Role of colectomy in preventing recurrent primary sclerosing cholangitis in liver transplant recipients

AIM To study the published evidence on the impact of colectomy in preventing recurrent primary sclerosing cholangitis(r PSC). METHODS An unrestricted systematic literature search in Pub Med, EMBASE, Medline Ovid SP, ISI Web of Science, Lista(EBSCO) and the Cochrane library was performed on clinical studies investigating colectomy in liver transplantation(LT) recipients with and without r PSC in the liver allograft. Study quality was evaluated according to a modification of the methodological index for non-randomized studies(MINORS) criteria. Primary endpoints were the impact of presence, timing and type of colectomy on r PSC. Overall presence of inflammatory bowel disease(IBD), time of IBD diagnosis, posttransplant IBD and immunosuppressive regimen were investigated as secondary *** The literature search yielded a total of 180 publications. No randomized controlled trial was identified. Six retrospective studies met the inclusion criteria of which 5 studies were graded as high quality articles. Reporting of IBD was heterogenous but in four publications, either inflammatory bowel disease, ulcerative colitis or in particular active colitis post-LT significantly increased the risk of r PSC. The presence of an intact(i.e., retained) colon at LT was identified as risk factor for r PSC in two of the high quality studies while four studies found no effect. Type of colectomy was not associated with r PSC but this endpoint was underreported(only in 33% of included studies).Neither tacrolimus nor cyclosporine A yielded a significant benefit in disease recurrence of primary sclerosing cholangitis(PSC).CONCLUSION The data favours a protective role of pre-/peri-LT colectomy in r PSC but the current evidence is not strong enough to recommend routine colectomy for r PSC prevention.

Balancing nutrients in a toxic environment: the challenge of eating

Insect herbivores can regulate their food intake by mixing food sources with different nutrient content, but face the resulting challenge of ingesting various plant secondary metabolites. How insects deal with toxins in a complex nutrient environment is unclear. Here we investigated the influence of a classic plant secondary metabolite, allyl glucosinolate (sinigrin), and its hydrolyzed product allyl isothiocyanate (AITC), on the development of Helicoverpa armigera (Hübner) (Lepidoptera: Noctuidae) when fed on diets with different protein-to-carbohydrate (p : c) ratios. We also examined the effects of these toxins on larval biochemistry, by chemically analyzing the frass produced by insects feeding on the different diets. As expected, AITC had a greater negative effect than sinigrin on H. armigera life-history traits. However, AITC at low concentration appeared to have a positive effect on some traits. Both sinigrin and AITC-induced detoxification activity in the gut, and the reaction was related to diet protein concentration. High-protein diets can provide the required free amino acid, especially cysteine, needed for the detoxification process. The nutrient content of the diet influences how plant secondary metabolites are handled, and the use of artificial diets in experiments investigating the metabolic fate of plant secondary compounds needs to be carefully evaluated.

As the first and rate-limiting enzyme of proline degradation, PROLINE DEHYDROGENASE1 （PDH1） is tightly regulated during plant stress responses, including induction under hypoosmolarity and repression under water deficit. The plant receptor histidine kinases AHKs, elements of the two-component system （TCS） in Arabidopsis thaliana, are pro- posed to function in water stress responses by regulating different stress-responsive genes. However, little information is available concerning AHK phosphorelay-mediated downstream signaling. Here we show that the Arabidopsis type-B response regulator 18 （ARR18） functions as a positive osmotic stress response regulator in Arabidopsis seeds and affects the activity of the PDH1 promoter, known to be controlled by C-group bZlP transcription factors. Moreover, direct physical interaction of ARR18 with bZIP63 was identified and shown to be dependent on phosphorylation of the conserved aspar- tate residue in the ARR18 receiver domain. We further show that bZlP63 itself functions as a negative regulator of seed germination upon osmotic stress. Using reporter gene assays in protoplasts, we demonstrated that ARR18 interaction negatively interferes with the transcriptional activity of bZIP63 on the PDH1 promoter. Our findings provide new insight into the function of ARR18 and bZIP63 as antagonistic regulators of gene expression in Arabidopsis.

目的:盾构隧道在成层土中掘进时,地下水渗流容易引起盾构开挖面失稳破坏。本文提出考虑地下水渗流的机动场模型,探讨成层土中地下水渗流规律,研究渗流对开挖面稳定性的影响,并提出渗流条件下成层土中盾构支护压力的计算方法。创新点:1.提出考虑地下水渗流盾构开挖面失稳机动场的模型;2.建立成层土中地下水渗流模型;3.推导考虑地下水渗流的盾构开挖面极限支护压力上限解。方法:1.根据已有工程案例,对渗流条件下成层土中盾构开挖面失稳进行受力分析(图5),并提出开挖面失稳机动场模型(图6);2.通过上限分析,推导得到盾构开挖面失稳极限支护压力计算公式(公式29);3.对成层土中地下水渗流进行数值模拟,并采用理论模型(图15)对渗流规律进行表征;4.研究极限支护压力对地下水渗流因素的敏感性。结论:1.地下水渗流在失稳土体内部产生渗流力作用,在盾构开挖面上也对支护压力产生抵消作用。2.提出成层土中考虑地下水渗流的失稳机动场模型,并推导出极限支护压力上限解;3.在盾构土舱未进行渗透性改良的条件下,成层土中地下水渗流在1200 s内达到稳定,其中,穿越层渗流方向主要为水平向,而覆土层中主要为竖向渗流;4.考虑渗流影响,本文上限解预测的支护压力值更为合理。

