Proton-pump inhibitors （PPIs） are among the most widely used drugs worldwide. PPI use has recently been linked to adverse changes in semen quality in healthy men; however, the effects of PPI use on semen parameters remain largely unknown specifically in cases with male factor infertility. We examined whether PPI use was associated with detrimental effects on semen parameters in a large population of subfertile men. We retrospectively reviewed data from 12 257 subfertile men who had visited our fertility clinic from 2003 to 2013. Patients who reported using any PPIs for 〉3 months before semen sample collection were included; 7698 subfertile men taking no medication served as controls. Data were gathered on patient age, medication use, and conventional semen parameters; patients taking any known spermatotoxic medication were excluded. Linear mixed-effect regression models were used to test the effect of PPI use on semen parameters adjusting for age. A total of 248 patients （258 samples） used PPIs for at least 3 months before semen collection. In regression models, PPI use （either as the only medication or when used in combination with other nonspermatotoxic medications） was not associated with statistically significant changes in semen parameters. To our knowledge, this is the largest study to compare PPI use with semen parameters in subfertile men. Using PPIs was not associated with detrimental effects on semen quality in this retrospective study.

Crystal and phase morphologies and structures determined by self-organization of crystalline-amorphous diblockcopolymers, crystallization of the crystallizable blocks, and vitrification of the amorphous blocks are reviewed through asystematic study on a series of poly(ethylene oxide)-b-polystyrene (PEO-b-PS) diblock copolymers. On the base ofcompetitions among these three processes, molecular and supramolecular ordering in confined environments can beinvestigated. In a concentration-fluctuation-induced disordered (D_(CF)) diblock copolymer, the competition between crystalli-zation of the PEO blocks and vitrification of the PS blocks is momtored by time-resolved simultaneous small angle X-rayscattering (SAXS) and wide angle X-ray diffraction (WAXD) techniques. In the case of T_cblocks is observed to be confined within the bicontinuous D_(CF) phase due to the rapid vitrification of the PS-rich *** crystallization rate, crystal melting behavior, and crystallinity results further confirm the competition betweencrystallization and vitrification at different temperatures. In an ordered lamellar PEO-b-PS diblock copolymer, the crystalli-zation of the PEO blocks is completely confined within the ordered lamellae due to rapid vitrification of the PS layers asobserved by time-resolved SAXS and WAXD experiments. From the combined two-dimensional SAXS and WAXDmeasurements, the crystal orientation within the confined lamellar geometry is found parallel to the lamellar surface normalwhen it is isothermally crystallized at 35℃. In an ordered cylindrical PEO-b-PS sample, the crystallization of the PEO blocksis also observed to be confined within the ordered cylinders because of rapid vitrification of the PS matrix. The crystalorientation within the confined cylinders is found perpendicular to the cylinder axis as it is crystallized at 35℃.

This paper reports the experimental investigation of flow pattern, preferential regions of deposition and trap efficiency as a function of the length of rectangular shallow reservoirs. Four flow patterns were identified (from longer to shorter reservoirs): an asymmetric flow with two reattachment points, an asymmetric flow with one reattachment point, an unstable flow, and a symmetric flow without any reattachment point. Using dye visualizations, the median value and the temporal variability of the reattachment lengths were precisely measured for the asymmetric flows. For each stable flow, sediment tests with plastic particles were carried out. The regions of deposition on the bed of the reservoir were clearly a function of the flow pattern. The transition from an asymmetric flow pattern to a symmetric flow pattern was responsible for an abrupt decrease of the trap efficiency; a number of regression laws were discussed to take it into account.

