Pancreatic head cancer in patients with chronicpancreatitis

BACKGROUNd: Chronic pancreatitis (CP) is a risk factor of pancreatic adenocarcinoma (PA). The discovery of a pancreatic head lesion in CP frequently leads to a pancreaticoduo denectomy (Pd) which preceded by a multidisciplinary meeting(MM). The aim of this study was to evaluate the relevance between this indication of Pd and the definitive pathological ***: Between 2000 and 2010, all patients with CP who underwent Pd for suspicion of PA without any histological proof were retrospectively analyzed. The operative decision has always been made at an MM. The definitive pathological finding was retrospectively confronted with the decision made at an MM, and patients were classified in two groups according to this concordance (group 1) or not (group 2). Clinical and biological parameters were analyzed, preoperative imaging were reread and confronted to pathological findings in order to identify predictive factors of malignant ***: during the study period, five of 18 (group 1) patients with CP had Pd were histologically confirmed to have PA, and the other 13 (group 2) did not have PA. The median age was52.5 ±8.2 years (gender ratio 3.5). The main symptoms were pain (94.4%) and weight loss (72.2%). There was no patient’s death. Six (33.3%) patients had a major complication (Claviendindo classification ≥3). There was no statistical difference in clinical and biological parameters between the two groups. The rereading of imaging data could not detect efficiently all patients with ***: Our results confirmed the difficulty in detecting malignant transformation in patients with CP before surgery and therefore an elevated rate of unnecessary Pd was found. A uniform imaging protocol is necessary to avoid Pd as a less invasive treatment could be proposed.

Kinetic and binding equilibrium studies of dihydroflavonol 4-reductase from Vitis vinifera and its unusually strong substrate inhibition

dihydroflavonol 4-reductase (dFR), a member of the short-chain dehydrogenase family, catalyzes the last common step in the biosynthesis of flavan-3-ols and condensed tannins. Initial rates of dFR were measured by monitoring the 340-nm absorbance decrease resulting from the joint consumption of dihydroquercetin (dHQ) and NAdPH, as a function of pH, temperature and ionic strength. At pH 6.5 and 30o C, substrate inhibition was observed above 30 μM dHQ. At lower/non-inhibitory dHQ concentrations, NAdP+ behaves as a competitive inhibitor with respect to NAdPH and as a mixed inhibitor with respect to dHQ, which supports a sequential ordered mechanism, with NAdPH binding first and NAdP+ released last. Binding-equilib-rium data obtained by means of the chromatographic method of Hummel and dreyer at pH 7.5 and by isothermal calorimetric titration at pH 6.5 led to the conclusion that ligands of the apoenzyme included NAdPH, NAdP+ and dHQ. The mechanism which best accounts for substrate inhibition at pH 6.5 in the absence of product involves the formation of a binary non-productive *** complex. Thus, a productive ternary complex cannot be formed when dHQ binds first. This mechanism of inhibition may prevent the accumulation of unstable leucoanthocyanidins within cells.

Low-to-Moderate Alcohol Consumption May Be Safe When Taking Methotrexate for Rheumatoid Arthritis or Psoriasis

It is unclear if consumption of small to moderate amounts of alcohol is safe while taking methotrexate. We set out to determine whether there is an association between liver enzyme abnormalities and alcohol consumption in subjects taking methotrexate. The study sample was identified from the database of the South Central Veteran Affairs Healthcare Network, consisting of 10 hospitals in Texas, Oklahoma, Mississippi and Louisiana. From a cohort of 2443 eligible Veterans with rheumatoid arthritis and/or psoriasis taking methotrexate from 10/1/2007 to 9/30/2009, 120 cases with abnormal liver-enzyme elevation were randomly selected to compare with 120 controls. data were collected from mailed survey forms that inquired about alcohol consumption habits, physicians’ advice on alcohol, and methotrexate compliance. There was no significant difference in the number of non-drinkers and low-to-moderate alcohol drinkers between cases and controls (p = 0.217). Few persons identified themselves as heavy drinkers. Our data suggest that it is likely safe for patients with rheumatoid arthritis and psoriasis to consume low-to-moderate amounts of alcohol while taking methotrexate. However, alternative methods to improve capturing alcohol consumption in heavy drinkers are needed for more comprehensive results.

Engineered Biofluids are paving the way to industrialized and specifically produced and tailored functional fluids such as coolant and dielectric, high purity traceable media, for the electrical and electronic industries. dielectric fluid compositions are commonly used in electrical devices, particularly in transformers. These liquids have the aim to isolate the various conductive elements of the device and to limit the heating of the equipment during its operation;in order to minimize, or maintain at higher power, the size of the device and to increase the lifespan of it. Concomitantly a number of bio and traditional processing advancements are made associated with pioneering process technologies which are outlined within the prime context of this paper. Enzyme Engineering and Cocktailing add “A New dimension to Softer Greener Chem-Bio Approaches” which are referenced beyond curiosity purpose. Some of them are possibly “revolutionary” more than evolutionary. Testing has to evolve accordingly to appreciate the challenges ahead in semi-extreme conditions which are relevant to climate changes as well. A significant part of this orientation work addresses and exemplifies these necessary testing innovations, likely adaptable to smart and responsive connecting, and further down the line bigger data role and learning machine evolutionary concepts. One other innovative part of this exploratory work, is the influence that such illustrative localized testing, with integrated sensors/induced-tracers, and online interpretation, may have on the smart grid developments;whereby production, transmission, distribution and consumption of energy can be made more reliable, more effective and more predictive and can also have an impact on the performance, reliability and sustainability of the equipment itself. Long Term Survival after Resection of a Small Bowel Metastasis of Lung Adenocarcinoma: A Case Report

