Multiple hepatic infarctions secondary to diabetic ketoacidosis:A case report

BACKGROUND Hepatic infarctions(HI)are ischemic events of the liver in which a disruption in the blood flow to the hepatocytes leads to focal ischemia and necrosis.Most HI are due to occlusive events in the liver’s blood vessels,but non-occlusive HI may occur.They are associated with disruption of microvasculature,such as in diabetic ketoacidosis.While HI usually presents as peripheral lesions with clear borders,irregular nodular lesions may occur,indistinguishable from liver neoplasms and presenting a diagnostic challenge.CASE SUMMARY We report a case of multiple extensive HI in a patient with poorly controlled diabetes mellitus,who first presented to the emergency room with diabetic ketoacidosis.He then developed jaundice,thrombocytopenia,and a marked elevation of serum aminotransferases.An ultrasound of the liver showed the presence of multiple irregular lesions.Further investigation with a computerized tomography scan confirmed the presence of multiple hypoattenuating nodules with irregular borders and heterogeneous appearance.These lesions were considered highly suggestive of a primary neoplasm of the liver.While the patient was clinically stable,his bilirubin levels remained persistently elevated,and he underwent an ultrasound-guided percutaneous biopsy of the largest lesion.Biopsy results revealed extensive ischemic necrosis of hepatocytes,with no signs of associated malignancy.Three months after the symptoms,the patient showed great improvement in all clinical and laboratory parameters and extensive regression of the lesions on imaging exams.CONCLUSION This case highlights that diabetic ketoacidosis can cause non-occlusive HI,possibly presenting as nodular lesions indistinguishable from neoplasms.

BACKGROUND Superior mesenteric artery syndrome(SMAS)is a rare condition,characterized by duodenal obstruction caused by compression of its third part by the superior mesenteric artery(SMA).Most cases of SMAS are associated with weight loss,and the most frequent clinical manifestations are nausea,vomiting,postprandial fullness,and abdominal pain.Treatment of SMAS is usually conservative,consisting mainly of adequate nutritional support,but in refractory cases surgery may be necessary,with gastrojejunostomy and duodenojejunostomy being the most commonly performed procedures.CASE SUMMARY We describe the case of a man in his forties with a pre-existing diagnosis of esophageal stricture due to sodium hydroxide ingestion,who suffered significant weight loss after replacement of his jejunostomy tube.He was admitted to the hospital due to pain and abdominal distension.A computerized tomography scan showed significant distension of the stomach and duodenum with narrowing of the duodenum at the point at which it is crossed by the superior mesenteric artery,thus establishing the diagnosis of SMAS.Due to the presence of the esophageal stricture,the patient was incapable of emesis;however,passage of a nasogastric tube for decompression was not possible.Considering the risk of gastric perforation due to distention,we opted for surgical treatment in the form of a surgical gastrojejunostomy after which he showed complete resolution of all symptoms and was discharged from the hospital 5 d after the procedure.CONCLUSION Diagnosis of SMAS can be challenging in patients with esophageal stenosis,and risk of gastric perforation may preclude conservative treatment.

Oxidative stress,inflammation,and gut microbiota impairments have been implicated in the development and maintenance of diabetes mellitus.Strategies capable of recovering the community of commensal gut microbiota and controlling diabetes mellitus have increased in recent years.Some lactobacilli strains have an antioxidant and anti-inflammatory system capable of protecting against oxidative stress,inflammation,and diabetes mellitus.Experimental studies and some clinical trials have demonstrated that Limosilactobacillus fermentum strains can beneficially modulate the host antioxidant and anti-inflammatory system,resulting in the amelioration of glucose homeostasis in diabetic conditions.This review presents and discusses the currently available studies on the identification of Limosilactobacillus fermentum strains with anti-diabetic properties,their sources,range of dosage,and the intervention time in experiments with animals and clinical trials.This review strives to serve as a relevant and well-cataloged reference of Limosilactobacillus fermentum strains capable of inducing anti-diabetic effects and promoting health benefits.

