Comparison of contrast sensitivity and visual acuity between deep anterior lamellar keratoplasty and penetrating keratoplasty in patients with keratoconus

AIM: To evaluate postoperative visual acuity and contrast sensitivity results following deep anterior lamellar keratoplasty (DALK) and penetrating keratoplasty (PK) in patients with keratoconus (KC). METHODS: All the patients’ records with KC who had PK or DALK surgery between May 2010 and May 2011 were retrospectively reviewed. Sixty patients who underwent successful corneal transplantation for KC: 30 eyes underwent DALK and 30 eyes underwent PK were included in this study. Preoperative and postoperative mean logarithm of the minimum angle of resolution (logMAR) uncorrected visual acuity (UCVA), logMAR best spectacle-corrected visual acuity (BSCVA) and intraocular pressure (IOP) were evaluated. Contrast sensitivity tests (CS) were done preoperative and 2 months after all sutures had removed. All surgeries were performed under regional anesthesia (retrobulbar anesthesia) by 1 surgeon (B.K.) who was experienced in penetrating and lamellar keratoplasty techniques. RESULTS: The mean age of the DALK group was 29.67±4.95 (range 18-40) years and the PK group was 28.7±3.53 (range 18-39) years. Preoperatively there was no significant difference in the logMAR UCVA, logMAR BSCVA and IOP between the DALK (1.281±0.56; 0.97±0.85; 12.07±2.12mmHg) and PK (1.34±0.21; 0.98±0.21; 13±2.12mmHg) groups. One-year after surgery there was no significant difference in the mean logMAR UCVA and IOP between the DALK (0.46±0.37; 11.73±2.1mmHg) and PK (0.38±0.21; 12±2.12mmHg) groups. The mean contrast sensitivity was evaluated by CC-100 Topcon LCD at 1.5, 2.52, 4.23, 7.10 and 11.91 cycles per degree (cs/deg) spatial frequencies before and 2 months after the all sutures had removed. CONCLUSION: All patients with keratoconus in both DALK and PK groups performed good visual function postoperatively. AIM：To compare visual,surgical and topographic outcomes of deep anterior lamellar keratoplasty（DALK）and penetrating keratoplasty（PK）for keratoconus（KC）.METHODS：In this multicenter,prospective,randomized clinical trial 76 eyes of 71 KC patients operated between January 2011 and July 2014 in 2 tertiary referral hospitals were included. Consecutive patients were alternately selected to receive one of the two surgical methods. Thirty eight eyes underwent DALK with the big-bubble technique and 38 eyes underwent ***：Mean best spectacle corrected visual acuity（BSCVA）at the first postoperative week（P=0.012）and the first postoperative month（P〈0.001）was statistically significantly higher in DALK group. The mean BSCVA at12 mo was not significantly different for DALK（0.30±1.99 log MAR）versus PK（0.40±0.33 log MAR）（P=0.104）. The76.3% of the eyes had a BSCVA over 0.5 in DALK and 47.4%in PK group（P=0.009）. The 7.9% of the eyes had a BSCVA of 1.0 in DALK and 5.3% in PK group（P=0.644）. Mean spherical equivalent was-2.94 D in DALK and-3.09 D in PK *** topographic astigmatism was 4.62 D and 4.18 D respectively. Regular topographic patterns were observed in 31（81.6%）of DALK and 29（76.3%）of PK（P=0.574）. The most frequent topographic pattern was oblate asymmetric bow tie,seen in 39.5% in DALK and 23.7% in PK. CONCLUSION：Big bubble DALK provides an earlier visual improvement compare to PK. However,visual and topographic outcomes are similar to those in PK at 1y. Postoperative complications including rejection and intraocular pressure elevation are more frequent in PK. DALK is a safer alternative to PK for KC. However,intraoperativeperforation of the Descemet＇s membrane is a significant complication.

目的：评估圆锥角膜患者配戴Orbiflex K角膜接触镜在不同疾病阶段的视力康复结果。方法：选取研究对象为2012-11/2013-12在 Dr. LUtfi Kιrdar Kartal培训和研究医院确诊为圆锥角膜的52例患者80眼。所有患者接受硬性角膜接触镜治疗,术后随访12 mo。患者的平均年龄为26.1±6.9岁（范围：15-43岁）。患者依照Amsler-Krumeich分型以及各阶段角膜曲率值进行分组。在配戴角膜接触镜治疗前后进行裸眼视力（ UCVA）,最佳矫正视力（ BCVA）,主观验光,角膜形态,角膜曲率,中央角膜厚度和生物显微镜检查。结果：角膜横轴屈光度（K1）平均值为50.25±4.17D,而角膜纵轴屈光度（K2）平均值为53.82±4.81D。 K的平均值为52.03±4.42 D。 UCVA的平均值为1.31±0.21 LogMAR （ Snellen 0.05±0.04）,戴眼镜和戴角膜接触镜下BCVA的平均值分别为0.79±0.33 LogMAR（Snellen 0.21±0.17）和0.05±0.08 LogMAR（Snellen 0.91±0.13）。比较角膜接触镜治疗前后的视力结果,差异有统计学意义（P〈0.001）。第1,2,3和4阶段圆锥角膜患者视力提高平均行数分别为5.18±1.38（范围：2-8）行,5.86±1.79（范围：2-8）行,6.32±2.16（范围：3-9）行和6.92±2.35（范围：3-9）行。结论：OrbiflexK角膜接触镜显著地改善了所有阶段的圆锥角膜患者的视力。

