Background Combined emphysema and pulmonary fibrosis,including idiopathic pulmonary fibrosis （IPF）,is a distinct disorder described with upper-lobe emphysema and lower-lobe fibrosis on chest computed tomography.Smoking appears to be the predominant risk factor for this disorder.We aimed to compare clinical features,smoking history,physiological and radiological findings between IPF with and without emphysema.Methods A sample of 125 IPF patients over a period of 48 months were evaluated.High resolution CT scans were reviewed blinded to clinical data.The IPF patients with or without emphysema were classified accordingly.Results The prevalence of emphysema in this IPF sample was 70/125.IPF with emphysema was significantly associated with smoking status （OR 63; 95％ CI 4.4 to 915; P=0.002） and smoking pack year （OR 1.1; 95％ CI 1.05 to 1.13; P=-0.000）.The patients with IPF and emphysema had a higher decrease in carbon monoxide diffusing capacity adjusted for alveolar volume （（58±19）％ pred vs.（66：±：21）％ pred; P=-0.021） and a higher prevalence of pulmonary hypertension （24/70 vs.7/55; P=0.006）.The two groups of patients had similar forced and residual volumes.No significant differences were found in cell differentials of bronchoalveolar lavage or the scores of fibrosis on chest CT.Survival of the patients with emphysema was significantly less than that of patients with IPF alone.Conclusions Cigarette smoking induces IPF combined with emphysema.Emphysema further impairs physiological function and increases the prevalence of pulmonary hypertension that leads to poor prognosis.The inclusion of the patients with combined pulmonary fibrosis and emphysema in IPF clinical trials may lead to under evaluation of the effect of treatment in patients.

为定量评价大兴区地下水开采强度,识别引起地下水开采强度增加的主要因素,基于集对分析构建了地下水开采强度评价模型,对研究区2006—2016年地下水开采强度进行了动态评价,并利用减法集对势识别了影响地下水开采强度的主要因子。结果表明:研究区2006—2016年地下水平均开采强度为中等强度,且有逐步恶化的趋势;2010年、2013年地下水开采强度平均联系数的减法集对势分别处于偏反势、反势,是地下水开采强度重点调控的年份;2010年有3个指标联系数的减法集对势处于偏反势或反势,分别为工业产值、第三产业产值和水浇地面积,涉及乡镇分别为3个、3个、5个,2013年有5个指标联系数的减法集对势处于偏反势或反势,分别为水浇地面积、菜田面积、设施农业面积、果园面积、机井数,涉及乡镇分别为5个、3个、7个、1个、4个。

Background:The prognosis of patients with small cell lung cancer(SCLC)and brain metastases(BM)was poor.This study aimed to explore the efficacy and safety of anlotinib as third-line or above treatment in SCLC with BM.Methods:This was a subgroup analysis of the ALTER1202 trial,which was a randomized,placebo-controlled trial aimed to evaluate the role of anlotinib as third-line treatment or above in patients with SCLC.This study included patients with BM at baseline.The efficacy and safety outcomes included progression-free survival(PFS),overall survival(OS),central nervous system(CNS),objective response rate(ORR),CNS disease control rate(DCR),time to CNS progression,and adverse events(AEs).Results:Twenty-one and nine patients with BM were included in the anlotinib and placebo groups,respectively.The median PFS and OS were 3.8 months(95%confidence interval[CI]:1.8-6.1)and 6.1 months(95%CI:4.1-8.0)in the anlotinib group.Anlotinib was associated with a significant improvement in PFS(hazard ratio[HR]=0.15,95%CI:0.04-0.51,p=0.0005)and OS(HR=0.26,95%CI:0.09-0.73,p=0.0061)than placebo.Anlotinib significantly prolonged the time to CNS progression(p<0.0001).The anlotinib group had a higher CNS DCR than placebo(95.2%vs.22.2%,p=0.0001).The most common grade 3 or higher AEs were increased lipase(19.0%),hypertension(14.3%),and hyponatremia(14.3%)in the anlotinib group.Conclusions:Anlotinib proved to have potential CNS activity and a manageable toxicity profile in patients with SCLC and BM,significantly delaying CNS progression.

Implantable cervical vagus nerve stimulation(i VNS) is a representative and promising neuromodulation.However,the invasive nature restricts its application.Traditional auricular acupuncture treatment has a long history.The auricular branch of the vagus nerve(ABVN) is a branch on the surface of the ear.Some studies demonstrates that transcutaneous auricular vagus nerve stimulation(ta VNS) would achieve similar effects as i VNS.Ta VNS and i VNS share a common anatomical basis and acting mechanism.In this article,we made a comparison between i VNS and ta VNS in indications and efficacy.The recent studies have revealed similar clinical efficacy of ta VNS,ta VNS would expand the indication of i VNS.Highquality clinical evidences are needed before ta VNS become be an alternative of i VNS.