To determine the role of corticotropin releasing factor receptor (CRF2) in epithelial permeability and enterocyte cell *** this purpose, we used rat Sprague Dawley and various colon carcinoma cell lines (SW620, HCT8R, HT-29 and Caco-2 cell lines). Expression of CRF2 protein was analyzed by fluorescent immunolabeling in normal rat colon and then by western blot in dissociated colonic epithelial cells and in the lysates of colon carcinoma cell lines or during the early differentiation of HT-29 cells (ten first days). To assess the impact of CRF2 signaling on colonic cell differentiation, HT-29 and Caco-2 cells were exposed to Urocortin 3 recombinant proteins (Ucn3, 100 nmol/L). In some experiments, cells were pre-exposed to the astressin 2b (A2b) a CRF2 antagonist in order to inhibit the action of Ucn3. Intestinal cell differentiation was first analyzed by functional assays: the trans-cellular permeability and the para-cellular permeability were determined by Dextran-FITC intake and measure of the transepithelial electrical resistance respectively. Morphological modifications associated to epithelial dysfunction were analyzed by confocal microscopy after fluorescent labeling of actin (phaloidin-TRITC) and intercellular adhesion proteins such as E-cadherin, p120ctn, occludin and ZO-1. The establishment of mature adherens junctions (AJ) was monitored by following the distribution of AJ proteins in lipid raft fractions, after separation of cell lysates on sucrose gradients. Finally, the mRNA and the protein expression levels of characteristic markers of intestinal epithelial cell (IEC) differentiation such as the transcriptional factor krüppel-like factor 4 (KLF4) or the dipeptidyl peptidase IV (DPPIV) were performed by RT-PCR and western blot respectively. bACKGROUND Asymptomatic children with Crohn's disease(CD) require ongoing monitoring to ensure early recognition of a disease *** In a cohort of pediatric CD patients, we aimed to assess the utility of serial fecal calprotectin measurements to detect intestinal inflammatory activity and predict disease *** In this prospective longitudinal cohort study, children with CD on infliximab therapy in clinical remission were included. Fecal calprotectin levels were assessed at baseline and at subsequent 2-5 visits. Clinical and biochemical disease activity were assessed using the Pediatric Crohn's Disease Activity Index, Creactive protein and erythrocyte sedimentation rate at baseline and at visits over the following 18 *** 53 children were included and eighteen patients(34%) had a clinical disease relapse during the study. baseline fecal calprotectin levels were higher in patients that developed symptomatic relapse [median(interquartile range), relapse 723μg/g(283-1758) vs 244 μg/g(61-627), P = 0.02]. Fecal calprotectin levels > 250μg/g demonstrated good predictive accuracy of a clinical flare within 3 mo(area under the receiver operator curve was 0.86, 95% confidence limits 0.781 to 0.937).CONCLUSION Routine fecal calprotectin testing in children with CD in clinical remission is useful to predict relapse. Levels > 250 μg/g are a good predictor of relapse in the following 3 mo. This information is important to guide monitoring standards used in this population.

Oocyte cryopreservation has recently emerged as an option for women to preserve their fertility for medical (e.g. treatable malignancy) or elective indications (e.g. advancing age). This report describes an IRb-approved study of over 200 oocyte cryopreservation cycles at one center. Patients presenting for oocyte cryopreservation (January 2005 to 2010) were analyzed for day 3 follicle stimulating hormone (FSH), basal antral follicle count (bAFC), gonadotropin usage and the number of oocytes retrieved and cryopreserved. New patient consultations were performed on 516 women, of whom 175 (34%) proceeded to initiate a total of 233 cryopreservation cycles. Twenty-four cycles were cancelled (10%) after starting follicular stimulation due to poor ovarian response or self-withdrawal of the patients. Patients whose cycles were cancelled demonstrated a higher Day 3 FSH and a lower bAFC than patients who completed cycles (p bAFC (r = 0.36), FSH (r = –0.25) and age (r = –0.18) with the mean number of oocytes cryopreserved at 13.6 ± 8.8. Information about long-term fertility preservation must reach both patients and health care providers so that more women can be educated about the benefits of proactive early physiological reproductive assessment and possible interventions available.
