Qingchang suppositry(清肠栓)induced remission in patients with mild-to-moderate ulcerative proctitis:a multicenter,prospective,randomized,parallel-controlled clinical trial

OBJECTIVE:To evaluate the efficacy and safety of Qingchang suppository(清肠栓,QCS),a preparation of Chinese herbal medicine,in the induction of remission in patients with mild-to-moderate ulcerative proctitis(UP).METHODS:We performed a multicenter,prospective,randomized,parallel-controlled trial to evaluate the efficacy of QCS induction therapy in 140 adult patients with mild-to-moderate UP and TCM syndrome of dampness-heat in large *** patients were randomized to receive QCS(study group)or Salicylazosulfapyridine(SASP)suppository(control group)one piece each time,twice a day,per anum for 12 *** score and main symptoms score were evaluated at weeks 0,2,4,8 and 12,rectosigmoidscopy was taken at weeks 0,4,8 and 12,Geboes score,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)and safety indexes were assessed at weeks 0 and *** primary efficacy endpoint is clinical remission rate,the secondary efficacy endpoints are clinical response rate,mucosa healing rate,Geboes score,the remission rates of the main symptoms,the median day to the remission of the symptom,***:There were no statistical difference in the clinical remission rates,the clinical response rates,the mucosa healing rates,Geboes score,ESR and CRP between the two *** remission rates of tenesmus and anal burning sensation of the study group were significantly higher than those of the control group(76.5%vs 25.0%,P=0.009;74.51%vs 29.63%,P=0.003).The median day to the remission of purulent bloody stool of the study group was significantly less than that of control group[11(1,64)vs 19(2,67),P=0.007].The patients receiving QCS had a significantly higher mucosa healing rate at week 4 than the patients receiving SASP suppository(71.42%vs 52.85%,P=0.023).No adverse event occurred in the study group while the adverse events incidence of the control group was 5.7%(P=0.049).CONCLUSIONS:QCS could induce the remission of UP as effectively and safely as SASP suppository,and was superio

【目的】明确农业技术扩散与市场集中度对我国农业全要素生产率的影响效应及其作用机制，并探究不同地区农业技术扩散与市场集中度对农业全要素生产率的影响是否存在异质性，为我国农业发展相关政策的制定提供理论参考。【方法】选取农业全要素生产率作为被解释变量，农业技术扩散和市场集中度作为核心解释变量，农业机械化水平和劳动生产率作为中介变量，采用系统GMM两步法进行基准回归模型构建，并基于2014—2022年我国30个省（市、自治区）（由于数据不可得性，香港、澳门、台湾和西藏未纳入研究）的面板数据进行实证分析，考察农业技术扩散、市场集中度对农业全要素生产率的影响与机制作用。【结果】在加入相关控制变量的条件下，农业技术扩散、市场集中度对农业全要素生产率的影响系数分别为0.076和0.003，分别达极显著（P中西部地区>东部地区。【建议】深入认识农业技术扩散与市场集中度对农业全要素生产率的影响，并从以下3个方面着手，提升农业全要素生产率：（1）强化农业技术创新与推广体系，促进农业技术快速扩散；（2）优化农业市场结构，适度提高市场集中度；（3）实施差异化区域农业发展战略，充分发挥各地区比较优势。

目的分析2型糖尿病患者常用口服降血糖药物的合理用药情况。方法以扬州市江都人民医院2023年6月-2023年11月收治2型糖尿病患者500例作为此次研究对象,总结常用口服降血糖药物的种类及名称,归纳给药方案,分析不合理用药的具体情况,统计患者发生药物不良反应的概率。结果临床较为常用的口服降血糖药物类型包括:双胍类、磺脲类等,常用的降血糖药物名称包括:二甲双胍、格列美脲、达格列净、格列吡嗪控释片等,其中达格列净使用频率位列第一,二甲双胍位列第二。双药联用降糖药物的占比率高于单独用药及多药联用(P<0.05)。目前临床不合理用药的情况包括:药物剂量不合理、药物种类不合理、药物联用不合理。此次共计21例患者出现不合理用药情况,占比4.20%。共计25例患者用药后出现不良反应,发生率为5.00%。结论该院2型糖尿病患者常用的口服降血糖药物以双胍类、磺脲类为主,达格列净、二甲双胍均为常用降糖药物,且该院口服降血糖药物使用基本合理,比较常见的不合理用药情况为联合用药不当,应予以重视。

