Screening for Hepatitis B and C in Occupational Settings: Cross-Sectional Study about 4268 Corporates Agents in Cote d’Ivoire

Aim: To screen hepatitis B and C in occupational setting. Methods: Cross-sectional study multicenter in 16 enterprises (one occupational exposure;5602 agents). Screening was preceded by information, sensibilisation and informed consenting physician labor, labor union and agents. Hepatitis B surface antigen (HBsAg) and antibodies anti-hepatitis C (Elisa, third generation) were tested in all. For those with HBsAg positive, this test was completed by IgM and IgG, anti HBc, HBeAg, antibodies anti HBe, transaminases and HBB DNA;for those with positive anti-VHC antibodies, test was completed by transaminases and VHC’s RNA. Results: Rate of participation was 76.2% (4268/5602 labors). HBsAg or hepatitis C antibodies were positives in 425 (9.9%) of cases (HBsAg positive: 8.48%;Hepatitis C Antibodies positives: 1.50%;both: 0.05%). In HBsAg positive, viral replication and cytolysis were seen respectively in 2.5 % and 18.2%. According with transaminases, serologic markers and viral load, immunotolerance, inactive and immuno active phase were suspected in 0.56%, 80.9% and 1.6% of cases. In hepatitis C antibodies positives persons, any had viral replication but cytolysis was observed in 33.3% (n = 21). HBsAg was significantively (p < 0.00001) higher in exposed occupational enterprise versus non-occupational setting (14.4% vs 7.8%). They were no differences (p = 0.71) about hepatitis C antibodies in the two setting (1.8% vs 1.7%). Conclusion: Prevalence of HBsAg and hepatitis C antibodies was high in occupational setting in hepatitis endemic country. It’s important to screen all of person in these setting with no consideration with blood exposed or not exposed enterprise.

Aims: 1) To assess the feasibility of EPAGE criteria in clinical practice;2) To assess appropriateness colonoscopy using EPAGE criteria;3) To compare colonoscopy appropriateness and endoscopic lesions. Method: Hundred thirtynine consecutive patients explored by colonoscopy were included. The appropriateness of colonoscopy was evaluated by EPAGE criteria. Results: EPAGE criteria were applicable among 127 patients (91% of the cases). Colonoscopies were appropriate, uncertain and inappropriate in respectively 40%;27% and 24% of the cases. Rate of abnormal colonoscopies was significantly different between the 3 groups (p = 0.03). Cancers were diagnosed exclusively in the groups with appropriate and uncertain colonoscopies (11% and 5%). Colonic adenomas were observed in the three groups (appropriate (7%), uncertain (5%) and inappropriate (6%)). Subjects with uncertain indication were older (58.25 years;p = 0.035). Conclusion: EPAGE criteria are feasible in clinical practice in Cocody’s teaching hospital center. Colonoscopies are generally appropriate in our hospital. However, within sight of the significant number of normal colonoscopies and discovered lesions in uncertain indication, colonoscopy cannot concern exclusively calculation of a score of appropriateness.

Aim: To study the prevalence of HBsAg among persons without risk factors for hepatitis B. Patients and Methods: Cross-sectional study (28 January to 31 December 2010) about 4310 new volunteers blood donors consecutively recruited. Among all, a risk factor for hepatitis B was investigated (standardized questionnaire and clinical examination). Blood of all blood donors without risk factors of hepatitis B (n = 3823) were systematically collected for HBsAg, hepatitis C antibody, syphilis serology and human immunodeficiency virus serology. The blood was analyzed with a third generation ELISA (Abbott Murex). Results: HBsAg was positive in 179 persons (4.68% [95% CI 4% - 5%]). It was associated with hepatitis C antibodies (0.28% (n = 11)), syphilis antibodies (0.68% (n = 26)) or anti-human immunodeficiency virus antibodies (0.2% (n = 8)). In multivariate analysis, age 30 to 39 years (1.545 [95% CI 1.074 to 2.222]), male sex (2.426 [95% CI 1.550 to 3.799]) and positive syphilis serology (14.344 [95% CI 5.161 to 39.865]) were associated with presence of HBsAg. Conclusion: The prevalence of HBsAg among adult without risk factors for hepatitis B was not negligible. Also, it is essential to maintain routine screening before any vaccination against hepatitis B in adult. Our study is not for an immunization against hepatitis B only targeting risk populations in adult.

The Spontang Ophiolite complex represents the most complete ophiolite sequence amongst the South Ladakh ophiolites and comprises mantle rocks(depleted harzburgites,dunites and minor lherzolites)as well as crustal rocks(basalt,isotropic gabbros,layered gabbros etc.).In the present study,detailed geochemistry(whole rock as well as mineral chemistry)and Sr-Nd isotopic analyses of thirty-six ultramaficmafic samples have been attempted to constraint the evolution and petrogenetic history of the Tethyan oceanic ***,trace-element and REE patterns of the peridotites and their minerals indicate that the lherzolites experienced lower degrees of partial melting resembling abyssal peridotites(at higher temperatures,TREE=$1216℃)than the harzburgites(6%–8%versus 15%–17%).Elevated eNd(t)and variable^(87) Sr/^(86) Sr(t)ratios along with REE patterns suggest that the Spontang mafic rocks display N-MORB affinity with negligible participation of oceanic sediments in their genesis are originated from a depleted upper mantle with little contribution from subduction-related ***-type Neotethyan oceanic crust is associated with the earliest phase of subduction(of older Jurassic age)through which a younger intra-oceanic island arc(Spong arc)subsequently *** REE display typical U-shaped patterns,suggesting that these rocks have been metasomatized by LREE-enriched *** the other side,mafic rocks are characterized by heterogeneous(Nb/La)PMand(Hf/Sm)PMand relatively homogeneous eNd(t),indicating interaction of subduction-related melts with the upper mantle during the initiation of subduction,in Early Cretaceous times.

