Clinical presentations of gastric small gastrointestinal stromal tumors mimics functional dyspepsia symptoms

AIM: To explore whether clinical presentations of gastric small gastrointestinal tumors(GISTs) mimics gastrointestinal dyspepsia symptoms.METHODS: The endosonographic data of 167 patients who underwent endoscopic submucosal dissection at the Tianjin Medical University General Hospital, China between 2009 and 2011 were analyzed. GISTs and leiomyomas had a similar intragastric distribution and similar locations within the gastric wall. Therefore, patients with GISTs were chosen as the study group and those with leiomyomas were chosen as the control group. Dyspepsia symptom questionnaires were used to investigate and compare the gastrointestinal symptoms of patients with GISTs and those with gastric leiomyomas before and after endoscopic submucosal dissection(ESD). The questionnaires evaluated symptoms such as epigastric pain, heartburn, regurgitation, epigastric discomfort, nausea and vomiting, abdominal bloating, and eructation. Symptoms were assessed using a four-point scoring scale.RESULTS: GISTs were the most common gastric submucosal lesion(67 cases, 40.12%), followed by leiomyomas(38 cases, 22.75%). Both groups were similar in terms of gender distribution(P = 0.49), intragastric location(P = 0.525), and originating layer within the gastric wall(P = 0.449), but leiomyomas were more commonly found in the proximal fundus(P < 0.05). Overall, 94.2% of the patients with small GISTs and 93.5% of those with gastric leiomyomas experienced some dyspepsia; however, total symptom scores were significantly lower in the GIST group than in the leiomyoma group(1.34 ± 1.27 vs 2.20 ± 1.70, P < 0.05). Each component of the symptom score demonstrated a statistically significant improvement in the GIST patients after ESD(P < 0.05), including epigastric pain(0.80 ± 0.90 vs 0.13 ± 0.46), heartburn(0.63 ± 1.08 vs 0.13 ± 0.41), regurgitation(0.55 ± 0.87 vs 0.22 ± 0.57), epigastric discomfort(0.70 ± 0.98 vs 0.32 ± 0.47), nausea and vomiting(0.27 ± 0.62 vs 0.05 ± 0.21), abdominal bloating(0.70 ± 0.90 vs 0.27 ± 0.49), and eructation(0.3

Fecal microbiota transplantation broadening its application beyond intestinal disorders

Intestinal dysbiosis is now known to be a complication in a myriad of diseases.Fecal microbiota transplantation(FMT),as a microbiota-target therapy,is arguably very effective for curing Clostridium difficile infection and has good outcomes in other intestinal diseases.New insights have raised an interest in FMT for the management of extra-intestinal disorders associated with gut microbiota.This review shows that it is an exciting time in the burgeoning science of FMT application in previously unexpected areas,including metabolic diseases,neuropsychiatric disorders,autoimmune diseases,allergic disorders,and tumors.A randomized controlled trial was conducted on FMT in metabolic syndrome by infusing microbiota from lean donors or from self-collected feces,with the resultant findings showing that the lean donor feces group displayed increased insulin sensitivity,along with increased levels of butyrate-producing intestinal microbiota.Case reports of FMT have also shown favorable outcomes in Parkinson's disease,multiple sclerosis,myoclonus dystonia,chronic fatigue syndrome,and idiopathic thrombocytopenic purpura.FMT is a promising approach in the manipulation of the intestinal microbiota and has potential applications in a variety of extra-intestinal conditions associated with intestinal dysbiosis.

基于深度学习的方法在关系提取中通常只侧重于细粒度文本单元的表示,导致学习文本特征不足.提出了一种融合知识增强的ERNIE模型与神经网络相结合的方式去进行关系提取.该方法分为两个部分:首先通过知识增强来对文本向量化,具体是将细粒度文本单元与粗粒度文本单元进行加权平均的操作使其达到知识增强的效果,再将该单元进行预测后的结果进行RTD判断有无替代词产生.最后将文本特征向量输入到BiLSTM网络中,使其得到词的上下文语义信息,并进行句子序列打分,选择分数最高的即可.实验结果表明,该方法在进行关系提取时,得到准确率为95%,精确率为91%,召回率为92%,f1-score为92%,与已有的方法进行对比,均提升了5%以上,因此提出的方法效果明显.

