Effect of oral premedication of midazolam, ketamine, and dexmedetomidine on pediatric sedation and ease of parental separation in anesthesia induction for elective surgery: A randomized clinical trial

Objective:To compare effect of midazolam,dexmedetomidine,and ketamine as oral premedication on pediatric sedation and ease of parental separation anxiety in anesthesia induction.Methods:This multicenter,prospective,randomized,double-blind,clinical trial focused on a pediatric population aged 2-7 years(n=153)with the American Society of AnesthesiologistsⅠ-Ⅱwho required elective surgery.The patients were stratified into three intervention groups:midazolam,ketamine,and dexmedetomidine.Hemodynamic parameters(blood pressure,heart rate,and oxygen saturation)every 5 min until induction of anesthesia along with non-hemodynamic factors,comprised of sedation score before the administration and at the time of being separated from the parents,as well as parental separation anxiety scale,acceptance of anesthesia induction,and side effects were recorded and compared.Results:No statistically significant difference in oxygen saturation,heart rate,blood pressure,duration of surgery,time to achieve an Aldrete score of 9-10,or sedation score was noted in the study groups.More patients in the dexmedetomidine and midazolam groups could better ease parental separation anxiety than the ketamine group(P=0.001).Moreover,fewer patients accept anesthesia induction(P=0.001)and more had side effects in the ketamine group(P=0.047).Conclusions:Our findings indicate that compared to the ketamine group,dexmedetomidine and midazolam are better in easing parental separation anxiety and accepting induction of anesthesia with fewer side effects.Dexmedetomidine and midazolam may be considered better choices.However,the final choice hinges on the patient's specific physical condition and the anesthesiologist's preference.Clinical registarion:This study is registered in the Iranian Registry Clinical Trial center with the clinical trial code of IRCT20211007052693N1.

Pure annihilation decay of strange beauty meson into two charm heavy mesons

In this study,the heavy to heavy decay of D^(0)_(s)→D^(*+)D^(-)is evaluated through the factorization approach by using the final state interaction as an effective correction.Under the factorization approach,this decay mode occurs only through the annihilation process,so a small amount is produced.Feynman's rules state that six meson pairs can be assumed for the intermediate states before the final meson pairs are produced.By taking into account the effects of twelve final state interaction diagrams in the calculations,a significant correction is obtained.These effects correct the value of the branching ratio obtained by the pure factorization approach from(2.41±1.37)×10^(-5)to(8.27土2.23)×10^(-5).The value obtained for the branching ratio of the D^(0)_(s)→D^(*+)D^(-)decay is consistent with the experimental results.

Mechanistic simulation of fracture effects on miscible CO_(2)injection

Miscible injection of carbon dioxide(CO_(2))into oil reservoirs as an Enhanced Oil Recovery(EOR)method has proved to be highly advantageous.According to the volume of the world's recoverable oil resides inside the fractured reservoirs,investigation of the controlling parameters in the efficient injection of miscible CO_(2) is of paramount importance,mainly owing to the intricacies and complexities associated with this process.This complexity in fractured reservoirs arises due to the presence of two distinct media for fluid transfer(i.e.,matrix and fracture network)and the corresponding differences in fluid velocities.Accordingly,performance of miscible carbon dioxide injection in these reservoirs was investigated through mechanistic simulation model in the form of dual-porosity(DP),and dual porosity-dual permeability(DPP).Moreover,due to limited supply and high injection costs of this gas in its pure form,performance of the miscible CO_(2) injection combined with C_(1),N_(2),and H_(2)S was also surveyed and compared to pure gas injection case.A sensitivity analysis was also performed based on fracture porosity,fracture horizontal and vertical permeability,matrix horizontal permeability,block height shape factor,matrix capillary pressure,and impure injected components in DP and DPP models,showing that matrix horizontal permeability and capillary pressure have the greatest,and porosity has the lowest impact on miscibility performance and oil recovery in these models.In the end,after investigating the effect of different injection cases on miscibility performance and oil recovery,it was concluded that the highest oil recovery in miscible gas injection obtained through optimization of a gas composition having the lowest minimum miscibility pressure(MMP)and the lowest density.

Morphological peculiarities of the lithium electrode from the perspective of the Marcus-Hush-Chidsey model

This study employs the kinetics framework of Marcus-Hush-Chidsey(MHC)to investigate the charge transfer at the interface of lithium electrode and electrolyte in lithium(ion)-batteries.The chargetransfer rate constant is evaluated for different facets of lithium,namely(100),(110),(101),and(111)as a function of surface charge density with the aid of density functional theory(DFT)calculations.The results highlight and quantify the sensitivity of the rate of lithium plating and stripping to the surface orientation,surface charge density,and charge-transfer over-potential.An intrinsic kinetics competition among the different surface orientations is identified together with an asymmetry between the lithium plating and stripping and showcased to influence the deposit morphology and surface protrusions and indentations.