The present study seeks the feasibility of using short carbon fibres recycled from polymer matrix composites as alternative to virgin carbon fibres in the reinforcement of magnesium *** microstructures,high temperature mechanical and creep properties of AZ91 alloy and its composites with various recycled carbon fibre contents(2.5 and 5 wt.%)and lengths(100 and 500μm)were investigated in the temperature range of 25-200℃.The microstructural characterization showed that the high shear dispersion technique provided the cast composites with finer grains and relatively homogenous distribution of *** materials tested displayed different behaviour depending on the type of *** general,while enhancements in the mechanical properties of composites is attributed to the load bearing and grain refinement effects of fibres,the fluctuations in the properties were discussed on the basis of porosity formation,relatively high reinforcement content leading to fibre clustering and interlayer found between the matrix and reinforcement compared to those of AZ91 *** compressive creep tests revealed similar or higher minimum creep rates in the recycled carbon fibre reinforced AZ91 in comparison to the unreinforced AZ91.

In this study,the recycled short carbon fiber(CF)-reinforced magnesium matrix composites were fabricated using a combination of stir casting and hot *** objective was to investigate the impact of CF content(2.5 and 5.0 wt.%)and fiber length(100 and 500μm)on the microstructure,mechanical properties,and creep behavior of AZ91 alloy *** microstructural analysis revealed that the CFs aligned in the extrusion direction resulted in grain and intermetallic refinement within the *** comparison to the unreinforced AZ91 alloy,the composites with 2.5 wt.%CF exhibited an increase in hardness by 16-20%and yield strength by 5-15%,depending on the fiber length,while experiencing a reduction in *** the reinforcement content was increased from 2.5 to 5.0 wt.%,strength values exhibited fluctuations and decline,accompanied by decreased *** divergent outcomes were discussed in relation to fiber length,clustering tendency due to higher reinforcement content,and the presence of interfacial products with micro-cracks at the CF-matrix *** creep tests indicated that CFs did not enhance the creep resistance of extruded AZ91 alloy,suggesting that grain boundary sliding is likely the dominant deformation mechanism during creep.

In this study, the boundary-value problem with eigenvalue parameter generated by the differential equation with discontinuous coefficients and boundary conditions which contains not only endpoints of the considered interval, but also point of discontinuity and linear functionals is investigated. So, the problem is not pure boundary-value. The authors single out a class of linear functionals and find simple algebraic conditions on coefficients, which garantee the existence of infinit number eigenvalues. Also the asymptotic formulas for eigenvalues are found.

The order Onygenales is classified in the class Eurotiomycetes of the subphylum *** in this order have classically been isolated from soil and dung,and two lineages contain causative agents of superficial,cutaneous and systemic infections in *** ecology and habitat choices of the species are driven mainly by the keratin and cellulose degradation *** present study aimed to investigate whether the ecological trends of the members of Onygenales can be interpreted in an evolutionary sense,linking phylogenetic parameters with habitat preferences,to achieve polyphasic definitions of the main taxonomic *** processes were estimated by multiple gene genealogies and divergence time *** described families,namely,Arthrodermataceae,Ajellomycetaceae,Ascosphaeraceae,Eremascaceae,Gymnoascaceae,Onygenaceae and Spiromastigoidaceae,were accepted in Onygenales,and two new families,Malbrancheaceae and Neogymnomycetaceae,were introduced.A number of species could not be assigned to any of the defined *** study provides a revised overview of the main lines of taxonomy of Onygenales,supported by multilocus analyses of ITS,LSU,TUB,TEF1,TEF3,RPB1,RPB2,and ribosomal protein 60S L10(L1)(RP60S)sequences,combined with available data on ecology,physiology,morphology,and genomics.

目的:研究剥脱综合征对结膜细胞和泪液功能的影响。方法:研究共纳入剥脱综合征患者60例86眼作为试验组,年龄相仿正常人55例89眼作为对照组。所有115例175眼均接受Schirmer泪试验及泪膜破裂时间试验,并对上方和鼻下方睑裂区球结膜进行印迹细胞学检查。印迹细胞学检查结果根据Nelson方法分级。对TBUT,Schirmer泪试验及印迹细胞学结果进行相关性分析。结果:剥脱综合征患者的Schirmer试验、TBUT均较正常组显著降低,结膜细胞也有显著改变。剥脱综合征患者的印迹细胞学分级中位数为3级,而正常人组为0级。在试验组中发现45例样本出现了两种以上分级共存的现象,而对照组仅发现3例。此外,实验组中的45例样本结膜上皮细胞和杯状细胞分级出现明显差异,且均表现出杯状细胞的破坏甚于上皮细胞,而对照组未发现此种现象。结论:剥脱综合征患者的结膜细胞受到破坏,泪液分泌受到影响,极易造成眼表疾病的出现。
