Autoimmune Hepatitis Prevalence among Patients from Saudi Arabia with Chronic Liver Disease Referred for Transient Elastography

Background and study aim: Autoimmune hepatitis (AIH) is a chronic liver disease that can lead to progressive liver damage. The prevalence of AIH among the general population and among chronic liver disease (CLD) patients is variable worldwide. Currently, no published data on the prevalence of AIH among Saudi or among liver disease patients from the region. In this study, we aimed to assess the prevalence of AIH among CLD patients who were referred for transient elastography (FibroScan) in Saudi Arabia. Patients and methods: A retrospective study was conducted among CLD patients who had a transient elastography (FibroScan) during the 3-year study period. We obtained demographic data and test results for serum alanine aminotransferase (ALT), bilirubin, hemoglobin (Hgb), platelets, and international normalized ratio (INR) from all patients. We compared the results of the AIH patients to those of the non-AIH patients with CLD. In addition, for AIH patients, we measured serum immunoglobulin G (IgG), antinuclear antibody (ANA), smooth muscle antibody (SMA) and ALT at 2 - 6 weeks and at 3 - 4 months. Results: We included 494 patients, who were predominantly female (271 (60%)) and Saudi (299 (60.5%)). Thirty patients (6.1%) had AIH, which represented the 4th most common liver disease. Compared to non-AIH patients, AIH patients were younger (mean ages 49.9 years, SD 14.22 years, and 40.4 years, SD 13.94 years, respectively, P = 0.001). Patients with AIH had significantly lower Hgb and platelets (P = 0.008 for both) and higher ALT, bilirubin and INR (P = 0.05, 0.047 and 0.019, respectively). The Incidence and Risk Factors of Third- and Fourth-Degree Perineal Tears in Ministry of Health in Bahrain over 5 Years

Background: Perineal trauma and vaginal laceration are considered a common complication associated with vaginal delivery. Well established risk factors, recognized by the Royal College of Obstetricians and Gynecologists, are ethnicity, birth weight over 4 kg persistent occipital posterior position, null parity, induction of labor, shoulder dystocia, instrumental delivery. There are other risk factors that were suggested in the literature, but data are conflicting, such as prolonged second stage of labor, episiotomy and obesity. Objective: To evaluate third- and fourth-degree perineal rears rates and the impact of related risk factors on perineal tears in Ministry of health in Bahrain over 5 years (which includes Salmanyia Medical complex (SMC) and Jidhafs maternity hospital (JMH)). Methods: This retrospective descriptive cross-sectional study analyzed all vaginal deliveries from January 2015 to December 2019 in Obstetrics and Gynecology department in Salmanyia Medical Complex (the main hospital in Kingdom of Bahrain which received all kinds of cases including low and high risks) and Jidhafs Maternity Hospital (tertiary hospital which received only low risk cases), Kingdom of Bahrain. During the period of interest 33,694 records were identified. Data were extracted from observational recording from SMC and JMH labour registry books. Results: There was no statistically significant difference between groups according to age (p = 0.199). On the other hand, there was statistically significant higher cases of >40 weeks at gestational age, obesity > 35 kg/mr, vacuum delivery, pushing stage > 90 min, birth weight > 4 kg, head circumference > 34 cm, fetal length at birth > 50 cm, episiotomy and lower cases of nulliparity in study group compared to control group 16 (66.7%) vs. 13,805 (41.0%), 3 (12.5%) vs. 1448 (4.3%), 3 (12.5%) vs. 1414 (4.2%), 4 (16.7%) vs. 1751 (5.2%), 3 (12.5%) vs. 1751 (5.2%), 12 (50.0%) vs. 15,926 (47.3%), 15 (62.5%) vs. 20,135 (59.8%) and 17 (70.8%) vs

Invasive Procedures for Prenatal Diagnosis in Salmaniya Medical Complex in Bahrain: A Retrospective Cross-Sectional Descriptive Study

Background: Prenatal diagnosis is the process of evaluating the presence of disease or potential disease in the fetus, this enables families to be better prepared before the birth of the baby. There are non-invasive prenatal diagnosis procedures and invasive prenatal diagnosis procedures. The invasive prenatal diagnosis procedures are CVS (chorionic villus sampling) and amniocentesis. The American College of Obstetricians and Gynecologists states that invasive diagnostic testing should be available to all women, regardless of age or risk. Objective: To determine the indications, outcome and results of diagnostic invasive prenatal procedures. Study setting: The obstetrics and Gynecology Department in Salmaniya Medical Complex in Kingdom of Bahrain. Study design: Retrospective descriptive study. Study subjects and Methods: This retrospective descriptive study was conducted on 175 pregnant women who underwent invasive prenatal procedures (CVS and amniocentesis) between January 2013 and December 2018 at SMC in Kingdom of Bahrain. All medical records of the participants were reviewed and entered the study. According to the implemented procedures, medical records were categorized into two chorionic villus sampling (CVS) and amniocentesis groups. The study subject will include indications of the procedures which are advanced maternal age, hematological disorders, genetic disorders, metabolic disorders, abnormal structural findings in fetal ultrasound and previous child with aneuploidy. In addition, the study will address the complications, outcome and results of procedures. Results: About half of our indications of the procedures were due to hematological disorders (47.6%) followed by abnormal structural findings in fetal ultrasound (30.1%) then genetic disorders (15.7%), metabolic disorders (4.8%) and advanced maternal age (1.8%). Regarding complications of the procedure;threatened miscarriage or loss of pregnancy within 3 weeks was (2.3%), amniotic fluid leakage (0.7%),

Postpartum Quality of Life in Primiparous Women after Normal Vaginal Delivery versus Caesarean Section

