Disease dependent qualitative and quantitative differences in the inflammatory response to ascites occurring in cirrhotics

AIm:To assess differing patterns and levels of ascitic fluid cyctokine and growth factors exist between those with a high risk and low risk of spontaneous bacterial peritonitis(SBP). mETHODS: A total of 57 consecutive patients with ascites requiring a large volume paracentesis were studied. Their age, gender, specific underlying disease conditions were recorded after a review of their clinical records. Each underwent a routine assessment prior to their paracentesis consisting of a complete blood count, complete metabolic profile and prothrombin time/international normalized ratio(INR) determination. The ascitic fluid was cultured and a complete cellcount and albumin determination was obtained on the fluid. In addition, blood and ascitic fluid was assessed for the levels of interleukin interleukin(IL)-1A, IL-1B, IL-2, IL-4, IL-8, IL-10, monocyte chemotactic protein(mCP)-1, tumor necrosis factor(TNF)-α, interferon(IFN)-γ, vascular endothelial growth factor(VEGF) and epidermal growth factor(EGF) utilizing the Randox Biochip platforms(Boston, mA). A serum-ascites gradient, for each cytokine and growth factor was calculated. The results are reported as mean ± SEm between disease groups with statistical analysis consisting of the student t-test(two tailed) with a P value of 0.05 defining significance. RESULTS: No clinically important demographic or biochemical differences between the 4 groups studied were evident. In contrast, marked difference in the cytokine and growth factors levels and pattern were evident between the 4 disease groups. Individuals with alcoholic cirrhosis had the highest levels of IL-1A, IL-1B, IL-4, IFNγ. Those with malignant disease had the highest levels of IL-2. Those with hepatitis C virus(HCV) associated cirrhosis had the highest value for IL-6, IL-8, IL-10, mCP-1 and VEGF. Those with cardiac disease had the highest level of TNF-α and EGF. Hepatitis C virus: A time for decisions. Who should be treated and when?

Cirrhosis is the most important risk factor for hepatocellular carcinoma(HCC) regardless of the etiology of cirrhosis. Compared to individuals who are antihepatitis C virus(HCV) seronegative, anti-HCV seropositive individuals have a greater mortality from both hepatic as well as nonhepatic disease processes. The aim of this paper is do describe the burden of HCV infection and consider treatment strategies to reduce HCV-related morbidity and mortality. The newly developed direct acting antiviral(DAA) therapies are associated with greater rates of drug compliance, fewer adverse effects, and appear not to be limited by the presence of a variety of factors that adversely affect the outcome of interferon-based therapies. Because of the cost of the current DAA, their use has been severely rationed by insurers as well as state and federal agencies to those with advanced fibrotic liver disease(metavir fibrosis stage F3-F4). The rationale for such rationing is that many of those recognized as having the disease progress slowly over many years and will not develop advanced liver disease manifested as chronic hepatitis C, cirrhosis, and experience any of the multiple complications of liver disease to include HCC. This mitigation has a short sided view of the cost of treatment of hepatitis C related disease processes and ignores the long-term expenses of hepatitis C treatment consisting of the cost of treatment of hepatitis C, the management of cirrhosis with or without decompensation as well as the cost of treatment of HCC and liver transplantation. We believe that treatment should include all HCV infected patients including those with stage F0-F2 fibrosis with or without evidence of coexisting liver disease. Resveratrol engages selective apoptotic signals in gastric adenocarcinoma cells

AIm: To investigate the intracellular apoptotic signals engaged by resveratrol in three gastric adenocarcinoma cancer cell lines, two of which (AGS and SNU-1) express p53 and one (KATO-Ⅲ) with deleted p53. mETHODS: Nuclear fragmentation was used to quanti- tate apoptotic cells; caspase activity was determined by photometric detection of cleaved substrates; formation of oxidized cytochrome C was used to measure cytochrome C activity, and Western blot analysis was used to determine protein expression. RESULTS: Gastric cancer cells, irrespective of their p53 status, responded to resveratrol with fragmentation of DNA and cleavage of nuclear lamins A and B and PARP. Resveratrol, however, has no effect on mitochondria-associated apoptotic proteins Bcl-2, Bcl- xl, Bax, Bid or Smac/Diablo, and did not promote sub- cellular redistribution of cytochrome C, indicating that resveratrol-induced apoptosis of gastric carcinoma cells does not require breakdown of mitochondrial membrane integrity. Resveratrol up-regulated p53 protein in SNU-1 and AGS cells but there was a difference in response of intracellular apoptotic signals between these cell lines. SNU-1 cells responded to resveratrol treatment with down-regulation of survivin, whereas in AGS and KATO- Ⅲ cells resveratrol stimulated caspase 3 and cytochrome C oxidase activities. CONCLUSION: These findings indicate that even within a specific cancer the intracellular apoptotic signals engaged by resveratrol are cell type dependent and suggest that such differences may be related to differentiation or lack of differentiation of these cells.

