Obje.tive. To inve.tigate.the.clinical fe.ture. of the.multiple.cutane.us and ute.ine.le.omyomatosis (MCUL) syndrome. including the.he.e.itary le.omyomatosis and re.al ce.l cance. syndrome. De.ign: A case.se.ie. of patie.ts with multiple.skin le.omyomas solicite. via a circular le.te. to de.matologists. Se.ting: Re.e.rch institute. Patie.ts: A total of 108 affe.te. individuals, including 46 probands and 62 affe.te. re.ative.. Main Outcome.Me.sure.: The.proportion of probands with unde.lying fumarate.hydratase.(FH) mutations, the.pe.e.rance.of FH mutations, and clinicopathologic fe.ture. of MCUL. Re.ults: Forty-one.(89% ) of 46 probands with multiple.skin le.omyomas had e.ide.ce.of ge.mline.FH mutations, which we.e.highly pe.e.rant. All 26 male.mutation carrie.s had skin le.omyomas. Of 67 wome. with FH mutations, 46 (69% ) had both skin and ute.ine.le.omyomas; 10 (15% ) had only skin le.omyomas;5(7% )had only ute.ine.le.omyomas;and 6(9% )we.e.clinically unaffe.te.. Patie.ts pre.e.te. with skin le.omyomas at a me.n age.of 24 ye.rs and had a me.n of 25 le.ions. Forty-one.individuals (89% ) re.orte. painful le.ions, particularly in re.ponse.to cold or trauma. Fibroids we.e.histologically unre.arkable. highly symptomatic, and associate. with a high risk of e.rly hyste.e.tomy. One.individual had a ve.y aggre.sive.colle.ting duct re.al cance.. The.G354R FH mutation pre.ispose. patie.ts to ute.ine.fibroids without skin le.omyomas (P=.03). Many patie.ts with skin le.omyomas had not pre.iously pre.e.te. for me.ical atte.tion. Fibroids we.e.rare.y re.ognize. as case. of MCUL. Conclusions: Highly pe.e.rant FH mutations unde.lie.MCUL. Incre.se. clinical aware.e.s is important be.ause.of the.associate. risk of se.e.e.ute.ine.fibroids and, in some.case., aggre.sive.re.al cance..

The.calamitous conse.ue.ce. of 2017 Hurricane.Maria for the.Caribbe.n island of Dominica highlighte. the.acute.and incre.sing susce.tibility of the.re.ion to disaste.s. De.pite.incre.sing inte.national atte.tion to disaste. risk re.uction, re.ove.y from hazard e.e.ts can be.e.pe.ially le.gthy and difficult for small island de.e.oping state.. In this article. we.build on e.isting unde.standings of disaste. risk as a physical and social condition, showing that historical proce.se. are.fundame.tal to unde.standing how conditions of risk e.e.ge.and pe.sist ove. time. We.take.an inte.rate. approach to analyzing the.drive.s of risk accumulation, using the.e.ample.of Dominica, whe.e.proce.se. se. in motion during the.colonial pe.iod have.shape. the.location of pe.ple.and asse.s, the.de.re. to which the. might be.harme., the.socie.al re.e.cussions of that harm and the.prospe.ts for re.ove.y. We.focus on the.unde.lying e.onomic vulne.abilitie. and physical e.posure.to hazards cre.te. by agricultural, e.onomic, and social practice., and succe.sive.disaste. re.ponse. that have.constraine. re.ove.y. Uncove.ing the.e.historical drive.s and pe.siste.t issue., e.ucidate. le.sons for pursuing a more.re.ilie.t de.e.opme.t traje.tory, including through the.promotion of e.onomic re.tructuring and dive.sification, and land re.orm.
