Diurnal changes in postural control in normal children: Computerized static and dynamic assessments

Mild traumatic brain injury (mTBI) causes postural control deficits and accordingly comparison of aberrant postural control against normal postural control may help diagnose mTBI. However, in the current literature, little is known regarding the normal pattern of postural control in young children. This study was therefore conducted as an effort to fill this knowledge gap. Eight normal school-aged children participated. Posture assessment was conducted before (7-8 a.m. in the morning) and after (4-7 p.m. in the after-noon) school on regular school days using the Balance Master? evaluation system composed of 3 static tests and 2 dynamic balance tests. A significant difference in the weight-bearing squats was detected between morning hours and afternoon hours (P < 0.05). By end of afternoon, the body weight was borne mainly on the left side with the knee fully extended and at various degrees of knee flexion. A significantly better directional control of the lateral rhythmic weight shifts was observed at the end of the afternoon than at morning hours (P < 0.05). In summary, most of our findings are inconsistent with results from previous studies in adults, sug-gesting age-related differences in posture control in humans. On a regular school day, the capacity of postural control and laterality or medio-lateral balance in children varies between morning and afternoon hours. We suggest that posturographic assessment in children, either in normal (e.g., physical education and sports training) or in abnormal conditions (e.g., mTBI-associated balance disorders), be better performed late in the afternoon.

New Use of Mn-Zn Ferrite Material in Power Electronics Integrated LC Filters

A new structure of integrated low-pass LC filter of DC-DC power converter is proposed in this paper. This filter consists in a circular planar coil enclosed between two ferrites substrates. Mn-Zn ferrite has been chosen because of its high permeability and permittivity. In this filter Mn-Zn substrates act not only as a magnetic core but also as a capacitor. In order to reduce the conduction losses in the part of the ferrite used as a capacitor, a particular topology using a blocking layer is proposed. A modelling of the dielectric behaviour of the materials has been performed and injected in a simulation in order to find the resulting LC filter performances and its power range of use. In order to increase the filter efficiency, different solutions have been explored. In particular the inter-turn gap evolution has been optimized to reduce the inter-turn losses. Regarding the bulk losses, BaTiO3?blocking layers have?been added, either upon the ground or the conductor. In this last case a co-firing ferrite tape has been inserted between turns to increase the LC product. Finally the use of low losses Mn-Zn and BaTiO3?has been proposed and the final characteristics (both electrical and dimensional) of our filter have been compared toconventional ones.

Background: Scant data on upper gastrointestinal cancers in Rwanda exist to guide potential prevention efforts. We evaluated the epidemiological, clinical and histopathological data among patients with gastric and esophageal tumors at a tertiary Referral Hospital in Rwanda. Methodology: We performed a retrospective review of histologically-confirmed esophageal and gastric cancers in adults age ≥ 18 yrs. old presenting to a university teaching hospital (Centre Hospitalier Universitaire de Butare) from 2014-2019. Variables included age at diagnosis, sex, clinical presentation, tumor location and histopathological type. Results: There were 149 upper gastrointestinal cancers, of which 137 (92%) were gastric and 12 (8%) were esophageal. Gastric cancer patients had a mean age at presentation of 56.9 ± 12.3 years (range 21 - 87). Presenting symptoms were epigastric pain (78.8%), weight loss (53.3%), post-prandial vomiting (52.6%), early satiety (29.9%), epigastric mass (24.8%), hematemesis (19.7%) and melena (16.8%). The location was antrum 50.3%, corpus 21.8%, fundus 8%, and cardia 8%. Tumor type was adenocarcinoma in 94.1%. Helicobacter pylori infection was present in 108 (78.8%). Esophageal cancer patients had a mean age of 54.4 ± 9.5 years (range 35 - 72). Presenting symptoms were dysphagia (100%) and weight loss (83%). The most common site was lower third esophagus (9/12), and adenocarcinoma cancer subtype accounted for 5/12 (41.6%) cases. Conclusion: Gastric adenocarcinoma was the most commonly diagnosed upper gastrointestinal cancers and was associated with a high prevalence of H. pylori infection. This study lays the foundation for future work to improve cancer outcomes in Rwanda.

How Polydopamine Modulates Biological Responses to PTFE Prostheses

Diaphragm repair after congenital diaphragmatic hernia is associated with hernia recurrence due to prosthesis failure. Expanded polytetrafluoroethylene (e-PTFE), a synthetic non-degradable biomaterial, is currently used for those diaphragmatic defect repairs. The drawback of e-PTFE is its poor wettability that leads to coating difficulties, bonding that could favor implant integration. However, polydopamine (PDA) can be deposited as well on organic as on inorganic substrates. Therefore, we assessed the biological responses of a clinically used e-PTFE biomaterial treated with PDA in two different manners: one impregnated with PDA and the other coated with a one side PDA film. Mechanical properties of the raw e-PTFE, the PDA soaked biomaterial and the PDA coated surface were characterized by colloidal probe atomic force microscopy. Behaviors of primary human fibroblasts and Wharton’s jelly stem cells were investigated by electron microscopy. Findings reveal that the mechanical properties at the microscopic scale are not modified by the PDA treatments. Cells spread onto both PDA functionalized substrates. In addition, microscopic observations disclose numerous focal cell contacts, evidencing cell attachment, and cytoplasmic projections particularly with the nanoscale PDA coating. Results clearly suggest that PDA in general but above all the PDA coating enhance cellular colonization of the implant material.

