Patients on chronic hemodialysis (HD) required efficient arterio-venous access. Different types of vascular access are commonly used, such as native arteriovenous (AV) fistula, prosthetic AV graft, central venous catheter, or temporary and tunnelled cuffed catheters. Vascular access—related morbidity remains the major issue for patients requiring long-term therapy. Vascular access infection constitutes the most challenging and life-threatening complication of vascular access and causes significant morbidity, loss of access and mortality. Vascular access infection is defined as local signs at the vascular access site or a positive blood culture with no know source other than the vascular access. The critical issue in the management of AV graft infection is the need to remove the infection and to maintain HD access with reduced morbidity. The diagnosis of infected AV graft is not always easy, especially with low-grade infection or atypical presentation. Fluorodeoxyglucose positron emission tomography (FDG-PET) is an imaging method that uses a radioactively labelled tracer (18 fluorodeoxy glucose) which is taken up by tissue with high metabolic activity, such as neoplastic and inflammatory lesions. FDG-PET has been proposed to visualise localisation of infection in patients with suspected prosthetic graft infection. We report a case of methicilin resistant staphylococcus aureus (MRSA) hemodialysis access infection without local inflammatory signs diagnosed on the PET-CT.

Arthroscopically assisted reconstruction of the anterior cruciate ligament (LCA) is generally a safe and effective procedure with a low rate of vascular complications. We report on a case of a 33-year-old woman with a combined arteriovenous fistula (AVF) and venous popliteal aneurysm that developed 6 months after arthroscopic anterior cruciate ligament reconstruction. At clinical exam the patient presented with left popliteal and calf pain, a tender pulsatile mass posterior to her left knee, popliteal bruit and a thrill at the popliteal fossa. CT scan showed an AVF arising from the left popliteal artery. An aneurysm was seen to fill at the same time as the popliteal vein. Findings at open surgical revision included AVF between the tibioperoneal trunk and the popliteal vein and a venous popliteal aneurysm at the level of the arteriovenous communication. The aneurysm and fistula were repaired. The patient had an uneventful follow up with complete recovery. We also report an endovascular treatment of a iatrogenic arterio-venous fistula.

水稻恶苗病是水稻生产中的重要种传真菌病害,其在我国的主要致病菌为藤仓镰孢菌Fusarium fujikuroi。本研究采用菌丝生长速率法和孢子萌发法,测定了102株藤仓镰孢菌对新型脱甲基酶抑制剂氯氟醚菌唑的敏感性,明确了其敏感性分布,同时测定了氯氟醚菌唑对藤仓镰孢菌菌丝和孢子形态、细胞膜通透性、细胞壁和细胞膜完整性、麦角甾醇和毒素合成的影响。结果表明:氯氟醚菌唑对藤仓镰孢菌菌丝生长和孢子萌发的EC50值范围分别在0.0305~0.7579μg/mL和0.1091~1.6870μg/mL,平均EC50值分别为(0.2469±0.0167)μg/mL和(0.6397±0.0324)μg/mL。此外,用EC50浓度的氯氟醚菌唑处理,可使藤仓镰孢菌菌丝扭曲破裂、孢子皱缩扁平,可破坏菌丝细胞壁和细胞膜完整性,显著降低了麦角甾醇含量,影响毒素合成。研究结果证实了氯氟醚菌唑对藤仓镰孢菌的生物活性,可为水稻恶苗病的田间防治以及氯氟醚菌唑的科学合理使用提供依据。

Efficacy of Cyanoacrylate Tissue Adhesive Application in the Management of Corneal Perforations at Preah Ang Duong Hospital in Cambodia

Purpose: The study was to evaluate the efficacy of cyanoacrylate tissue adhesive (CTA) application in corneal perforations. Method: This was a prospective study on 20 patients of corneal perforations who received cyanoacrylate tissue adhesive application as treatment between March 2021 and March 2022 at Preah Ang Duong Hospital. The primary outcome measure was success rate of CTA application, while the secondary outcome was to measure postoperative best-corrected visual acuity (BCVA) and ocular complications. Results: The mean age of patients was 44.15 ± 16.05 years old and 7 (35%) were female. Causes of perforation were microbial infection in 12 patients (60%), trauma in 5 patients (25%), and sterile melting in 3 patients (15%). The perforation of size smaller than 1.5 mm was in 8 patients (40%) while 12 patients (60%) had perforated size between 1.5 mm to 3 mm. The perforation was 60% (12 patients) central, 25% (5 patients) paracentral, and 15% (3 patients) peripherally. Out of 20 patients, 5 patients (25%) received CTA application more than 1 time. The mean glue retention was 57.60 ± 31.84 days. Success rate of glue application (defined as intact globe without surgical intervention regardless of number of CTA applications) was 85%. At the last visit, 7 patients (35%) had BCVA of 6/120 or better. Common complications were uveitis (45%), ocular hypertension (30%), cataract (25%) and neovascularization (20%). No serious complications were found. Conclusion: Cyanoacrylate tissue adhesive is an effective treatment option in sealing corneal perforations with no serious complications. .

