AIM: To study diagnostic laparoscopy as a tool for excluding donors on the day of surgery in living donor liver transplantation (LDLT).METHODS: This study analyzed prospectively collected data from all potential donors for LDLT. All of the donors were subjected to a three-step donor evaluation protocol at our institution. Step one consisted of a clinical and social evaluation, including a liver profile, hepatitis markers, a renal profile, a complete blood count, and an abdominal ultrasound with Doppler. Step two involved tests to exclude liver diseases and to evaluate the donor’s serological status. This step also included a radiological evaluation of the biliary anatomy and liver vascular anatomy using magnetic resonance cholan-giopanc reatography and a computed tomography (CT) angiogram, respectively. A CT volumetric study was used to calculate the volume of the liver parenchyma. Step three included an ultrasound-guided liver biopsy. Between November 2002 and May 2009, sixty-nine potential living donors were assessed by open exploration prior to harvesting the planned part of the liver. Between the end of May 2009 and October 2010, 30 potential living donors were assessed laparoscopically to determine whether to proceed with the abdominal incision to harvest part of the liver for donation. RESULTS: Ninety-nine living donor liver transplants were attempted at our center between November 2002 and October 2010. Twelve of these procedures were aborted on the day of surgery (12.1%) due to donor findings, and eighty-seven were completed (87.9%). These 87 liver transplants were divided into the following groups: Group A, which included 65 transplants that were performed between November 2002 and May 2009, and Group B, which included 22 transplants that were performed between the end of May 2009 and October 2010. A fistula formation between the esophagus and an aberrant right subclavian artery is a rare but fatal com-plication that has been mostly described in the setting of prolonged nasogastric intubation and foreign body erosion. We report a case of a young morbidly obese patient who underwent sleeve gastrectomy that was complicated by a postoperative leak at the level of the gastroesophageal junction. A covered esophageal stent was placed endoscopically to treat the leak. The pa-tient developed massive upper gastrointestinal bleeding secondary to the erosion of the stent into an aberrant retroesophageal right subclavian artery twelve days after stent placement. She was ultimately treated by endovascular stenting of the aberrant right subclavian artery followed by thoracotomy and esophageal repair over a T-tube. This case report highlights the multidis-ciplinary approach needed to diagnose and managesuch a devastating complication. It also emphasizes the need for imaging studies prior to stent deployment to delineate the vascular anatomy and rule out the possi-bility of such an anomaly in view of the growing popu-larity of esophageal stents, especially in the setting of a leak.

Background:?Ovarian cancer (OC) is the most lethal gynecologic malignancy. About 70% of ovarian cancer patients have advanced disease and often not totally resectable. Previous studies of neoadjuvant chemotherapy (NACT) followed by interval debulking surgery (IDS) versus primary debulking surgery (PDS) give inconsistent results. The aim of this retrospective study is to evaluate the outcome of the neoadjuvant chemotherapy followed by IDS versus PDS followed by adjuvant chemotherapy for the International Federation of Gynecology and Obstetrics (FIGO) stage III epithelial ovarian cancer.?Methods:?This study was performed on eligible patients with ovarian cancer admitted in Surgical Oncology and Medical Oncology departments at South Egypt Cancer Institute-Assiut University in the period from January 2010 to December 2015. Patients were divided into two groups, the first group included those who have NACT and underwent IDS plus adjuvant chemotherapy, and the second group included those who underwent PDS followed by adjuvant chemotherapy.?Results:?This study included 380 cases. One hundred and fifty-four patients (40.53%) had IDS. The remaining two hundred and twenty-six patients (59.47%) underwent PDS. In this study, treatment modality was significant for both disease-free survival (DFS) and overall survival (OS). DFS was significantly reduced after IDS when compared to PDS (median DFS: 33.00 months vs. 45.00 months, respectively;p??0.001). Also, OS was significantly reduced after IDS when compared to PDS. (Median OS: 43 months vs. 46 months, respectively;p = 0.047). Moreover, this drop of the survivals mainly occurred in specific subgroups such as the elderly patients, patients with bad performance status, suboptimal cytoreduction, as well as high-grade tumors. 这项试验主要借助新型活性约束技术研究约束混凝土的单轴压缩性能。采用热触发的预应变形状记忆合金(SMA)螺线恢复应力来施加混凝土圆柱试块主动侧限压力。首先,利用热力试验确定选用的螺线恢复应力和预应变损失。其次,单独利用SMA螺线或与玻璃纤维增强复合材料(GFRP)共同约束混凝土柱块。对约束混凝土柱块进行压缩试验,比较它和仅用GFRP被动约束柱块性能的不同。结果表明,SMA螺线在单调及反复加载下具有稳定的恢复应力。试验中测到的预应变损失很少,因此,它对约束混凝土的受力没有影响。试验结果表明,SMA螺线对混凝土试块上主动侧限压力大及稳定的情况是有效的。此外,相比GFRP被动约束混凝土柱块,SMA螺线的极限应变得到明显提高,而强度只是略有提高。

This paper represents the interaction of well characterized Lewis base [（Pyr）nCuX]4O2, n = 1 or 2, X = Cl, Br or I, Pyr = pyrrolidine with CO2 as a Lewis acid to produce new series of oxidative coupling and catechol oxidase initiators [（Pyr）nCuX]4（CO3）2. These carbonato derivatives are isolated as stable solids. They are easily soluble in aprotic solvents as CH2Cl2 or PhNO2. Cryoscopic measurements support tetranuclear core structure for all of them. Infrared spectra show differences from their oxo analogous in the carbonato domains but those differences did not distinguish between tridentate bridging carbonato and bidentate one. Rate of oxidation of 2,6-dimethylphenol （DMP） by [（Pyr）CuCl]4（CO3）2, supports coordination number six for Cu（Ⅱ） centers in [（Pyr）CuCl]4（CO3）2. In order to fulfill coordination number six, for n = 1, carbonate will act as tridentate while for n = 2, it will act as bidentate, as shown in Scheme 4. Near infrared spectra indicate a [（3 halo） Cu（Ⅱ） charge transfer] for [（Pyr）nCuX]4（CO3）2, n = 1 or 2, X = Cl or Br. Low molecular absorptivities of the maxima at 825 nm and 730 nm for [（Pyr）nCuI]4（CO3）2, n = 1 or 2 with a minimum of high molecular absorptivities at 600 nm, comparing to X= CI or Br analogous, support a step structure for [（Pyr）,Cul]4（CO3）2, as shown in Scheme 5. Cyclic voltammograms for [（Pyr）nCuX]4（CO3）2; n = 1 or 2, X = CI or Br, are irreversible in characters.

This paper investigates the performability of hierarchical Wireless Networked Control Systems (WNCS). The WNCS studied can operate in two modes: passive supervisor and active supervisor. It is first shown that the Markov models for both modes are identical. Performability models are then developed and a case study shows how to use these models to help make design decisions. More specifically, it is observed that the performability of a passive supervisor system increases in time while that of an active supervisor system decreases in time.
