BACKGROUND Acromial and scapular spine fractures after reverse total shoulder arthroplasty(RTSA)are a well-known complication that may negatively impact the effects of long-term ***,to the best of our knowledge,there has been no report of simultaneous bilateral fractures of the acromion or scapular spine that occurred following staged *** SUMMARY A 79-year-old right-handed male visited our outpatient clinic with a chief complaint of pain and limited motion of both shoulder joints for a one-year *** on plain radiographs and magnetic resonance images,the preoperative diagnosis was bilateral cuff tear arthropathy with failed rotator cuff *** patient was treated with staged bilateral RTSA at a two-month *** 5 and 3 mo after right and left side surgery,the patient returned to the outpatient clinic with severe pain and limited motion of both shoulder joints for 2 wk without a traumatic event.A computed tomography scan revealed nondisplaced acromial base fractures of both *** bilateral involvement,fracture location,and patient’s demand,open reduction and internal fixation(ORIF)using plate for bilateral acromial base fractures were *** 2 years after ORIF,the fracture was completely healed,and the patient was satisfied with shoulder *** This report describes an extremely rare case of simultaneous bilateral acromial base fractures after staged RTSA managed successfully by ORIF with a precontoured plate designed for distal clavicle *** acromial fracture after RTSA can be treated conservatively,simultaneous bilateral fractures may warrant surgical intervention as a means of addressing difficulties in activities of daily living.

Abstract Tidal current energy is renewable and sustainable, which is a promising altemative energy resource for the future elec- tricity supply. The straight-bladed vertical-axis turbine is regarded as a useful tool to capture the tidal current energy especially under low-speed conditions. A 2D unsteady numerical model based on Ansys-Fluent 12.0 is established to conduct the numerical simulation, which is validated by the corresponding experimental data. For the unsteady calculations, the SST model, 2x 105 and 0.01 s are se- lected as the proper turbulence model, mesh number, and time step, respectively. Detailed contours of the velocity distributions around the rotor blade foils have been provided for a flow field analysis. The tip speed ratio （TSR） determines the azimuth angle of the appearance of the torque peak, which occurs once for a blade in a single revolution. It is also found that simply increasing the incident flow velocity could not improve the turbine performance accordingly. The peaks of the averaged power and torque coeffi- cients appear at TSRs of 2.1 and 1.8, respectively. Furthermore, several shapes of the duct augmentation are proposed to improve the turbine performance by contracting the flow path gradually from the open mouth of the duct to the rotor. The duct augmentation can significantly enhance the power and torque output. Furthermore, the elliptic shape enables the best performance of the turbine. The numerical results prove the capability of the present 2D model for the unsteady hydrodynamics and an operating performance analy- sis of the vertical tidal stream turbine.

Background Takayasu arteritis(TAK)is a disease associated with increased risk of cardiovascular *** aimed to evaluate the incidence,prevalence and risk of stroke in patients with *** Data from 1065 patients were obtained from a national database(2010–2018).The annual incidence and prevalence per 100000 persons were estimated using the registration population in the midst of every year,and the standardised incidence ratio(SIR)of stroke was compared with the general population based on the data from the 2006 national report for cardiovascular and cerebrovascular ***-adjusted incidence rate ratio(IRR)of stroke based on the time interval after diagnosis was also calculated.A time-dependent Cox regression was conducted to investigate predictive factors of *** The overall incidence rate of TAK ranged between 0.2 and 0.3/100000 person-years annually;the prevalence of TAK gradually increased,reaching 3.25/100000 person-years in ***-three(6.9%)patients experienced stroke during follow-up,and the risk of developing stroke was higher than the general population(overall SIR 7.39,95%CI 5.79 to 9.29;men:SIR 5.70,95%CI 2.84 to 10.20;women:SIR 7.06,95%CI 5.41 to 9.05).Most stroke events(90.9%)were cerebral infarction for men,whereas the proportion of cerebral infarction was lower(62.9%)in *** half of stroke events occurred within 6 months after diagnosis,and stroke was more common within 6 months of diagnosis compared with after 3 years in women(IRR 13.46,95%CI 6.86 to 26.40).In Cox regression analysis,age was the sole predictor of stroke(adjusted HR 1.02,95%CI 1.00 to 1.04,p=0.043).Conclusions The annual incidence of TAK was similar to the previous studies from Asia,and the risk of stroke increased in *** patterns of subtype and incidence of stroke were found according to sex,although age was the only predictor.

Multicast technology application has been widely utilized in broadband internet. Source authentication is one of the most needs for many multicast applications transferring real-time information such as stream video and online news. Because multicast current services provided to the group members are changed dynamically, data transferring by a group member is not used by the recipient. In order to verify the identity of the sender who sent the packet and to make sure that the data have not been tampered, an optimized source authentication scheme has been proposed to transfer the authentication information not to the next-door packet. The proposed method for multiple packets authenticates the source with a limited number of electronic signatures. The proposed method can reduce overhead compared to the method by adding a digital signature for every packet. In addition, by sending the generated electronic signature to the first packet and the last packet, it prevents the loss of consecutive packets, as well as a source authentication can be provided in real-time services.

The use of nanofillers with high surface area and extreme purity in polymer composite is an effective strategy to obtain high performance polymeric ***,the effect of nanofillers such as carbon nanotubes(CNT),titanium dioxide(TiO_(2)),and their hybrid on rubber-based composites was *** this study,rubber nanocomposites were fabricated by using room temperature vulcanized(RTV)silicone rubber matrix and nanofillers(***,TiO_(2),and CNT-TiO_(2))through solution casting ***,the purity and surface area of CNT(purity:>96%and BET surface area:300 m2/g)and TiO_(2)(purity:>98%and BET surface area:165 m2/g)were estimated by field emission scanning electron microscopy/energy dispersive X-ray(FESEM-EDX)and adsorption *** mechanical properties of the rubber nanocomposites were enhanced by incorporating *** compressive modulus was 2.18 MPa for unfilled composites and increased to 6.8 MPa(CNT),3.95 MPa(CNT-TiO_(2)),and 2.44 MPa(TiO_(2))at 5 phr,***,the tensile strength was 0.54 MPa for unfilled composites and increased to 1.37 MPa(CNT),1.33 MPa(CNT-TiO_(2))and 0.61 MPa(TiO_(2))at 5 phr,***,the actuation displacement was improved with increasing input voltage and it was 2 mm for CNT,1.6 mm for CNT-TiO_(2) hybrid and 0.5 mm for TiO_(2) at 10 ***,a series of experiments show the potential application in piezoelectric actuation.