Background: Small bowel metastases of lung adenocarcinoma represent a rare clinical entity, often diagnosed after complications due to obstruction, bleeding or bowel perforation. Case Report: We report a case of ileal metastasis discovered during an intestinal perforation. The patient was a 47-year-old man hospitalized in emergency for acute abdominal pain, with right lower quadrant abdominal defense and a clinical history of left lower pulmonary lobectomy for lung adenocarcinoma 6 months before. Exploratory laparotomy for peritonitis revealed a single ileal tumor which was resected with lymph nodes excision. Pathology finding revealed that it was compatible with an ileal metastasis of his lung adenocarcinoma. Conclusions: The suspicion of a small bowel metastasis of lung carcinoma, although rare, must be taken in consideration and to carry out an extensive surgery, because only this will have an improvement on the survival of these patients. The purpose of this investigation is to add a new case report to the few that exist in order to provide a therapeutic approach to these exceptional cases.

Assessment of a Public Participation Tool by Experts: A Case Study of Quebec, Canada

In theory, sustainable forest management (SFM) puts public participation at the epicenter of the decision-making process. However, in reality, the situation is different. The growing importance of public participation seems essential for the implementation of sustainable forest management. The aim of this study is to assess the implementation of a theoretical concept closely related to SFM, integrated land and resource management (ILRM). This Canadian concept aims to integrate many forest functions using participatory tools. Using semi-structured interviews with experts, we clarify our theoretical concept and adapt it to reality. Our results show that better communication between stakeholders is necessary. It is also important to increase the flexibility of the timber harvest system and adapted forest zoning should be considered. A new concept, the forest culture, seems to be a key element that needs to be considered upstream of forest management. A new definition of ILRM is proposed in order to better dana;">dana;">dana;">reach social values.

AIM: To investigate the conformity of management practices of gastrointestinal hemorrhage in cirrhotic patients with relevant guidelines. METHOdS: A questionnaire on the management of digestive bleeding was completed for all consecutive cirrhotic patients admitted to 31 French hospitals. RESULTS: One hundred and twenty-six bleeding events were recorded. It was the first bleeding episode in 79 patients (63%), of whom 40 (51%) had a prior diagnosis of cirrhosis and 25 (32%) had previously undergone an endoscopy. The bleeding episode was a recurrence in 46 patients (37%). The median time between onset and admission was 4 h, but exceeded 12 h in 42% of cases. There was an agreement between centers forearly vasoactive drug administration (87% of cases), association with ligation (42%) more often than sclerosis (21%) at initial endoscopy, and antibiotic prophylaxis (64%). By contrast, prescription of beta-blockade alone or in combination (0 to 100%, P = 0.003) for secondary prophylaxis and lactulose (26% to 86%, P = 0.04), differed among centers. CONCLUSION: In French hospitals, management of bleeding related to portal hypertension in cirrhotic patients is generally in keeping with the consensus. Broad variability still remains concerning beta-blockade use for secondary prophylaxis. Screening for esophageal varices, the use of antibiotic prophylaxis and patients information need to be improved.

Introduction: We sought to explore iodine status (expressed as urine iodine excretion) in a group of women with a variety of breast diseases to characterize the relationship between iodine and breast physiology. Methods: demographic and clinical data were recorded and random urine iodine levels were checked in 415 euthyroid patients over 5 years during clinic visits. Results: Pre menopausal women excreted less iodine in their urine than post menopausal women. Post menopausal women who took hormone supplementation had an excretion level similar to pre menopausal women. Women with breast cancer excreted less iodine than those without, and iodine excretion increased after treatment with aromatase inhibitors. Conclusions: These results demonstrate that there is a relationship between estrogen status and iodine excretion in euthyroid women. This suggests that there may be a threshold for total body iodine required for prevention of breast disease.

despite its proximity to other well studied islands, Cebu has received little attention from herpetologists, most likely because of early deforestation and the perception very little natural habitat remains for amphibians and reptiles. In this study, we present a preliminary assessment of island＇s herpetofauna, focusing our field work on Cebu＇s last remaining forest fragments and synthesizing all available historical museum distribution data. We surveyed amphibians and reptile populations using standardized methods to allow for comparisons between sites and assess sufficiency of sampling effort. Fieldwork resulted in a total of 27 species recorded from five study sites, complementing the 58 species previously known from the island. Together, our data and historical museum records increase the known number of Cebu＇s resident species to 13 amphibians（frogs） and 63 reptiles（lizards, snakes, turtle, crocodile）. We recorded the continued persistence Cebu＇s rare and endemic lizard（Brachymeles cebuensis） and secretive snakes such as Malayotyphlops hypogius, and Ramphotyhlops cumingii, which persist despite Cebu＇s long history of widespread and continuous habitat degradation. Most species encountered, including common and widespread taxa, appeared to persist at low population abundances. To facilitate the immediate recovery of the remaining forest fragments, and resident herpetofauna, conservation effort must be sustained. However, prior to any conservation interventions, ecological baselines must be established to inform the process of recovery.

Impinging jets are frequently adapted for cooling overheated parts. With the film cooling technique, this process improves thermal exchanges between walls and fluid. However if many works have concerned only the thermal aspect of this problem [1], its dynamic field has been rarely studied especially for multiple impingements. As the two phenomena cannot be totally dissociated, we have undertaken the aerodynamic and thermal study of jets impinging on a plane wall. Various techniques have been used as visualizations (spreading over method, LASER sheet visualizations), LdA measurements to propose a topology schema of the flow and infrared thermography.
