Morbidity analysis of left hepatic trisectionectomy for hepatobiliary disease and live donor

Background:Despite remarkable advances in surgical techniques and perioperative management,left hepatic trisectionectomy(LHT)remains a challenging procedure with a somewhat high postoperative morbidity rate compared with less-extensive *** study aimed to analyze the short-and long-term outcomes of LHT and identify factors associated with the postoperative morbidity of this technically demanding surgical ***:The medical records of 53 patients who underwent LHT between June 2005 and October 2019 at a single institution were retrospectively *** independent prognostic factor of postoperative morbidity was analyzed using the logistic regression ***:Hepatocellular carcinoma was the most common indication for surgery(n=21),followed by hilar cholangiocarcinoma(n=14),intrahepatic cholangiocarcinoma(n=10),and other pathologies(including colorectal liver metastasis,hepatolithiasis,gallbladder cancer,living donor,hemangioma,and multilocular biliary cyst;n=8).The rates of postoperative morbidities of Clavien-Dindo grade 3 or higher and 90-day mortality were 39.6% and 1.9%,*** 1-,3-,and 5-year overall survival rates were 81.1%,61.4%,and 44.6%,*** analysis revealed that preoperative jaundice[hazard ratio(HR)=6.15,95%confidence interval(CI):1.57-24.17,P=0.009]and operative time>420 min(HR=4.66,95%CI:1.27–17.17,P=0.021)were independent predictors of postoperative ***:The in-hospital mortality of LHT surgery can be minimalized by a reliable preoperative evaluation of liver function and selection of the dominant anatomic features of right posterior sector,active and appropriate preoperative management for obstructive cholangitis and compensatory hypertrophy of the future remnant posterior sector,and the experience of the surgeon.

Microbiota-based treatments in alcoholic liver disease

Gut microbiota plays a key role in the pathogenesis of alcoholic liver disease(ALD). Consumption of alcohol leads to increased gut permeability, small intestinal bacterial overgrowth, and enteric dysbiosis. These factors contribute to the increased translocation of microbial products to the liver via the portal tract. Subsequently, bacterial endotoxins such as lipopolysaccharide, in association with the Toll-like receptor 4 signaling pathway, induce a gamut of damaging immune responses in the hepatic milieu. Because of the close association between deleterious inflammation and ALD-induced microbiota imbalance, therapeutic approaches that seek to reestablish gut homeostasis should be considered in the treatment of alcoholic patients. To this end, a number of preliminary studies on probiotics have confirmed their effectiveness in suppressing proinflammatory cytokines and improving liver function in the context of ALD. In addition, there have been few studies linking the administration of prebiotics and antibiotics with reduction of alcoholinduced liver damage. Because these preliminary results are promising, large-scale randomized studies are warranted to elucidate the impact of these microbiotabased treatments on the gut flora and associated immune responses, in addition to exploring questions about optimal delivery. Finally, fecal microbiota transplant has been shown to be an effective method of modulating gut microbiota and deserve further investigation as a potential therapeutic option for ALD.

Effect of citrulline on post-exercise rating of perceived exertion,muscle soreness,and blood lactate levels:A systematic review and meta-analysis

Impacts of weaning age on dietary needs of whey permeate for pigs at 7 to 11 kg body weight

Background:Whey permeate is an effective lactose source for nursery pigs and the most benefits are obtained when pigs are at 7 to 11 kg *** weaning ages could cause different length of early-weaner phases until 7 kg BW and thus it would influence the dietary need of whey permeate during 7 to 11 kg BW of *** study aimed to evaluate if weaning ages would affect the dietary needs of whey permeate for optimum growth performance of pigs at 7 to 11 kg ***:A total of 1,632 pigs were weaned at d 21(d 21.2±1.3)or d 25(d 24.6±1.1)after *** pigs had a common early-weaner feeds until they reached 7 kg *** pigs reached 7 kg BW within a weaning age group,they were allotted in a randomized complete block design(2×4 factorial).Two factors were weaning age groups(21 and 25 d of age)and varying whey permeate levels(7.50%,11.25%,15.00%,and 18.75%).Data were analyzed using the GLM and NLIN procedures of SAS for slope-ratio and broken-line analyses to determine the growth response to whey permeate and optimal daily whey permeate intake for the growth of the pigs weaned at different ***:Pigs weaned at 21 d of age had a common diet for 11 d to reach 7 kg BW whereas pigs weaned at 25 d of age needed 2 *** G:F of pigs weaned at 25 d of age responded to increased daily whey permeate intake greater(P<0.05)than pigs weaned at 21 d of *** were obtained(P<0.05)at 88 and 60 g/d daily whey permeate intake or 17.0%and 14.4%of whey permeate for G:F of pigs weaned at 21 and 25 d of age,***:Pigs weaned at an older age with a short early-weaner phase had a greater growth response to whey permeate intake compared with pigs weaned at a younger age with a long early-weaner *** weaning ages affected dietary needs of whey permeate for optimum growth performance of pigs from 7 to 11 kg BW.