Background:The expression of pyruvate kinase muscle 2(PKM2)is augmented in macrophages of patients with atherosclerotic coronary artery disease.The role of PKM2 in atherosclerosis is to be determined.Methods:Global and myeloid cell-specific PKM2 knock-in mice with ApoE^(-/-)background(ApoE^(-/-),PKM2^(KI/KI)and Lyz2-cre,ApoE^(-/-),and PKM2^(flox/flox))were produced to evaluate the clinical significance of PKM2 in atherosclerosis development.Wild-type and PKM2 knock-in macrophages were isolated to assess the function of PKM2 in macrophage phagocytosis.Atherosclerotic mice were treated with PKM2 inhibitor shikonin(SKN)to evaluate the therapeutic potential of PKM2 suppression in atherosclerosis.Results:Oxidized low-density lipoprotein(oxLDL)upregulated PKM2 in macrophages.PKM2 in return promoted the uptake of oxLDL by macrophages.Overexpressed PKM2 accelerated atherosclerosis in mice.SKN blocked the progress of mouse atherosclerosis.Conclusions:PKM2 accelerates macrophage phagocytosis and atherosclerosis.Targeting PKM2 is a potential therapy for atherosclerosis.

Flowering time and stem growth habit determine inflorescence architecture in soybean, which in turn influences seed yield. Dt1, a homolog of Arabidopsis TERMINAL FLOWER 1(TFL1), is a major controller of stem growth habit, but its underlying molecular mechanisms remain unclear.Here, we demonstrate that Dt1 affects node number and plant height, as well as flowering time,in soybean under long-day conditions. The b ZIP transcription factor FDc1 physically interacts with Dt1, and the FDc1-Dt1 complex directly represses the expression of APETALA1(AP1). We propose that FT5 a inhibits Dt1 activity via a competitive interaction with FDc1 and directly upregulates AP1. Moreover, AP1 represses Dt1 expression by directly binding to the Dt1 promoter, suggesting that AP1 and Dt1 form a suppressive regulatory feedback loop to determine the fate of the shoot apical meristem. These findings provide novel insights into the roles of Dt1 and FT5 a in controlling the stem growth habit and flowering time in soybean, which determine the adaptability and grain yield of this important crop.

Flowering time and plant height are key agronomic traits that directly affect soybean(Glycine max)yield.APETALA1(AP1)functions as a class A gene in the ABCE model for floral organ development,helping to specify carpel,stamen,petal,and sepal identities.There are four AP1 homologs in soybean,all of which are mainly expressed in the shoot apex.Here,we used clustered regularly interspaced short palindromic repeats(CRISPR)–CRISPR-associated protein 9 technology to generate a homozygous quadruple mutant,gmap1,with loss-of-function mutations in all four GmAP1 genes.Under short-day(SD)conditions,the gmap1 quadruple mutant exhibited delayed flowering,changes in flower morphology,and increased node number and internode length,resulting in plants that were taller than the wild type.Conversely,overexpression of GmAP1a resulted in early flowering and reduced plant height compared to the wild type under SD conditions.The gmap1 mutant and the overexpression lines also exhibited altered expression of several genes related to flowering and gibberellic acid metabolism,thereby providing insight into the role of GmAP1 in the regulatory networks controlling flowering time and plant height in soybean.Increased node number is the trait with the most promise for enhancing soybean pod number and grain yield.Therefore,the mutant alleles of the four AP1 homologs described here will be invaluable for molecular breeding of improved soybean yield.

为减少人类活动对地下水环境的影响,实现地下水资源可持续利用,需从时空尺度上对地下水开采强度进行动态评价。基于集对分析构建了地下水开采强度时空动态评价模型,并基于二阶偏联系数挖掘了同一度、差异度、对立度的动态演化过程,识别了影响地下水开采强度等级的主要因素。结果表明:2006-2016年,北京市大兴区地下水开采强度趋于增大,空间分布上中部地区>南部地区>北部地区;人口数、工业产值、第三产业产值对地下水开采强度等级影响的时空分布特征为北部地区>中部地区>南部地区。农业种植面积、机井数对地下水开采强度等级影响的时空分布特征为南部地区>中部地区>北部地区。依据二阶偏联系数识别结果,人口数、工业产值、第三产业产值对地下水开采强度的影响程度在增强,农业种植面积、机井数对地下水开采强度的影响程度在减弱。本文构建的地下水开采强度时空动态评价模型能够较好地反映研究区的地下水开采强度等级及动态演化过程,且计算简便,有一定的应用推广价值。

根据风电场并网要求,同时计及双馈风机（double-fed induction generator,DFIG）转子侧变流器最大输出电压和电流容量的限制,讨论DFIG的低电压穿越控制策略。在此基础上,分析DFIG故障电流的特性,进而考察90°接线方式方向元件在双馈式风电场汇集线路中的适用性。研究结果表明,非故障相方向元件在反方向两相短路时不能正确闭锁,造成方向性电流保护误动。最后,利用数模混合仿真试验和现场录波数据对方向元件进行测试,验证该文提出结论的准确性。
