AIM: To investigate the age differences in the risk factors, clinicopathological characteristics and patterns of treatment of female breast cancer patients. METHODS: Seven thousand one hundred and fiftytwo women with primary breast cancer from the Hong Kong Breast Cancer Registry were recruited after receiving patients' consent, they were asked to complete standardized questionnaires which captured their sociodemographic characteristics and risk factors associated with breast cancer development. Among them, clinicopathological data and patterns of treatment were further collected from medical records of 5523 patients with invasive breast cancers. Patients were divided into two groups according to the age at diagnosis: younger(< 40 years old) vs older patients(≥ 40 years old) for subsequent analyses.RESULTS: Analysis on the sociodemographic characteristics and exposure to risk factors were performed on 7152 women with primary breast cancer and the results revealed that younger patients were more likely to have unhealthy lifestyles; these include a lack of exercise(85.4% vs 73.2%, P < 0.001), having high stress in life(46.1% vs 35.5%, P < 0.001), having dairy/meat-rich diets(20.2% vs 12.9%, P < 0.001),having alcohol drinking habit(7.7% vs 5.2%, P = 0.002). Younger patients were also more likely to have hormone-related risk factors including nulliparity(43.3% vs 17.8%, P < 0.001) and an early age at menarche(20.7% vs 13.2%, P < 0.001). Analyses on clinicopathological characteristics and patterns of treatment were performed on 5523 women diagnosed with invasive breast cancer. The invasive tumours in younger patients showed more aggressive pathological features such as having a higher percentage of grade 3 histology(45.7% vs 36.5%, P < 0.001), having a higher proportion of tumours with lymphovascular invasion(39.6% vs 33.2%, P = 0.003), and having multifocal disease(15.7% vs 10.3%, P < 0.001); they received different patterns of treatment than their older counterparts.CONCLUSION: Younger patients in Hong Kong are more

Virus evolution is a common process of pathogen adaption to host population and environment.Frequently,a small but important fraction of virus mutations are reported to contribute to higher risks of host infection,which is one of the major determinants of infectious diseases outbreaks at population scale.The key mutations contributing to transmission advantage of a genetic variant often grow and reach fixation rapidly.Based on classic epidemiology theories of disease transmission,we proposed a mechanistic explanation of the process that between-host transmission advantage may shape the observed logistic curve of the mutation proportion in population.The logistic growth of mutation is further generalized by incorporating time-varying selective pressure to account for impacts of external factors on pathogen adaptiveness.The proposed model is implemented in real-world data of COVID-19 to capture the emerging trends and changing dynamics of the B.1.1.7 strains of SARS-CoV-2 in England.The model characterizes and establishes the underlying theoretical mechanism that shapes the logistic growth of mutation in population.

Epidemiological study of hypertensive retinopathy in the primary care setting: Retrospective cross-sectional review of retinal photographs

Objective:The objective is to estimate the prevalence and grading of hypertensive retinopa-thy in the primary care setting;examine the patient characteristics associated with hypertensive retinopathy;and examine the association of hypertensive retinopathy and other hypertension complications.Methods:This is a retrospective cross-sectional study.Subjects included adult hypertensive patients with available and gradable retinal photographs.Results:Two hundred fifty-six male hypertensive patients(34.3%)and 491 female hyper-tensive patients(65.7%)were included.The average duration of hypertension was 7.2 years,and 49.8%and 41.2%of patients were taking one or two antihypertensive medications respectively.Among 1491 qualified retinal photographs(744 right eye and 747 left eye),24.9%,62.6%,and 12.5%were classified as showing normal,mild,and moderate hypertensive retinopathy respec-tively.The three commonest retinal signs were generalized or focal arteriolar narrowing(650 cases,43.6%),hard exudates(168 cases,11.3%),and opacity(copper or silver wiring)of the arteriolar wall(166 cases,11.1%).Patients older than 61 years,having hypertension for more than 15 years,or taking three or more antihypertensive medications were significantly associated with hyperten-sive retinopathy(P<0.05).Conclusion:In a primary care clinic in Hong Kong,77.1%of hypertensive patients had hypertensive retinopathy.Advanced hypertensive retinopathy was the commonest target organ damage for hypertensive patients in a primary care clinic.

Impact of climate change on dengue fever epidemics in South and Southeast Asian settings:A modelling study

The potential for dengue fever epidemic due to climate change remains uncertain in tropical areas.This study aims to assess the impact of climate change on dengue fever transmission in four South and Southeast Asian settings.We collected weekly data of dengue fever incidence,daily mean temperature and rainfall from 2012 to 2020 in Singapore,Colombo,Selangor,and Chiang Mai.Projections for temperature and rainfall were drawn for three Shared Socioeconomic Pathways(SSP126,SSP245,and SSP585)scenarios.Using a disease transmission model,we projected the dengue fever epidemics until 2090s and determined the changes in annual peak incidence,peak time,epidemic size,and outbreak duration.A total of 684,639 dengue fever cases were reported in the four locations between 2012 and 2020.The projected change in dengue fever transmission would be most significant under the SSP585 scenario.In comparison to the 2030s,the peak incidence would rise by 1.29 times in Singapore,2.25 times in Colombo,1.36 times in Selangor,and>10 times in Chiang Mai in the 2090s under SSP585.Additionally,the peak time was projected to be earlier in Singapore,Colombo,and Selangor,but be later in Chiang Mai under the SSP585 scenario.Even in a milder emission scenario of SSP126,the epidemic size was projected to increase by 5.94%,10.81%,12.95%,and 69.60%from the 2030se2090s in Singapore,Colombo,Selangor,and Chiang Mai,respectively.The outbreak durations in the four settings were projected to be prolonged over this century under SSP126 and SSP245,while a slight decrease is expected in 2090s under SSP585.The results indicate that climate change is expected to increase the risk of dengue fever transmission in tropical areas of South and Southeast Asia.Limiting greenhouse gas emissions could be crucial in reducing the transmission of dengue fever in the future.

