Tacrolimus and mycophenolate mofetil as second-line treatment in autoimmune hepatitis:Is the evidence of sufficient quality to develop recommendations?

BACKGROUND The standard management of autoimmune hepatitis(AIH)is based on corticosteroids,alone or in combination with ***-line treatments are needed for patients who have refractory ***,high-quality data on the alternative management of AIH are *** To evaluate the efficacy and safety of tacrolimus and mycophenolate mofetil(MMF)and the quality of evidence by using the Grading of Recommendations Assessment,Development and Evaluation approach(GRADE).METHODS A systematic review and meta-analysis of the available data were *** calculated pooled event rates for three outcome measures:Biochemical remission,adverse events,and mortality,with their corresponding 95%confidence intervals(CI).RESULTS The pooled biochemical remission rate was 68.9%(95%CI:60.4-76.2)for tacrolimus,and 59.6%(95%CI:54.8-64.2)for MMF,and rates of adverse events were 25.5%(95%CI:12.4-45.3)for tacrolimus and 24.1%(95%CI:15.4-35.7)for *** pooled mortality rate was estimated at 11.5%(95%CI:7.1-18.1)for tacrolimus and 9.01%(95%CI:6.2-12.8)for *** biochemical remission rates for tacrolimus and MMF in patients with intolerance to standard therapy were 56.6%(CI:43.4-56.6)vs 73.5%(CI:58.1-84.7),and among non-responders were 59.1%(CI:48.7-68.8)vs 40.8%(CI:32.3-50.0),***,the overall quality assessments using GRADE proved to be very low for all our outcomes in both treatment *** Tacrolimus and MMF are in practice considered effective for patients with AIH who are non-responders or intolerant to first-line treatment,but we found no high-quality evidence to support this statement.

In this study,we developed multiple hybrid machine-learning models to address parameter optimization limitations and enhance the spatial prediction of landslide susceptibility *** created a geographic information system database,and our analysis results were used to prepare a landslide inventory map containing 359 landslide events identified from Google Earth,aerial photographs,and other validated sources.A support vector regression(SVR)machine-learning model was used to divide the landslide inventory into training(70%)and testing(30%)*** landslide susceptibility map was produced using 14 causative *** applied the established gray wolf optimization(GWO)algorithm,bat algorithm(BA),and cuckoo optimization algorithm(COA)to fine-tune the parameters of the SVR model to improve its predictive *** resultant hybrid models,SVR-GWO,SVR-BA,and SVR-COA,were validated in terms of the area under curve(AUC)and root mean square error(RMSE).The AUC values for the SVR-GWO(0.733),SVR-BA(0.724),and SVR-COA(0.738)models indicate their good prediction rates for landslide susceptibility ***-COA had the greatest accuracy,with an RMSE of 0.21687,and SVR-BA had the least accuracy,with an RMSE of *** three optimized hybrid models outperformed the SVR model(AUC=0.704,RMSE=0.26689),confirming the ability of metaheuristic algorithms to improve model performance.

Analyzing Some Behavior of a Beam with Different Crack Positions Transversely inside It

Vertical displacement, critical Euler buckling load and vibration behavior of a cracked beam are considered in this research. The crack inside the beam is placed in different positions and results compared for each crack position. On first Eigenvalue of free vibration results, there is a border that first Eigenvalue of free vibration does not change if center of crack is located on that border, and after that border, the first Eigenvalue of free vibration is increased that is a counterexample relation of critical Euler buckling load and first Eigenvalue of free vibration.

Effect of bariatric surgery on adiposity and metabolic profiles: A prospective cohort study in Middle-Eastern patients

AIM To investigate changes in adiposity and cardio-metabolic risk profile following Roux-en-Y gastric bypass in patients of Middle Eastern ethnicity with severe *** This prospective cohort study involved 92 patients who met the indications of bariatric surgery. Post-procedure markers of obesity and cardiometabolic profile were monitored regularly for a *** Mean body mass index decreased by 29.5% from 41.9 to 29.5 kg/m^2 between baseline and 12-mo follow-up, while mean fat mass decreased by 45.9% from 64.2 kg to 34.7 kg. An improvement was also observed in the glucometabolic profile with both fasting glucose and HbA 1csubstantially decreasing(P < 0.001).CONCLUSION The present study shows the short to medium term(1 year) health benefits of bariatric surgery for patients of Middle Eastern ethnicity.

Crash Severity Modeling in Urban Highways Using Backward Regression Method

Identifying and classifying intersections according to severity is very important problem for implementation of safety related counter measures and effective models are needed to compare and assess the severity. Highway safety organizations have considered intersection safety among their priorities. In spite of significant advances in highways safety, the large numbers of crashes with high severities still occur in the highways. Investigation of influential factors on crashes enables engineers to carry out calculations in order to reduce crash severity. In the previous studies, there are no perfect models which are capable to illustrate the influence of human factors, road, vehicle, weather conditions and traffic features including traffic volume and flow speed on the crash severity. Therefore, this paper is aimed to develop the models for illustration of the simultaneous influence of these variables on the crash severity in urban highways. The models represented in this study have been developed using binary Logit Models. SPSS software has been used to calibrate the models. It must be mentioned that backward regression method in SPSS was used to identify the significant variables in the model. Consider to obtained results it can be concluded that the main factor in increasing of crash severity in urban highways are driver age, movement with reverse gear, technical defect of the vehicle, vehicle collision with motorcycle and bicycle, bridge, frontal impact collisions, frontal-lateral collisions and multi-vehicle crashes in urban highways which always increase the crash severity in urban highways. Obtained results show the effectiveness and capability of the developed models.

