Pain synd.omes are often associated.with d.pression. In a prospective stud. we analysed.if d.terminants of d.pression d.ffer among patients with d.fferent primary head.ches and.between head.ches and.non-head.che pain. d.ring a 2-year period.between 1 February 2002 and.31 January 2004, 635 subjects (migraine n=231; tension-type head.che n=176; cluster head.che n=11; patients with low back pain n=103; and.healthy subjects n=114) seen by two neurologists filled.in a questionnaire on pain characteristics, the MId.S questionnaire and.the Beck d.pression Inventory. A multivariate general regression mod.l was used.to id.ntify ind.pend.nt pred.ctors of the severity of d.pressive symptoms. Pain was most frequent in chronic tension-type head.che and.most intense in the cluster subgroup (P d.r, age, pain frequency, pain intensity and.d.sability were all significantly associated.with the severity of d.pressive symptoms. In the multivariate mod.l d.sability was the most important ind.pend.nt d.terminant of the severity of d.pressive symptoms in the pooled.head.che group as well as in the migraine and.tension-type head.che subgroups. In contrast to patients with head.che, pain frequency and.pain intensity were the significant ind.pend.nt pred.ctors of the severity of d.pressive symptoms in patients with low back pain. In a multivariate mod.l, after controlling for other factors, d.terminants of the severity of d.pressive symptoms were d.fferent in head.che and.non-head.che pain subjects, suggesting a d.fferent mechanism for d.veloping d.pression in primary head.ches and.in other pain synd.omes.

AIM: To evaluate the role of micro RNA(mi R)-146 a,-155 and.122 in the d.od.nal mucosa of ped.atric patients with Crohn's d.sease(Cd. and.the effect of transforming growth factor-β(TGF-β) on these mi Rs in d.od.nal epithelial and.fibroblast cells.METHOd.: Formalin-fixed. paraffin-embed.ed.biopsies d.rived.from the macroscopically inflamed.Cd.inflamed. n = 10) and.intact(Cd.intact: n = 10) d.od.nal mucosa of ped.atric Cd.patients and.control child.en(C: n = 10) were examined. Expression of mi R-146 a,-155 and.122 was d.termined.by realtime polymerase-chain reaction(PCR). The expression of the above mi Rs was investigated.in recombinant human TGF-β(1 nmol/L, 24 h) or vehicle treated.small intestinal epithelial cells(CCL-241) and.primary d.od.nal fibroblast cells d.rived.from healthy child.en as well.RESULTS: Expression of mi R-146 a was significantly higher in the inflamed.d.od.nal mucosa compared.to the intact d.od.nal mucosa of child.en with Cd.Cd.inflamed. 3.21 ± 0.50 vs Cd.intact: 0.62 ± 0.26, p ≤ 0.01) and.to the control group(Cd.inflamed. 3.21 ± 0.50 vs C: 1.00 ± 0.33, p ≤ 0.05). The expression of mi R-155 was significantly increased.in the inflamed.region of the d.od.num compared.to the control group(Cd.inflamed. 4.87 ± 1.02 vs Control: 1.00 ± 0.40, p ≤ 0.001). The expression of mi R-122 was unchanged.in the inflamed.or intact mucosa of Cd.patients compared.to controls. TGF-β treatment significantly d.creased.the expression of mi R-155 in small intestinal epithelial cells(TGF-β: 0.7 ± 0.083 vs Control: 1 ± 0.09, p ≤ 0.05) and.also the expression of mi R-146a(TGF-β: 0.67 ± 0.04 vs Control: 1 ± 0.15, p ≤ 0.01) and.mi R-155(TGF-β: 0.72 ± 0.09 vs Control: 1 ± 0.06, p ≤ 0.05) in primary d.od.nal fibroblasts compared.to correspond.ng vehicle treated.controls. TGF-β treatment d.d.not influence the expression of mi R-122.CONCLUSION: The elevated.expression of mi R-146 a and.155 in the inflamed.d.od.nal mucosa of Cd.patients suggests the role of these mi Rs in the pathomechanism of inflammatory bowel d.sease.
