A Simple Combined Antegrade Radiological and Retrograde Endoscopic Procedure to Recanalise Fibrotic Hypopharyngo-Oesophageal Occlusions: Technical Description and Lessons from Clinical Outcome in Three Cases

Background: Complete hypopharyngo-oesophageal occlusion is a rare complication of head and neck radiotherapy and a range of other conditions. Absolute dysphagia is accompanied by aspiration and dependence on gastrostomy feeding. The condition presents a substantial management challenge. Surgical approaches to re-establish pharyngo-oesophageal continuity are varied, highly invasive and are associated with unpredictable outcomes. Minimally invasive techniques employing endoscopic and radiological techniques are emerging. This report describes a multidisciplinary approach which translates two interventional radiology techniques used in the management of central venous occlusions and biliary strictures to the management of three cases of complete hypopharyngo-oesophageal occlusion. Methods: Three cases with different underlying aetiologies had treatment initiated between 2009 and 2011. Antegrade pharyngoscopic access to the occlusions was accompanied by retrograde endoscopic access via a small gastrostomy. Luminal continuity was re-established by the interventional radiology technique of “sharp recanalisation” followed by passage of a wide bore nasogastric tube which was maintained in situ for 4-6 months, a duration of treatment analogous to that applied in the radiological management of fibrotic biliary strictures. After treatment a radiological contrast swallows examination was performed to gauge the calibre of the re-established lumen, assess functionality and to rule out aspiration. Results: Pharyngo-oesophageal continuity was re-established in all three cases on the first attempt. No complications occurred as a result of the procedures. In two cases, the excellent swallowing function was re-established, although one of these required prolonged post-treatment adjuvant interventions. In one case no swallowing function resulted, despite apparently successful re-establishment of luminal continuity. Conclusions: Complete fibrotic occlusion of the hypopharyngo-oesophagea

Ecotoxicological assessment of diffuse pollution using biomonitoring tool for sustainable land use in Thailand

As a developing country, Thailand has a significant issue with diffuse pollution of the soil ecosystem due to an indiscriminate use of agrichemicals and poorly regulated disposal of a wide variety of hazardous wastes. Practical risk assessment tools based on locallyoccurring species are needed to assess the effects of diffuse pollutants on the soil ecosystem in Thailand because reliance on soil criteria developed for overseas conditions may provide inadequate protection. Native soil organisms in Thailand may be more or less sensitive to contaminants compared to overseas test species. This article described a biological indicator approach for ecological risk assessment of diffuse pollution in the soil ecosystem of Thailand from pesticide application with the aim of developing standardized protocols using native species and locally generated data to better evaluate the ecological risks of non-point source soil pollution. It was found that ecotoxicological assessment provided a better understanding of the ecological impacts that diffuse pollution induced on Thai environmental conditions. Thai soil biota species were more sensitive to soil contaminants than similar species overseas. Soil series also had an influence on the ecotoxicology of contaminants to soil biota. Collembolan, Cyphoderus sp., was demonstrated as a useful alternative test species to Folsomia candida （international test species） for terrestrial ecotoxicological testing of Thai soils. In addition, the soil biota activities such as soil respiration and earthworm avoidance including soil biodiversity and the litter bag decomposition technique are also good tools to assess the effects of diffuse pollution by pesticides on the soil ecosystem of Thailand.

Differences between lower extremity joint running kinetics captured by marker-based and markerless systems were speed dependent

Background:The development of computer vision technology has enabled the use of markerless movement tracking for biomechanical *** research has reported the feasibility of markerless systems in motion analysis but has yet to fully explore their utility for capturing faster movements,such as *** studies using markerless systems in clinical and sports settings are still ***,the present study compared running biomechanics estimated by marker-based and markerless *** running speed not only affects sports performance but is also associated with clinical injury prevention,diagnosis,and rehabilitation,we aimed to investigate the effects of speed on the comparison of estimated lower extremity joint moments and powers between markerless and marker-based technologies during treadmill running as a concurrent validating ***:Kinematic data from marker-based/markerless technologies were collected,along with ground reaction force data,from 16 young adults running on an instrumented treadmill at 3 speeds:2.24 m/s,2.91 m/s,and 3.58 m/s(5.0 miles/h,6.5 miles/h,and 8.0 miles/h).Sagittal plane moments and powers of the hip,knee,and ankle were calculated by inverse dynamic *** series analysis and statistical parametric mapping were used to determine system ***:Compared to the marker-based system,the markerless system estimated increased lower extremity joint kinetics with faster speed during the swing phase in most ***:Despite the promising application of markerless technology in clinical settings,systematic markerless overestimation requires focused *** on segment pose estimations,the centers of mass estimated by markerless technologies were farther away from the relevant distal joint centers,which led to greater joint moments and powers estimates by markerless ***-based *** differences were amplified by running speed.