Faunal data and envenomation emergency first aid of cone snails (Conus spp.) in Qeshm Island, the Persian Gulf

Objective: To investigate the fauna of a highly venomous marine species group, the cone snails(Family Conidae), in the shores of Qeshm Island, of evaluating the possibility of envenomation in the area and summarize recommendations for emergency first ***: Shores surrounding Qeshm Island were surveyed to collect cone snails during cold(February and March) and warm(May and June) seasons of 2017. Collected snails were identified to the species level. Abundance and species richness were estimated in shores of different structures, including muddy and sandy-rocky shores. Also, the most updated medical literature was reviewed to summarize related emergency first ***: Three cone snail species were recorded from southern sandy-rocky shores of the Island, in decreasing order of abundance, included crowned cone(Conus coronatus)(65%), feathered cone(Conus pennaceus)(28%), and frigid cone(Conus frigidus)(7%).Abundance of these species were significantly higher in cold season compared to the warm season(P < 0.05). No cone snails were recorded along the northern muddy shores of the ***: Envenomation can cause various symptoms ranging from minor local pain to systemic paralysis and death due to respiratory failure. We recommend an awareness programme for the seashore visiting public.

Performance Analysis of Listed Companies in the UAE-Using DEA Malmquist Index Approach

Data Envelopment Analysis (DEA) is becoming an increasingly popular tool for assessing the relative performance of industries and companies. By applying DEA theory to the non-financial sector, the relative efficiency of 27 listed corporations in the United Arab Emirates (UAE) has been analyzed in this paper. The focus of the study has been on the impact of the financial crisis and the recovery thereafter. Further, the productivity change was decomposed into technical efficiency change and technological change by using the non-parametric Malmquist Productivity Index (MPI) over the period from 2007 to 2014. Based on Malmquist analysis, we find that the most efficient industries during the post-crisis period were food and beverages, telecommunication and pharmaceuticals. In contrast, the sectors that were adversely affected by the crisis were services, real estate, construction and cements. The break-up of the TFP indicated that the efficiency indices in the top performing industries were driven by technological improvements or frontier effects. The top-performing companies in the UAE during the 2007-14 period demonstrated innovation-led growth, aided by the use of better technology, investments in capital equipment, and adoption of new production processes.

Estimate Erosion in Ladiz Dam in Sistan and Baluchistan with Use of Trace Minerals—South East Iran

A survey uses this new information: the statistical calculation and interpretation to determine the characteristics of sediments in the reservoir’s “Dodar Ladiz” dam. It is a useful and efficient way to use clay minerals as mineral tracer for identifying the sources and assessing erosion rate in the Dodar Ladiz dam watershed in a statistical method. The existence of four main clay minerals includes Kaolinite, Montmorillonite, Illite and Chlorite. The estimated watershed erosion rate is 2220.64 tons per square kilometer per year. Data ANOVA table in the inter/intra group communication of the clay minerals in the Dodar Ladiz dam watershed samples is shown by F-test statistics;there is a significant difference between the frequency of clay minerals in the samples with 95% confidence rate (P value < 0.05). Therefore, they can be divided into six groups. The average rates of the clay minerals are not equal in any of homogeneous hydrological reservoir units of the dam. Kruskal-Wallis test shows that the rate of frequency of Iolite mineral is identical among all samples of the hydrologic units and the dam reservoir;this mineral cannot be used as a tracer to identify the source. Reviewing the obtained data from statistical tests and studying clay minerals as a trace in determining the rate of degradation of upstream catchment “Dodar Ladiz” dam, it shows that the maximum annual erosion rate is 994.85 tons per sq/km, in the basin A within a 105.13 square kilometers area. Among the most important factors involving erosion and production of basin sediments, we can refer to lithology as the most critical factor and tectonic features, topography and climate as minor factors.

Antibacterial resistance patterns of Acinetobacter baumannii complex:The results of Isfahan Antimicrobial Resistance Surveillance-1 Program

Objective: To determine the antibiotic resistance patterns of the Acinetobacter(A.) baumannii complex isolates that cause the confirmed infection. Methods: The present descriptive study was performed from March 2016 to March 2018 in three referral hospitals in Isfahan, Iran. All A. baumannii complex strains isolated from different clinical samples were identified by conventional phenotypic methods and antibiotic susceptibility pattern was detected. After the clinical investigation, contaminated samples were excluded and the source(hospital/community) and site of the infection were determined. Data on antibiotic susceptibility testing were extracted from WHONET software and analysis was done with ***: From 254 patients who had confirmed A. baumannii complex infection, 158(62.20%) cases were male, 27(10.63%) were less than 20 years old, 172(67.72%) had healthcare-associated infections and 96(37.79%) were admitted in intensive care units. The most frequent infection was bloodstream infections(111, 43.70%). Our results showed that most of the isolates were resistant to most of the antibiotics(more than 75.00%) and a lower rate of non-susceptibility was observed against minocycline(20, 44.44%) and colistin(0%). The rate of multidrug-resistant isolates was 88.97%. There was no significant difference between resistance of A. baumannii complex isolates according to age. However, the resistance to amikacin and minocycline and the rate of multidrug resistance(MDR) were significantly different between males and females. In patients with healthcare associated infection(HAI), MDR isolates were significantly different regarding admission in ICU ward. Resistance to levofloxacin and ciprofloxacin were lower in isolates from patients with bloodstream infections in comparison to other ***: In our study, a high level of antibiotic resistance was detected in both community-acquired and healthcare-associated A. baumannii complex infections.