Development of tenofovir disoproxil fumarate resistance after complete viral suppression in a patient with treatment-na?ve chronic hepatitis B: A case report and review of the literature

Tenofovir disoproxil fumarate(TDF) is a potent nucleo-tide analogue that is recommended as first-line therapy for patients with chronic hepatitis B. The results of a longitudinal study of TDF treatment demonstrated no development of resistance. We observed one treatment-na?ve chronic hepatitis B(CHB) patient who developed TDF resistance after complete viral suppression during long-term TDF treatment. A 37-year-old HBe Ag-positive man received TDF 300 mg/d for 43 mo. The hepatitis B virus(HBV) DNA titer was 8 log_(10) copies/m L at baseline and became undetectable at 16 mo after treatment. However, the HBV DNA titer rebounded to 7.5 log_(10) copies/m L at 43 mo after treatment. We performed full sequencing to find mutation sites associated with virologic breakthrough. The results showed 9 mutation sites, most of which had not been well-known as mutation sites. We changed the therapy from tenofovir to entecavir with a regimen of 0.5 mg once daily. After 4 mo, the HBV DNA titer decreased to 267 copies/m L, and the liver enzyme levels were normalized.

Among the environmentally friendly marine energies,tidal current power plants require low cost because they do not need to construct a large *** power is particularly reliable energy source because the power generation capacity is predictable regardless of weather or *** materials or stainless steel have been used as materials of blades for current power ***,their strength and welding performance generated many problems in field *** alloys with excellent cavitation resistance and corrosion resistance were applied as blade materials to improve the durability of copper *** were coated with WC-27NiCr and WC-10Co4Cr using the high velocity oxy-fuel (HVOF) *** metal spray coating technology has the advantage in terms of the selection of materials for cost effectiveness and environmental effects of corrosion because the properties of the coat layer can be controlled *** with WC-27NiCr and WC-10Co4Cr by HVOF,WC-27NiCr shows better corrosion resistance *** reason for this seems to be that corrosion resistance improves and a stable passive film forms due to the effects of Ni and Cr.

Abscission is the process by which plants discard organs in response to environmental cues/stressors, or as part of their normal development. Abscission has been studied throughout the history of the plant sciences and in numerous species. Although long studied at the anatomical and physiological levels, abscission has only been elucidated at the molecular and genetic levels within the last two decades, primarily with the use of the model plant Arabidopsis thaliana. This has led to the discovery of numerous genes involved at all steps of abscission, including key pathways involving receptor-like protein kinases （RLKs）. This review covers the current knowledge of abscission research, highlighting the role of RLKs.

QKD system with fast active optical path length compensation

We develop a quantum key distribution (QKD) system with fast active optical path length compensation. A rapid and reliable active optical path length compensation scheme is proposed and applied to a plug-and-play QKD system. The system monitors changes in key rates and controls it is own operation automatically. The system achieves its optimal performance within three seconds of operation, which includes a sifted key rate of 5.5 kbps and a quantum bit error rate of less than 2% after an abrupt temperature variation along the 25 km quantum channel. The system also operates well over a 24 h period while completing more than 60 active optical path length compensations.

Analysis of expanded criteria to select candidates for active surveillance of low-risk prostate cancer

We aimed to analyze the value of each criterion for clinically insignificant prostate cancer （PCa） in the selection of men for active surveillance （AS） of low-risk PCa. We identified 532 men who were treated with radical prostatectomy from 2006 to 2013 who met 4 or all 5 of the criteria for clinically insignificant PCa （clinical stage 〈 T1, prostate specific antigen [PSA] density 0.15, biopsy Gleason score 〈 6, number of positive biopsy cores 〈 2, and no core with 〉 50% involvement） and analyzed their pathologic and biochemical outcomes. Patients who met all 5 criteria for clinically insignificant PCa were designated as group A （n -- 172）, and those who met 4 of 5 criteria were designated as group B （n = 360）. The association of each criterion with adverse pathologic features was assessed via logistic regression analyses. Comparison of group A and B and also logistic regression analyses showed that PSA density 0.15 ng ml-1 and high （〉7） biopsy Gleason score were associated with adverse pathologic features. Higher （〉 Tlc） clinical stage was not associated with any adverse pathologic features. Although 〈 3 positive cores were not associated with any adverse pathology, 〉4 positive cores were associated with higher risk of extracapsular extension. Among potential candidates for AS, PSA density ~ 0.15 ng ml-~ and biopsy Gleason score ~ 6 pose significantly higher risks of harboring more aggressive disease. The eligibility criteria for AS may be expanded to include men with clinical stage T2 tumor and 3 positive cores.