Water use efficiency and yield of winter wheat under different irrigation regimes in a semi-arid region

In irrigation schemes under rotational water supply in semiarid region, the water allocation and irrigation scheduling are often based on a fixed-area proportionate water depth with every irrigation cycle irrespective of crops and their growth stages, for an equitable water supply. An experiment was conducted during the 2004- 2005 season in Haouz irrigated area in Morocco, which objective was i) to evaluate the effects of the surface irrigation scheduling method (existing rule) adopted by the irrigation agency on winter wheat production compared to a full irrigation method and ii) to evaluate drip irrigation versus surface irrigation impacts on water saving and yield of winter wheat. The methodology was based on the FAO-56 dual approach for the surface irrigation scheduling. Ground measurements of the Normalized Difference Vegetation Index (NDVI) were used to derive the basal crop coefficient and the vegetation fraction cover. The simple FAO-56 approach was used for drip irrigation scheduling. For surface irrigation, the existing rule approach resulted in yield and WUE reductions of 22% and 15%, respectively, compared with the optimized irrigation scheduling proposed by the FAO-56 for full irrigation treatment. This revealed the negative effects of the irrigation schedules adopted in irrigation schemes under rotational water supply on crops productivity. It was also demon-strated that drip irrigation applied to wheat was more efficient with 20% of water saving in comparison with surface irrigation (full irrigation treatment). Drip irrigation gives also higher wheat yield compared to surface irrigation (+28% and +52% for full irrigation and existing rule treatments respectively). The same improvement was observed for water use efficiency (+24 and +59% respectively).

Because the elderly account for 80% of deaths from COVID-19 and they may be more vulnerable to air pollution, in this retrospective study we aimed to explore individual and environmental risk factors for COVID-19 mortality in the geriatric departments of seven European University hospitals, between February and May 2020. Long-term exposure to air pollution was estimated through annual pollutant concentrations at the residential address over the last two years. Short-term variations in air pollutants and weather parameters were also examined through a 20-day period before the confirmed PCR diagnostic of COVID-19. We found positive associations for diabetes and COVID-19 mortality (OR 2.2 CI 95%: 1.1, 4.4). Regarding environmental factors, we found no association between COVID-19 mortality and air pollutants and weather parameters;however, our study suffers from strong disparities—such as patient characteristics—between fairly polluted and less polluted cities. In order to overcome those disparities between cities, we aimed to explore the relationship between air pollution and COVID-19 mortality within each city, but even with the high-efficiency modelisation systems, differences in air pollutants were too small to estimate the effect of air pollution at the city level. Thus, this study highlights the need to improve the estimation of individual exposure to air pollution. To address this issue, solutions exist such as the increase of the number of fixed air monitors, or even better, through the use of individual markers of air pollution exposure such as urinary black carbon or passive individual samplers. Furthermore, we underline that outdoor air pollutant concentrations may not be representative of individual exposure, especially in the elderly, thus, we suggest that further studies focus on indoor air pollution. Use of a Model of a Blood-Induced Bruise for the Evaluation of Formulations on Bruising

Esthetic treatments can induce swelling and bruises. Thus, a treatment that would prevent or hasten the resolution of bruising should be very useful. Generally, the regression of bruising was conducted with patients or animals models. So we decided firstly to develop an ex vivo model in order to test antibruising properties of topical formulations and secondly to evaluate a curative effect of a cream (mixture of arnica extract and apigenin) in comparison with a positive control (vulnerary cream) and also to estimate the preventive interest of this cream. The results showed that the injection of 25 μl of blood into the dermis of skin fragments was sufficient to create a model of induced-bruise. The duration of 24 hours was chosen to compare the effects of actives on the decrease in the size of the bruise. Joint effects of a pretreatment and a treatment of a mixture of arnica extract and apigenin decreased significantly the area of bruising compared to the treatment group, the control group and the positive control group. Many topical products claim to improve bruising on their package label. Our model can demonstrate their efficacy and determinate the best topical antibruising formulation. The mechanism involved in anti-inflammatory activity of active compounds of topical formulations is often not fully understood. Our blood-induced model may bring some responses through the study of mediators of the inflammation.

背景尽管有报道说皮肤温度的变化比针刺痛或冷温度感觉对阻滞效果的评定更具敏感性和特异性，但以前应用的方法是计算机辅助的红外温度记录法，该技术花赞大且需要一定的人员培训。在这项前瞻性的观察研究中，我们评估简易的红外温度计是否能可靠地预测锁骨下臂丛神经的阻滞效果。方法30例在锁骨下臂丛神经阻滞下行上技手术的患者纳入本研究。局部麻醉药（以下简称局麻药）注射后在4个主要神经分布区域监测皮肤温度，且5分钟评价一次感觉阻滞的效果（用无痛温觉为0到正常感觉为2）共记录30分钟。注入局麻药后30分钟内4个主要神经分布区域（桡神经、尺神经、正中神经和肌皮神经）的痛温觉消失评分为0视为阻滞成功。皮肤温度的测定采用非接触的温度传感器。结果阻滞120支神经（每例患者4支神经，共30例患者）。其中25例患者成功，4例患者因阻滞失败需补充给药，1例患者改用了全麻。不同神经分布区域的皮肤温度变化没有差异。和阻滞前相比，神经阻滞区域皮肤温度的升高有统计学意义（P〈0．0001，T5-T30）。同一时间神经阻滞区域和非神经阻滞区域的皮肤温度变化有明显的差异（P〈0．05，T5且P〈0．0001，T10～T30）。在某个特定的感觉区域，当5分钟和10分钟时皮肤温度升高1℃或者更高，该神经即被阻滞（评分为0）。因此，5分钟和10分钟时4个神经分布区域的皮肤温度变化明显，30分钟时神经即阻滞成功。监测显示对侧上肢或中心体温没有变化。结论用红外线温度计监测皮肤温度是判断神经阻滞成功与否的可靠、简单且及时的指标。