Background: Studies on either postnatal quality of life in general or studies that compare quality of life in new mothers after different mode of delivery are limited. An investigation on health related quality of life measures in women after different type of delivery showed that women who had vaginal delivery had better health related quality of life compared to those who had caesarean section. However, the best method of delivery, vaginal or caesarean for postpartum quality of life is a difficult question as it is a matter of controversy both from professional’s perspectives and from women’s experience during childbirth. Objective: To compare postpartum quality of life in primiparous women after vaginal delivery versus caesarean section. Methods: Prospective cross-sectional study was conducted at the department of Obstetrics and Gynecology, Salmaniya medical complex in Kingdom of Bahrain. 500 primiparous women who gave birth either vaginally or by cesarean section answered a questionnaire designed to include general information, questions from short form health survey questionnaire (SF-36) and specific questions regarding postnatal related symptoms. Results: Body pain, fatigue, wound pain, headache and backache were significantly higher in cesarean section group as compared to vaginal delivery group (p-value Conclusion: Cesarean delivery has negatively affected the quality of life (QOL) of primiparous women. More studies with larger sample sizes should be conducted to examine the effects of cesarean delivery on QOL in both primiparous and multiparas within a shorter period after delivery. .

Parallel Image Processing: Taking Grayscale Conversion Using OpenMP as an Example

In recent years, the widespread adoption of parallel computing, especially in multi-core processors and high-performance computing environments, ushered in a new era of efficiency and speed. This trend was particularly noteworthy in the field of image processing, which witnessed significant advancements. This parallel computing project explored the field of parallel image processing, with a focus on the grayscale conversion of colorful images. Our approach involved integrating OpenMP into our framework for parallelization to execute a critical image processing task: grayscale conversion. By using OpenMP, we strategically enhanced the overall performance of the conversion process by distributing the workload across multiple threads. The primary objectives of our project revolved around optimizing computation time and improving overall efficiency, particularly in the task of grayscale conversion of colorful images. Utilizing OpenMP for concurrent processing across multiple cores significantly reduced execution times through the effective distribution of tasks among these cores. The speedup values for various image sizes highlighted the efficacy of parallel processing, especially for large images. However, a detailed examination revealed a potential decline in parallelization efficiency with an increasing number of cores. This underscored the importance of a carefully optimized parallelization strategy, considering factors like load balancing and minimizing communication overhead. Despite challenges, the overall scalability and efficiency achieved with parallel image processing underscored OpenMP’s effectiveness in accelerating image manipulation tasks.

Analysis of the land suitability for paddy fields in Tanzania using a GIS-based analytical hierarchy process

The importance of irrigation development is considered a key factor for food security and poverty reduction because it improves crop productivity,and ensures stable expansion of agricultural ***,irrigation development requires understanding of the avail-able resources including the suitability of the land for *** this study,the land suitability for paddy fields was evaluated within the United Republic of Tanzania mainland by integrating the geographic information system(GIS)and analytical hierarchy process(AHP).In this study,11 criteria based on various sources(soil type,soil drainage,soil organic carbon,soil pH,soil depth,elevation,slope,land use,topographic wetness index,temperature,and precipitation)were *** criteria were used within the GIS-based AHP to identify the most suitable land for sustainable paddy field cultivation considering the preservation of the natural environment of forests and protected areas by examining two scenarios:rainfed condition and irrigation *** former ten criteria were assumed to be constant in both scenarios and were assigned the same scores,while the latter criterion(precipitation)was assigned different scores for varying amounts to plan new irrigation *** land represents 72.8%of the study area,reducing the potential agriculture land(PAL)appropriate for cultivation to 27.2%.In the rainfed condition scenario,the very high and high suitability classes represent 17.6%of the total land of the study area and 64.7%of the *** the irrigation priority scenario,the same classes represent 21.4%of the total land of the study area and 78.6%of the ***,the distribution of the land suitability for both scenarios was analyzed within eight administrative irrigation zones to determine the irrigation zone with the greatest potential for paddy field cultivation.

The electrochemical oxidation of 1,4-dihydroxyanthraquinone has been studied in the presence of acetylacetone and benzoylacetone as nucleophiles in a mixture of ethanol/water by means of cyclic voltammetry as a diagnostic *** results indicate the participation of electrochemically produced anthraquinone in the Michael addition reaction with acetylacetone and benzoylacetone to form the corresponding new anthraquinone *** the basis of the EC mechanism,the observed homogeneous rate constants（kobs）of the reaction of anthraquinone with acetylacetone and benzoylacetone were estimated by comparing the experimental cyclic voltammograms with the digitally simulated results.

The reaction of electrochemically generated anthradiquinone as a Michael acceptor with 2-mercaptobenzothiazole and 2- mercaptobenzoxazole,as nucleophiles in ethanol/water mixtures has been studied by means of cyclic *** voltammetric data indicate that produced anthradiquinone participates in Michael addition reaction with nucleophiles and via an ECEC mechanism converts to the new anthraquinonethioether *** on an EC mechanism,the observed homogeneous rate constant of the Michael reaction of anthradiquinone with nucleophiles were estimated by comparing the experimental cyclic voltammograms with the digital simulated results.

Pathogenesis of columnar-lined esophagus

Since its initial description, the pathogenesis of the columnar-lined esophagus （CLE） has been surrounded by many controversies. The first controversy is related to the existence of the condition itself. The second controversy centers on whether the CLE is a congenital or an acquired condition. In this article, we review the congenital and acquired theories of development of CLE and discuss the various factors in acquisition of CLE. The bulk of evidence in the literature suggests that CLE is an acquired condition.
