Objectives:To detail the US multi-institutional experience with the Occlutech©(Occlutech International AB,Helsingborg,Sweden)atrial flow regulator(AFr)in children and adults with acquired or congenital heart ***:The creation of a long-term atrial communication is desirable in several cardiovascular disease phenotypes,most notably pulmonary arterial hypertension,disorders of increased left ventricular filling and increased cavopulmonary pressures in patients with a Fontan type ***:Patients were identified for inclusion from the AFr device manufacturer *** was collected using a redCap database following IrB approval.8 weeks of follow up data was sought for each patient based on available *** was analyzed and summarized using ***:We report the experience of 6 US centers in the implantation of AFr devices in 15 patients,across a wide age range,with different disease phenotypes and a variety of *** was technically successful in all patients and improvement was noted in both clinical and hemodynamic *** were no immediate or intermediate term complications reported.3 patients died remote from *** deaths were not felt to be related to the AFr device or related procedural ***:Compassionate use of the AFr device in children and adults with congenital&acquired heart disease is technically feasible and produces beneficial short term hemodynamic and symptomatic *** uptake of this technique and treatment at specialist centers has the potential to provide significant benefits to a variety of complex patients with currently limited treatment options and indeterminate prognosis.

OBJECTIVE:To determine the difference in prevention of chemotherapy-induced fatigue between time-acupointsspace acupuncture(ATAS)administered weekly compared to sham acupuncture and non-acupuncture in patients with early breast cancer receiving adjuvant chemotherapy by epirubicine-cyclophosphamide(EC)followed by paclitaxel,as measured by the multidimensional fatigue inventory(MFI)over the previous week and Visual Analogue Scale measuring fatigue(VAS-F),and to evaluate the effects of ATAS on selfreported neuropathy pain,sleep,anxiety and ***:In this multicenter clinical trial,we have randomized patients into 3 groups:ATAS,Sham and nonacupuncture with an unequal randomization of 2∶1∶1.A cloud related electronical clinical report form and smartphone platform was established for data *** with a history of stageⅠ-Ⅲbreast cancer scheduled to receive adjuvant *** will be delivered once a week during chemotherapy with VAS-F *** order to qualify and quantify the mechanism of fatigue induced by chemotherapy with or without acupuncture,an evaluation of immune profiling was incorporate in this ***:The presence and seriousness of chemotherapy-induced fatigue should be considered in therapeutic programs for early breast cancer *** induced by adjuvant chemotherapy in patients with breast cancer remains a major concern affecting the quality of life ***,we do not have effective pharmacological interventions *** clinical trials of acupuncture in preventing chemotherapyinduced fatigue in patients with early breast cancer have not been ***:The findings of the trial will allow us to determine the effects of acupuncture treatment *** will also be able to confirm whether ATAS is better than sham acupuncture and non-acupuncture treatments.

The majority of spatial data reveal some degree of spatial dependence. The term “spatial dependence” refers to the tendency for phenomena to be more similar when they occur close together than when they occur far apart in space. This property is ignored in machine learning (ML) for spatial domains of application. Most classical machine learning algorithms are generally inappropriate unless modified in some way to account for it. In this study, we proposed an approach that aimed to improve a ML model to detect the dependence without incorporating any spatial features in the learning process. To detect this dependence while also improving performance, a hybrid model was used based on two representative algorithms. In addition, cross-validation method was used to make the model stable. Furthermore, global moran’s I and local moran were used to capture the spatial dependence in the residuals. The results show that the HM has significant with a r2 of 99.91% performance compared to rBFNN and rF that have 74.22% and 82.26% as r2 respectively. With lower errors, the HM was able to achieve an average test error of 0.033% and a positive global moran’s of 0.12. We concluded that as the r2 value increases, the models become weaker in terms of capturing the dependence.

Preparing Health Professionals to Provide Care to Individuals with Disabilities

Aim To review the perceptions of dental/medical educators and their students in the United States on the adequacy of didactic and clinical preparation to provide service for individuals with disabilities. Methodology An e-mailed questionnaire with follow-up was sent to 198 deans of dental/medical schools, 1,628 directors of residency programs in nine medical/dental residency programs, 427 medical students in 12 medical schools, and 368 health related organizations, facilities and programs. results More than half （58%） of the responding deans of medical schools and 50% of the deans of dental schools reported that a curriculum for patients with disabilities was not a high priority at their school. A majority （61%） of deans of medical schools, and 47% of the deans of dental schools, reported that their graduates were competent to treat patients with disabilities. However, majorities of dental/ medical school seniors and graduates expressed inadequate competency in the care of these patients. A majority of the directors of medical/dental residencies indicated a need for additional training for their residents. Conclusion There is need for increased didactic and clinical preparation of dental/medical school graduates in the care of individuals with special health needs. The interest expressed by health profession educators in an effort to develop appropriate curriculum modules provides an opportunity to prepare new graduates for the care of an increasing population of individuals with disabilities.