AIm:To quantitate the simultaneous serum and ascitic fluid levels of procalcitonin and inflammatory markers in cirrhotics with and without ***:A total of 88 consecutive severe cirrhotic patients seen in a large city hospital liver clinic were studied and divided into two groups,those with and without *** 1 consisted of 41 cirrhotic patients with massive ascites,as demonstrated by necessity for therapeutic large-volume *** 2consisted of 47 cirrhotic patients without any clinically documented ascites to include either a recent abdominal computed tomography scan or ultrasound *** and ascitic fluid levels of an array of inflammatory markers,including procalcitonin,were measured and compared to each other and a normal plasma panel(NPP).RESULTS:The values for inflammatory markers assayed in the serum of Groups 1 and 2,and ascitic fluid of the Group *** plasma levels of the inflammatory cytokines interleukin(IL)-2,IL-4,IL-6,IL-8,interferon gamma(IFNγ)and epidermal growth factor(EGF)were all significantly greater in the serum of Group 1as compared to that of the serum obtained from the Group 2 subjects(all Pm levels of IL-6,IL-8,IL-10,monocyte chemoattractant protein-1,tumor necrosis factor-α,vascular endothelial growth factor and EGF when comparing Group 2 to the *** was no significant difference for IL-1A,IL-1B,IL-2,IL-4 and IFNγlevels between these two *** procalcitonin levels were increased in cirrhotics with ascites compared to cirrhotics without ascites,but serum levels were similar to ascites levels within the ascites ***,many of these cytokines,but not procalcitonin,demonstrate an ascites-to-serum *** procalcitonin does not demonstrate any significant difference segregated by liver etiology in the ascites group;but ascitic fluid procalcitonin is elevated significantly in car-diac cirrhosis/miscellaneous subgroup compared to the hepatitis

Endoscopic retrograde cholangiopancreatography under moderate sedation and factors predicting need for anesthesiologist directed sedation: A county hospital experience

AIm: To evaluate variables associated with failure of gastroenterologist directed moderate sedation (GDS) during endoscopic retrograde cholangiopancreatography (ERCP) and derive a predictive model for use of anesthesiologist directed sedation (ADS) in selected patients. mETHODS: With institutional review board approval, we retrospectively analyzed consecutive records of all patients who underwent ERCPs between July 1, 2009 to October 1, 2011 to identify patient related and procedure related factors which could predict failure of GDS. For patient related factors, we abstracted and analyzeddata regarding the age, gender, ethnicity, alcohol and illicit drug use habits. For procedure related factors, we abstracted data regarding initial or repeat procedures, indication for performing ERCP, the interventions performed during ERCP, and the grade d difficulty of cannulation as defined in the American Society for Gastrointestinal Endoscopy guidelines. Our outcome of interest was procedural success. If the procedure was not successful, the reasons for failure of procedures were recorded along with immediate post procedure complications. multivariate analysis was then performed to define factors associated with failure of GDS and a model constructed to predict requirement of ADS. RESULTS: Fourteen percent of patients undergoing GDS could not complete the procedure due to intolerance and 2% due to cardiovascular complications. Substance abuse, male gender, black race and alcohol use were significant predictors of failure of GDS on univariate analysis and substance abuse and higher grade of procedure remained significant on multivariate analysis. Using our predictive model where the presence of substance abuse was given 1 point and planned grade of intervention was scored from 1-3, only 12% patients with a score of 1 would require ADS due to failure of GDS, compared to 50% with a score of 3 or higher. The well-known frictional effect related to liquid-liquid two-phaseﬂow in pipelines can be reduced using drag-reducing *** this study,such an effect has been investigated experimentally using a mixture of oil and ***,numerical simulations have been carried out using the COmSOL simulation *** mea-surements were taken in a horizontal pipe with the length and diameter equal to 3 and 0.125 m,***,Polyethylene oxide with 150 ppm was exploited to reduce the drag effect while considering different water-to-oil fractions(0.3,0.4,0.5,and 0.7)and a constant totalﬂow velocity of 2.3 m/*** made evident by the results,a signiﬁcant reduction can be obtained in terms of pressure drop,which becomes even more signiﬁcant as the water to oil fraction is *** maximum achieved drag reduction is 70%with a water fraction of *** results also show that the addition of polymer additives can also have an impact on theﬂow ***-parison of experimental and numerically determined pressure drop indicates that the error is smaller than 7%.

In this paper we present a full-geometry Computational Fluid Dynamics (CFD) modeling of air flow distribution from an automotive engine cooling fan. To simplify geometric modeling and mesh generation, different solution domains have been considered, the Core model, the Extended-Hub model, and the multiple Reference Frame (mRF) model. We also consider the effect of blockage on the flow and pressure fields. The Extended-Hub model simplifies meshing without compromising accuracy. Optimal locations of the computational boundary conditions have been determined for the mRF model. The blockage results in significant difference in pressure rise, and the difference increases with increasing flow rates. Results are in good agreement with data obtained from an experimental test facility. Finally, we consider Simplified Fan models which simplifies geometric modeling and mesh generation and significantly reduce the amount of computer memory used and time needed to carry out the calculations. Different models are compared in regards to efficiency and accuracy. The effect of using data from different planes is considered to optimize performance. The effect of blockage on simplified models is also considered.

The behavior of the mixed amphiphilic drug promethazine hydrochloride(PmT) and cationic as well as nonionic surfactants was studied by *** cmc values of the PmT-surfactant systems decrease at a surfactant mole fraction of 0.1 and it then becomes *** critical micelle concentration(cmc) values are lower than the ideal cmc(cmc*) values for PmT/TX-100,PmT/TX-114,PmT/Tween 20,and PmT/Tween 60 *** the PmT/Tween 40,PmT/Tween 80,PmT/CPC,and PmT/CPB systems the cmc values are close to the cmc* *** indicates that PmT forms mixed micelles with these surfactants by attractive *** surface excess(Γmax) decreases in the presence of *** rigid structure of the drug makes adsorption easier and the contribution of the surfactant at the interface *** interaction parameters βm(for the mixed micelles) and βσ(for the mixed monolayer) are negative indicating attraction among the mixed components.