Electric field in two-dimensional complex plasma crystal:Simulated lattices

We focus on molecular dynamics simulated two-dimensional complex plasma crystals. We use rigid walls as a confinement force and produce square and rectangular crystals. We report various types of two-row crystals. The narrow and long crystals are likely to be used as wigglers; therefore, we simulate such crystals. Also, we analyze the electric fields of simulated crystals. A bit change in lattice parameters can change the internal structures of crystals and their electric fields notably. These parameters are the number of grains, grains charge, length, and width of the crystal. With the help of electric fields, we show the details of crystal structures.

Empirical models and design codes in prediction of modulus of elasticity of concrete

Modulus of Elasticity (MOE)is a key parameter in reinforced concrete design.It represents the stressstrain relationship in the elastic range and is used in the prediction of concrete structures.Out of range estimation of MOE in the existing codes of practice strongly affect the design and performance of the concrete structures.This study includes: (a)evaluation and comparison of the existing analytical models to estimating the MOE in normal strength concrete,and (b)proposing and verifying a new model.In addition,a wide range of experimental databases and empirical models to estimate the MOE from compressive strength and density of concrete are evaluated to verification of the proposed model. The results show underestimation of MOE of conventional concrete in majority of the existing models.Also,considering the consistency between density and mechanical properties of concrete,the predicted MOE in the models including density effect,are more compatible with the experimental results.

Expression of antioxidant genes in broiler chickens fed nettle(Urtica dioica)and its link with pulmonary hypertension

Nettle(Urtica dioica)contains a wide range of chemical constituents that confer a strong antioxidant capacity to the plant.The present study was to investigate the antioxidant gene expression and pulmonary hypertensive responses of broiler chickens to U.dioica.A total of 240 one-d-old broilers(Ross308)were randomly assigned to 4 dietary levels of U.dioica(0.0.5%.1% and 1.5%).Birds were reared for6 wk in a high altitude region(2.100 m).The results showed a significant relative overexpression(target gene/β-actin as the arbitrary unit)of catalase(CAT)and superoxide dismutase 1(SOD1)in the liver and lung of the chickens fed U.dioica.Lipid peroxidation was significantly suppressed,as reflected in reduced circulatory concentrations of malondialdehyde(MDA)in the birds fed U.dioica.These birds also had significantly(P<0.05)higher serum nitric oxide(NO)concentrations than those in the control group.Feeding U.dioica at 1% and 1.5% also attenuated the right ventricular hypertrophy(reflected in the lower right to total ventricular weight ratio),which was associated with a significant lower rate of mortality from pulmonary hypertension syndrome.Feeding U.dioica led to an upregulation of hepatic and pulmonary antioxidant genes.

Plug-in electric vehicle(PEV) load modeling is very important in the operation and planning studies of modern power system nowadays. Several parameters and considerations should be taken into account in PEV load modeling, making it a complex problem that should be solved using appropriate techniques. Different techniques have been introduced for PEV load modeling and each of them has individual specifications and features. In this paper, the most popular techniques for PEV load modeling are reviewed and their capabilities are evaluated. Both deterministic and probabilistic methods are investigated and some practical and theoretical hints are presented. Moreover, the characteristics of all techniques are compared with each other and suitable methods for unique applications are proposed. Finally, some potential research areas are presented for future works.

本文研究了减少切削液消耗对刀具表面质量和刀具磨损的影响。利用响应面方法(RSM)建立了预测表面粗糙度的数学模型,使用方差分析(ANOVA)来研究所建立回归模型的有效性。结果表明,拟合模型的R^2分别为dry型的97.46%、FM型的89.32%、MQL型的99.44%,具有较高的拟合精度。在MQL型条件下,表面粗糙度较dry型和FM型分别提高了23%~44%和19%~41%,加工表面的SEM图像证明了这一说法。刀具前端面扫描电镜图像表明粘着磨损明显减少。粘着磨损的主要产物是粘着边和粘着层,利用X射线能谱分析刀具表面特定点,确定粘着材料的化学成分。将dry工艺和FM工艺改为MQL工艺,显著降低了铝合金加工过程中刀具磨损的程度。导致切削刀具早期失效的断裂磨损通过MQL技术得到了控制。

Chemical-only reprogramming to pluripotency

Direct reprogramming technology has emerged as an outstanding technique for the generation of induced pluripotent stem cells （iPSCs） and various specialized cells directly from somatic ceils of different species. Reprogramming techniques conventionally use viral vectors encoding transcription factors to induce fate conversion. However, the introduction of transgenes limits the therapeutic applications of the reprogrammed cells. To overcome safety-related concerns, small molecules offer some advantages over the existing methods for the control of gene expression and induction of cell fate conversion. Technical advances in optimizing concentrations, durations, structures, and combinations of small molecules make chemical reprogramming a safe and feasible method. This review provides a concise overview of cutting-edge findings regarding chemical-only reprogramming as one of the integration-free approaches to iPSC generation.
