The parathyroid glands are the main regulator of plasma calcium and have a direct influence on the digestive tract.Parathyroid disturbances often result in unknown long-standing symptoms.The main manifestation of hypoparathyroidism is steatorrhea due to a deficit in exocrine pancreas secretion.The association with celiac sprue may contribute to malabsorption.Hyperparathyroidism causes smooth-muscle atony,with upper and lower gastrointestinal symptoms such as nausea,heartburn and constipation.Hyperparathyroidism and peptic ulcer were strongly linked before the advent of proton pump inhibitors.Nowadays,this association remains likely only in the particular context of multiple endocrine neoplasia type 1/Zollinger-Ellison syndrome.In contrast to chronic pancreatitis,acute pancreatitis due to primary hyperparathyroidism is one of the most studied topics.The causative effect of high calcium level is confirmed and the distinction from secondary hyperparathyroidism is mandatory.The digestive manifestations of parathyroid malfunction are often overlooked and serum calcium level must be included in the routine workup for abdominal symptoms.

Lineament extraction and analysis is one of the routine work in mapping medium and large areas using remote sensing data, most of which are satellite images. Landsat Enhanced Thematic Mapper （ETM） of 945×1 232 pixels subscene acquired on 21 March 2000 covering the northwestern part of Yunnan Province has been digitally processed using ER Mapper software. This article aims to produce lineament density map that predicts favorable zones for hydrothermal mineral occurrences and quantify spatial associations between the known hydrothermal mineral deposits. In the process of lineament extraction a number of image processing techniques were applied. The extracted lineaments were imported into MapGIS software and a suitable grid of 100 m×100 m was chosen. The Kriging method was used to create the lineament density map of the area. The results show that remote sensing data could be useful to extract the lineaments in the area. These lineaments are closely correlated with the faults obtained through other geological investigation methods. On comparing with field data the lineament-density map identifies two important high prospective zones, where large-scale deposits are already existing. In addition the map highlights unrecognized target areas that require follow up investigation.

Repositioning of dipeptidyl peptidase-4 inhibitors and glucagon like peptide-1 receptor agonists is a breakthrough in the field of neural regeneration research increasing glucagon like peptide-1 bioavailability, hence its neuroprotective activities. In this article, the authors suggest not only crossing blood-brain barrier and neurodegenerative disease as off target for dipeptidyl peptidase-4 inhibitors and glucagon like peptide-1 receptor agonists, but also for ophthalmic preparations for diabetic retinopathy, which may be the latest breakthrough in the field if prepared and used in an appropriate nano-formulation to target the retinal nerves. The relation of neurodegenerative diseases' different mechanisms to the dipeptidyl peptidase-4 inhibitors and glucagon like peptide-1 receptor agonists should be further examined in preclinical and clinical settings. The repositioning of already marketed antidiabetic drugs for neurodegenerative diseases should save the high cost of the time-consuming normal drug development process. Drug repositioning is a hot topic as an alternative to molecular target based drug discovery or therapeutic switching. It is a relatively inexpensive pathway due to availability of previous pharmacological and safety data. The glucagon like peptide-1 produced in brain has been linked to enhanced learning and memory functions as a physiologic regulator in central nervous system by restoring insulin signaling. Intranasal administration of all marketed gliptins(or glucagon like peptide-1 receptor agonists) may show enhanced blood-brain barrier crossing and increased glucagon like peptide-1 levels in the brain after direct crossing of the drug for the olfactory region, targeting the cerebrospinal fluid. Further blood-brain barrier crossing tests may extend dipeptidyl peptidase-4 inhibitors' effects beyond the anti-hyperglycemic control to intranasal spray, intranasal powder, or drops targeting the blood-brain barrier and neurodegenerative diseases with the most suitable formula. Identification of unknown remains recovered from marine and terrestrial locations is a significant humanitarian problem.This investigation proposes a simple method applicable to fragmentary femora for a more refined level of ancestry and/or sex estimation.To that end,we re-examined Purkait’s triangle which involves three inter-landmark distances between the traction epiphyses and the articular rim of femoral head.A large sample(n=584)from geographically diverse(Egyptian,Indian and Greek)populations was compiled.Additionally,shape(n=3)and trigonometrically derived variables and ratios(n=9 variables)were employed to detect any geographically-clustered morphological differences between these populations.Random forest modelling(RFM)and linear discriminant function analysis(LDA)were employed to create classification models in instances where sex was known or unknown.The sample was apportioned into training and test sets with a ratio 70/30.The classification accuracies were evaluated by means of k fold cross-validation procedure.In sex estimation,RFM showed similar performance to LDA.However,RFM outperformed LDA in ancestry estimation.Ancestry estimation was satisfactory in the Indian and Egyptian samples albeit the Greek sample was problematic.The Greek samples presented greater morphological overlap with the Indian sample due to high within-group variation.Test samples were accurately assigned to their ancestral category when sex was known.Generally,higher classification accuracies in the validation sample were obtained in the sex-specific model of females than in males.Using RFM and the linear variables,the overall accuracy reached 83%which is distributed as 95%,71%and 86%for the Egyptian,Indian and Greek females,respectively;whereas in males,the overall accuracy is 72%and is distributed as 58%,87%and 50%for the Egyptian,Indian and Greek males,respectively.Classification accuracies were also calculated per group in the test data using the 12 derived variables.For the females,the accuracies using the medians model wa

Abdominal cocoon,the idiopathic form of sclerosing encapsulating peritonitis,is a rare condition of unknown etiology that results in an intestinal obstruction due to total or partial encapsulation of the small bowel by a f ibrocollagenous membrane.Preoperative diagnosis requires a high index of clinical suspicion.The early clinical features are nonspecif ic,are often not recognized and it is diff icult to make a def inite pre-operative diagnosis.Clinical suspicion may be generated by the recurrent episodes of small intestinal obstruction combined with relevant imaging f indings and lack of other plausible etiologies.The radiological diagnosis of abdominal cocoon may now be conf idently made on computed tomography scan.Surgery is important in the management of this disease.Careful dissection and excision of the thick sac with the release of the small intestine leads to complete recovery in the vast majority of cases.

