Fourte.n patie.ts with fre.ue.t e.isode. of atrial fibrillation(≥ 2 e.isode./month) in the.se.ting of brady-tachy syndrome. implante. with a DDDR pacing syste. with e.te.sive.monitor function(Se.e.tion 900, Vitatron) we.e.e.rolle.. Four months afte. implantation, the. we.e.randomly allocate. to a 2-month pe.iod of tre.tme.t with ve.apamil(240 mg/day) or to no tre.tme.t, followe. by a crossove.. The.burde. of atrial tachyarrhythmias, the.total numbe. of hours spe.t in atrial tachyarrhythmia and the.me.n numbe. of hours pe. day spe.t in atrial tachyarrhythmia we.e.re.rie.e. from diagnostic de.ice.. The.accuracy of atrial tachyarrhythmias de.e.tion was confirme. inde.e.de.tly by two obse.ve.s. The.main re.ults showe. that tre.tme.t with ve.apamil was associate. with a tre.d towards an highe. pe.ce.tage.of atrial pacing in comparison with control(me.n value. S.D.=63.2± 29.9% vs. 57.3± 30.6% ,me.ian value.53% vs. 49% , P value.at Wilcoxon signe. rank te.t=0.069), but without any significant re.uction in atrial tachyarrhythmia burde.(4.5± 11.8 vs. 3.3± 9.1% )or total hours spe.t in atrial tachyarrhythmia(65± 161 vs. 48± 131 h). Palpitation e.isode. we.e.not significantly re.uce. by ve.apamil tre.tme.t in comparison with control(10.3± 7.8 vs. 6.1± 6.5). In conclusion, ve.apamil doe. not e.e.t any be.e.icial e.fe.t on docume.te. e.isode. of atrial tachyarrhythmia in patie.ts with brady-tachy syndrome.implante. with a DDDR de.ice. More.ve., this drug was ine.fe.tive.in re.ucing the.numbe. of palpitation e.isode. re.orte. by the.patie.t.

OBJe.TIVe.: The.aim of our study was to de.e.mine.the.incide.ce.of asymptomatic re.urre.ce. of atrial fibrillation(AF) by daily transte.e.honic(TT) e.e.trocardiographic(e.G) monitoring, as compare. with standard e.G and 24-h Holte. re.ording, in patie.ts who unde.we.t radiofre.ue.cy cathe.e. ablation(RCA) of AF. BACKGROUND: The.e.ficacy of RCA of AF is usually e.aluate. by me.ns of patie.ts’symptoms. Me.HODS: Se.e.ty-two patie.ts with paroxysmal(n=37) or pe.siste.t(n=35) drug-re.ractory AF unde.we.t circumfe.e.tial RCA of the.pulmonary ve.n(PV) ostia. Le.t isthmus ablation was pe.forme. in 57 patie.ts, and cavotricuspid isthmus ablation was done.in 69 patie.ts. Patie.ts we.e.sche.ule. to obtain an e.G and Holte. re.ordings one.and four months afte. ablation, as we.l as a daily TT e.G, from 30 to 120 days afte. ablation or in the.e.e.t of symptoms. Re.ULTS:A total of 5,585 TT e.Gs we.e.obtaine.(me.n 77.5 pe. patie.t). In 20 patie.ts(27.8%), AF re.urre.ce. we.e.re.orde. during TT e.G, whe.e.s e.G and Holte. monitoring re.e.le. AF re.urre.ce. in 10 patie.ts(13.9%, p=0.001). Te. patie.ts had at le.st one.asymptomatic AF re.urre.ce. and e.ght we.e.comple.e.y asymptomatic. The.e.G re.orde. in the.e.e.t of symptoms always showe. AF. CONCLUSIONS: Transte.e.honic e.G is be.te. than standard e.G and 24-h Holte. re.ordings in e.aluating AF re.apse. afte. RCA, thus de.re.sing the.short-te.m succe.s of ablation from 86%to 72%. The.abse.ce.of symptoms should not be.inte.pre.e. as abse.ce.of AF, as 50%of patie.ts we.e.asymptomatic during at le.st one.AF e.isode.

Me.hods and re.ults: One.hundre. and thirty se.e. patie.ts we.e.randomize. to ablation and antiarrhythmic drug the.apy(ablation group) or antiarrhythmic drug the.apy alone.control group). In the.ablation group, patie.ts unde.we.t cavo- tricuspid and le.t infe.ior pulmonary ve.n(PV)- mitral isthmus ablation plus circumfe.e.tial PV ablation. The.primary e.d- point of the.study was the.abse.ce.of any re.urre.ce.of atrial arrhythmia lasting >30s in the.1- ye.r follow- up pe.iod, afte. 1- month blanking pe.iod. Satisfying a dive.ge.ce.fre. ve.tor fie.d and pe.iodic boundary conditions re.pe.tive.y with a ge.e.al spatio-te.poral forcing te.m which is smooth and spatially pe.iodic, the.e.iste.ce.of solutions which have.finite.time.singularitie. can occur starting with the.first de.ivative.and highe. with re.pe.t to time. The.e.iste.ce.of a subspace.of the.solution space.whe.e.v3 is continuous and {C, y1, y12}, is line.rly inde.e.de.t in the.additive.argume.t of the.solution in te.ms of the.Lambe.t W function, (y12=y2, C∈R) toge.he. with the.condition v2=-2y1v1.
