Adult-to-adult living-donor liver transplantation: the experience of the Université catholique de Louvain

Background: Liver transplantation is the treatment for end-stage liver diseases and well-selected malignancies. the allograft shortage may be alleviated with living donation. the initial UCLouvain experience of adult living-donor liver transplantation(LDLt) is presented. Methods: A retrospective analysis of 64 adult-to-adult LDLts performed at our institution between 1998 and 2016 was conducted. the median age of 29(45.3%) females and 35(54.7%) males was 50.2 years(interquartile range, IQR 32.9–57.5). twenty-two(34.4%) recipients had no portal hypertension. three(4.7%) patients had a benign and 33(51.6%) a malignant tumor [19(29.7%) hepatocellular cancer, 11(17.2%) secondary cancer and one(1.6%) each hemangioendothelioma, hepatoblastoma and embryonal liver sarcoma]. Median donor and recipient follow-ups were 93 months(IQR 41–159) and 39 months(22–91), respectively. Results: Right and left hemi-livers were implanted in 39(60.9%) and 25(39.1%) cases, respectively. Median weights of right-and left-liver were 810 g(IQR 730–940) and 454 g(IQR 394–534), respectively. Graft-to-recipient weight ratios(GRWRs) were 1.17%(right, IQR 0.98%-1.4%) and 0.77%(left, 0.59%-0.95%). One-and five-year patient survivals were 85% and 71%(right) vs. 84% and 58%(left), respectively. Oneand five-year graft survivals were 74% and 61%(right) vs. 76% and 53%(left), respectively. the patient and graft survival of right and left grafts and of very small( ts was 86% and 86% at 3-and 12-month. No donor died while five(7.8%) developed a Clavien–Dindo complication IIIa, IIIb or IV. Recipient morbidity consisted mainly of biliary and vascular complications; three(4.7%) recipients developed a small-for-size syndrome according to the Kyushu criteria. Conclusions: Adult-to-adult LDLt is a demanding procedure that widens therapeutic possibilities of many hepatobiliary diseases. the donor procedure can be done safely

the present aim is to update, upon arrival of new learning data, the parameters of a score constructed with an ensemble method involving linear discriminant analysis and logistic regression in an online setting, without the need to store all of the previously obtained data. Poisson bootstrap and stochastic approximation processes were used with online standardized data to avoid numerical explosions, the convergence of which has been established theoretically. this empirical convergence of online ensemble scores to a reference “batch” score was studied on five different datasets from which data streams were simulated, comparing six different processes to construct the online scores. For each score, 50 replications using a total of 10N observations (N being the size of the dataset) were performed to assess the convergence and the stability of the method, computing the mean and standard deviation of a convergence criterion. A complementary study using 100N observations was also performed. All tested processes on all datasets converged after N iterations, except for one process on one dataset. the best processes were averaged processes using online standardized data and a piecewise constant step-size.

Using robotic devices might improve recovery post-stroke, but the optimal way to apply robotic assistance has yet to be determined. the current study aimed to investigate whether training under the robotic active-assisted mode improves bimanual motor skill learning(biMSkL) more than training under the active mode in stroke patients. twenty-six healthy individuals(HI) and 23 chronic hemiparetic stroke patients with a detectable lesion on MRI or Ct scan, who demonstrated motor deficits in the upper limb, were randomly allocated to two parallel groups. the protocol included a two-day training on a new bimanual cooperative task, LIFt-tHE-tRAY, under either the active or activeassisted modes(where assistance decreased in a pre-determined stepwise fashion) with the bimanual version of the REAplan? robotic device. the hypothesis was that the active-assisted mode would result in greater biMSkL than the active mode. the biMSkL was quantified by a speed-accuracy trade-off(SAt) before(t1) and immediately after(t2) training on days 1 and 2(t3 and t4). the change in SAt after 2 days of training(t4/t1) indicated that both HI and stroke patients learned and retained the bimanual cooperative task. After 2 days of training, the active-assisted mode did not improve biMSkL more than the active mode(t4/t1) in HI nor stroke patients. Whereas HI generalized the learned bimanual skill to different execution speeds in both the active and active-assisted subgroups, the stroke patients generalized the learned skill only in the active subgroup. taken together, the active-assisted mode, applied in a pre-determined stepwise decreasing fashion, did not improve biMSkL more than the active mode in HI and stroke subjects. Longitudinal trajectories of athlete burnout among young table tennis players: A 3-wave study

Purpose:the purposes of this study were to examine the trajectories of athlete burnout across a 2-month period characterized by high physical,psychological,and social demands to explore(1)whether several subgroups of athletes representing distinct burnout trajectories emerged from the analyses and(2)whether athlete burnout symptoms(reduced accomplishment,sport devaluation,and exhaustion)developed in tandem or whether some burnout dimensions predicted downstream changes in other dimensions(causal ordering model).Methods:One hundred and fifty-nine table tennis players in intensive training centers completed a self-reported athlete burnout measure across 3 time points within a 2-month period characterized by high *** were analyzed through latent class growth ***:Results of latent class growth analysis showed 3 distinct trajectories for each athlete burnout dimension,indicating not only linear or quadratic change but also stability in longitudinal athlete burnout *** also suggested that the 3 dimensions of athlete burnout did not develop in ***,the likelihood of belonging to particular emerging trajectories of sport devaluation and physical/emotional exhaustion was significantly influenced by the athletes’perception of reduced accomplishment assessed at time ***,reduced accomplishment predicted downstream changes in the 2 other athlete burnout ***:As a whole,these results highlighted that the multinomial heterogeneity in longitudinal athlete burnout symptoms needs to be accounted for in future research.

Hollow-core photonic crystal fibre for high power laser beam delivery

We review the use of hollow-core photonic crystal fibre(HC-PCF)for high power laser beam delivery.A comparison of bandgap HC-PCF with Kagome-lattice HC-PCF on the geometry,guidance mechanism,and optical properties shows that the Kagome-type HC-PCF is an ideal host for high power laser beam transportation because of its large core size,low attenuation,broadband transmission,single-mode guidance,low dispersion and the ultra-low optical overlap between the core-guided modes and the silica *** power handling capability of Kagome-type HC-PCF is further experimentally demonstrated by millijoule nanosecond laser spark ignition and^100μJ sub-picosecond laser pulse transportation and compression.