Water quality impact due to excessive nutrients has been extensively studied. In recent years, however, micro-pollutants such as pharmaceuticals and hormonal products used in animal agriculture have added an additional impact to overall water quality. Pharmaceuticals used in the poultry, swine, beef, and dairy industries have been detected in various environmental matrices such as, soil, groundwater and surface water. In this study, 26 surface water samples were collected throughout the Bosque river Watershed （BrW） with samples representing a range of land use conditions and locations of major dairy operations. Samples were analyzed using commercially available Enzyme-Linked Immunosorbent Assay test. Of the 26 samples, three samples consistently tested positive for monensin antibiotic with concentration ranging from 0.30 to 3.41 μg/L. These three samples were collected from sites that received varying amount of agriculture wastes （11.7% to 31.3%） and located downstream from sites associated with moderate levels of animal agriculture. The preliminary results suggest that there is a potential for monensin occurrence in the BrW, although initial findings indicate only very low levels.

Silicon containing materials have traditionally been used in microelectronic fabrication. Semiconductor devices often have one or more arrays of patterned interconnect levels that serve to electrically couple the individual circuit elements forming an integrated circuit. These interconnect levels are typically separated by an insulating or dielectric film. Previously, a silicon oxide film was the most commonly used material for such dielectric films having dielectric constants( k ) near 4 0. However, as the feature size is continuously scaling down, the relatively high k of such silicon oxide films became inadequate to provide efficient electrical insulation. As such, there has been an increasing market demand for materials with even lower dielectric constant for Interlayer Dielectric(ILD) applications, yet retaining thermal and mechanical integrity. We wish to report here our investigations on the preparation of ultra low k ILD materials using a sacrificial approach whereby organic groups are burnt out to generate low k porous OrMOSIL films. We have been able to prepare a variety of organically modified silicone resins leading to highly microporous thin films, exhibiting ultra low k from 1 80 to 2 87, and good to high modulus, 1 5 to 5 5 GPa. Structure property influences on porosity, dielectric constant and modulus will be discussed.

Pubo-Penile Testicular Ectopia (ETPP) of the Infant of 4 Months about a Case

Pubo-penile testicular ectopia is a rare congenital malformation whose etiopathogenesis remains poorly understood. It represents other testicular ectopias less than 1% of all testicular migration disorders. We report a clinical observation of a 4-month-old infant who consulted for swelling at the root of the penis associated with vacuity of the right hemi scrotum. An inguinal ultrasound was performed which confirmed the presence of the right testicle. An orchidopexy was performed at 4 months of life by an inguinal approach, the postoperative course was simple with a follow-up of 6 months.

Abdominal Mass revealing a right Ovarian Cyst in a 2-Year-Old Infant: A Case report

Introduction: Ovarian cysts are rare pathologies in infants. They represent 1% to 2% of all abdominal tumors in children. Abdominal pain is the most frequent initial sign. Sometimes the discovery is fortuitous, revealed by an abdominal ultrasound. We report here the case of an ovarian cyst in an infant in order to clarify the diagnostic and therapeutic particularities. Patient and observation: 2-year-old infant, female, weighing 12 kg and with no particular pathological history, was admitted to our department for pain plus abdominal mass evolving for 2 months. The clinical examination had noted: a mass ranging from FID to hypogastrium, painless, of firm consistency, with regular contour, with a smooth and mobile surface in relation to the deep plane. The abdominal ultrasound performed had concluded to a mesenteric cyst. The treatment consisted of a monobloc total cystectomy with preservation of the healthy ovarian tissue by a transverse laparotomy under the umbilical. The postoperative course was simple;the histological study of the surgical specimen had concluded to a serous cystadenoma. Conclusion: Ovarian cysts are benign tumors in most cases. Abdominal pain is the most common initial symptom at any age. Sometimes they can be incidentally discovered by ultrasound. The preservation of healthy ovarian tissue in the rules of oncological surgery allows the preservation of subsequent fertility.
