AIM To examine healthcare resource utilizationpatterns and costs accrued by carcinoid syndrome （Cs）patients with and without ***： We conducted a retrospective cohort studyusing Marketscan data from 1/1/2002-12/31/*** diagnosed Cs patients had 1 medical claim forCs （ICD-9-CM code 259.2） plus either ≥ 1 additionalclaim for Cs or for carcinoid tumors （ICD-9-CM 209.x）, and had no evidence of Cs for 1 year prior to index Cs diagnosis, in commercially-insured patients 〈 65years old. Patients were required to have continuousenrollment one year prior and after index date （firstclaim with Cs diagnosis in the ID period）. We identifiedpatients with evidence of non-infectious diarrhea （ICD-9-CM codes 564.5 and 787.91） within one year from theindex date. Overall and Cs-related healthcare resourceutilization and costs were compared between patientswith and without non-infectious diarrhea during theone year period after the index ***： There were 2822 newly diagnosed Cspatients; 534 （18.9%） had evidence of non-infectiousdiarrhea. Compared to patients without non-infectiousdiarrhea, non-infectious diarrhea patients morecommonly had at ≥ 1 Cs-related hospitalization（13.7% vs 7.2%）, ≥ 1 Cs-related ED visit （11.0% vs4.4%）, and Cs-related office visits in one year （6.9 vs4.1; all p 〈 0.001）. After adjusting for demographics,region, number of chronic conditions and the CharlsonComorbidity Index, the proportions of patients withany and with Cs-related hospitalizations were 9.7%and 6.8% higher, respectively, among non-infectiousdiarrhea patients compared to those with without noninfectiousdiarrhea （p 〈 0.001）. Unadjusted costs weresignificantly higher among non-infectious diarrheapatients vs those without non-infectious diarrhea. Thenon-infectious diarrhea group was also more costly,with adjusted mean annual costs of $81610, comparedto $51719 in the group without non-infectious diarrhea（p 〈 0.001）.CONCLUsION： Diarrhea is burdensome and costlyin Cs patients. Cyclin dependent kinases (CDKs) have recently raised considerable attention because of their central role in the regulation of cell cycle progression. A high incidence of genetic mutation of CDK substrates and deregulaton of CDK modulators were found in a number of disease states,particularly in cancer. A novel series of unsymmetrical substituted indolocarbazoles were synthesized and their kinase inhibitory capability was evaluated in vitro. 6-substtuted indolocarbazoles were found to be highly potent and selective D1/CDK4 inhibitors. These indolocarbazoles exhibited ATP competitive D1/CDK4 activity and inhibited tumor cell growth,arrested tumor cell at G1 phase. These molecules demonstrated potent anti-tumor activity and inhibited pRb phosphorylation at s780 in the human lung carcinoma (Calu6) and non-small cell lung carcinoma (NCI-H460) xenograft models. The results indicate that these small molecules have potential as therapeutic agents in cancer chemotherapeutc agents.

OBJECTIVEs: The purpose of this study was to assess the relationship between b ody mass index(BMI) and the prognostic value of myocardial perfusion single-pho ton emission computed tomography(MPs). BACKGROUND: The prognostic value of MPs i n the obese has not been evaluated. METHODs: We studied 4,720 patients with and 10,019 patients without known coronary artery disease(CAD) who underwent rest Tl -201/stress Tc-99m sestamibi MPs, including 5,233 gated MPs studies and follow ed up(mean 2.7 to 3.2 years). Patients were categorized as normal weight(BMI 18. 5 to 24.9 kg/m2), overweight(BMI 25.0 to 29.9 kg/m2), or obese(BMIsULTs: Unadjusted annual rates of cardiac death(CD) rose versus stress MPs abn ormalities in all weight groups(pse or overweight patients with or without known CAD who had normal MPs were at low CD risk(similar to normal weight patients. In CAD, obese and overweight patients with abnormal MPs had lower rates of CD compared with normal weight patients(ps with low ejection fraction(EF) by gated MPs, those with normal weight had high est CD rate(p=0.001). Multivariable models revealed that BMI was not a predictor of CD in suspected CAD patients(hazard ratio[HR] 0.99; 95%confidence interval[ CI] 0.95 to 1.02) but was an independent inverse predictor of CD in known CAD pa tients(HR 0.95; 95%CI 0.92 to 0.98), especially in women, adenosine stress, low EF, or abnormal perfusion. CONCLUsIONs: Normal MPs was associated with low risk of CD in patients of all weight categories. In patients with known CAD undergoing MPs, obese and overweight patients were at lower risk of CD over three years than normal weight patients.
