Intra-procedural arrhythmia during cardiac catheterization: A systematic review of literature

BACKGROUND Cardiac catheterization is among the most performed medical procedures in the modern *** were sporadic reports indicating that cardiac arrhythmias are common during cardiac catheterization,and there are risks of developing serious and potentially life-threatening arrhythmias,such as sustained ventricular tachycardia(VT),ventricular fibrillation(VF)and high-grade conduction disturbances such as complete heart block(CHB),requiring immediate ***,there is lack of systematic overview of these *** To systematically review existing literature and gain better understanding of the incidence of cardiac arrhythmias during cardiac catheterization,and their impact on outcomes,as well as potential approaches to minimize this *** We applied a combination of terms potentially used in reports describing various cardiac arrhythmias during common cardiac catheterization procedures to systematically search PubMed,EMBASE and Cochrane databases,as well as references of full-length *** During right heart catheterization(RHC),the incidence of atrial arrhythmias(premature atrial complexes,atrial fibrillation and flutter)was low(<1%);these arrhythmias were usually transient and *** associated with the development of a new RBBB at a rate of 0.1%-0.3%in individuals with normal conduction system but up to 6.3%in individuals with pre-existing left bundle branch *** patients may require temporary pacing due to transient *** premature ventricular complexes or non-sustained VT are common during RHC(up to 20%of cases).Sustained ventricular arrhythmias(VT and/or VF)requiring either withdrawal of catheter or cardioversion occurred infrequently(1%-1.3%).During left heart catheterizations(LHC),the incidence of ventricular arrhythmias has declined significantly over the last few decades,from 1.1%historically to 0.1%*** overall reported rate of VT/VF in diagnostic LHC and coronary angiography is 0.8

Purpose in life among very old men

This paper provides very old men’s experiences of and reflections on purpose in life. The answers from an interview question about purpose in life from 23 men were analyzed by using qualitative content analysis. The results revealed three content areas: talking of purpose of life in general, talking of own purpose in life and reflections on purpose in life. Our findings showed that very old men experienced purpose in life most strongly when remembering the past and describing their earlier work. The old men reflected on purpose in life not just from their own individual perspectives, but also from a more reflective and analytic perspective.

Cardiac implications of thrombotic thrombocytopenic purpura

Thrombotic thrombocytopenic purpura(TTP) is a multisystem disorder that essentially can affect any organ in the human body. The hallmark of the pathogenesis in TTP is the large von Willebrand factor multimers on plateletmediated micro-thrombi formation, leading to microvascular *** studies showed that cardiac arrest and myocardial infarction are the most common immediate causes of death in these patients. Clinical manifestations of cardiac involvement in TTP vary dramatically, from asymptomatic elevation of cardiac biomarkers, to heart failure, MI and sudden cardiac death. There is limited knowledge about optimal cardiac evaluation and management in patients with TTP. The absence of typical cardiac symptoms,combined with complicated multi-organ involvement in TTP, may contribute to the under-utilization of cardiac evaluation and treatment. Prompt diagnosis and timely initiation of effective therapy could be critically important in selected cases. Based on our experience and this review of the literature, we developed several recommendations for focused cardiac evaluation for patients with acute TTP:(1) patients with suspected or confirmed TTP should be screened for the potential presence of cardiac involvement with detailed history and physical,electrocardiogram and cardiac enzymes;(2) clinical deterioration of TTP patients warrants immediate cardiac reevaluation;(3) TTP patients with clinical evidence of cardiac involvement should be monitored for telemetry, cardiac biomarkers and evaluated with transthoracic echocardiography. These patients require urgent targeted TTP treatment as well as cardiac-specific treatment. Aspirin therapy is indicated for all TTP patients. Since epicardial coronary artery involvement is rare, cardiac catheterization is usually not required, given the high risk for hemorrhage and kidney injury;(4) we recommend evidence-based medical therapy for ischemic symptoms and heart failure. STIM1 expression is associated with osteosarcoma cell survival

Objective: To examine the role of store-operated calcium entry(SOCE) and stromal interaction molecule 1(STIM1) in survival and migration of osteosarcoma cells and investigate what blockade of store-operated Ca^(2+)contributes to the regulation of osteosarcoma ***: First, we examined the expression levels of STIM1 in osteosarcoma cell lines by Western analysis and in tissue specimens by immunohistochemistry. Second, we investigated the effect of SOCE and STIM1 on osteosarcoma cell viability using MTS assays and on cell proliferation using colony formation. Third, we investigated the role of SOCE and STIM1 in cell migration using wound healing assays and Boyden chamber assays. Finally, we studied the effect of SOCE on the nuclear factor of activated T-cells cytoplasmic 1(NFATc1)activity by luciferase ***: STIM1 was overexpressed in osteosarcoma cell lines and tissue specimens and was associated with poor survival of osteosarcoma patients. Also, inhibition of SOCE and STIM1 decreased the cell viability and migration of osteosarcoma cells. Furthermore, our results showed that blockade of store-operated Ca^(2+) channels involved down-regulation of NFATc1 in osteosarcoma ***: STIM1 is essential for osteosarcoma cell functions, and STIM1 and Ca^(2+) entry pathway could be further explored as molecular targets in the treatment of osteosarcoma.

AIM:To investigate the effect of short-chain fatty ac-ids (SCFAs) on production of prostaglandin E2 (PGE2),cytokines and chemokines in human ***:Human neutrophils and monocytes were isolated from human whole blood by using 1-Step Polymorph and RosetteSep Human Monocyte Enrich-ment Cocktail,respectively. Human GPR41 and GPR43 mRNA expression was examined by quantitative real-time polymerase chain reaction. The calcium flux assay was used to examine the biological activities of SCFAs in human neutrophils and monocytes. The effect ofSCFAs on human monocytes and peripheral blood mononuclear cells (PBMC) was studied by measuring PGE2,cytokines and chemokines in the supernatant. The effect of SCFAs in vivo was examined by intraplantar injection into rat ***:Human GPR43 is highly expressed in human neutrophils and monocytes. SCFAs induce robust calcium flux in human neutrophils,but not in human monocytes. In this study,we show that SCFAs can in-duce human monocyte release of PGE2 and that this effect can be enhanced in the presence of lipopolysac-charide (LPS). In addition,we demonstrate that PGE2 production induced by SCFA was inhibited by pertussis toxin,suggesting the involvement of a receptor-mediated mechanism. Furthermore,SCFAs can specifi cally inhibit constitutive monocyte chemotactic protein-1 (MCP-1) production and LPS-induced interleukin-10 (IL-10) production in human monocytes without affecting the secretion of other cytokines and chemokines examined. Similar activities were observed in human PBMC for the release of PGE2,MCP-1 and IL-10 after SCFA treatment. In addition,SCFAs inhibit LPS-induced production of tumor necrosis factor-α and interferon-γ in human PBMC. Finally,we show that SCFAs and LPS can induce PGE2 production in vivo by intraplantar injection into rat paws (P < 0.01). CONCLUSION:SCFAs can have distinct antiinflammatory activities due to their regulation of PGE2,cytokine and chemokine release from human immune cells.
