Epidemiological and Anatomo Path Ological Aspects of Stomach Cancers at the Koutiala District Hospital

Aim: This paper aims to evaluate the epidemiological and pathological aspects of stomach cancer at Koutiala District Hospital. Methods: This was a retrospective study carried out in the General Surgery Department of the Koutiala District Hospital Health Centre. The records of patients admitted for stomach cancer in the period from August 1, 2017 to December 31, 2022 were collected. Patients who underwent surgery for stomach cancer were included in the study. Patients who had not undergone surgery were excluded. Results: We collected records from 40 patients operated on for stomach cancer. The median age was 58.4 years ± 5.7. Men were in the majority with 72.5% The sex ratio was 2.6. The incidence rate of stomach cancer in the general population in the circle was 4.9 cases per 100,000 population. Patients had a history of gastric ulcer in 90%, Helicobacter pylori infection in 47.5% and a notion of excessive salt consumption in 47.5%. The median duration of disease progression was 5 years ± 2.3. The seat was antro-pyloric in 77.5%. The cancer was ulcerative budding in 23 patients. Adenocarcinoma was the histological type in all patients. The cancer was classified as stage III in 30 cases and stage IV in 10 cases. Palliative surgical treatment was the most performed (47.5%). Postoperative morbidity was 15% and mortality was 42.5% at one year. Conclusion: Stomach cancer is common in the ward. Most patients consult at the stage of metastasis. Palliative surgery remains the most practical. Postoperative morbidity and mortality is very high.

Gomphrena serrata L. (Amaranthaceae) is a medicinal plant widely used traditionally in rural areas to treat many diseases, including gastrointestinal helminthiasis of small ruminants. This work aims to estimate the plant extracts phytochemical potential and study the activities of the aqueous decoction (GS_AD), aqueous macerate (GS_AM), and hydroalcoholic macerate (GS_HEM) of Gomphrena serrata extracts on two developmental stages (eggs and adult worms) of Haemonchus contortus. Phytochemical screening was carried out. The acute toxicity test was performed according to the Organisation for Economic Co-operation and Development (OECD) test guideline 423. The Haemonchus contortus eggs hatching inhibition test was performed left to incubate for 48 hours in the presence of the different plant extracts at increasing concentrations of 0.125;0.25;0.5;0.625;1;2;3 mg/mL. For the motility inhibition test on adult worms, the worms were contacted with the extracts at concentrations of 0.75;1;1.25;1.5;2;3;4 mg/mL and left to incubate for 24 hours. Albendazole and levamisole were used as standard. Phytochemical screening revealed the presence of tannins, saponins, reducing compounds, coumarins and derivatives, anthocyanins, steroids, triterpenes, and flavonoids. An acute toxicity study showed that the oral LD50 value of G. serrata was estimated to be 5000 mg/kg. All extracts showed an effect on both stages of parasite development. The hatching inhibition rate was 100% from 3 mg/mL for all three extracts. The rate of inhibition of motility was 100% from 4 mg/mL for each extract. HEM is more efficient on eggs and Haemonchus contortus adult worms than other extracts with IC50 of 0.457 mg/mL and LC50 of 1.329 mg/mL, respectively. These results suggest that Gomphrena serrata, a plant used by farmers, has anthelmintic properties.

Incidences and Risk Factors for Surgical Site Infections in Koutiala, Mali

Purpose: This paper aims to assess the incidences and risk factors for surgical site (ISO) infections in the general surgery department of the Koutiala District Hospital. Patients and Methods: This was a prospective and descriptive study from August 1, 2017 to October 31, 2020 involving all patients who underwent laparotomy in the general surgery department of the Koutiala District Hospital. Patients who were not operated on and who did not have a laparotomy were not included. Age, sex, frequency, patient history, National Nosocomial Infections Surveillance (NNIS) index, time to occur, bacteriological test results and course of infection were analyzed. Results: Fifty patients were registered. The average age was 34.2 ± 21.2 years old. Eleven patients were 60 years old or older. The hospital incidence rate was 4.3% and the community incidence rate was 6.1 cases per 100,000 population. The concept of smoking was noted in 15 patients. Patients were operated on in emergency 84% of cases. Peritonitis was the most common initial lesion with 32% of cases. Our patients had an NNIS index greater than 0 in 84% of cases. The time to develop infection from the surgical site was less than 3 days in 8 patients (16% of cases). Depending on the depth of the infection, it was superficial (cutaneous) in 39 patients (78%) and deep (subcutaneous and organic) in 11 patients (22%). Escherichia coli was the most common germ with 72% followed by staphylococcus aureus at 24%. We noted 22% morbidity. The median length of hospitalization was 9 ± 2.2 days. Conclusion: ISO is common in our service. Prevention and mastery require knowledge of risk factors.

