primary Hyperparathyroidism:A Tale of Two Cities Revisited-New York and Shanghai

In the 1970s, with the advent of biochemical multichannel screening in the United States and other western countries, the clinical presentation of primary hyperparathyroidism （pHpT） changed from a symptomatic to an asymptomatic disorder. However, in Asian countries, like China, pHpT did not show this evolution, but rather continued to be a symptomatic disease with target organ involvement. In this paper, we revisit the clinical features of pHpT in New York and Shanghai, representative United States and Chinese cites, over the past decade. The questions we address are whether the disease evolved in China to a more asymptomatic one and, whether in the United States further changes are evident. The results indicate that while pHpT con- tinues to present primarily as an asymptomatic disease in the United States, a new phenotype characterized by normal serum calcium and high parathyroid hormone levels, normocalcemic pHpT, has emerged. Data from Shanghai demonstrates a trend for pHpT to present more commonly as an asymptomatic disorder in China. However, most patients with pHpT in China still manifest classical symptoms, i.e. nephrolithiasis and fractures. A comparison of the two cohorts shows that Chinese patients with pHpT are younger, with higher serum calcium and pTH levels, and lower 25-hydroxyvitamin D levels than patients in New York. Normocalcemic pHpT has not yet been recognized in Shanghai. In summary, although the phenotypes of pHpT in both cities are evolving towards less evident disease, sharp clinical and biochemical differences are still apparent in pHpT as expressed in China and the United States.

Maturation of Osteoblast-Like Saos-2 on Hydroxyapatite Ceramics

Hydroxyapatite ceramics (HA) has been proved to be excellent in biocompatibility and bioactivity. However, limited information is available concerning how HA ceramics affects the maturation of osteoblasts in molecular biological level in vitro. This study examines the mRNA expression and protein production of bone-related genes in osteoblast-like cell line (Saos-2) cultured on HA disks. Saos-2 cells are seeded onto the substrates and cultured for 18 days. Harvested cells are tested for the cell growth rate, expression of mRNAs for osteocalcin and alkaline phosphatase, and protein production of bone sialoprotein and osteocalcin. MTS assay shows that cell proliferates well on HA ceramic substrate. After 9d, bone sialoprotein and osteocalcin protein production in SapS-2 increases more on HA surfaces than on control material. As bone sialoprotein and osteocalcin are the genes to be highly expressed at the late stage of osteoblast differentiation, this study reveals that after long time culture in HA, HA can induce Saos-2 maturation. The behavior of Saos-2 on HA surfaces revealed in this study provides valuable information for the understanding of the biocompatibility and bioactivity of HA ceramics.

Controlled grazing is considered a good management strategy to maintain or increase the live weight of livestock and to reduce vegetation degradation of rangelands. The present study investigated soil characteristics, aboveground vegetation biomass dynamics and controlled grazinginduced changes in the live weight of local ewes in the semi-arid rangeland of Ahmadun, Ziarat, Balochistanprovince of pakistan. An area of 115 ha was protected from livestock grazing in April 2014. In June 2015, soil characteristics within 0-30 cm depth i.e. soil organic matter(SOM), mineral nitrogen, p H and texture in controlled and uncontrolled grazing sites were assessed. Aboveground vegetation biomass measured in early(June) and late summer(August) in 2015 and 2016. The nutritional value i.e. crude protein, phosphorus(p), neutral detergent fiber(NDF), acid detergent fiber(ADF), calcium(Ca), magnesium(Mg) and potassium(K) of dominantplant species were assessed at the beginning of experiment in 2015. Vegetation cover of controlled and uncontrolled grazing sites was also measured during the two years of the study period using the Veg Measure software. From June to November in2015 and 2016, controlled and uncontrolled livestock grazing sites were grazed on a daily basis by local ewes with a stocking rate of 2 and 1 head ha^(-1) respectively. Results reveal that the organic matter contents of coarse-textured, slightly alkaline soil of the study site were in the range of 9.4-17.6 g kg^(-1) soil and showed a strong positive correlation with aboveground vegetation biomass. Donor-to-recipient gender match in liver transplantation: A systematic review and meta-analysis

