth factor,c-Myc,C-kit and basal cytokeratins,Poly(ADP-ribose) polymerase-1,p53,tyrosinase kinases,m-TOR,heat and shock proteins and TOP-2A in TNBC. We also looked for studies published on synthetic lethality and inhibition of angiogenesis,growth,and survival pathways. TNBC is a complex disease subtype with many subclasses. Majority TNBC have a basal-like molecular phenotype by gene expression profiling. Their clinical and pathologic features overlap with hereditary BRCA1 related breast cancers. Management of these tumours is a challenge to the clinician because of its aggressive behaviour,poor outcome,and absence of targeted therapies. As the complexity of this disease is being simplified over time new targets are also being discovered for the treatment of this disease. There are many biomarkers in TNBC being used in clinical practice. Biomarkers may be useful as prognostic or predictive indicators as well as suggest possible targets for novel therapies. Many targeted agents are being studied for treatment of TNBC.

Systemic treatment strategies for triple-negative breast cancer

Triple-negative breast cancer(TNBC) is defined by the lack of immunohistochemical expression of the estrogen and progesterone receptors and human epidermal growth factor receptor 2(EGFR2). Most TNBC has a basal-like molecular phenotype by gene expression profiling and shares clinical and pathological features with hereditary BRCA1 related breast cancers. This review evaluates the activity of available chemotherapy and targeted agents in TNBC. A systematic review of PubM ed and conference databases was carried out to identify randomised clinical trials reporting outcomes in women with TNBC treated with chemotherapy and targeted agents. Our review identified TNBC studies of chemotherapy and targeted agents with different mechanisms of action, including induction of synthetic lethality and inhibition of angiogenesis, growth and survival pathways. TNBC is sensitive to taxanes and anthracyclins. Platinum agents are effective in TNBC patients with BRCA1 mutation, either alone or in combination with poly adenosine diphosphate polymerase 1 inhibitors. Combinations of ixabepilone and capecitabine have added to progression-free survival(PFS) without survival benefit in metastatic TNBC. Antiangiogenic agents, tyrosine kinase inhibitors and EGFR inhibitorsin combination with chemotherapy produced only modest gains in PFS and had little impact on survival. TNBC subgroups respond differentially to specific targeted agents. In future, the treatment needs to be tailored for a specific patient, depending on the molecular characteristics of their malignancy. TNBC being a chemosensitive entity, combination with targeted agents have not produced substantial improvements in outcomes. Appropriate patient selection with rationale combinations of targeted agents is needed for success.