本文采用一种简单、有效的规模化溶液化学策略,在基底(如商用氟掺杂氧化锡透明导电涂层玻璃(FTO))和光活性薄膜(如赤铁矿)之间形成丰富的背接触界面,并用于低成本水氧化反应.高分辨率电子显微镜(扫描电镜、透射电镜、扫描透射电镜)、原子力显微镜、元素成像(电子能量损失谱和能量色散谱)和光电化学研究表明,可通过前驱体溶液的化学成分工程来有效降低机械应力、晶格失配、电子势垒和FTO与赤铁矿在背面接触界面之间的空隙以及FTO与电解液之间的短路和有害反应,进而提升这些低成本光阳极对水氧化反应以及PEC水分解清洁、可持续地生产氢气的整体效率.本研究对通过最小化在介孔电极的背接触界面和晶粒边界上的电子-空穴复合,进而提高电荷收集效率具有重要意义,可提高低成本PEC水裂解装置的整体效率和规模化的能力.

Development of a novel in vivo lung perfusion(IVLP)procedure allows localized delivery of high-dose doxorubicin(DOX)for targeting residual micrometastatic disease in the lungs.However,DOX delivery via IVLP requires careful monitoring of drug level to ensure tissue concentrations of this agent remain in the therapeutic window.A small dimension nitinol wire coated with a sorbent of biocompatible morphology(Bio-SPME)has been clinically evaluated for in vivo lung tissue extraction and determination of DOX and its key metabolites.The in vivo Bio-SPME-IVLP experiments were performed on pig model over various(150 and 225 mg/m^(2))drug doses,and during human clinical trial.Two patients with metastatic osteosarcoma were treated with a single 5 and 7 μg/mL(respectively)dose of DOX during a 3-h IVLP.In both pig and human cases,DOX tissue levels presented similar trends during IVLP.Human lung tissue concentrations of drug ranged between 15 and 293 μg/g over the course of the IVLP procedure.In addition to DOX levels,Bio-SPME followed by liquid chromatography-mass spectrometry analysis generated 64 metabolic features during endogenous metabolite screening,providing information about lung status during drug administration.Real-time monitoring of DOX levels in the lungs can be performed effectively throughout the IVLP procedure by in vivo Bio-SPME chemical biopsy approach.Bio-SPME also extracted various endogenous molecules,thus providing a real-time snapshot of the physiology of the cells,which might assist in the tailoring of personalized treatment strategy.

Corynebacterium pseudotuberculosis is an infectious agent that occurs in small ruminants causing caseous lymphadenitis, and more rarely in humans causing lymphadenitis and pneumonia. The breeding small ruminants have great economic importance in Brazil. Rural farm workers and veterinary students who acquired this disease suffered from weakening symptoms for weeks, and the identification of the etiological agent was time-consuming and complex. Due to the low prevalence of case records, there is probably no available commercial diagnostic kit for C. pseudotuberculosis infection in humans. This study aimed to describe human seroreactivity to secreted antigens from C. pseudotuberculosis. Reactivity of serum from farm workers (n = 14), individuals who work with the bacillus at laboratory (n = 8) or individuals without contact (n = 25) was tested with secreted proteins from PAT10 strain of C. pseudotuberculosis by Western blotting. Samples of all (100%) farm workers showed reactivity to 31 kDa, 71 kDa and 164 kDa proteins, while laboratory workers showed 87.5%, 62.5 % and 37.5%, and no-contact 20%, 0% and 16%, respectively. All sera recognized the 275 kDa protein. Our data suggest that C. pseudotuberculosis secreted proteins are antigenic in humans and the recognition profiles allowed the identification of individuals with and without prior contact with this bacillus. This is the first paper which describes human reactivity to C. pseudotuberculosis in serum samples of workers in Brazil.

Use of Intragastral Balloon and Increase in Fertility： Case Report

The IGB (intragastric balloon) is a silicone device, inflated by endoscopy in the gastric cavity, with physiological saline and a blue dye, up to the volume of 700 mL, and used for a maximum of 6 months. It is indicated for patients with BMI ≥27 kg/m² that have not undergone previous gastric surgery. In obese women there is a decrease in peripheral resistance fertility, and the weight loss achieved in bariatric surgery improves fertility due to reduced subcutaneous fat, with lower insulin resistance. Similar results could be achieved through the use of IGB, a less invasive method. The objective of the article is to report the case of a patient who became pregnant using IGB, considering the hypothesis that the increase in fertility was due to weight loss and/or due to the improvement in the self-esteem (and sexual life) of patient. We selected 13 articles from PubMed, UpToDate, JAMA and a case report. Results: Balloon with drawal occurred on 05/02/16 after positive pregnancy test with final weight 65.5 kg and BMI 25.91 kg/m². Conclusions: Weight loss actually favors fertility, but self-esteem may have been instrumental in improving the sex life, enabling conception.

