Backgrounds and Aims: Several epidemiological investigations revealed that the prevalence of Type 2 Diabetes Mellitus (T2DM) has been increasing in the adult population of Bangladesh. But the prevalence of diabetes among the children and adolescents in Bangladesh has not been reported. This study addressed the prevalence of diabetes among the younger people in Bangladesh. Subjects and Methods: We investigated school children of age group 10 - 18 years in rural, suburban and urban communities. Investigations included socio-demographic information, height, weight, mid-upper-arm circumference (MUAC), blood pressure (SBP, DBP) and fasting plasma glucose (FPG). We used WHO criteria (1999) for impaired fasting glucose (IFG) and diabetes mellitus (T2DM). Results: A total of 2152 students (boys/girls: 1064/1088) volunteered the study. Their mean (SD) age was 13.3 (2.0) y, BMI was 18.5 (3.1) and MUAC was 21.2 (3.4) cm. The mean (SD) of FPG was 4.6 (0.87) mmol/l. The prevalence of IFG (95% CI) was 3.4% (2.63 - 4.17) and T2DM was 1.8% (1.23 - 2.37). BMI showed no association with FPG in either sex. In assessing risk for hyperglycemia (FPG > = 5.6mmol/l), logistic regression showed [odds ratio (OR) with 95% CI] that compared with lower age (16 y) higher age had excess risk (OR 5.2, 2.92 - 9.23). Compared with the rural the urban children had higher risk (OR 14.7, 6.41 - 33.78). Higher family income was also found to have higher risk (BDT 8000: OR 2.03, 1.30 - 3.18);whereas, higher BMI and MUAC were proved to be not significant. Conclusions: The prevalence of IFG and T2DM in Bangladeshi children and adolescents appears to be high. The urban children from the higher family income are the most vulnerable for developing diabetes, obesity and hypertension. A prospective cohort including other variables like physical activities, waist-girth, insulin or macro-/micronutrients may explain which risk factors are significant for developing diabetes among children and adolescents in Bangla

Gait Based Human Recognition with Various Classifiers Using Exhaustive Angle Calculations in Model Free Approach

Human Gait recognition is emerging as a supportive biometric technique in recent years that identifies the people through the way they walk. The gait recognition in model free approaches faces the challenges like speed variation, cloth variation, illumination changes and view angle variations which result in the reduced recognition rate. The proposed algorithm selected the exhaustive angles from head to toe of a person, and also height and width of the same subject. The experiments were conducted using silhouettes with view angle variation, and cloth variation. The recognition rate is improved to the extent of 91% using Support vector machine classifier. The proposed method is evaluated using CASIA Gait Dataset B (The institute of Automation, ChineseAcademy of Sciences), China. Experimental results demonstrate that the proposed technique shows promising results using state of the art classifiers.

Genotype Environment Interaction (G ×E) of Heat Tolerant Wheat Genotypes over Locations and Years

Twenty heat tolerant wheat genotypes were evaluated in three heat stress environments of Bangladesh such as Wheat Research Center (WRC), Nashipur, Dinajpur, Regional Wheat Research Center, BARI, Gazipur, Regional Wheat Research Center, RARS, Jashore in 2015-2016. The experiments were conducted in RCBD with three replications in 2015-2016. The stability and response to change of location and sowing time of the genotypes were assessed through AMMI model and regression coefficient analysis on seven characters, viz. days to heading, days to maturity, plant height (cm), spikes m-2, grains spike-1, 1000-grain weight (g) and yield (kg·ha-1). Among the linear interactions, sowing time alone exerted the maximum sum of square (335,388,000.00**) and among the non-linear interactions, location × year paid the highest effect (7,676,490.00**), which was followed by year × location × sowing time interaction (3,956,500.00**). The highest grain yield (4475.3 kg·ha-1) was obtained from the genotype G13, followed by the genotypes G17 (4460.8 kg·ha-1) and G19 (4404.7 kg·ha-1). The genotype G8 had the smallest interaction effect, while the genotype G14 followed by G10 responded vigorously to amelioration. The genotypes G13, G18 and G19 showed averagely high yield and showed stability over the environmental modifications. The other two genotypes G20 and G12 exhibited great reduction of yield potential under unfavorable environment. The environment 121 proceeded by J121 and G131 appeared as favorable environments. The genotypes G13 and G19 produced high yield (kg·ha-1) in the favorable environments. The genotypes G16 and G18 high IPCa1 scores, indicated sensitive to mega-environmental changes. The best genotype with respect to environment was genotype G13. Genotypes G17 and G19 were best for site E1;genotypes G14, G16 and G18 were best for site E2.

