Analysis of core protein clusters identifies candidate variable sites conferring metronidazole resistance in Helicobacter pylori

Burnout in the emergency department: Randomized controlled trial of an attention-based training program

Background: Burnout(encompassing emotional exhaustion, depersonalization and personal accomplishment) in healthcare professionals is a major issue worldwide. Emergency medicine physicians are particularly affected, potentially impacting on quality of care and attrition from the ***: The aim of this study was to apply an attention-based training(ABT) program to reduce burnout among emergency multidisciplinary team(MDT) members from a large urban ***, setting, participants and interventions: Emergency MDT members were randomized to either a notreatment control or an intervention group. Intervention group participants engaged in a four session(4 h/session) ABT program over 7 weeks with a practice target of 20 min twice-daily. Practice adherence was measured using a smart phone application together with a wearable Charge 2 *** outcome measures: The primary outcome was a change in burnout, comprising emotional exhaustion, depersonalization and personal achievement. The secondary outcomes were changes in other psychological and biometric ***: The ABT program resulted in a significant reduction(P < 0.05; T1 [one week before intervention]vs T3 [follow-up at two months after intervention]) in burnout, specifically, emotional exhaustion, with an effect size(probability of superiority) of 59%. Similar reductions were observed for stress(P < 0.05) and anxiety(P < 0.05). Furthermore, ABT group participants demonstrated significant improvements in heart rate variability, resting heart rate, sleep as well as an increase in pro-inflammatory cytokine ***: This study describes a positive impact of ABT on emergency department staff burnout compared to a no-treatment control group.

Objectives:To detail the US multi-institutional experience with the Occlutech©(Occlutech International AB,Helsingborg,Sweden)atrial flow regulator(AFR)in children and adults with acquired or congenital heart ***:The creation of a long-term atrial communication is desirable in several cardiovascular disease phenotypes,most notably pulmonary arterial hypertension,disorders of increased left ventricular filling and increased cavopulmonary pressures in patients with a Fontan type ***:Patients were identified for inclusion from the AFR device manufacturer *** was collected using a RedCap database following IRB approval.8 weeks of follow up data was sought for each patient based on available *** was analyzed and summarized using ***:We report the experience of 6 US centers in the implantation of AFR devices in 15 patients,across a wide age range,with different disease phenotypes and a variety of *** was technically successful in all patients and improvement was noted in both clinical and hemodynamic *** were no immediate or intermediate term complications reported.3 patients died remote from *** deaths were not felt to be related to the AFR device or related procedural ***:Compassionate use of the AFR device in children and adults with congenital&acquired heart disease is technically feasible and produces beneficial short term hemodynamic and symptomatic *** uptake of this technique and treatment at specialist centers has the potential to provide significant benefits to a variety of complex patients with currently limited treatment options and indeterminate prognosis.

Splenomegaly, hypersplenism, and hereditary disorders with splenomegaly

Splenomegaly, sometimes of massive extent, occurs in a large number of hereditary diseases, some relatively prevalent and others, rare to ultra-rare. Because physicians are often unfamiliar with the less common disorders, patients may suffer because of diagnostic delay or diagnostic error and may undergo invasive, non-innocuous procedures such as splenectomy that are potentially avoidable were the correct diagnosis suspected. In this review article, we discuss the definition and clinical ramifications of “massive” splenomegaly and describe several rare genetic disorders that are sometimes associated with marked splenic enlargement as well as four additional hereditary “splenomegalic” lysosomal storage diseases (cholesterol esterase storage disease, Niemann-Pick C disease, acid sphingomyelinase deficiency disease, Gaucher disease) in which approved or promising experimental treatments should generally obviate the need for palliative splenectomy. We also summarize current concepts about the appropriate use of splenectomy in patients with β-thalassemia, hereditary spherocytosis and Gaucher disease and discuss surgical alternatives to classical total splenectomy for these disorders.

Sanitary Hazards and Microbial Quality of Open Dug Wells in the Maldives Islands

Concern for saline and microbial quality post-December 2004 tsunami, led to a field based surveillance study to systematically investigate the sanitary hazards which cause faecal contamination of groundwater. In seven islands, two duplicate sample sets, in two surveys, revealed that only 6.4% of the 173 well water samples (combining both surveys) satisfied the WHO Drinking Water Quality Guideline for 44°C thermo-tolerant (Faecal) Coliform (FC) indicator value (zero cfu/100 ml sample). Based on a combined risk analysis of Sanitary Hazard Score (SHS) and FC counts, more than 57.7% of the study wells were classified as at very high (FC: 100 to >1,000 cfu/100ml;and SHS: ≥ 9) microbial health risk. During this study, fundamental changes were made to the published generic sanitary inspection method (WHO, 1997) for identifying sanitary hazards, for its application in the extremely vulnerable hydro-geological setting of the Maldives. However, the most important hazard controlling the intensity of faecal contamination in the Maldives is the safe separation distance between a latrine seepage point and the well. It was demonstrated that, due to the prevailing hydro-geological conditions and the well and sanitation system densities, safe separation distance cannot be achieved. Consequently, septic tank effluent quality must be greatly improved.