Carbon isotope compositions of both sedimentary carbonate and organic matter can be used as key proxies of the global carbon cycle and of its evolution through time,as long as they are acquired from waters where the dissolved inorganic carbon(DIC)is in isotope equilibrium with the atmospheric CO2.However,in shallow water platforms and epeiric settings,the influence of local to regional parameters on carbon cycling may lead to DIG isotope variations unrelated to the global carbon cycle.This may be especially true for the terminal Neoproterozoic,when Gondwana assembly isolated waters masses from the global ocean,and extreme positive and negative carbon isotope excursions are recorded,potentially decoupled from global signals.To improve our understanding on the type of information recorded by these excursions,we investigate the pairedδ^13Ccarb andδ^13Corg evolution for an increasingly restricted late Ediacaran-Cambrian foreland system in the West Gondwana interior:the basal Bambui Group.This succession represents a 1~(st)-order sedimentary sequence and records two majorδ^13Ccarb excursions in its two lowermost lower-rank sequences.The basal cap carbonate interval at the base of the first sequence,deposited when the basin was connected to the ocean,hosts antithetical negative and positive excursions forδ^13Ccarb andδ^13Corg,respectively,resulting inΔ^13C values lower than 25‰.From the top of the basal sequence upwards,an extremely positiveδ^13Ccarb excursion is coupled toδ^13Corg,reaching values of+14‰and-14‰,respectively.This positive excursion represents a remarkable basin-wide carbon isotope feature of the Bambui Group that occurs with only minor changes inΔ^13C values,suggesting change in the DIC isotope composition.We argue that this regional isotopic excursion is related to a disconnection between the intrabasinal and the global carbon cycles.This extreme carbon isotope excursion may have been a product of a disequilibria between the basin DIC and atmospheric CO2 induced by an active methanogenesis,favored by t

Tree structure and diversity of lowland Atlantic forest fragments：comparison of disturbed and undisturbed remnants

This study describes the tree community structure of three moist lowland Atlantic Forest fragments in Rio de Janeiro State, southeastern Brazil. Two fragments were disturbed and an undisturbed one was used as reference. Our hypothesis was that disturbed fragments show distinct structural patterns in comparison with undisturbed stands due to past disturbance practices and forest fragmentation. Four 100 ×5 m sampling plots were demarcated in each fragment and all live and dead trees with DBH C 5 cm were located, measured and identified. The results supported our hypothesis, due to the high values found for standing dead trees, an increase of dominance of a few pioneer species, lower values of large trees and species richness in disturbed fragments in comparison with the undisturbed one. The advanced fragmentation process in the Southern Brazilian lowland areas and the high species richness in undisturbed areas highlight these forest fragments as priority areas for conservation and management.

Mesenteric Adenitis as a Differential Appendicitis Diagnosis: Case Report

Mesenteric adenitis consists of an important differential diagnosis of potentially serious diseases such as acute appendicitis, since they manifest themselves with a similar clinical picture. Case report: A 17-year-old male patient presented with periumbilical abdominal pain for 2 days, of abrupt onset and moderate intensity, who gave away with a common analgesic, which progressed to the right iliac fossa (RIF) and increased of intensity. He also reported liquid diarrhea, with several episodes a day, without mucus and/or blood, 38°C fever, anorexia and vomiting. He was hospitalized for investigating a possible diagnostic of Acute Appendicitis. In the exams, it was considered mesenteric adenitis, and antibiotic therapy was prescribed. The patient had complete improvement at the end of treatment. Conclusions: Given its importance as a differential diagnosis of numerous pathologies, as well as controversial before the therapeutic and diagnostic conduct, it is essential to report these cases in order to elucidate the specificities of this disease.
