Conduit necrosis following esophagectomy: An up-to-date literature review

Esophageal conduit ischaemia and necrosis is an uncommon but devastating complication of esophagectomy and remains one of the most challenging issues in surgical practice. The incidence, time interval to develop symptoms, and clinical presentation are highly variable with no predictable pattern. Evidence comes from case reports and case series rather than randomized controlled *** describe the issues surrounding conduit necrosis affecting the stomach,jejunum and colon as an esophageal replacement and the advantages,disadvantages and challenges of each type of reconstruction. Diagnosis is challenging for the most experienced surgeon. Upper gastrointestinal endoscopy and computed tomography thorax with both oral and intravenous contrast is the gold standard. Management, either conservative or interventional is also a difficult decision. Management options include conservative treatment and more aggressive treatments such as stent insertion, surgical debridement and repair of the esophagus using jejunum, colon or a musculocutaneous flap. In spite of recent advances in surgical techniques, there is no reliable strategy to manage esophageal conduit necrosis. Our review covers the pathophysiology and clinical significance of esophageal necrosis while highlighting current techniques of prevention, diagnosis and treatment of this life-threatening condition.

Peripheral nerve injuries induce a severe motor and sensory deficit. Since the availability of autologous nerve transplants for nerve repair is very limited, alternative treatment strategies are sought, including the use of tubular nerve guidance conduits(tNGCs). however, the use of tNGCs results in poor functional recovery and central necrosis of the regenerating tissue, which limits their application to short nerve lesion defects(typically shorter than 3 cm). Given the importance of vascularization in nerve regeneration, we hypothesized that enabling the growth of blood vessels from the surrounding tissue into the regenerating nerve within the tNGC would help eliminate necrotic processes and lead to improved regeneration. In this study, we reported the application of macroscopic holes into the tubular walls of silk-based tNGCs and compared the various features of these improved silk^(+) tNGCs with the tubes without holes(silk^(–) tNGCs) and autologous nerve transplants in an 8-mm sciatic nerve defect in rats. Using a combination of micro-computed tomography and histological analyses, we were able to prove that the use of silk^(+) tNGCs induced the growth of blood vessels from the adjacent tissue to the intraluminal neovascular formation. A significantly higher number of blood vessels in the silk^(+) group was found compared with autologous nerve transplants and silk^(–), accompanied by improved axon regeneration at the distal coaptation point compared with the silk^(–) tNGCs at 7 weeks postoperatively. In the 15-mm(critical size) sciatic nerve defect model, we again observed a distinct ingrowth of blood vessels through the tubular walls of silk^(+) tNGCs, but without improved functional recovery at 12 weeks postoperatively. Our data proves that macroporous tNGCs increase the vascular supply of regenerating nerves and facilitate improved axonal regeneration in a short-defect model but not in a critical-size defect model. Catalytic Oxidative Coupling of 1-Phenyl-1h-tetrazole-5-thiol(hl) and Complex [Cu(I)_6l_6] with a Chair Configuration

Both a persulfide crystal of l-l（1）, the oxidative coupling product of l-phenyl-lh-tetrazole-5-thiol（hl）, and a neutral copper（1） complex of Cu6l6（2） were self-assembled and their crystal architectures were characterized by CCD method. hl was converted into the persulfide l-l（1） over a metalloporphyrin catalyst with enzymatic characters under ambient conditions. Whlie in the crystal architecture of Cu6l6（2）, 6 copper（1） ions were ligated by 6l anions to construct a Cu6 ring, which just resembled the chair configuration of cyclohexane. Notably, both compounds 1 and 2 exhibit strong photoluminescence in solid state.

Synthesis and characterization of CNT/Ce-TiO_2 nanocomposite for phenol degradation