This research evaluated the efficiency of Atrium in hot,arid regions and investigated the parameters influencing the total energy consumption of an existing shopping mall in Dubai,UAE using the IES-VE energy modeling software.The parameters tested included:Roof profile,Roof ventilation,Number of floors,glazing’s Light to Solar Gain ratio(LSG)and the Thermal insulation of the building envelope.The impact of using light sensors was also studied.The light sensors and the roof’s thermal insulation resulted in the highest reduction of the atrium’s energy,8.5%and 7.5%,respectively.Increasing the number of floors reduced the energy consumption by 5%per unit area.The roof profile,external wall’s thermal insulation and height of the atrium resulted in similar energy reductions of 3.5%each.The external glazing’s LSG reduced the energy consumption by 3.25%while the roof ventilation resulted in a reduction of 1.3%.The internal glazing’s LSG did not affect the atrium’s total energy consumption.An optimal combination of the parameters studies resulted in an energy reduction of about 20%.Thus,this paper presents the optimal design configuration to reduce the energy consumption of an atrium located in a hot and arid climate.

Feasibility of using ultrasound in ambulances in Saudi Arabia

BACKGROUND In developed countries,the economic feasibility of using ultrasound in ambulances before arriving at the hospital has been achieved through comprehensive studies,and this of course does not apply to Arab countries,as there are no current studies to support this.Therefore,this study is a preliminary measure of the economic feasibility of using ultrasound in ambulances in Saudi Arabia.AIM To measure the demand for ultrasound equipment in ambulances in Saudi Arabia.METHODS A cross-sectional study of five different groups of participants including radiation technologists,emergency physicians,paramedics,Red Crescent managers and the public.Email and social media were used to deliver a questionnaire to these groups.The questionnaire included specific questions to measure the purpose of ultrasound use in each group of participants.RESULTS Each group had some knowledge on ultrasound and its benefits.More than 50%in each study group supported the availability of ultrasound in ambulances.Additionally,60%of emergency physicians reported that they had difficulties in venous access,checking the presence of internal bleeding,recognizing pregnancy in trauma cases,and inserting endotracheal tubes,and the majority of them confirmed the effective role of ultrasound in achieving such tasks.Almost all paramedics(93.33%),physicians(98.89%),and Saudi Red Crescent managers(96.3%)emphasized the importance of communication between ambulance staff and emergency departments.Moreover,most physicians(77.78%),and technologists(82.73%)supported the presence of paramedics in ambulances to operate ultrasound in order to improve patient outcomes.CONCLUSION Most of the study groups evaluated had knowledge on ultrasound and supported the presence of ultrasound devices in ambulances.

Landsat TM digital spectral data of Lancang Jinghong area (Yunnan P ro vince) has been used for the purpose of geological interpretation. To meet this object, different image processing techniques including selected band color comp osites, principal component analysis and IHS decorrelation stretching are used t o improve the discrimination of different lithological and structural features i n the area.It was found that IHS decorrelation stretching images obtained from t he transformation of false color composite 741 (in red, green and blue) prov ided the best results based on the original data.By combining the characteristic s of images produced by different approaches and other canonically transformed i mages with available geological data and surface observations, the geological in terpretation could be done with satisfactory degree of accuracy.

深水桥梁矩形围堰水流荷载的分流板抑制方法

围堰是深水桥梁建设顺利开展的重要平台和前提,在其节段施工中动水荷载是影响钢围堰精确定位、下沉及着床的关键因素。为减小水流作用下的钢围堰动水荷载,针对矩形围堰提出一种在尾端增加一定长度分流板的抑制方法。首先采用计算流体力学(CFD)方法基于开源分析平台OpenFOAM建立钢围堰的二维简化模型,引入修正的k-ε湍流模型用以闭合求解钢围堰超高雷诺数效应下的N-S方程,通过方形截面和矩形截面柱验证所提方法的准确性;然后针对足尺钢围堰模型分流板的设置对于钢围堰动水阻力和横向摆动力的影响进行分析,明确不同分流板长度以及前/后端分流板对动水荷载的抑制作用,提出优化可行的分流板设置方案。研究结果表明:在高雷诺数条件下标准的k-ε模型不能准确捕捉方形及矩形钝体的涡旋脱落过程,会严重低估动水荷载,采用修正的k-ε模型可准确模拟矩形钝体的绕流特性,与试验值吻合良好,可用于钢围堰的计算分析;矩形钢围堰采用圆角设置可有效减小动水阻力和横向摆动力,横向摆动频率提高;在钢围堰后端增加中央分流板可显著降低动水阻力和横向摆动力,当分流板长度在1/10钢围堰纵边长时,抑制效率最高;分流板长度超过钢围堰纵边长的75%时,尾流出现摆动,导致横向摆动力增大,此时增加前端分流板可抑制摆动作用;仅设置前端分流板对动水作用的影响可忽略;通过现场流迹线试验,验证了采用尾端分流板方案的有效性,其可减小并推后尾流涡旋的形成,有效抑制了钢围堰受到的动水阻力和摆动力。