康复训练对老年心肺功能受损患者心肺功能及心理状态的影响研究

目的观察康复训练对老年心肺功能受损患者心肺功能恢复及心理状态的影响。方法72例老年心肺功能受损的住院患者,以随机数字表法分为对照组(予以常规治疗及干预)和研究组(在对照组基础上配合康复训练干预),各36例。比较两组患者干预前后心肺功能指标、不良情绪评分及满意度。结果两组患者干预前的心率、呼吸频率、血氧饱和度、每分钟最大通气量及步行6 min距离、抑郁自评量表(SDS)评分、焦虑自评量表(SAS)评分比较,无显著差异性(P>0.05);干预后,两组心率、呼吸频率、SDS评分、SAS评分均较干预前降低,血氧饱和度、每分钟最大通气量及步行6 min距离均较干预前更高,研究组患者心率(69.86±3.22)次/min、呼吸频率(19.79±1.31)次/min、SDS评分(49.58±6.61)分、SAS评分(49.15±3.99)分低于对照组的(76.17±7.55)次/min、(22.38±2.38)次/min、(54.17±5.62)分、(54.02±4.74)分,血氧饱和度(98.49±0.45)%、每分钟最大通气量(85.58±6.73)L/min及步行6 min距离(235.27±14.01)m高于对照组的(95.18±1.31)%、(80.42±7.91)L/min、(208.54±10.11)m(P<0.05)。研究组总满意率100.00%较对照组的83.33%更高(P<0.05)。结论老年心肺功能受损的住院患者在常规治疗基础上配合康复训练,可明显提升心肺功能,改善不良情绪,获得患者一致满意,效果理想。

The TaFIM1 gene mediates wheat resistance against Puccinia striiformis f. sp. tritici and responds to abiotic stress

Fimbrin, a regulator of actin cytoskeletal dynamics that participates in numerous physiological and biochemical processes, controls multiple developmental processes in a variety of tissues and cell types. However, the role of fimbrin in pathogen defense of wheat and the mechanisms have not been well studied. Here, we investigated that the expression of TaFIM1 gene of wheat was significantly induced in response to avirulent race of Puccinia striiformis f. sp. tritici(Pst) and silencing of TaFIM1 by virus-induced gene silencing method. The results show that silencing of TaFIM1 resulted in a reduction of resistance against the stripe rust indicated by both phenotypes and a histological examination of Pst growth. Additionally, the expression level of Ta FIM1 gene was up-regulated under abiotic stresses. These findings suggest that Ta FIM1 functions as a positive regulator of pathogen resistance of wheat plants and response to abiotic stress. Our work may show new light on understanding the roles of fimbrin in wheat.

Aspirin and pravastatin reduce lectin-like oxidized low density lipoprotein receptor-1 expression, adhesion molecules and oxidative stress in human coronary artery endothelial cells

Background Oxidative stress and inflammation are important steps in the pathogenesis of atherosclerosis. We postulated that therapeutic concentrations of aspirin and pravastatin, especially in combination, may suppress oxidative stress and inflammation in endothelial cells, and this concept was examined in human coronary artery endothelial cells （HCAECs）. Methods Human coronary artery endothelial cells were cultured and treated with oxidized-low density lipoprotein （ox-LDL, 60 ug/ml for 24 hours） alone, or pre-treated with aspirin （1, 2 or 5 mmol/L）, pravastatin （1, 5 or 10 umol/L） or their combination （1 mmol/L aspirin and 5umol/L pravastatin）, followed by ox-LDL treatment. After respective treatment, superoxide anion production, p38 mitogen activated protein kinase and transcription factor NF-KB activation, protein expression of lectin-like ox-LDL receptor-1 （LOX-1） and adhesion molecules, and monocyte adhesion were measured. Results Ox-LDL treatment greatly elicited its receptor LOX-1 expression, superoxide anion production and inflammatory response, which were minimally affected by low concentration of aspirin （1 mmol/L） or pravastatin （5umol/L）, but were markedly decreased by their combination. Activation of p38 mitogen activated protein kinase and NF-KB, the expression of intercellular adhesion molecule-1 and monocyte chemotactic protein-I, which were only mildly affected by aspirin or pravastatin alone, were significantly attenuated by their combination. As a consequence, monocyte adhesion to endothelial cells was markedly attenuated by the combination of the two agents. Well-known anti-oxidants a-tocopherol and y-tocopherol had similar inhibitory effects on ox-LDL-mediated oxidative stress and LOX-1 expression as well as monocyte adhesion as did the combination of aspirin and pravastatin. Conclusions These studies point to a positive interaction between aspirin and pravastatin with regard to endothelial bioloqy. Anti-oxidant and subsequent anti-inflammatory