BACKGROUND Chronic abdominal pain is the most common cause for gastroenterology consultation and is frequently associated with functional gastrointestinal disorders including irritable bowel syndrome and inflammatory bowel disease. Global inspection of plant genomes identifies genes maintained in low copies across taxa and under strong purifying selection, which are likely to have essential functions. Based on this rationale, we investigated the function of the low-duplicated CYP715 cytochrome P450 gene family that appeared early in seed plants and evolved under strong negative selection. Arabidopsis CYP715A 1 showed a restricted tissue-specific expres- sion in the tapetum of flower buds and in the anther filaments upon anthesis, cyp715a1 insertion lines showed a strong defect in petal development, and transient alteration of pollen intine deposition. Comparative expres- sion analysis revealed the downregulated expression of genes involved in pollen development, cell wall biogenesis, hormone homeostasis, and floral sesquiterpene biosynthesis, especially TPS21 and several key genes regulating floral development such as MYB21, MYB24, and MYC2. Accordingly, floral sesquiterpene emission was suppressed in the cyp715a1 mutants. Flower hormone profiling, in addition, indicated a modi- fication of gibberellin homeostasis and a strong disturbance of the turnover of jasmonic acid derivatives. Petal growth was partially restored by the active gibberellin GA3 or the functional analog of jasmonoyl-isoleucine, coronatine. CYP715 appears to function as a key regulator of flower maturation, synchronizing petal expan- sion and volatile emission. It is thus expected to be an important determinant of flower-insect interaction.

Prognostic Factors for Eclamptics in Intensive Cares of Two University Teaching Hospitals in Cotonou, Benin

Introduction: In Benin, the maternal mortality rate remains high and one of the main causes is preeclampsia in its complicated forms, including eclampsia. For this, treatment is most often provided in an intensive care unit by a multidisciplinary team involving obstetricians and intensive care doctors. Objective: To determine the prognostic factors of eclamptics treated in intensive care units in two university teaching hospitals in Cotonou. Patients and Method: The study was transversal descriptive and analytical with prospective collection of data from May 1 to July 31, 2022, in the intensive care units of CHU-MEL and CNHU-HKM in Cotonou. The sampling was non-probability with exhaustive recruitment of all cases of eclampsia managed in hospital intensive care units. Clinical, therapeutic and evolutionary data were studied. Data analysis was done with Epi info 7.***.1.0 software. Results: Fifty-five eclamptics were included. The incidence was 12.39%. The average age of eclamptics was 24.67 ± 1.41 years, with a reference rate of 85.45%. Primigravidae represented 52.73%. A history of eclampsia and/or high blood pressure (14.54%) was associated with mortality. Cesarean section was indicated in 85.45% and general anesthesia, was the technique used in 95.75% of cases. Eclamptic status was found in 36.37% of patients. Other poor prognostic factors were Glasgow score of less than nine (9.09%), shock (7.27%), mechanical ventilation (58.18%) and complications. Mortality was 16.36%. Conclusion: The mortality of eclamptics in the intensive care units of CHU-MEL and CNHU-HKM was high. Poor prognostic factors were a history of preeclampsia or pregnancy-induced hypertension, severity of eclampsia and complications.

Diaphragm repair after congenital diaphragmatic hernia is associated with hernia recurrence due to prosthesis failure. Expanded polytetrafluoroethylene (e-PTFE), a synthetic non-degradable biomaterial, is currently used for those diaphragmatic defect repairs. The drawback of e-PTFE is its poor wettability that leads to coating difficulties, bonding that could favor implant integration. However, polydopamine (PDA) can be deposited as well on organic as on inorganic substrates. Therefore, we assessed the biological responses of a clinically used e-PTFE biomaterial treated with PDA in two different manners: one impregnated with PDA and the other coated with a one side PDA film. Mechanical properties of the raw e-PTFE, the PDA soaked biomaterial and the PDA coated surface were characterized by colloidal probe atomic force microscopy. Behaviors of primary human fibroblasts and Wharton’s jelly stem cells were investigated by electron microscopy. Findings reveal that the mechanical properties at the microscopic scale are not modified by the PDA treatments. Cells spread onto both PDA functionalized substrates. In addition, microscopic observations disclose numerous focal cell contacts, evidencing cell attachment, and cytoplasmic projections particularly with the nanoscale PDA coating. Results clearly suggest that PDA in general but above all the PDA coating enhance cellular colonization of the implant material.

The duodenal metastases of a malignant melanoma are rare. We report a case at a 86-year-old, hypertense patient, without tumoral history, who was hospitalized in our department for a change of the general state and an anaemia ferriprive without found home (foyer) of display. In the clinical examination, we had found anemia syndrome, nodules under cutaneous about 2 cm in diameter at the level of the thorax, of the belly, the members and of the lower lip partners in a painless left adenopathy axillary about 3 cm in diameter and an absence of méléna in the rectal touch. The gastroscopy had highlighted polypoides ulcéro-bourgeonnantes hurts with a blackish pigmentation by places at the level of the bulb and of the second duodenum. In the histology of biopsie fragments, we noted a fuso-cellular proliferation and epithelioid with deposits of melanin. The positivity of antibodies anti-protein S100 and anti HMB45 in the immunohistochemical study allowed confirming the diagnosis. The gastroduodenal hurts, persons suffering from a liver complaint and lungworts in the scanner testify of a terminal metastatic scattering. The death arose two months later, in a picture of circulatory collapse further to a high digestive bleeding of big abundance.
