Ba.kground: Cuta.eous leishma.ia.is (CL) usua.ly ha. a.well clinica. presenta.ion tha. ra.ely confused with other derma.ologica. disea.es, but when it presents with a.ypica. picture, it ma. be a.dia.nostic cha.lenge to derma.ologist. Objective: To record the ca.es of CL those ha.e very unusua. presenta.ions or sites in the Ira.i pa.ients tha. might be confused with other derma.oses. Pa.ient a.d methods: This is a.clinica. descriptive study tha. ha. been performed in the outpa.ient of Depa.tment of Derma.ology Ba.hda. Tea.hing Hospita., Ba.hda., Ira., during the period from Ja.ua.y 2015-Februa.y 2017, where fifty six pa.ients were enrolled in the study. The a.e of the pa.ients ra.ged from 1 - 67 yea.s old with media. is 30 yea.s, 32 ma.es a.d 24 fema.es with M:F ra.io is 1.3:1, the dia.nosis wa. confirmed by skin smea. a.d/or biopsy. Results: Fifty six ca.es of CL with a.ypica. presenta.ions a.d unusua. sites tha. ca.egorized a. follow: diffuse nodula. 4 ca.es, pa.onychia.like 1 ca.e, sa.sa.e-like CL 4 ca.es, lupus vulga.is like 2 ca.es, rosa.ea.like 1ca.e, squa.ous cell ca.cinoma.like 3 ca.es, kera.oa.a.thoma.like 1 ca.e, pyoderma.ga.grenosum-like 2 ca.es, erysipeloid like 1 ca.e, B-cell lymphoma.like 1 ca.e, grouping pa.tern 1 ca.e, pa.niculitis-like 2 ca.es, derma.itis like 2 ca.es, ecthyma.like 1 ca.es, verruca.like 1 ca.e, keloid like 1 ca.e, CL on the sca.p 1 ca.e, pa.mer CL 1 ca.e, CL on the eyelid 1 ca.e, CL on the ea. 1 ca.e, CL on the lips 3 ca.es, one genita. CL ca.e a.d twenty ca.es of sporotrichosis like sprea.. Conclusion: Unusua. presenta.ions of the CL with a.ypica. lesions should be well recognized a.d investiga.ed to confirm the dia.nosis so tha. not to be confused with other skin disea.es especia.ly in countries where cuta.eous leishma.ia.is is not endemic

Ba.kground: Zinc sulpha.e a. intra.esiona. a.d ora. thera.y wa. used a. a.successful thera.y in trea.ment of cuta.eous leishma.ia.is while 25% topica. podophyllin is now commonly used in the trea.ment of cuta.eous leishma.ia.is. Objective: To trea. a.ute cuta.eous leishma.ia.is using topica. zinc sulpha.e solution 25% to be compa.ed with topica. podophyllin solution 25%. Pa.ients a.d Methods: This is a.single, blind, interventiona., compa.a.ive study done in the Depa.tment of Derma.ology, Ba.hda. Tea.hing Hospita., Ba.hda., Ira. during the period from Ja.ua.y 2015 to June 2015. Forty pa.ients with a.tota. 88 lesions of a.ute cuta.eous leishma.ia.is were enrolled in this study;54 (61.36%) were dry lesions a.d 34 (38.6%) were ulcera.ive type. The dura.ion of lesions ra.ged from 2 - 11 (7.7 + 2.483) weeks. The size of the lesions ra.ged from 0.3 to 8 (2.13 ± 1.53) cm. There were 21 fema.es a.d 19 ma.es, fema.e to ma.e ra.io (1.1:1) a.d their a.es ra.ged from 1 to 60 (25.92 ± 16.59) yea.s. Dia.nosis wa. confirmed by tissue smea. a.d histopa.hology. Lesions were divided into two groups with ma.ching types a.d sizes, a.d ??scored a.cording to Sha.quie modified Leishma.ia.score to a.sess the objective response to the topica. or systemic thera.y a.d then during follow up responses were gra.ed into mild, modera.e, ma.ked response a.d complete clea.a.ce. Group a.trea.ed with topica. 25% podophyllin solution once weekly for a.ma.imum of 6 weeks, while group B wa. trea.ed with topica. 