Prevalence and Contributing Factors of Orthostatic Hypotension in the Cardiology Department of the CHU Ignace Deen in Conakry

Introduction: Orthostatic Hypotension (OH) is a frequent situation during consultation in hypertensive patients. The prevalence in the general population increases with age and it is recommended to systematically search for it in patients with multiple comorbidities. The objective of this study was to determine the prevalence of orthostatic hypotension;describe their socio- demographic profile, the various clinical and paraclinical aspects and the predisposing factors of orthostatic hypotension in treated hypertensives, controlled or not. Method: This was a descriptive cross-sectional study, from January 2 to June 30, 2022 in the cardiology department of the Ignace Deen National Hospital. Included in this study were all patients who presented with orthostatic hypotension under antihypertensive treatment, regardless of age and sex, and who agreed to participate in the study. The non-inclusion criteria were hypertensive patients without OH and those who had not agreed to participate in the study. Each patient had blood pressure and heart rate measured in the supine position at room temperature and with an empty bladder. Then the measurement is taken again three (3) minutes after the switch to orthostatism. We retained the diagnosis of OH if the SBP drops by at least 20 mmHg and/or the PAD by at least 10 mmHg three (3) min after the transition to orthostatism. Our data obtained were analyzed in the Epi-info 7.4.0 software. Results: During our study period, we investigated 385 presented with OH, 12.2%. The mean age of the patients was 60.83 years ± 10.01 years and hypertensive patients, 47 of whom the M/F sex ratio was 0.81. In our study, blood pressure was not controlled in 36.2% of our patients with a predominance of grade 3 hypertension, a rate of 55.32%. Renal failure, anemia, heart failure and stroke were the main comorbidities associated with the occurrence of HO. The present study reports the titanium dioxide(TiO_(2))films synthesized from TiCl3 precursor on ITO glass substrates using two electrochemical techniques,namely direct electro-deposition(DE)and pulse electro-deposition(PE).The synthesis potential during the time-on(Ton)period was fixed at-1.5 ***,the open-circuit potential was applied during the time-off(Toff)*** effect of the technique of electro-deposition and Toff duration on the properties of TiO_(2)films and their photoelectron-catalytic activity were *** obtained films were investigated by X-ray diffraction(XRD),scanning electron microscopy(SEM),UV-VIS spectrometer,and photocurrent measurement *** is found that the use of the PE technique at different Toff improves the properties of TiO_(2)films compared to the DE *** XRD patterns show the anatase phase with a marked preferential orientation along the(101)direction for all *** the SEM analysis,it is seen a significant change from big multigrain agglomerates at DE to a dense film structure and small multigrain agglomerates at different *** the Toff decreases from 3 s to 1 s,the photocurrent response rises and reaches a high value of about 12 m A/cm^(2).Compared with DE,and under UV light the photocatalytic property of TiO_(2)film synthesis via PE has been improved in the degradation of methyl orange(MO).Finally,the films deposited at low Toff(Toff=1 s)show a faster degradation of MO.

Diagnostic Rigid Urethrocystoscopy: Indications, Results and Pain Assessment

Background: Over the last 50 years, endourology has revolutionized urological practices worldwide. This is not so in many urological centers in West Africa. Although, some centers have made progress in the level of urological services that they offer;many of such centers provide rigid urethrocystoscopy services. Rigid urethrocystoscopy is an endoscopic examination that explores the urethra, bladder and prostate in men. Aims: We report the indications, results and pain assessment of rigid urethrocystoscopy in our center. Patients and Methods: We carried out a retrospective, descriptive, monocentric study in our center between January 2016 and June 2018. The study included all patients who had a rigid urethrocystoscopy under local anaesthesia. We studied the following parameters: sex, age, indications for the examination, outcomes and the pain assessment. Results: Five hundred and forty-one patients were included. The sex-ratio was 1.49. The mean age was 49.47 ± 18.48 years (12 years and 91 years). Lower urinary tract symptoms (29%) and hematuria (28%) were the most common indications. The rigid urethrocystoscopy was normal in 26.8% of patients. Bladder tumors (21.2%) were the most frequent lesions. In men, prostate tumors were more common (21%) followed by bladder tumors (17.9%), while in women, bladder tumors (26.3%) were predominant followed by cystopathy lesions (12.4%).The mean Simple Verbal Scale (SVS) score was 1.25 in women and 2.1 in men. Conclusion: Rigid urethrocystoscopy was relatively well tolerated by our patients. Hematuria was the primary indication and the main etiologies were bladder and prostate tumors.

