This article compliments the S18 session( transmission of traditional ecological knowledge and conservation of medicinal plants) at the 13th International Congress of Ethnobiology( ISE) in Montpellier,France( May2012). The aim of the article is to provide an insight into the livelihoods of a Berber community,a village of the High Atlas Mountains of Morocco,of the local biodiversity and most importantly,the prevalent use of medicinal plants for primary health care. It seeks to demonstrate the value of traditional ecological and herbal knowledge for the subsistence of the community and emphasises the vital role that family and community connections play for the transmission of traditional herbal knowledge within the community.

BACKGROUND Glomerulopathy with fibrillary deposits is not uncommon in routine nephropathology practice,with amyloidosis and fibrillary glomerulonephritis being the two most frequently encountered entities.Renal amyloid heavy and light chain(AHL)is relatively uncommon and its biopsy diagnosis is usually limited to cases that show strong equivalent staining for a single immunoglobulin(Ig)heavy chain and a single light chain,further supported by mass spectrometry(MS)and serum studies for monoclonal protein.But polyclonal light chain staining can pose a challenge.CASE SUMMARY Herein we present a challenging case of renal AHL with polyclonal and polytypic Ig gamma(IgG)staining pattern by immunofluorescence.The patient is a 62-yearold Caucasian male who presented to an outside institution with a serum creatinine of up to 8.1 mg/dL and nephrotic range proteinuria.Despite the finding of a polyclonal and polytypic staining pattern on immunofluorescence,ultrastructural study of the renal biopsy demonstrated the presence of fibrils with a mean diameter of 10 nm.Congo red was positive while DNAJB9 was negative.MS suggested a diagnosis of amyloid AHL type with IgG and lambda,but kappa light chains were also present supporting the immunofluorescence staining results.Serum immunofixation studies demonstrated IgG lambda monoclonal spike.The patient was started on chemotherapy.The chronic renal injury however was quite advanced and he ended up needing dialysis shortly after.CONCLUSION Tissue diagnosis of AHL amyloid can be tricky.Thorough confirmation using other available diagnostic techniques is recommended in such cases.

BACKGROUND Perioperative surgical home(PSH)is a novel patient-centric surgical system developed by American Society of Anesthesiologist to improve outcomes and patient satisfaction.PSH has proven success in large urban health centers by reducing surgery cancellation,operating room time,length of stay(LOS),and readmission rates.Yet,only limited studies have assessed the impact of PSH on surgical outcomes in rural areas.AIM To evaluate the newly implemented PSH system at a community hospital by comparing the surgical outcomes using a longitudinal case-control study.METHODS The research study was conducted at an 83-bed,licensed level-III trauma rural community hospital.A total of 3096 TJR procedures were collected retrospectively between January 2016 and December 2021 and were categorized as PSH and non-PSH cohorts(n=2305).To evaluate the importance of PSH in the rural surgical system,a case-control study was performed to compare TJR surgical outcomes(LOS,discharge disposition,and 90-d readmission)of the PSH cohort against two control cohorts[Control-1 PSH(C1-PSH)(n=1413)and Control-2 PSH(C2-PSH)(n=892)].Statistical tests including Chi-square test or Fischer’s exact test were performed for categorical variables and Mann-Whitney test or Student’s t-test were performed for continuous variables.The general linear models(Poisson regression and binomial logistic regression)were performed to fit adjusted models.RESULTS The LOS was significantly shorter in PSH cohort compared to two control cohorts(median PSH=34 h,C1-PSH=53 h,C2-PSH=35 h)(P value<0.05).Similarly,the PSH cohort had lower percentages of discharges to other facilities(PSH=3.5%,C1-PSH=15.5%,C2-PSH=6.7%)(P value<0.05).There was no statistical difference observed in 90-d readmission between control and PSH cohorts.However,the PSH implementation reduced the 90-d readmission percentage(PSH=4.7%,C1-PSH=6.1%,C2-PSH=3.6%)lower than the national average 30-d readmission percentage which is 5.5%.The PSH system was effectively established at the rural community hospital

Delivery of acupuncture in the setting of a clinical trial is a unique practice that diverges significantly from the delivery of acupuncture in a real-world clinical setting. Research acupuncturists, particularly those trained in traditional Chinese medicine(TCM), are often required to set aside valued precepts of traditional care, including diagnosing imbalances, individualizing treatment, and forging a therapeutic relationship with patients. TCM-trained acupuncturists express mixed feelings about participating in clinical trials. Many are eager to play a vital role in the advancement of acupuncture science and appreciate the need for strict protocol adherence to minimize bias. However, the acupuncturist(s) may also have concerns about clinical trial methodology, including but not limited to the delivery of a control condition, e.g., sham acupuncture. Investigators should anticipate certain questions and even a level of resistance to the requirements of research among acupuncturists and be prepared to address them. This manuscript presents a brief review of the subjective experience of the research acupuncturist within the available scientific literature as it pertains to the delivery of active and sham clinical research protocols. Our goals are to better understand the perspectives of acupuncturists who may participate in clinical research, so that their concerns may be addressed in study design and methodology. To that end, we suggest the creation of a novel training program specifically for clinical trial acupuncturists, intended for qualified TCM-and Western-trained practitioners, that would help to standardize the research acupuncturist’s role and help to strengthen the design and execution of acupuncture studies.

