Bone Dysmorphia-Induced Blindness Following a Secondary Hyperparathyroidism: A Case Report

Introduction: Conjunctival-corneal or choroidal calcifications are frequent in SHPT, blindness is however exceptional. We report a case of blindness secondary to compressive ischemic optic neuropathy. Case Report: Mr. B.E.K., 49 years old, has a chronic renal failure secondary to unlabeled glomerular nephropathy for 17 years. He has been on chronic hemodialysis for 12 years and has had SHPT for nine years. He secondarily developed disabling segmental osteoarticular deformities associated with kyphoscoliosis, “drumstick” fingers and facial dysmorphism. Five months before admission he developed eye pain and reduced visual acuity progressing within one month to blindness. Biology noted: serum creatinine at 726 umol/l (60 - 120 umol/L), azotemia at 14.3 mmol/l (2.5 - 7.5 mmol/L), serum calcium at 2.25 (2.25 - 2.55 mmol/L), phosphatemia at 1.13 (0.8 - 1.35 mmol/L), alkaline phosphatases at 2196 (5 - 270 IU/L) and parathyroid hormone level at 2257 (10 - 60 pg/mL). Retinal angiography revealed lesions suggestive of ischemic neuropathy. The orbit CT scan with 3D coronal reconstruction revealed narrowing of the caliber of the optical channels with dystrophic thickening of the skull base and cranial vault. Cranioencephalic and orbital MRI revealed diffuse brown tumors and pre-chiasmatic optic atrophy. Discussion: The most frequent ocular complications of SHPT are conjunctival-corneal or sclero-choroidal calcifications, asymptomatic, associated with hypercalcemia. Compressive manifestations are rarer, represented mainly by an amputation of the visual field, diplopia, ptosis or blindness, as described in our patient. The main cause is osteodystrophy and brown tumors of the skull base (1% - 2%). Incision Site in Manual Small Incision Cataract Surgery in Case of Preoperative Direct Astigmatism

We compared surgically induced astigmatism (SIA) by a superior incision with a temporal incision in manual small incision cataract surgery (MSICS), in patients with bilateral direct (regular) preoperative corneal astigmatisms. Patients and method: We carried out a prospective study from July 1st 2018 to September 30th 2019 in the department of ophthalmology at the University Hospital of Brazzaville. Keratometric readings were recorded before surgery to assess preoperative corneal astigmatisms. Keratometric evaluation was done 45 days post-operatively. Patients were followed 90 days after surgery in order to assess the healing of the surgical site. The surgically induced astigmatism (SIA) is the difference in the magnitude vectors between the preoperative and postoperative astigmatism. The result was positive if the postoperative astigmatism was greater than the preoperative astigmatism and negative if the postoperative astigmatism was less than the preoperative astigmatism. Results: Our study sample included 48 eyes from 24 patients, including 24 operated on temporal incision and 24 in superior incision. The average SIA for superior incisions was 0.33 ± 1.55 diopters (D), versus 0.33 ± 1.44 for temporal incisions. For superior incisions the SIA was 0.81 for astigmatisms lower than 2D, against 0.16D for the temporal incisions. On the other hand, for preoperative astigmatisms greater than 2D, the surgically induced astigmatism was ±0.62D, marking a decrease in preoperative astigmatism for the superior incisions against an increase of 0.5D for the temporal incisions. The healing was delayed for the temporal incisions responsible for discomfort persisting beyond 45 days. Conclusion: The temporal incision had better results than the superior incision for astigmatisms lower than 2D, and less good for astigmatisms higher than 2D. The temporal incision healed less well.

Ametropias in Kinkala: Epidemiological and Clinical Aspects

Ametropia is one of the leading causes of visual impairment. This study describes epidemiological and clinical aspects of ametropia as part of a free medical care program in Kinkala, Republic of Congo. Patients and Methods: We conducted a cross-sectional study from August 25th to September 24th, 2019;patients presenting complaints suggestive of ametropia were evaluated. The ametropia was determined by the objective method under cyclopentolate eye drops according to the following protocol: 3 applications of one drop every 5 minutes, followed by auto-refractometry at least 45 minutes after the first instillation. The auto-refractometer gave us the objective refraction. An anatomical examination was performed in order to eliminate any organic pathology. Results: Of the 2352 patients received, only 137 patients were included in our survey. Of the 137, 14 were emmetropic. Astigmatism was the most common ametropia. Its axis reversed with age, with more than 65% of direct astigmatism before age 40. There was nearly 65% of reversed astigmatism at 60 years and over. In spherical equivalents, hyperopia accounted for more than 80% of ametropia. Conclusion: Ametropia in Kinkala is dominated by astigmatism. Spherical equivalent, hyperopia represents more than 80% of refractive error.

Hybrid aerogels have been prepared by freeze-drying technique after mixing water dispersions of cellulose microfibers or cellulose nanofibers and silica(SiO2)of type SBA-15(2D-hexagonal).The prepared composites were characterized by different analysis techniques such as SEM,hot-filament,DMA,*** composites are compared to those previously prepared using nanozeolites(NZs)as mineral *** morphology studied by SEM indicated that both systems have different structures,i.e.,individual fibers for cellulose microfibers WP-based aerogels and films for nanofibrillated cellulose NFC-based ones....These differences seem to be driven by the charge of the particles,their aspect ratio and *** hybrid materials exhibit tunable thermal conductivity and mechanical *** thermal conductivity values range between^18 to 28 mW.m^-1.K^-1 and confirm the superinsulation ability of these fibrous *** on the thermal insulation properties and mechanical properties was shown by adjunction of mineral particles to both cellulose-based aerogels by reaching pore size lower than 100 *** significantly reduces the thermal conductivity of the hybrid aerogels as predicted by Knudsen et ***,the addition of mineral fillers to aerogels based on cellulose microfibers induced a significant increase in stiffness.

Numerical Analysis of the Structure of High-Strength Double-Layer Steel Plate Concrete Shaft by Drilling Method of Water-Bearing Weak Rock Formation

In order to ensure the safety of coal mine shaft construction, a double-layer steel plate concrete composite shaft wall structure was proposed. However, fewer studies were conducted on this structure, which made engineers too confused to fully recognize its feasibility of this structure. Hence, based on the previous experimental research on the Taohutu mine construction project in Ordos in Inner Mongolia, this research paper aims to provide a widely deep numerical analysis by the usage of the finite element software, in fact, to establish the corresponding numerical analysis model and make a comparison with the experimental data to get the rationality of the verified model. The influence of the composite characteristics of the steel plate and concrete on the ultimate bearing capacity and stress field of the shaft wall structure is studied here through the method of multi-factor analysis. Also, the optimal design scheme of the double-layer steel plate and concrete composite shaft wall structure is proposed in this research paper.

Cotunneling transport in ultra-narrow gold nanowire bundles

We investigate the charge transport in close-packed ultra-narrow （1.5 nm diameter） gold nanowires stabilized by oleylamine ligands. We give evidence of charging effects in the weakly coupled one-dimensional （1D） nanowires, monitored by the temperature and the bias voltage. At low temperature, in the Coulomb blockade regime, the current flow reveals an original cooperative multi-hopping process between 1D-segments of Au-NWs, minimising the charging energy cost. Above the Coulomb blockade threshold voltage and at high temperature, the charge transport evolves into a sequential tunneling regime between the nearest- nanowires. Our analysis shows that the effective length of the Au-NWs inside the bundle is similar to the 1D localisation length of the electronic wave function （of the order of 120 nm _＋ 20 nm）, but almost two orders of magnitude larger than the diameter of the nanowire. This result confirms the high structural quality of the Au-NW segments.