Contribution of electric Tomography, Chemical and Isotopic Tools to the Understanding of the Resilience and Salinization of Wetlands along the Northern Coast of the Cape-Verde Peninsula: The Case of the Retba Lake

On the northern coast of the Cape Verde peninsula, Lake Retba is one of the few depressions that have preserved its water body due to the drought that plagued the Sahel in the 1960s. The aim of this article is to understand the resilience of the lake to the drought. For this purpose, a deep knowledge of interactions between the different components of the hydro-system, namely: lake, dune tablecloth, ponds and ocean is necessary. The use of electrical resistivity tomography (ETR) and chemical and isotopic tools yielded conclusive results. electrical tomography of resistivity (ETR) shows throughout the coastal dune a superposition between fresh and salt water, with even a predominance of salt water at the level of the old canal. The chemistry of the major ions and isotopes confirms this contact between the saltwater bison of the sea and the lake by showing a superposition of the fresh/brackish waters on the salty waters of the bevel, which, in turn, are found in the very salty waters influenced by the lake. This translates a feeding of the lake by the sea through the bevel. At the canal level, this feeding is done, on the surface, by means of ponds whose water characteristics show that they constitute an outcrop of the bevel. The freshwater body of the coastal dune that floats on salt water in the form of a lens also flows into the lake. The feeding of the lake, by the sea through the bevel and the ponds, and by the water table, is therefore effective and attested by all the methods used in this work. It allowed the resilience of the lake against the drought. This inflow of seawater into the lake is at the origin of its salinization and therefore of the salt that settles on the bottom. This study shows that the survival of the lake also depends on safeguarding the ponds located in the north of the lake.

Metastatic Prostate Cancer under Androgen Deprivation Therapy: Factors Influencing Castration Resistance

Objective: To evaluate the factors predicting the time to progression to castration-resistant in metastatic prostate cancer under Androgen Deprivation Therapy (ADT) in our center. Patients and Methods: This is a retrospective, descriptive, analytical study in a single center over a period of 2 years. It has interest patients followed for metastasized prostate cancer under ADT. The parameters studied were: epidemiological, clinical, paraclinical, prostate specific antigen (PSA) nadir, time to nadir (TTN) and their link with the castration resistance. Results: The frequency of castration resistant prostate cancer was 28 patients per year. The mean age was 70.4 ± 7.9 years. An ECOG score ≥ 3 was more common as was the cT2c stage. The median of the initial total PSA was 489.6 ng/ml (203.3;1653.2). All patients had adenocarcinoma. The International Society of Urological Pathology (ISUP) 1 was more frequent. Bone metastases were more frequent. The medians of nadir, TTN and the castration resistance were 19.3 ng/ml (3.7;102.1), 5.5 months (3;9) and 11 months (6;15.3), respectively. The Eastern Cooperative Oncology Group (ECOG) score, clinical stage, metastatic site, the nadir and its TTN influenced the DSR. Age, lymph node involvement, initial total PSA and Gleason score did not influence the castration resistance. Conclusion: ADT should be initiated as soon as possible before an attack of general and/or clinical stage advanced to delay resistance. A drilling should be associated with this hormone therapy as much as possible because of its gain on resistance.

Plantago notata Lagasca (Plantaginaceae), a spontaneous plant used as a traditional medicine in Gharda a (Septentrional Sahara Algerian). This paper reports the extraction and partial characterization of water-soluble polysaccharides from Plantago notata leaves. These polysaccharides were obtained by sequential extraction in distilled water, followed by precipitation in 75% ethanol. The yield of extract is 2.0% (w/w). The crude water soluble polysaccharide extracts were further characterized and revealed the average values12.49±1.61% proteins and 85.03±1,13% carbohydrates, among them 20,34 ± 0,38%are uronic acid and79.65 ± 0.77% are neutral *** identification of monosaccharide composition by high performance anion exchange chromatography with pulsed amperometric detection (HPAEC-PAD) method shows 43.95% of galactose, 20.28% of rhamnose, 11.30% of glucose 9.55% of arabinose and 12.57% of galacturonic acid.