The Effects of Clamp and Shunt Usage on Vascular Endothelium in Coronary Artery Bypass Grafting Surgery

In this review we aimed to present the effects of clamp and shunt usage on vascular endothelium in coronary artery surgery under the light of current literature data. Each surgical team or even each surgeon uses different methods in order to create bloodless surgical site during beating heart surgical procedures. Every method has its own advantages and disadvantages. There is no formed consensus regarding this issue. Surgeons still want to operate on bloodless surgical fields. But, they have concerns about continuity of blood flow and vascular endothelial injury during off-pump coronary surgery. In order to achieve a consensus statement about this issue, multicentric larger studies are needed.

Diabetes and Hypertension in a Santhal Tribe in Bangladesh: A Population Based Study

BACKGROUND: Santhal (Santals) tribe is one of the oldest and largest aboriginal pre Aryan populations in India and Bangladesh. There was no published report on the prevalence of diabetes and hypertension. OBJECTIVE: To determine the prevalence of type 2 diabetes mellitus (T2DM) and hypertension in a Santhal tribe of Bangladesh. RESEARCH DESIGN AND METHODS: Eight villages inhabited largely by Santhal tribe were purposively selected. All Santhals aged 20 years or more were considered eligible and enlisted for the study. Investigations included socio-demographic information (age, sex, education, income), clinical history (general illness), anthropometry (height, weight, waist-girth, hip-girth) and blood pressure. Body mass index (BMI) and waist-to-hip ration (WHR) were calculated. Blood samples were collected for fasting plasma glucose (FPG), total cholesterol (Chol), triglycerides (TG), urea and creatinine. RESULTS: Thirteen hundred eligible Santhals were enlisted. Of them, 1049 (80.7%) participated in the study. The male and female participants were 40% and 60%, respectively. The prevalence of T2DM was 0.6% and hyperglycemia (FPG > 5.5 mmol/l) was 10.0%. The prevalence of systolic hypertension (sHTN) was 24.4% and diastolic hypertension (dHTN) was 24.6%. Compared with the males the females had significantly higher prevalence of sHTN (OR, 2.20 with 95% CI, 1.62 - 3.02) and dHTN (OR, 1.81 with CI, 1.34 - 2.0);whereas, the prevalence of T2DM and IFG did not differ. Regarding obesity 45% of the participants had BMI 23.1. Logistic regression estimated that the increasing age, female sex, higher FPG (>5.5 mmol/l) and higher Chol (>160 mg/dl) had independent risk for sHTN and dHTN. CONCLUSIONS: The Santhals had less risk for diabetes but increased risk for hypertension. The Santhal females had excess risk of hypertension. Advancing age, female sex, hyperglycemia and hypercholesterolemia were found to have significant risk for hypertension. Obesity had no effect on diabetes

Intercropping Empower Reduces Insect Pests and Increases Biodiversity in Agro-Ecosystem

Currently insect pest management solely depends on chemical pesticide that continuously affects on environment, biodiversity, animal as well as human health. Outbreak of secondary insect pest is also the cost of pesticide use in field leading crop more vulnerable to more pests. These negative impacts of pesticides have provoked growing interest in the adoption of multi-function agricultural biodiversity that promote pest management, creating interesting challenge for traditional approaches to regulatory compliance. To address multi-function agricultural practice, we tested several intercropping systems with mustard and their effect on pest management. Our results revealed that intercropping systems mustard with onion, garlic, radhuni and coriander significantly reduced pest population over sole crop. However, intercropping mustard with wheat and gram increased pest population in mustard field. This result indicated that all crops are not suitable for intercropping system. Among the tested intercropping systems, mustard with onion and coriander significantly reduced branch and flower infestation and increased pod formation per plant. These four intercropping systems did not significantly affect on honeybee pollinator which are crucial for mustard crop yield. A significant linear relationship was also found between honeybee population and pod formation. Our results indicate that suitable intercropping system can be a potential multi-functional agricultural practice for pest management in mustard crop.