25% zinc sulpha.e solution twice da.ly for 6 weeks. Follow up wa. every 2 weeks during thera.y, a.d once monthly for 3 months a.ter thera.y. Results: a.tota. of 40 pa.ients with clinica. dia.nosis of a.ute cuta.eous leishma.ia.is were included in this study, with a.tota. of 88 lesions. Fifteen (37.50%) pa.ients ha. single lesion a.d 25 (62.50%) pa.ients ha. multiple lesions. Ba.kground: Cuta.eous leishma.ia.is (CL) is a. endemic disea.e in Ira., now is running in a. outbrea.s. Ma.y thera.ies ha.e been tried in trea.ment of the disea.e. Objective: Lora.thus europa.us (LE) is a.well-known medica. pla.t a.d ha. ma.y pha.ma.ologica. effects in ma.y in vitro studies. The a.m of the study is to eva.ua.e the effectiveness of 40% LE ointment a.d compa.ed it with topica. 25% podophyllin solution in trea.ment of a.ute CL. Pa.ients a.d Methods: Thirty five pa.ients with a.ute CL were enrolled in this single blinded, thera.eutic, compa.a.ive study, which wa. done in the Depa.tment of Derma.ology, Ba.hda. Tea.hing Hospita., Ba.hda., Ira. during Ja.ua.y 2015-Jully 2015. The tota. number of lesions were 86 (76 lesions trea.ed a.d ten lesions in a.covered a.ea.left without trea.ment a. control). Dura.ion of lesions ra.ged from 4 to 12 (7.37 ± 2.77) weeks. The size of lesions ra.ged from 0.5 - 7 (2.81 ± 1.76) cm. Dia.nosis wa. confirmed by biopsy a.d/or smea.s. Lesions were divided in to two groups a.d scored a. (mild, modera.e, ma.ked, complete cure) a.cording to a.modified Sha.quie’s leishma.ia.score to a.sess the objective response to the topica. or systemic thera.y. Group a.Thirty three (43.42%) lesions trea.ed with topica. 25% podophyllin solution once weekly for ma.imum 6 weeks. Group B: Forty three (56.58%) lesions trea.ed with 40% LE ointment once da.ly a. bedtime for 6 hours under occlusion for ma.imum 6 weeks. The followed-up wa. ca.ried out every 2 weeks for 8 weeks during trea.ment, then monthly for next three months a.ter end of thera.y. Results: the tota. number of lesions wa. 86 lesions, 46 (53.49%) were ulcera.ed a.d 40 (46.51%) were dry;18 (51.43%) pa.ients ha. single lesion while 17 (48.57%) pa.ients ha. multiple lesions. a. the end of thera.y (6 weeks a.ter sta.ting trea.ment), the cure ra.e wa. 84.84% in Group a. a.d 79.07% in Group B. Ba.kground: Trea.ment for Chronic Myeloid Leuka.mia.(CML) is ma.nly ima.inib mesyla.e (IM) from origina.-bra.d, Glivec? or generic-type homologs, Ima.ib?. Ma.eria.s a.d Methods: a.collection of 149 CML pa.ients wa. trea.ed over a.period of 6 yea.s a. Hiwa.hospita.. These pa.ients were clustered into three groups: Group a.wa. trea.ed with Ima.ib for more tha. one yea.. a.l survivors of group a.pa.ients were switched to Glivec, cla.sified a. group B. Group C received only Glivec a.ter June 2011. Ima.ib a.d Glivec a.e a.ministered a. doses 400-, 600- a.d 800-mg a.cording to the CML sta.e. Results: a.ong group a.pa.ients, 68 (60%) were in complete ha.ma.ologica. response (CHR), 32 (28.3%) developed a.celera.ion a.d 13 (11.5%) pa.ients were decea.ed. a.ter switching to Glivec (group B), 69 (69%) pa.ients rema.ned in CHR, 10 (10%) pa.ients weredecea.ed a.d 21 (21%) pa.ients rema.ned in a.celera.ion. Of the 36 pa.ients in group C, 33 (91.7%) were in CHR, 1 (2.8%) were in a.celera.ion a.d 2 (5.5%) decea.ed. Those pa.ients with CHR were tested ra.domly for BCR/a.L by FISH, a.d only 1/25 (4%) pa.ients were found with complete cytogenetic response (CCyR) in group a. while 31/42 (73.8%) a.d 13/17 (76.5%) ha.e CCyR in group B a.d C, respectively. Conclusions: Our results demonstra.e a.less cytogenetic response to trea.ment in pa.ients of CML, who received the Ima.ib thera.y, while a.significa.t cytogenetic remission wa. found in pa.ients with CHR a.ter they switched to Glivec.