BACKGROUND Tuberculosis(TB)remains one of the highest Asia’s health problems.Spondylitis TB in diabetes mellitus(DM)and hypothyroidism patients is a rare case of extrapulmonary tuberculosis.However,there is a lack of therapeutic guidelines to treat spondylitis TB,particularly with type 2 DM(T2DM)and hypothyroidism as comorbidities.Here we present a case of spondylitis TB with T2DM and hypothyroidism in a relatively young patient and its therapeutic procedure.CASE SUMMARY We report the case of a 35-year-old male patient from Surabaya,Indonesia.Based on anamnesis,physical examination,and magnetic resonance imaging,the patient has been categorized in stage II of spondylitis TB with grade 1 paraplegia.Surprisingly,the patient also had a high HbA1c level,high thyroid stimulating hormone,and low free T_(4)(FT_(4)),which indicated T2DM and hypothyroidism.A granulomatous process was observed in the histopathological section.The antituberculosis drugs isoniazid and rifampicin were given.In addition,insulin,empagliflozin,and linagliptin were given to control hyperglycemia conditions,and also levothyroxine to control hypothyroidism.CONCLUSION The outcome was satisfactory.The patient was able to do daily activities without pain and maintained normal glycemic and thyroid levels.For such cases,we recommend the treatment of spondylitis TB by spinal surgery,together with T2DM and hypothyroidism therapies,to improve the patients’condition.Prompt early and non-invasive diagnoses and therapy are necessary.

The aim of this study was to develop near infrared spectroscopy(NIRS)calibrations to predict quality parameters,dry matter(DM,g kg1)and crude protein(CP,g kg1 DM),in fresh un-dried grass.Knowledge of these parameters would enable more precise allocation of quality herbage to grazing livestock.Perennial ryegrass samples(n=1615)were collected over the 2017 and 2018 grazing seasons at Teagasc Moorepark to develop a NIRS calibration dataset.Additional samples were collected for an independent validation dataset(n=197)during the 2019 grazing season.Samples were scanned using a FOSS 6500 spectrometer at 2 nm intervals in the range of 1100~2500 nm and absorption was recorded as log 1/Reflectance.Reference wet chemistry analysis was carried out for both parameters and the resultant data were calibrated against spectral data by means of modified partial least squares regression.A range of mathematical spectral treatments were examined for each calibration,which were ranked in order of standard error of prediction(SEP)and ratio of percent deviation(RPD).Best performing calibrations achieved high predictive precision for DM(R2=0.86 SEP=9.46 g kg1,RPD=2.60)and moderate precision for CP(R2=0.84 SEP=20.38 g kg1 DM,RPD=2.37).These calibrations will aid the optimisation of grassland management and the development of precision agricultural technologies.

Given the failure to develop disease-modifying therapies for Alzheimer’s disease(AD),strategies aiming at preventing or delaying the onset of the disease are being prioritized.While the debate regarding whether depression is an etiological risk factor or a prodrome of AD rages on,a key determining factor may be the timing of depression onset in older adults.There is increasing evidence that untreated early-onset depression is a risk factor and that late-onset depression may be a catalyst of cognitive decline.Data from animal studies have shown a beneficial impact of selective serotonin reuptake inhibitors on pathophysiological biomarkers of AD including amyloid burden,tau deposits and neurogenesis.In humans,studies focusing on subjects with a prior history of depression also showed a delay in the onset of AD in those treated with most selective serotonin reuptake inhibitors.Paroxetine,which has strong anticholinergic properties,was associated with increased mortality and mixed effects on amyloid and tau deposits in mice,as well as increased odds of developing AD in humans.Although most of the data regarding selective serotonin reuptake inhibitors is promising,findings should be interpreted cautiously because of notable methodological heterogeneity between studies.There is thus a need to conduct large scale randomized controlled trials with long follow up periods to clarify the dose-effect relationship of specific serotonergic antidepressants on AD prevention.

Background:Effective communication of research findings on vector-borne diseases in Africa is challenging for a number of reasons.Following the experiences of a number of researchers over the life of a project,this article looks for lessons that can be shared with the wider research community.Main body:Between 2014 and 2017,a set of five inter-disciplinary teams from seven African countries collaborated on a project focusing on vector-borne diseases in the context of climate change.A central objective of this work was to influence policy and programming with relevant research findings.This article examines how principles of research communication,derived from the literature and current guidelines,can be applied in practice.Several challenges and lessons are highlighted,showing that research communication takes place within difficult constraints and in complex,fluid institutional and political environments.The processes of communication between policymakers and researchers including stakeholder mapping,defining research communication plans and tailoring communication products are discussed.Conclusions:The article concludes that while guidelines and frameworks for research communication are helpful,they should not detract from the ability of local teams to adapt to circumstances.Of key importance are the relationships and networks of local research teams.

Sensorial information is very difficult to elicit, to represent and to manage because of its complexity. Fuzzy logic provides an interesting means to deal with such information, since it allows us to represent imprecise, vague or incomplete descriptions, which are very common in the management of subjective information. Aggregation methods proposed by fuzzy logic are further useful to combine the characteristics of the various components of sensorial information.