Sperm banking before gonadotoxic treatment:is it worth the effort?

We aimed to compare the sperm quality in different cancer types and benign diseases before gonadotoxic treatment,and assess the usage rate of cryopreserved sperm for assisted reproductive treatment(ART).This retrospective study was conducted at two university clinics between January 2008 and July 2018.A total of 545 patients suffering from cancer or benign diseases were included in the *** pretreatment sperm analyses were based on the World Health Organization(WHO)*** with testicular malignancy(TM)showed a significantly lower sperm count(median[interquartile range]:18.7×10^(6)[5.3×10^(6)-43.0×10^(6)]ml^(-1);P=0.03)as well as total sperm count(42.4×10^(6)[13.3×10^(6)-108.5×10^(6)]per ejaculate;P=0.007)compared to other malignant and benign *** addition,patients with nonseminomatous TM showed the lowest sperm count(14.3×10^(6)[6.0×10^(6)-29.9×10^(6)]ml^(-1),vs seminomas:16.5 x 106[4.6×10^(6)-20.3×10^(6)]ml^(-1);P=0.001).With reference to the WHO 2010 guidelines,approximately 48.0%of the patients with TM and 23.0%with hematological malignancies(HM)had *** the observation period,only 29 patients(5.3%)used their frozen sperms for 48 ART cycles,resulting in 15 clinical pregnancies and 10 live *** sperm quality varies with the type of underlying disease,with TM and HM patients showing the lowest sperm *** to the observed low usage rate of cryopreserved sperm,further patient interviews and sperm analyses should be included in the routine oncologic protocols to avoid unnecessary storage ***,sperm banking is worth the effort as it provides hope for men who cannot reproduce naturally after gonadotoxic treatment.

Conceptual Design of a Butyl-levulinate Reactive Distillation Process by Incremental Refinement

Butyl-levulinate has been identified as a promising fuel candidate with high oxygen content. Its com- bustion in diesel engines yields very low soot and NOx emissions. It can be produced by the esterification of butanol and levulinic acid, which themselves are platform chemicals in a biorenewables-based chemical supply chain. Since the equilibrium of esterification limits the conversion in a conventional reactor, reactive distillation can be applied to overcome this limitation. The presence of the high-boiling catalyst sulfuric acid requires a further separation step downstream of the reactive distillation column to recover the catalyst for recycle. Optimal design specifications and an optimal operating point are determined using rigorous flowsheet optimization. The challenging optimization problem is solved by a favorable initialization strategy and continuous reformulation. The design identified has the potential to produce a renewable transportation fuel at reasonable cost.

Characteristics of Momentum and Heat Transfer over Semiarid Grasslands with Different Grazing Intensities in Inner Mongolia,China

The drag coefficient （Ca） and heat transfer coefficient （Ch） with the bulk transfer scheme are usually used to calculate the momentum and heat fluxes in meteorological models. The aerodynamic roughness length （z0m） and thermal roughness length （z0h） are two crucial parameters for bulk transfer equations. To improve the meteorological models, the seasonal and interannual variations of Z0m, Z0h, coefficient kB-1, Cd, and Ch were investigated based on eddy covariance data over different grazed semiarid grasslands of Inner Mongolia during the growing seasons （May to September） from 2005 to 2008. For an ungrazed Leymus chinensis grassland （ungrazed since 1979）, Z0m and z0h had significant seasonal and in- terannual variations. Zorn was affected by the amount and distribution of rainfall, kB 1 exhibited a relatively negative variation compared with z0h, which indicates that the seasonal variation of z0h cannot be described by kB 1. To parameterize Zorn and z0h, the linear regressions between ln（z0m）, ln（z0h）, and the leaf area index （LAI） were performed with R2=0.71 and 0.83. The monthly average kB-1 was found to decrease linearly with LAI. The four-year averaged values of Ca and Ch were 4.5 × 10^-3 and 3.9× 10^-3, respectively. The monthly average Cd only varied by 8% while the variation of Ch was 18%, which reflects the dif- ferent impacts of dead vegetation on momentum and heat transfer at this natural grassland. Moreover, with the removal of vegetation cover, grazing intensities reduced Z0m, Z0h, Cd, and Ch.