An innovative photocatalyst, Carbon nanotube （CNT） supported Ce-TiO2 nanocomposite was successfully synthesized via modi-fied sol gel method and investigated in a batch reactor for abolition of phenol under UV light spectrum. Characterization of catalyst micro-structure and internal properties were done by means of X-ray diffraction （XRD）, Brunauer-Emmett-Teller （BET）, scanning electron micros-copy （SEM）, transmission electron microscopy （TEM） and UV-vis diffuse reflectance spectra （DRS）. Ce doping can inhibit phase transfor-mation from anatase to rutile and eliminate the recombination of electron-hole pairs in the catalyst. The presence of CNT in TiO2 composite can both increase the photoactivity under UV and change surface properties to achieve sensitivity to visible light. The optimum mass ratio of CNT support and cerium （Ce） dopant in TiO2 was the prominent factor to harvest CNT/Ce-TiO2 photocatalyst nanocomposite. The results demonstrated that optimum mass ratio of CNT：TiO2：Ce was 0.02：1.0：0.06, which resulted in the great performance of the photocatalyst to de-grade about 94% of phenol in a 50 mg/l solution in only 3 h. In this paper, dissimilar role of CNT support and Ce dopant in the TiO2 photo-catalysis of phenol was also discussed.

Phase transition and dynamics of iron under ramp wave compression

Micro-Photoluminescence Confocal Mapping of Single V-Grooved GaAs Quantum Wire

We perform the micro-photoluminescence measurement at low temperatures and a scanning optical mapping with high spatial resolution of a single V-grooved GaAs quantum wire modified by the selective ion-implantation and rapid thermally annealing. While the mapping shows the luminescences respectively from the quantum wires and from quantum well areas between quantum wires in general, the micro-photoluminescence at liquid he temperatures reveals a plenty of spectral structures of the Pl band for a single quantum wire. The spectral structures are attributed to the inhomogeneity and non-uniformity of both the space structure and compositions of real wires as well as the defects nearby the interface between quantum wire and surrounding quantum well structures. All these make the excitons farther localized in quasi-zero-dimensional quantum potential boxes related to these non-uniformity and/or defects. The results also demonstrate the ability of micro-photoluminescence measurement and mapping for the characterization of both opto-electronic and structural properties of real quantum wires.

low-temperature heat capacities of the solid compound Zn（C4h7O5）2（s） were measured in a temperature range from 78 to 374 K, with an automated adiabatic calorimeter. A solid-to-solid phase transition occurred in the temperature range of 295？322 K. The peak temperature, the enthalpy, and entropy of the phase transition were determined to be （316.269±1.039） K, （11.194±0.335） kJ？mol-1, and （35.391±0.654） J？K-1？mol-1, respectively. The experimental values of the molar heat capacities in the temperature regions of 78？295 K and 322？374 K were fitted to two polynomial equations of heat capacities（Cp,m） with reduced temperatures（X） and [X = f（T）], with the help of the least squares method, respectively. The smoothed molar heat capacities and thermodynamic functions of the compound, relative to that of the standard reference temperature 293.15 K, were calculated on the basis of the fitted polynomials and tabulated with an interval of 5 K. In addition, the possible mechanism of thermal decomposition of the compound was inferred by the result of TG-DTG analysis.

Tree stand falls: A persistent cause of neurological injury in hunting

AIM: To characterize and compare our current series of patients to prior reports in order to identify any changes in the incidence of neurological injury related to hunting accidents in Rochester, New ***: All tree stand-related injuries referred to our regional trauma center from September 2003 through November 2011 were reviewed. Information was obtained from the hospital's trauma registry and medical records were retrospectively reviewed for data pertaining to the ***: Fifty-four patients were identified. Ninetysix percent of patients were male with a mean age of 47.9 years(range 15-69). The mean Injury Severity Score was 12.53 ± 1.17(range 2-34). The average height of fall was 18.2 feet(range 4-40 feet). All patients fell to the ground with the exception of one who landed on rocks, and many hit the tree or branches on the way down. A reason for the fall was documented in only 13 patients, and included tree stand construction(3), loss of balance(3), falling asleep(3), structural failure(2), safety harness breakage(3) or lightheadedness(1). The most common injuries were spinal fractures(54%), most commonly in the cervical spine(69%), followed by the thoracic(38%) and lumbar(21%) spine. Eight patients required operative repair. head injuries occurred in 22%. Other systemic injuries include rib/clavicular fractures(47%), pelvic fractures(11%), solid organ injury(23%), and pneumothorax or hemothorax(19%). No patient deaths were reported. The average hospital length of stay was 6.56 ± 1.07 d. Most patients were discharged home without(72%) or with(11%) services and 17% required rehabilitation. CONClUSION: Falls from hunting tree stands are still common, with a high rate of neurological injury. Compared to a decade ago we have made no progress in preventing these neurological injuries, despite an increase in safety advances.