Regulation of the serotonin transporter in the pathogenesis of irritable bowel syndrome

Serotonin(5-HT) and the serotonin transporter(SERT) have earned a tremendous amount of attention regarding the pathogenesis of irritable bowel syndrome(IBS). Considering that enteric 5-HT is responsible for the secretion, motility and perception of the bowel, the involvement of altered enteric 5-HT metabolism in the pathogenesis of IBS has been elucidated. Higher 5-HT availability is commonly associated with depressed SERT mR NA in patients with IBS compared with healthy controls. The expression difference of SERT between IBS patients and healthy controls might suggest that SERT plays an essential role in IBS pathogenesis, and SERT was expected to be a novel therapeutic target for IBS. Progress in this area has begun to illuminate the complex regulatory mechanisms of SERT in the etiology of IBS. In this article, current insights regarding the regulation of SERT in IBS are provided, including aspects of SERT gene polymorphisms, microR NAs, immunity and inflammation, gut microbiota, growth factors, among others. Potential SERT-directed therapies for IBS are also described. The potential regulators of SERT are of clinical importance and are important for better understanding IBS pathophysiology and therapeutic strategies.

Pterygium is a prevalent ocular disease that can cause discomfort and vision impairment.Early and accurate diagnosis is essential for effective management.Recently,artificial intelligence(AI)has shown promising potential in assisting clinicians with pterygium diagnosis.This paper provides an overview of AI-assisted pterygium diagnosis,including the AI techniques used such as machine learning,deep learning,and computer vision.Furthermore,recent studies that have evaluated the diagnostic performance of AI-based systems for pterygium detection,classification and segmentation were summarized.The advantages and limitations of AI-assisted pterygium diagnosis and discuss potential future developments in this field were also analyzed.The review aims to provide insights into the current state-of-the-art of AI and its potential applications in pterygium diagnosis,which may facilitate the development of more efficient and accurate diagnostic tools for this common ocular disease.

Hemostatic effect of topical hemocoagulase spray in digestive endoscopy

AIM: To evaluate the hemostatic effect of topical hemocoagulase spray in digestive endoscopy.METHODS: Eighty-nine patients who developed oozing bleeding during endoscopic treatment from September 2014 to October 2014 at Center for Digestive Endoscopy, Tianjin Medical University General Hospital were randomly divided into either a study group(n = 39) or a control group(n = 50). The study group was given topical hemocoagulase spray intraoperatively, while the control group was given traditional 8% norepinephrine spray. Hemostatic efficacy was compared between the two groups. Bleeding site, wound cleanliness and perforation were recorded, and the rates of perforation and late bleeding were compared.RESULTS: Successful hemostasis was achieved in 39(100%) patients of the study group and in 47(94.0%) patients of the control group, and there was no significant difference in the rate of successful hemostasis between the two groups. Compared with the control group, after topical hemocoagulase spray in the study group, the surgical field was clearer, the bleeding site was more easily identified, and the wound was cleaner. There was no significant difference in the rate of perforation between the study and control groups(16.7% vs 35.0%, P = 0.477), but the rates oflate bleeding(0% vs 15.8%, P = 0.048) and overall complications(P = 0.032) were significantly lower in the study group.CONCLUSION: Topical hemocoagulase spray has a definite hemostatic effect for oozing bleeding in digestive endoscopy, and this method is convenient, safe, and reliable. It is expected to become a new method for endoscopic hemostasis.

LIN28A attenuates high glucose-induced retinal pigmented epithelium injury through activating SIRT1-dependent autophagy

AIM:To evaluate the effects of LIN28A(human)on high glucose-induced retinal pigmented epithelium(RPE)cell injury and its possible mechanism.METHODS:Diabetic retinopathy model was generated following 48h of exposure to 30 mmol/L high glucose(HG)in ARPE-19 cells.Quantitative real-time polymerase chain reaction(qRT-PCR)and Western blot tested the expression of the corresponding genes and proteins.Cell viability as well as apoptosis was determined through cell counting kit-8(CCK-8)and flow cytometry assays.Immunofluorescence assay was adopted to evaluate autophagy activity.Caspase 3 activity,oxidative stress markers,and cytokines were appraised adopting their commercial kits,respectively.Finally,ARPE-19 cells were preincubated with EX527,a Sirtuin 1(SIRT1)inhibitor,prior to HG stimulation to validate the regulatory mechanism.RESULTS:LIN28A was downregulated in HG-challenged ARPE-19 cells.LIN28A overexpression greatly inhibited HGinduced ARPE-19 cell viability loss,apoptosis,oxidative damage as well as inflammatory response.Meanwhile,the repressed autophagy and SIRT1 in ARPE-19 cells challenged with HG were elevated after LIN28A overexpression.In addition,treatment of EX527 greatly inhibited the activated autophagy following LIN28A overexpression and partly abolished the protective role of LIN28A against HG-elicited apoptosis,oxidative damage as well as inflammation in ARPE-19 cells.CONCLUSION:LIN28A exerts a protective role against HG-elicited RPE oxidative damage,inflammation,as well as apoptosis via regulating SIRT1/autophagy.