Lawsonia inermis is a hairless plant growing in various regions of North Africa, the Indian subcontinent, and the Middle East. It possesses many medicinal attributes, including curative properties against infectious dermatoses. This study was carried out to evaluate the phytochemical profile of the crude ethanolic extract of the plant leaves and its fractions as well as their antimicrobial activities. The phytochemical profile was performed using high-performance thin-layer chromatography (HPTLC), gas chromatography-mass spectrometry (GC-MS), and high-performance liquid chromatography (HPLC). Additionally, the phenolic and flavonoid contents were determined using the Folin-Ciocalteu spectrophotometric and the aluminum trichloride methods. Antimicrobial activity was tested using disc diffusion and microdilution methods. The presence of flavonoids, tannins, sterols, and triterpenes was revealed. GC-MS detected twelve compounds main compounds consisting of saturated and unsaturated fatty acids and phenolic and terpenoid compounds among twenty-seven components. HPLC also detected high contents of phenolic acids and flavonoids. The most abundant triterpene and sterols were ursolic acid (around 43.14 g/100g DW, 13.9 g/100g dry weight (DW), and 0.68 g/100g DW) in the crude ethanolic extract of leaves (FeLi), hexane fraction (FHLi) and dichloromethane fraction (FDLi), respectively and, β-sitosterol in FeLi (56.7 mg/100g DW), FHLi (10.55 g/100g DW), FDLi (106.1 mg/100g DW) and butanol fraction (FBLi) (357.4 mg/100g DW). Among the flavonoids, rutin = 3.24 g/100g and quercetin = 0.63 g/100g in the ethanolic extract, rutin = 15.73 g/100g in the dichloromethane fraction, and rutin = 0.23 g/100g) in the aqueous fraction;and among phenolic compounds, caffeic acid (37.65 g/100g DW) and vanillic acid (22.70 g/100g DW) were the most important in the ethyl acetate fraction (FAeLi). Introduction: Anemia is one of the most common public health problems in the world and affects all ages. Objective: To describe the etiological and evolutionary profile of anemia in patients hospitalized in the Internal Medicine Unit of the Fousseyni Daou Hospital in Kayes. Methodology: It was a descriptive and cross-sectional study with retrospective data collection which took place from January 1 to December 31, 2020 at the Internal Medicine Unit of the Fousseyni Daou Hospital in Kayes. It covered all anemic patients hospitalized during the study period. Results: We identified 91 cases of anemia out of 200 patients admitted to the Unit that is a hospital prevalence of 45.5%. The age group of 26 to 35 years was the most represented, that is to say 29 cases (31.87%) with an average age of 43.55 years ± 17.48 years, the female sex predominated, 51 cases (56.04%) with a sex ratio of 0.78. Housewives represented 41 cases (45.05%). The majority of patients resided in rural areas 48 cases (52.75%). The main manifestations were: asthenia 75 cases (78.02%), vertigo 68 cases (74.72%), dyspnea 62 cases (68.13%), headaches 59 cases (64.83%), palpitations 55 cases (60.44%), conjunctival pallor 53 cases (58.24%), tachycardia 43 cases (47.25%), systolic murmur 18 cases (19.78%) and IMO 11 cases (12.09%). The associated pathologies were: infected diabetic wounds 25.27%, followed by bacterial pleuro-pneumopathy 18.68%. Biologically, microcytic anemia was the most frequent 49 cases (53.84%), followed by normocytic anemia 35 cases (38.46%) and macrocytic anemia 7 cases (7.7%). Anemia was hypochromic, 53 cases (58.24%) were more encountered compared to normochromic anemia 38 cases (41.76%). The anemia was: severe in 43 cases (47.25%), moderate 29 cases (31.87%) and mild 19 cases (20.88%). Introduction: Urinary tract infection is one of the most frequent public health problems in the world and concerns all ages. Objective: To describe the epidemiological, clinical and biological profile of urinary tract infection in the internal medicine unit of the Fousseyni Daou hospital in Kayes. Methodology: It was a descriptive and cross-sectional study with retrospective data collection which took place from January 1 to December 31, 2020 at the Internal Medicine Unit of the Fousseyni Daou Hospital in Kayes. It covered all outpatients and hospitalized patients presenting with a urinary tract infection during the study period. Results: We identified 79 cases of urinary tract infection out of 145 requests for ECBU, that is a positivity rate of 53.10% and out of a total of 1883 admissions that is a hospital prevalence of 4.19%. The age groups of 36 to 45 years and 46 to 55 years were more represented in 28 cases (35.44%) and 25 cases (31.64%) with an average age of 45 years ± 10. The female sex was predominant, 42 cases (53.16%) with a sex ratio of 0.88. Housewives were more represented 28 cases (34.44%). The majority of patients resided in rural areas 49 cases (62.03%). The history was dominated by the following pathologies: hypertension 48.10%, diabetes 36.70% and gastropathy 30.37%. The main clinical manifestations were: Urinary burning 67 cases (84.81%), Dysuria 65 cases (82.27%), Pelvic pain 59 cases (74.68%), Fever 57 cases (72.15%). The epidemiological profile of the isolated strains was dominated by enterobacteriaceae, followed by gram-positive cocci and gram-negative bacilli. The main bacteria responsible for urinary tract infections in order of frequency: Escherichia coli (45.56%), Klebsiella pneumoniae (11.39%), Pseudomonas aeruginosa (8.86%), Staphylococcus aureus (7.59%), Streptococcus sp (6.33%), Enterobacter cloacae (3.79%), Acinetobacter sp (3.79%). Trauma to the male genitalia is not very common and mainly affects a young population. There is probably a particular instinct in men to defend their external genitalia;indeed, in the flaccid state, the penis is little exposed, and fairly well protected and its mobility allows it to move with the impact of trauma and thus reduce the vulnating force. Open trauma is rare, as is testicular dislocation, the most common being closed trauma. Testicular trauma is an uncommon accident that affects young people. Trauma is most often caused by road and traffic accidents. Animal bites are rare, as is genital self-mutilation related to a psychiatric disorder. Surgical exploration is the rule except in cases of moderate testicular trauma. Our objective was to report our experience in an emergency context where treatment is poorly codified and to review the literature. We recorded four patients with external genitalia trauma managed in the urology department from April 2013 to March 2022. Our patients were children with open genitalia trauma following a traffic accident, scrotal injuries were encountered in three patients, and additional penile injuries in one patient. All patients were treated surgically. The follow-up was straightforward.

