Heparin-binding protein as a predictor of mortality in patients with diabetes mellitus and community-acquired pneumonia in intensive care unit:a propensity score matched study

BACKGROUND:Patients with diabetes mellitus(DM)are vulnerable to community-acquired pneumonia(CAP),which have a high mortality rate.We aimed to investigate the value of heparin-binding protein(HBP)as a prognostic marker of mortality in patients with DM and CAP.METHODS:This retrospective study included CAP patients who were tested for HBP at intensive care unit(ICU)admission from January 2019 to April 2020.Patients were allocated to the DM or non-DM group and paired with propensity score matching.Baseline characteristics and clinical outcomes up to 90 days were evaluated.The primary outcome was the 10-day mortality.Receiver operating characteristic(ROC)curves,Kaplan-Meier analysis,and Cox regression were used for statistical analysis.RESULTS:Among 152 enrolled patients,60 pairs were successfully matched.There was no significant difference in 10-day mortality,while more patients in the DM group died within 28 d(P=0.024)and 90 d(P=0.008).In the DM group,HBP levels at ICU admission were higher in 10-day non-survivors than in 10-day survivors(median 182.21[IQR:55.43-300]ng/ml vs.median 66.40[IQR:34.13-107.85]ng/mL,P=0.019),and HBP levels could predict the 10-day mortality with an area under the ROC curve of 0.747.The cut-off value,sensitivity,and specificity were 160.6 ng/mL,66.7%,and 90.2%,respectively.Multivariate Cox regression analysis indicated that HBP was an independent prognostic factor for 10-day(HR 7.196,95%CI:1.596-32.455,P=0.01),28-day(HR 4.381,95%CI:1.449-13.245,P=0.009),and 90-day mortality(HR 4.581,95%CI:1.637-12.819,P=0.004)in patients with DM.CONCLUSION:Plasma HBP at ICU admission was associated with the 10-day,28-day,and 90-day mortality,and might be a prognostic factor in patients with DM and CAP.

Lipid nanoparticles(LNPs)have emerged as highly effective delivery systems for nucleic acid-based therapeutics.However,the broad clinical translation of LNP-based drugs is hampered by the lack of robust and scalable synthesis techniques that can consistently produce formulations from early development to clinical application.In this work,we proposed a method to achieve scalable synthesis of LNPs by scaling inertial microfluidic mixers isometrically in three dimensions.Moreover,a theoretical predictive method,which controls the mixing time to be equal across different chips,is developed to ensure consistent particle size and size distribution of the synthesized LNPs.LNPs loaded with small interfering RNA(siRNA)were synthesized at different flow rates,exhibiting consistent physical properties,including particle size,size distribution and encapsulation efficiency.This work provides a practical approach for scalable synthesis of LNPs consistently,offering the potential to accelerate the transition of nucleic acid drug development into clinical application.

Single-pixel imaging(SPI)can capture 2D images of the target with only a nonpixelated detector,showing promising application potential in nonvisible spectral imaging,low-photon imaging,lidar,and other extreme imaging fields.However,the imaging mechanism of traditional SPI makes it difficult to achieve high imaging speed,which is a primary barrier for its widespread application.To address this issue,in this work,we propose and demonstrate a novel high-speed 2D and 3D imaging scheme based on traditional SPI,termed time-resolved single-pixel imaging(TRSPI).Previous SPI works mainly utilize correlation between a stable target and iterative illumination masks to reconstruct a single image.In TRSPI,by further exploiting correlation information between a dynamic scene and every static mask,we can reconstruct a series of time-varying images of the dynamic scene,given the dynamic scene is repetitive or reproducible.Experimentally,we conducted 2D and 3D imaging on a rotating chopper with a speed of 4800 revolutions per minute(rpm),and imaging speeds up to 2,000,000 fps.It is believed that this technology not only opens up a novel application direction for SPI,but also will provide a powerful solution for high-speed imaging.

Coronavirus disease 2019(COVID-19)has continued to spread globally since late 2019,representing a formidable challenge to the world's healthcare systems,wreaking havoc,and spreading rapidly through human contact.With fever,fatigue,and a persistent dry cough being the hallmark symptoms,this disease threatened to destabilize the delicate balance of our global community.Rapid and accurate diagnosis of COVID-19 is a prerequisite for understanding the number of confirmed cases in the world or a region,and an important factor in epidemic assessment and the development of control measures.It also plays a crucial role in ensuring that patients receive the appropriate medical treatment,leading to optimal patient care.Reverse transcription-polymerase chain reaction(RT-PCR)technology is currently the most mature method for detecting viral nucleic acids,but it has many drawbacks.Meanwhile,a variety of COVID-19 detection methods,including molecular biological diagnostic,immunodiagnostic,imaging,and artificial intelligence methods have been developed and applied in clinical practice to meet diverse scenarios and needs.These methods can help clinicians diagnose and treat COVID-19 patients.This review describes the variety of such methods used in China,providing an important reference in the field of the clinical diagnosis of COVID-19.

尘螨是主要的过敏原之一,广泛分布于全球各地,尤其是在家庭环境中,屋尘螨、粉尘螨和热带无爪螨是最常见的种类。已知的尘螨致敏组分超过35种,其中Der p 1、Der p 2和Der p 23是主要组分。目前,临床主要采用粗提物进行皮肤点刺试验和血清特异性免疫球蛋白E(specific immunoglobulin E,sIgE)检测进行尘螨过敏的初步诊断,不能精确识别具体的致敏组分。考虑到不同地区和人群对尘螨的致敏蛋白存在显著差异,尘螨过敏原组分诊断在精确确定致敏组分方面显得尤为重要。这不仅对于过敏原的回避有指导意义,也对确定尘螨免疫治疗方案至关重要。为加强对尘螨分子诊断的认识并促进我国变态反应学科与国际接轨,本文对近期欧洲过敏与临床免疫学会发布的《过敏原组分诊断指导建议2.0》进行内容解读与阐释。

A prospective cohort study of the presence of SARS-CoV-2 in clinical samples from multiple bodily sites:implications for transmission routes of COVID-19

Objective: The coronavirus disease 2019 (COVID-19) epidemic resulting from severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has still spread globally. The occurrence of the Delta variant, which is more infectious and spreads faster than earlier forms of the virus that causes COVID-19, makes infection prevention more challenging. Therefore, this study aimed to gain a comprehensive insight into the transmission routes of SARS-CoV-2 for curbing the propagation of SARS-CoV-2 in human populations.Methods: We studied a prospective cohort of 576 patients admitted consecutively to the First Affiliated Hospital of Guangzhou Medical University from January 21 to June 8, 2020. These patients were chosen based on their similar clinical phenotypes or imaging findings. There were 21 (3.6%) laboratory-confirmed COVID-19 patients (16 severe and 5 mild cases) and 555 non-COVID-19 patients. The antibody response and routes and duration of viral shedding were systematically evaluated in serial clinical specimens. Moreover, SARS-CoV-2 RNA was also detected in a mouth rinse, urine, and tear samples. This study was approved by the Medical Ethical Committee of The First Affiliated Hospital of Guangzhou Medical University (approval No. 2020-77).Results: SARS-CoV-2 mainly existed in sputum, nasal and throat swabs, and feces samples. Virus latency was longer in sputum and feces samples than in nasopharyngeal samples. IgG antibody response in respiratory samples was related to disease severity. Although droplets and aerosols are the major transmission routes for COVID-19, covert routes of transmission from asymptomatic patients, contaminated surfaces, and wastewater are also of interest.Conclusion: Our findings provide a solid foundation for developing prophylactic measures against SARS-CoV-2.