Segmentation and texture analysis with multimodel inference for the automatic detection of exudates in early diabetic retinopathy

Diabetic retinopathy (DR) is an eye disease caused by the increase of insulin in blood and may cause blindness if not treated at an early stage. Exudates are the primary sign of DR. Currently there is no fully automated method to detect exudates in the literature and it would be useful in large scale screening if fully automatic method is available. In this paper we developed a novel method to detect exudates that based on interactions between texture analysis and segmentation with mathematical morphological technique by using multimodel inference. The texture analysis involves three components: they are statistical texture analysis, high order spectra analysis, and fractal analysis. The performance of the proposed method is assessed by the sensitivity, specificity and accuracy using the public data DIARETDB1. Our results show that the sensitivity, specificity and accuracy are 95.7%, 97.6% and 98.7% (SE = 0.01), respectively. It is shown that the proposed method can be run automatically and also improve the accuracy of exudates detection significantly over most of the previous methods.

目的利用计算机辅助的全自动眼底照相分析技术,研究血管源性脑白质病变严重程度与眼底血管网络参数的相关性。方法纳入2015年3—10月于中山大学附属第一医院神经科住院的能完成颅脑MR和坐位眼底照相的脑小血管病患者106例.分别对其颅脑MR显示的脑室旁脑白质病变及深部脑白质病变程度进行Fazekas评分,按照评分将患者分别分为轻度及中重度病变组:比较不同部位不同病变程度患者的基线临床资料和眼底血管网络参数。结果依照脑室旁脑白质病变严重程度分组分析,校正基线信息后行Logistic回归分析显示.脑小动脉不对称性指数(OR=1.71,95%CI 1.02-2.88.P<0.05)的减小与中重度脑室旁脑白质病变风险增加有关。依照深部脑白质病变严重程度分析,校正基线信息后行Logistic冋归分析显示,视网膜中央动脉直径等效值(OR=5.19,95%CI 1.06-25.44,P<0.05)、脑小动脉不对称指数(OR=2.96,95%CI 1.42-6.17,P<0.05)、脑小静脉不对称指数(OR=2.99,95%CI 1.48-6.02,P<0.05)的减小和视网膜中央静脉直径等效值(OR=0.14,95%CI 0.03-0.67,P<0.05)的增加与中重度深部脑白质病变风险增加有关。结论不同部位的脑白质病变发病机制可能存在不同,故二者的早期诊断和观察随访适用于不同的眼底血管网络几何学参数。

AIM: To investigate the correlation between tissue ST6Gal I and serum msAFP in HCC patients, and to investigate their prognostic significance.METHODS: Preoperative sera,paired tumorous and non-tumorous tissues were collected from 19 consecutive patients who had undergone surgical resection of HCC.ST6Gal T activities in the tissues were measured by an in vitro microsomal enzyme activity assay. The percentages of tumor-specific msAFP in the sera were also estimated by an isoelectric focusing-immunoblotting assay.RESULTS: The tumor ST6Gal I activity was negatively correlated with serum msAFP percentage (r = -0.53,P = 0.019). Both decreased tumor ST6Gal I activity andincreased serum msAFP percentage were associatedwith poor tumor cell differentiation. Univariate analyses showed that both decreased tumor ST6Gal T activity (P = 0.028), increased serum msAFP percentage (P = 0.034) and poor tumor cell differentiation (P = 0.031)were associated with shorter overall survival. Multivariate analysis using the Cox regression model showed that the preoperative serum msAFP percentage (P = 0.022)and tumor cell differentiation status (P = 0.048) were independent prognostic indicators for patient overall survival.CONCLUSION: Our results indicate that the presence of msAFP in blood circulation is associated with a decreased activity of ST6Gal I activity in HCC. Both tissue ST6Gal I and serum msAFP are potential prognostic markers for patients with operable HCC.

目的利用计算机辅助的全自动眼底照相分析技术,研究脑小血管病与眼底血管网络参数的相关性。方法连续性纳入能坐位行眼底照相的脑小血管病患者80例,大动脉粥样硬化性脑梗死患者41例,比较两组患者的一般临床资料和眼底血管网络参数。同时行logistic回归分析脑小血管病患者的危险因素。结果脑小血管病组在男性比例、吸烟史发生率、血尿酸水平方面均低于大动脉粥样硬化性脑梗死组[(55%vs.75.6%);(20%vs.39%);(308.33±85.30 vs.367.79±113.60)],差异均有统计学意义(P<0.05)。脑小血管病组小静脉分支系数和不对称性指数均小于大动脉粥样硬化性脑梗死组,[(1.37±0.04 vs.1.39±0.05);(0.80±0.02 vs.0.81±0.02)],差异均有统计学意义(P<0.05)。Logistic回归分析显示,调整血管危险因素后,减小的小静脉不对称性指数与脑小血管病相关,是其危险因素(OR值=1.16,95%CI:1.05-1.38,P<0.05)。结论减小的小静脉不对称性指数与脑小血管病相关,是其危险因素,可作为其早期诊断指标。