目的研究生长素(IAA)、细胞分裂素(6-BA)、赤霉素(GA3)、脱落酸(ABA)和乙烯(ET)5种植物激素对白花前胡种子萌发的影响。方法以宁国产并在室温放置12个月的白花前胡种子为材料,把5种不同的植物激素均配制成4个梯度浓度(1×10^(-1)、1×10^(-2)、1×10^(-3)、1×10^(-4)mg·mL^(-1)),对白花前胡种子进行浸种处理。通过培养皿萌发试验,分析种子的发芽率、根长和鲜重的变化情况。结果在4个梯度范围内,6-BA对白花前胡种子的生长状态整体起抑制作用。把1×10^(-1)mg·mL^(-1)和1×10^(-2)mg·mL^(-1)设置为高浓度,1×10^(-3)mg·mL^(-1)和1×10^(-4)mg·mL^(-1)设置为低浓度。其中6-BA无论是高浓度还是低浓度均对白花前胡种子的生长状态整体起抑制作用。而GA3和ET在高浓度(1×10^(-1)mg·mL^(-1)和 1×10^(-2)mg·mL^(-1))及ABA和IAA在低浓度(1×10^(-3)mg·mL^(-1)和1×10^(-4)mg·mL^(-1))下均对白花前胡种子的萌发率、根长和鲜重都表达出明显的促进作用。结论植物激素对白花前胡种子的生长发育均有不同程度的影响,其中ET在1×10^(-2)mg·mL^(-1)时,对白花前胡种子的生长发育影响最为显著,白花前胡种子的萌发率,最大根长和最大鲜重均高于其他组。

基于卫星遥感的透明度(Secchi depth,SD)反演方法已经在大洋、近海、湖泊等不同水体开展了广泛应用。富营养化河口港湾等水体具有水域面积小且受陆地影响大等特点,并且在此类水体开展环境监测具有时效性要求。受时间空间分辨率以及云层覆盖、大气校正等影响,基于卫星遥感方法在河口港湾等小水域进行透明度反演会受到一定的应用限制,因此在此类特征水体建立一种高效便捷的透明度反演方法作为卫星遥感方法的有益补充就十分迫切。本研究尝试通过无人机和智能手机应用程序HydroColor APP搭载的普通光学相机构建基于红(red,R)绿(green,G)蓝(blue,B)三波段的象山港透明度反演方法。结果表明,无人机和HydroColor的红蓝波段比值(R/B)和红绿波段比值(R/G)与透明度具有显著负相关,相关系数R为–0.88~–0.93(n=16,P<0.001)。根据相关性分析结果构建透明度反演模型并基于独立数据库对模型进行精度评估。结果显示,(1)指数反演模型要优于线性经验模型,(2)基于R/G反演模型要优于R/B模型,(3)HydroColor反演模型要优于无人机反演模型。通过以上结果分别构建基于无人机DJI-R/G和HydroColor-R/G最优透明度指数反演模型。DJI-R/G模型平均相对误差和均方根误差为29%和0.3 m,HydroColor-R/G模型为21.9%和0.27 m。以上结果表明,通过无人机和手机获取RGB信息均可用于对象山港透明度进行反演。该方法的建立为快速便捷开展河流港湾等水体的水质监测和赤潮防控提供了新的技术支持。

Jasminum sambac(jasmine flower),a world-renowned plant appreciated for its exceptional flower fragrance,is of cultural and economic importance.However,the genetic basis of its fragrance is largely unknown.Here,we present the first de novo genome assembly of J.sambac with 550.12 Mb(scaffold N50=40.10 Mb)assembled into 13 pseudochromosomes.Terpene synthase(TPS)genes associated with flower fragrance are considerably amplified in the form of gene clusters through tandem duplications in the genome.Gene clusters within the salicylic acid/benzoic acid/theobromine(SABATH)and benzylalcohol O-acetyltransferase/anthocyanin O-hydroxycinnamoyltransferases/anthranilate N-hydroxycinnamoyl/benzoyltransferase/deacetylvindoline 4-O-acetyltransferase(BAHD)superfamilies were identified to be related to the biosynthesis of phenylpropanoid/benzenoid compounds.Several key genes involved in jasmonate biosynthesis were duplicated,causing an increase in copy numbers.In addition,multi-omics analyses identified various aromatic compounds and many genes involved in fragrance biosynthesis pathways.Furthermore,the roles of JsTPS3 in b-ocimene biosynthesis,as well as JsAOC1 and JsAOS in jasmonic acid biosynthesis,were functionally validated.The genome assembled in this study for J.sambac offers a basic genetic resource for studying floral scent and jasmonate biosynthesis,and provides a foundation for functional genomic research and variety improvements in Jasminum.
