There are several caval reconstruction techniques currently in use for orthotopic liver transplantation. These include caval replacement or the conventional technique, performed with or without venovenous bypass, piggyback technique with anastomosis with two or three hepatic veins with or without cavotomy and modifications of the piggyback technique including end-to-side and side-to-side cavocaval anastomosis. There are few randomized controlled trials comparing the use of these techniques and our knowledge of their comparability is based on a few multi- and many single-center retrospective and prospective reviews. Although there are advantages and disadvantages for each technique, it is advisable that the surgeon perform the technique with which they have the most the experience and at which they are the most skilled as excellent outcomes can be obtained with any of the caval reconstruction options discussed.

Lung transplant is the standard of care for patients with end-stage lung disease refractory to medical management. There is currently a critical organ shortage for lung transplantation with only 17% of offered organs being transplanted. Of those patients receiving a lung transplant, up to 25% will develop primary graft dysfunction, which is associated with an 8-fold increase in 30-d mortality. There are numerous mechanical lung assistance modalities that may be employed to help combat these challenges. We will discuss the use of mechanical lung assistance during lung transplantation, as a bridge to transplant, as a treatment for primary graft dysfunction, and finally as a means to remodel and evaluate organs deemed unsuitable for transplant, thus increasing the donor pool, improving survival to transplant, and improving overall patient survival.

The endosomal Toll-like receptors （TLRs） TLR3, TLR7, TLR8 and TLR9 are important in sensing foreign nucleic acids encountered by phagocytes. Because TLR8 was initially thought to be non-functional in mice, less is known about TLR8 than the genetically and functionally related TLR7. Originally associated with the recognition of single-stranded RNA of viral origin, there is now evidence that human TLR8 is also able to sense bacterial RNA released within phagosomal vacuoles, inducing the production of both nuclear factor （NF）-κB-dependent cytokines and type I interferons （IFNs）, such as IFN-β. The functions of TLR8 extend beyond the recognition of foreign pathogens and include cross-talk with other endosomal TLRs, a process that may also have a role in the generation of autoimmunity.

Background： This article traces the history of scientific ideas connecting forest cover with rainfall to inform ongoing debates about whether forests are net users or producers of water in the hydrological cycle. Scholars of the supply-side school argue that forests are net producers and magnifiers that increase rainfall at regional scales. Supply-side scholars seek to challenge the dominance of demand-side thinking. The demand-side school emphasizes that trees are net users of water within a catchment that decrease overall water available for other users. This scientific debate has significant implications for the development of policies to manage forests and water. Results： Scientists have debated the question of whether forests improve or worsen water balance for over two hundred years. connections between forests and rainfall gained prominence in scientific circles during the second half of the nineteenth century and again during the past three decades. The popularity of forest- rainfall connections has paralleled societal and scientific interest in anthropogenic climate change and deforestation. Theories connecting forests with rainfall peaked in popularity in the 1850s to 1880s, a period when scientists expressed alarm that deforestation caused regional declines in precipitation. Forests were understood to create rain within a locality and region. Scientific consensus shifted by the early twentieth century to the view that forests did not play a significant role in determining rainfall. The forest-rainfall connection reemerged in the 1980s alongside advances in climate modelling and growing fears of anthropogenic global warming and tropical deforestation. Using new data and theories, supply-side advocates have once again placed a strong forest-rainfall connection into scientific prominence. conclusion： Supply-side management policies have a checkered history that should elicit caution, while demand-side policies, which are based on almost a century of hydrolog

Intra- and interspecific variation in plant and insect traits can alter the strength and direction of insect-plant interactions, with outcomes modified by soil biotic and abiotic conditions. We used the potato aphid （Macrosiphum euphorbiae Thomas） feeding on cultivated Solanum tuberosum and wild Solanurn berthaulti to study the impact of water availability and plant mutualistic arbuscular mycorrhizal （AM） fungi on aphid performance and susceptibility to a parasitoid wasp （Aphidius ervi Haliday）. Plants were grown under glass with live or sterile AM fungal spores and supplied with sufficient or reduced water supply. Plants were infested with 1 of 3 genotypes ofM. euphorbiae or maintained as aphidfree controls; aphid abundance was scored after 1 week, after which aphid susceptibility to A. ervi was assayed explanta. Solarium tuberosum accumulated c. 20% more dry mass than S. berthaultii, and root mass of S. berthaultii was smallest under reduced water supply in the presence of AM fungi. Aphid abundance was lowest on S. berthaultii and highest for genotype ＂2＂ aphids; genotype ＂1＂ aphid density was particularly reduced on S. berthaultii. Aphid genotype ＂1＂ exhibited low susceptibility to parasitism and was attacked less frequently than the other two more susceptible aphid genotypes. Neither AM fungi nor water availability affected insect performance. Our study suggests a fitness trade-offin M. euphorbiae between parasitism resistance and aphid performance on poor quality Solarium hosts that warrants further exploration, and indicates the importance of accounting for genotype identity in determining the outcome of multitrophic interactions.