The paper considers three common snubber circuits used on gate turn-off thyristor and/or insulated gate bipolar transistor inverters. The three snubbers are passive lossless circuits for power bridge legs, and the improvements and modifications to these snubber circuits are presented. The comparative features and operation of the three improved energy recovery snubbers are discussed and supported by PSPICE simulations and experimental results.

Cell-Specific Effects of Ethanol on Nitric Oxide Synthase Induced by Inflammatory Mediators

To examine the pro- and anti-oxidant properties of nitric oxide (NO) production in alcoholic liver disease, we compared the effects of ethanol pretreatment (24 hrs, 100 mM concentration in a dedicated CO2 incubator) on the induction of inducible NO synthase and its activity in a cell culture system. We employed two types of liver cells as models for the intact liver parenchyma: the rat hepatoma cell FTO2B and rat hepatocytes in primary culture. Cells were incubated with a combination of cytokines (IFN-γ, TNF-α, IL-β) and LPS in the presence or absence of (85 - 93 mM) ethanol in the culture media. At series of time intervals, production of nitric oxide was measured as the accumulation of nitrite and nitrate, using Griess assay. The results revealed that 1) total NO formation was attenuated by ethanol in hepatocytes (ca. 50%) but augmented in FTO2B cells (ca. 37%) and 2) both pretreatment and co-treatment with ethanol were necessary for maximal ethanol effect. These results indicate that the effects of ethanol on inflammatory processes, such as induction of NO synthesis, are cell-type specific.

Multitarget stool DNA for colorectal cancer screening:A review and commentary on the United States Preventive Services Draft Guidelines

Multitarget stool DNA(mt-sDNA) testing was approved for average risk colorectal cancer(CRC) screening by the United States Food and Drug Administration and thereafter reimbursed for use by the Medicare program(2014).The United States Preventive Services Task Force(USPSTF) October 2015 draft recommendation for CRC screening included mt-s DNA as an "alternative" screening test that "may be useful in select clinical circumstances",despite its very high sensitivity for early stage *** evidence supporting mt-s DNA for routine screening use is *** clinical efficacy of mt-s DNA as measured by sensitivity,specificity,life-years gained(LYG),and CRC deaths averted is similar to or exceeds that of the other more specifically recommended screening options included in the draft document,especially those requiring annual testing *** a population with primarily irregular screening participation,tests with the highest point sensitivity and reasonable specificity are more likely to favorably impact CRC related morbidity and mortality than those depending on annual *** paper reviews the evidence supporting mt-s DNA for routine screening and demonstrates,using USPSTF's modeling data,that mt-s DNA at three-year intervals provides significant clinical net benefits and fewer complications per LYG than annual fecal immunochemical testing,high sensitivity guaiac based fecal occult blood testing and 10-year colonoscopy screening.

3d transition metal oxides based thin film coatings such as copper–cobalt oxides exhibit high absorption in the visible region and low emittance in the infrared to far-infrared region of the solar spectrum which is favourable for use as potential selective surface materials in photothermal devices. These materials have the potential to minimize heating while increasing absorption in the operative spectrum range and therefore achieve higher solar selectivity. A series of mixed copper–cobalt metal spinel oxides（Cu_xCo_yO_z）doped with graphene oxide thin films were deposited on commercial grade aluminium substrates using a sol-gel dip-coating technique at an annealing temperature of 500 °C in air for 1 h. Characterizations of the synthesized films were carried out by high temperature synchrotron radiation X-ray Diffraction（SRXRD）, UV–Vis, Fourier Transform infrared spectroscopy（FTIR） and X-ray photoelectron microscopy（XPS）techniques. High thermal stability of coatings with multiple phases, binary and ternary metal oxides, was defined through SR-XRD study. FTIR analysis shows moderate（〈80%） to high（up to 99%） reflectance in the infrared region while the UV–Vis investigations demonstrate that, in the visible region, solar absorption increases gradually（up to 95%） with the addition of graphene oxide to the Cu_x Co_y O_z *** the incorporation of 1.5 wt.% of graphene oxide to the copper–cobalt oxide coatings, a high solar selectivity of 29.01（the ratio of the average solar absorptance in visible and the average thermal emittance in infrared to far infrared region; α/ε） was achieved.