Hamiltonian analysis of 4-dimensional spacetime in Bondi-like coordinates

We discuss tile Hamiltonian formulation of gravity in four-dimensional spacetime under Bondi-like coordinates {v,r,xa,a=2,3}. In Bondi-like coordinates, the three-dimensional hypersurface is a null hypersurface, and the evolution direction is the advanced time v. The internal symmetry group SO（1,3） of the four-dimensional spacetime is decomposed into SO（1,1）, SO（2）, and T^±（2）, whose Lie algebra s0（1,3） is decomposed into s0（1,1）, s0（2）, and t^± （2） correspondingly. The SO（1,1） symmetry is very obvious in this type of decomposition, which is very useful in s0（1,1） BF theory. General relativity can be reformulated as the four-dimensional coframe （eμ^I） and connection （ωμ^IJ） dynamics of gravity based on this type of decomposition in the Bondi-like coordinate system. The coframe consists of two null 1-forms e-, e＋ and two spacelike 1-forms e2, e3. The Palatial action is used. The Hamiltonian analysis is conducted by Dirac＇s methods. The consistency analysis of constraints has been done completely. Among the constraints, there are two scalar constraints and one two-dimensional vector constraint. The torsion-free conditions are acquired from the consistency conditions of the primary constraints about πIJ^μg. The consistency conditions of the primary constraints πIJ^0= 0 can be reformulated as Gauss constraints. The conditions of the Lagrange multipliers have been acquired. The Poisson brackets among the constraints have been calculated. There are 46 constraints including 6 first-class constraints πIJ^0= 0 and 40 second-class constraints. The local physical degrees of freedom is 2. The integrability conditions of Lagrange multipliers no, 10, and eA are Ricci identities. The equations of motion of the canonical variables have also been shown.

Successful endoscopic sclerotherapy for bleeding gastric varices with combined cyanoacrylate and aethoxysklerol

Two patients with liver cirrhosis and portal hyper-tension related to hepatitis infection were admitted to Shanghai Ruijin Hospital due to recurrent melena and hematemesis. Isolated gastric varices were observed in the gastric fundus during the retroflexion of gastroscope. We carried out endoscopic sclerotherapy successfully for bleeding gastric varices with combined cyanoacrylate and aethoxysklerol, which disappeared dramatically several months after two courses of sclerotherapy for each patient. No complication and clinical signs of gastrointestinal re-bleeding were observed during the 6-mo endoscopic follow-up. CT portal angiography (CTPA) has been widely used in the assessment of variceal treatment and improves the results of endoscopic injection therapy.

Semiconductor lasers directly grown on silicon offer great potential as critical components in high-volume,lowcost integrated silicon photonics *** InAs/InP quantum dash(QDash)lasers on native InP substrate emitting at 1.5μm(C-band)have demonstrated notable performance,the growth of InAs/InP QDash lasers on silicon remains undeveloped because of the 8%lattice mismatch between InP and *** we report advances of growth techniques leading to the first C-band room-temperature continuous-wave electrically pumped QDash lasers on CMOS standard(001)silicon substrates by metalorganic chemical vapor deposition.A correlation between various material characterizations and device performance is analyzed for different QDash laser structures grown on planar nominal(001)*** the optimized QDash growth and improved fabrication process,the lowest threshold current density of 1.5 kA∕cm^(2) was determined on an 8μm×1.5 mmdevice on planar silicon with a single facet output power exceeding 14 *** device results illustrate the good material quality of the QDash lasers grown on silicon,suggesting potential applications for other active components of photonic integrated circuits,such as semiconductor optical amplifiers,modulators,and photodetectors.