Application and evaluation of a pattern-based building energy model calibration method using public building datasets

Building performance simulation has been adopted to support decision making in the building life *** essential issue is to ensure a building energy simulation model can capture the reality and complexity of buildings and their systems in both the static characteristics and dynamic *** energy model calibration is a technique that takes various types of measured performance data(e.g.,energy use)and tunes key model parameters to match the simulated results with the actual *** study performed an application and evaluation of an automated pattern-based calibration method on commercial building models that were generated based on characteristics of real buildings.A public building dataset that includes high-level building attributes(e.g.,building type,vintage,total floor area,number of stories,zip code)of 111 buildings in San Francisco,California,USA,was used to generate building models in *** level energy use calibrations were then conducted by comparing building model results against the actual buildings’monthly electricity and natural gas *** results showed 57 out of 111 buildings were successfully calibrated against actual buildings,while the remaining buildings showed opportunities for future calibration *** to the pattern-based model calibration method are identified to expand its use for:(1)central heating,ventilation and air conditioning(HVAC)systems with chillers,(2)space heating and hot water heating with electricity sources,(3)mixed-use building types,and(4)partially occupied buildings.

Simulation of the deformable lateral boundaries in biaxial test using DEM

Applying the discrete element method(DEM)in soil mechanics can provide abundant information at the particle-scale and facilitates illustration of the macro-mechanical behaviour of soils based on the inter-particle *** triaxial test is one of the most common laboratory methods to study the macro-mechanical behaviour of particulate materials such as ***,many problems in geotechnical design can be assumed and simplified as a plane strain *** a biaxial test can be conducted to reproduce the macro-mechanical behaviour of soil,where the sample is enclosed by two horizontal rigid platens and a vertical latex membrane,which is a deformable continuous element and allows the enclosed specimen to deform freely while maintaining confining stress during *** paper presents an algorithm to represent physical and mechanical characteristics of latex membrane in the 2D DEM simulation of biaxial test using the PFC^(2D) *** investigate the impact of the lateral boundary conditions on the micro-macro mechanical behaviours of soil samples,two sets of DEM biaxial tests are conducted,*** rigid and deformable lateral boundary *** DEM modeling results indicate that the lateral boundary conditions have a significant effect on the micro-scale fabric properties,thickness and inclination of the shear *** comparison between these two simulations also demonstrates that the lateral boundary conditions play a major role in the peak and post-peak stress-strain behaviours as well as the dilation and critical state behaviours of granular soils.

Coronaviruses in pregnant women in Saudi Arabia:A systematic comparative review of MERS-CoV and SARS-CoV-2

Objective:To compare the effects of coronaviruses,such as Middle East respiratory syndrome coronavirus(MERS-CoV)or coronavirus disease 2019(COVID-19)caused by severe acute respiratory syndrome coronavirus-2(SARS-CoV-2),on pregnant women in Saudi Arabia,and to understand the disease dynamics of these coronaviruses so as to konw how to provide care and management of infected pregnant women and ***:We conducted a systematic review of all published papers on MERS-CoV and SARS-CoV-2 infections in pregnant women in Saudi Arabia to identify knowledge *** and exclusion criteria were based on the PRISMA *** search included all papers which were published between September 1,2012 and November 29,2020 on pregnant women with MERS-CoV and/or COVID-19 in Saudi *** 26 papers screened,five were included in the ***:A total of 11 pregnant women with MERS-CoV and four with COVID-19 were reported in Saudi Arabia in the medical literature during the review *** mean ages of the women were 33 and 31 years old for MERS-CoV and COVID-19,*** and fetal mortality in cases of MERSCoV were 35%and 30%,respectively,while no maternal or fetal mortalities were reported in COVID-19 ***:Very limited data has emerged from Saudi Arabia on pregnant women MERS-CoV and *** such high mortality observed with MERS-CoV compared to COVID-19,there is a need for greater reporting of cases to truly grasp the extent of these infections in pregnant women in a country where both coronaviruses are circulating.

China, Asia, and the World Economy:The Implications of an Emerging Asian Core and Periphery

A growing body of evidence suggests that China＇s emergence is having differential effects on Asia＇s advanced and developing countries. The region＇s advanced countries are benefiting from the existence of a large and rapidly growing Chinese market for their capital goods, components and technology, whereas its developing countries compete head to head with China in third markets. These facts create additional challenges for late-industrializing Asian countries seeking to catch up with the region＇s industrial leaders. In turn, the emergence of an Asian core and periphery will not encourage the development of a cohesive Asian economic and political bloc. Asian regionalism will be open regionalism in order to prevent regional initiatives from giving rise to costly trade diversion. Efforts to promote Asian financial development and integration are unlikely to come at the expense of the region＇s financial links with the rest of the world. An early move toward a common exchange rate regime with the associated common monetary, stance will be problematic. And, in the absence of these common policies, pressure for the development of powerful regional institutions to formulate the common monetary stance will be at best modest.