Xanthomonas spp. cause severe bacterial diseases. However, effective strategies for prevention and management of these diseases are scarce. Thus, it is necessary to improve the efficiency of control of diseases caused by Xanthomonas. In this study, Xanthomonas oryzae pv. oryzae(Xoo), which causes rice bacterial leaf blight, has been studied as a representative. A transposon insertion library of Xoo, comprising approximately 200,000 individual insertion mutants, was generated. Transposon sequencing data indicated that the mariner C9 transposase mapped at 35.7–36.4% of all potential insertion sites, revealing 491 essential genes required for the growth of Xoo in rich media. The results show that, compared to the functions of essential genes of other bacteria, the functions of some essential genes of Xoo are unknown, 25 genes might be dangerous for the Xanthomonas group, and 3 are specific to Xanthomonas. High-priority candidates for developing broad-spectrum, Xanthomonas-specific, and environment-friendly bactericides were identified in this study. In addition, this study revealed the possible targets of dioctyldiethylenetriamine using surface plasmon resonance(SPR) in combination with high performance liquid chromatography–mass spectrometry(HPLC–MS). The study also provided references for the research of some certain bactericides with unknown anti-bacterial mode of action. In conclusion, this study urged a better understanding of Xanthomonas,provided meaningful data for the management of bacterial leaf blight, and disclosed selected targets of a novel bactericide.

Introduction: Like healthcare workers, medical students are also exposed to a risk of accidental contamination during their hospital internships. However, they do not benefit from prevention services in the same way as workers in this sector, although they actively participate in the routine care provided to patients in the various hospital departments. The objective of our study is to determine the incidence of these accidents and their medical and administrative management among these victims (medical students) not classified in the category of workers. Materials and Method: This was a cross-sectional study carried out over three months from January 15 to March 14, 2023. The study population consisted of all medical students at the Faculty of Health Sciences at the University of Bangui. Regularly enrolled students from 2nd to 7th year, on placement in hospitals in the Central African Republic and who gave their informed consent were included. Data were collected through a self-administered questionnaire. Results: A cross-sectional study was carried out among medical students concerning the incidence and management of accidents involving exposure to blood in the Central African Republic. Of the 254 students included, three (9%) had already been correctly vaccinated against the viral hepatitis B virus, 198 (77.9%) reported still having access to personal protective equipment, 133 students (52.4%) reported being victims of OBE in the past 12 months. The main types of OBE were stings (62%), followed by liquid splashes (32%) and cuts (6%). Among the 133 victims, 92.9% carried out immediate hand disinfection or abundant eye washing, 41.9% obtained the serology of the source patient and 37% reported the accident to their supervisor. Conclusion: It is important that students benefit from better prevention and management of these accidents.

Objectives: Sex work is not well documented among African men. The aim of this study was to describe the epidemiological profile of men who have sex with men (MSM) and eventually to determine the proportion of sex workers (SW) among them, as well as the proportion of MSM who have been victims of gender-based violence (GBV). Material and Methods: A cross-sectional study coordinated by the Direction de la Médecine du Travail was carried out at the headquarters of the association ALTERNATIVES in Bangui Bangui from July 1 to October 31, 2021. Consenting MSM present during the study period were systematically included. Sociodemographic variables, those relating to the future vision of the activity and to GBV were collected and analyzed using Epi-Info version 7 software. Results: Forty MSM with an average age of 23 years and mainly secondary education (75.0%) were included. The vast majority of MSM were unemployed (85.0%). 45.0% had been victims of GBV and wanted to stop working as an MSM (47.5%), and almost 2/3 (65.0%) would accept another income-generating activity in exchange for the MSM. Violence was sexual (32.5%), economic (22.5%), physical (20.0%), verbal (12.5%) and psychological (12.5%). Conclusion: The MSM were mainly young, poorly educated, unemployed, and almost half were victims of GBV. The desire to change MSM activity to another income-generating activity alongside that of MSM shows that many of them are SW, workers in the informal sector. This must be taken into account in prevention activities, even if the data needs to be confirmed on a much larger sample.

Objective: HIV-HBV co-infection is a major public health problem that has not been sufficiently explored in the Central African workplace. The aim of this study was to assess the frequency of HIV-HBV co-infection among people who living with HIV (PLHIV) in the infectious and tropical diseases department of the Centre Hospitalier Universitaire de lAmiti Sino-Centrafricaine in Bangui. Methods: A retrospective study was carried out from January 1, 2010 to December 31, 2021 in the Infectious and Tropical Diseases Department at the Amiti Sino-Centrafricaine University Hospital. It included the files of all PLHIV, which included the results of HBV serology. A standardized form was used to collect socio-demographic and professional data by documentary review. Data was analysed using Epi-Info 7 software. Means, proportions were calculated as well as Chi square witch was significant if p-value was below 0.05. Results: The study included 265 patients, 188 were women (70.1%) and 77 men (29.1%), giving a sex ratio of 0.45. Mean age was 35.8 years, higher in men (40 years) than in women (35.8 years) (p 0.0001). The age groups 25 to 34 (37.7%) and 35 to 44 (33.6%) were in the majority (71.3%). The majority of PLHIV were unemployed (57.1%), including housewives (43.0%). HBV prevalence was 14.3%, including 7.2% among the unemployed, who account for half of all co-infections. The search for associations between HIV-HBV co-infection and all socio-demographic characteristics (age, sex, marital status) and socio-professional categories showed no significant difference (p 0.05). Conclusion: PLHIV were predominantly young adults, female, and unemployed;no occupation was significantly associated with co-infection. The vast majority of co-infected people were not covered by the occupational health system (unemployed or informal sector). Urgent action is needed to improve workers access to occupational medicine in CAR.