AIM To perform a systematic review and meta-analysis on donor-to-recipient gender mismatch as a risk factor for post-transplant graft loss. METHODS A systematic literature search was performed using pubM ed, Cochrane Library database and EMBASE. The primary outcome was graft loss after liver transplantation. Odds ratios and 95% confidence intervals were calculated to compare the pooled data between groups with different donor-to-recipient gender matches. Three analyses were done considering(1) gender mismatches(F-M and M-F) vs matches(M-M and F-F);(2) Femaleto-Male mismatch vs other matches; and(3) Male-toFemale mismatch vs other *** A total of 7 articles were analysed. Gender mismatch(M-F and F-M) was associated with a significant increase of graft loss respect to match(M-M and F-F)(OR: 1.30; 95%CI: 1.13-1.50; p pecifically investigated, it confirmed its detrimental role in terms of graft survival(OR: 1.83; 95%CI: 1.20-2.80; p = 0.005). M-F mismatch failed to present a significant role(OR: 1.09; 95%CI: 0.73-1.62; p = 0.68).CONCLUSION Gender mismatch is a risk factor for poor graft survival after liver transplantation. Female-to-male mismatch represents the worst combination. More studies are needed with the intent to better clarify the reasons for these results.

Sonographic signs of neutropenic enterocolitis

AIM： To investigate the sonographic features at time of diagnosis and follow-up in patients with neutropenic enterocolitis. METHODS： The sonographic findings in 14 patients with neutropenic enterocolitis were described and evaluated regarding symptoms and clinical outcome. RESULTS： In all patients with neutropenic enterocolitis, the ileocoecal region was involved with wall thickening 〉10 mm. A transmural inflammatory pattern, hypervascularity of the thickened bowel wall and free abdominal fluid were the common findings. The sonographically revealed thickness of the bowel wall was associated with lethal outcome （p〈0.03）. In the 11 surviving patients, the improvement of clinical symptoms was accompanied by progressive reduction of intestinal wall thickness. CONCLUSION： High-end sonography of the bowel is a helpful tool for diagnosis, assessment of prognosis and follow-up of patients with neutropenic enterocolitis. The ultrasonographically revealed bowel thickness reflects the severity and the course of the disease, and seems to be predictive for the clinical outcome.

Androgen receptor deficiency in monocytes/ macrophages does not alter adiposity or glucose homeostasis in male mice

Androgen deprivation in men leads to increased adiposity, but the mechanisms underlying androgen regulation of fat mass have not been fully defined. Androgen receptor （AR） is expressed in monocytes/macrophages, which are resident in key metabolic tissues and influence energy metabolism in surrounding cells. Male mice bearing a cell-specific knockout of the AR in monocytes/macrophages （M-ARKO） were generated to determine whether selective loss of androgen signaling in these cells would lead to altered body composition. Wild-type （WT） and M-ARKO mice （12-22 weeks of age, n = 12 per group） were maintained on a regular chow diet for 8 weeks and then switched to a high-fat diet for 8 additional weeks. At baseline and on both the regular chow and high-fat diets, no differences in lean mass or fat mass were observed between groups. Consistent with the absence of differential body weight or adiposity, no differences in food intake （3.0 ± 0.5 g per day for WT mice vs 2.8 ± 0.4 g per day for M-ARKO mice） or total energy expenditure （0.6 ± 0.1 Kcal h-1 for WT mice vs 0.5 ± 0.1 Kcal h-1 for M-ARKO mice） were evident between groups during high-fat feeding. Liver weight was greater in M-ARKO than that in WT mice （1.5 ± 0.1 g vs 1.3 ± 0.0 g, respectively, p = 0.02）. Finally, M-ARKO mice did not exhibit impairments in glucose tolerance or insulin sensitivity relative to WT mice at any study time point. In aggregate, these findings suggest that AR signaling specifically in monocytes/macrophages does not contribute to the regulation of systemic energy balance, adiposity, or insulin sensitivity in male mice.

Litter removal impacts on soil biodiversity and eucalypt plantation development in the seasonal tropics