目的探讨肿瘤标记物SCC、NSE在感染性肺部疾病临床诊断中的价值。方法选择我院2015年4月至2017年4月收治的40例肺癌患者作为观察组,同期40例肺部感染性良性疾病住院患者作为对照组,检测并比较两组患者的血清SCC和NSE水平。结果 SCC在非小细胞肺癌患者、小细胞肺癌患者和对照组患者的阳性率分别为86.67%、33.33%、0.00%,NSE在非小细胞肺癌、小细胞肺癌和对照组患者中的阳性率分别0.00%、50.00%、0.00%。观察组和对照组的SCC水平分别为(6.06±10.20)ng/m L、(1.12±1.04)ng/m L,NSE水平分别为(3.43±2.78)ng/m L、(1.80±1.51)ng/m L,组间差异有统计学意义(均P<0.01)。结论肿瘤标记物SCC、NSE在肺部肿瘤患者的诊断、鉴别诊断和治疗中有较高的应用价值。

目的比较两种最常用的抗癫痫药物——苯巴比妥和卡马西平在孟加拉儿童癫痫患者中应用时的行为学副作用。设计前瞻性随机对照单中心研究。地点孟加拉 Dhaka 儿童专科医院。对象 108例2～15岁全身性强直-阵挛发作(51例)或局限性继发全身性发作(57例)的患儿。主要结局评估指标癫痫控制和行为学副作用。结果 91例患儿随访12个月。6例需改用其他抗癫痫药物。比较了85例患儿的药物副作用。药物治疗和随访1年,在随访中的后1/4时间,71例患儿未再癫痫发作。随机分组后,苯巴比妥组32例102天、卡马西平组39例74天未再癫痫发作。10例患儿行为学问题增多,其中4例不能接受(苯巴比妥组1例,卡马西平组3例)。但独立 t 检验显示,两组试验药物间没有显著性差异。结论在资源缺乏的国家,苯巴比妥不引起癫痫儿童过多的行为学副作用。临床试验注册号 NCT00381537[***]。

Angiotensin Ⅱ stimulates expression of transcription factors c-Jun and c-Fos in cyclosporine induced human gingival fibroblasts

The present study demonstrates that the expression of c-Jun and c-Fos are elevated in gingival fibroblast cells treated with angiotensin Ⅱ and *** healthy human gingival tissues were collected and gingival fibroblasts were isolated and *** used RT-PCR and Western blot analysis to identify the expression of c-Jun and c-Fos in cyclosporine and angiotensin II treated human gingival fibroblast *** found that angiotensin Ⅱ in combination with cyclosporine induces c-Jun and c-Fos expressions significantly;however,the angiotensin Ⅱ antagonist losartan inhibits the expression of c-Jun and c-Fos(p<0.01).The data suggest that angiotensin Ⅱ in combination with cyclosporine modulates the expression of c-Jun and c-Fos in human gingival fibroblast cells.

目的观察焦氏头针治疗CO中毒迟发性脑病（DEACMP）患者认知功能恢复的临床疗效。方法将60例DEACMP患者采用前瞻性研究方法 ,按临床就诊先后顺序随机分为两组,每组30例。A组（治疗组）：焦氏头针疗法加常规治疗。B组（对照组）：单纯常规治疗组。两组年龄均在19~60岁,分别于治疗前及治疗3个疗程后采用临床疗效标准、Barthel指数（BI）及成人简易智力测验（MMSE）观察指标,对两组患者疗效进行评定。结果治疗后治疗组总有效率为90.0%,对照组总有效率为66.7%,治疗组疗效优于对照组（χ2=8.043,P〈0.05）。BI及MMSE指数均有提高,BI与对照组比较（P〈0.05）,MMSE与对照组比较（P〈0.05）,差异有统计学意义。结论焦氏头针疗法加常规治疗DEACMP优于单纯常规治疗。