Hardware Security for IoT in the Quantum Era: Survey and Challenges

The Internet of Things (IoT) has become a reality: Healthcare, smart cities, intelligent manufacturing, e-agriculture, real-time traffic controls, environment monitoring, camera security systems, etc. are developing services that rely on an IoT infrastructure. Thus, ensuring the security of devices during operation and information exchange becomes a fundamental requirement inherent in providing safe and reliable IoT services. NIST requires hardware implementations that are protected against SCAs for the lightweight cryptography standardization process. These attacks are powerful and non-invasive and rely on observing the physical properties of IoT hardware devices to obtain secret information. In this paper, we present a survey of research on hardware security for the IoT. In addition, the challenges of IoT in the quantum era with the first results of the NIST standardization process for post-quantum cryptography are discussed.

Opilia amentacea (Opiliaceae) is a woody plant with multiple medicinal claimed effects. The present study aimed to assess the anti-inflammatory and antioxidant activities of the decoction and macerate extracts from the leaves of Opilia amentacea. Moreover, acute toxicity and phytochemical analysis were performed. The acute toxicity was evaluated on NMRI mice at 2000 mg/kg bw. The anti-inflammatory activity was studied using the carrageenan-induced mouse paw edema and the lipoxygenase inhibition assay. The radical scavenging (DPPH and ABTS), ferric-reducing antioxidant power (FRAP), and lipid peroxidation (LPO) assays were used to measure the antioxidant capacity of the extracts. Qualitative and quantitative methods served for identifying and quantifying the extract’s phytoconstituents. The decoction demonstrated low acute toxicity;the lethal dose was therefore estimated to be superior to 2000 mg/kg bw. The extracts significantly reduced the mouse paw’s thickness at 600 mg/kg bw. The extracts developed weak radical scavenging and lipid peroxidation inhibitory effects. However, the macerate showed a high ability (664.90 ± 0.71 mol Ascorbic Acid Equivalent/g dry extract) to reduce the ferric ions. Saponins, sterols, triterpenes, and flavonoids were qualitatively detected in the two extracts. Total phenolics (TP) and total flavonoids (TF) were found abundant in the extracts, especially the decoction (TP content (TPC) = 94.03 ± 2.66 mg GAE/g;TF content (TFC) = 35.05 ± 0.32 mg QE/g). Strong positive correlations existed between ferric-reducing capacity and TPC (r = 0.959) for the macerate, while TFC was mainly involved in the DPPH radical scavenging of the two extracts. Instead, most correlations were negative between the polyphenol compounds and the anti-inflammatory assays. The results indicate potent in vivo anti-inflammatory and in vitro antioxidant effects of the aqueous extracts from the leaves of Opilia amentacea. Emerging and rapidly spreading multidrug resistant bacteria constitute a rising public health concern worldwide. Acinetobacter baumannii is one of these bacteria that cause different infections including pneumonia, bacteremia, meningitis, soft-tissue, and urinary tract infections, and are associated with high mortality and economic burden. We present a case of a 43-year-old woman, admitted at the department of orthopedics, regional hospital of Ourossogui, North-East of Senegal for soft-tissue injuries. Initially diagnosed with Yersinia pestis infection, the patient was well managed before being released. Supplementary sampling for confirmatory tests allowed the detection of an extensively drug-resistant Acinetobacter baumannii clone.