Gastric Adenocarcinoma at the Joliot Curie Institute in Dakar: Epidemiological, Diagnostic and Therapeutic Aspects about 54 Cases

Objectives: To study the epidemiological, diagnostic, therapeutic and prognostic aspects of gastric adenocarcinoma at the Joliot Curie Institute in Dakar. Materials and Methods: We conducted a retrospective study over a seven-year period from January 2010 to December 2017 at the Joliot Curie Institute in Dakar. All cases of gastric adenocarcinoma proven by fibroscopy followed by histology or proven on the histological analysis of a surgical specimen were taken into account. The parameters studied were age, risk factors, stages of the disease, treatment and prognosis. Results: There were 54 cases of gastric adenocarcinoma over a period of 7 years. The average age was 54.74 years with extremes of 25 and 84 years. A male predominance was noted (35/54). The main risk factors found were alcohol (5/54), tobacco (13/54), Helicobacter pilori (4/54), gastric ulcer (12/54). Epigastralgia was the most frequent clinical manifestation. FOGD was carried out in 77.8% of patients. Histology was obtained before surgery in 40 patients (74.1% of cases) and

Disparities in Dental Caries among Students in Senegalese Koranic School: Epidemiological Study in Daara of Koki

Background: School is a space where children are exposed to the social and psychosocial environment that can affect their health. In Senegal, the prevalence of dental caries is high in classic schools according to fragmented data, and unknown in the koranic school (Daaras). Objective: The objective is to describe the disparity of dental caries in the schoolchildren (talibes) of Daara of Koki. Method: A cross-sectional study was carried out on 400 talibes aged 7 to 12 years. Dental caries and socio-demographic data were, respectively, collected by clinical examination and questionnaire. Results: The sex ratio was 9, the mean age of 9.9 ± 1.5 years and the mean length of stay in the Daara was 2.3 ± 1.1 years. The prevalence of dental caries was 47.4%. The average mixed DFMT index was 1.1 ± 1.5. The prevalence was 51.2% among boys, 60% among eight-year-old children and 37.4% among twelve-year-old children. By length of stay, the prevalence was 52.9% at the start and 22.2% at the end with a significant downward trend (p Conclusion: The study showed an unequally distributed of caries among talibes of Koki’s Daara. She suggests that oral health promotion policies are needed in Koranic schools and must rely on koranic teachers.

This paper aims to assess soil fertility and risk contamination parameters in response to increased amendment rates of sewage sludge application,under semi-arid climatic conditions of Morocco,using two soil textures:soil A(clay-silty)and soil B(sandy-clay)over two *** this end,sewage sludge and soils treatments physicochemical parameters,heavy metals content,degree of pollution and associated risks were evaluated and compared to controlled *** results showed that all studied parameters changed significantly(pPb>Zn>Ni>Cd>***,their concentrations were below the critical values established by the European Union for the agronomic use of *** obtained results of assessed parameters indicating SS degree of pollution and associated risks showed a low to a moderate contamination risks of heavy ***,environmental risks of sewage sludge application will be related to increase of heavy metals mainly in a long-term agricultural ***,optimal management of sewage sludge application is recommended to ensure sustainability and benefits at the same time to master the contamination risks.

Pulmonary Hypertension in Chronic Hemodialysis Patients at Aristide Le Dantec University Hospital

Introduction: Pulmonary hypertension (PH) is defined as high blood pressure in the lungs. It is recently described as a vascular disease entity in chronic kidney disease, particularly for chronic hemodialysis patients. The aims of this study were to determine the prevalence of PH, to describe clinico-biological and morphological characteristics and to identify risk factors associated with PH in hemodialysis patients. Patients and methods: This is a cross-sectional, descriptive, and analytical study during a period of 8 months from January 1, 2019, to August 31, 2019, in the hemodialysis center of nephrology department of Aristide Le Dantec Hospital. All chronic hemodialysis patients for more than 3 months and with PH confirmed by cardiac doppler ultrasound were included. Results: During the study period, 25/94 patients on chronic hemodialysis, presented with pulmonary hypertension (26.6%). The mean age was 49.3 ± 12.9 years. The sex ratio (M/F) was 0.92. Exertional dyspnea was found in 18 patients (72%). Cardiac auscultation revealed an arrhythmia in 3 patients (12%), augmented second heart sound in 12 patients (48%). On transthoracic doppler ultrasound, pulmonary systolic arterial pressure was on average 51.4 ± 13.2 mmHg. PH was moderate in 13 patients (52%), mild in 9 patients (36%) and severe in 3 patients (12%). The left ventricular ejection fraction was altered in 9 patients (36%). Three patients (12%) had valve calcifications. They were mainly located at aortic valve (8%) and mitral valve (4%). The major risk factors associated to PH in our patients are pathological fractures (p = 0.023), aortic calcifications (p = 0.023), ischemic heart disease (p = 0.023). The duration of hemodialysis represents another favoring factor (p = 0.042). Also implicated are arrhythmias (p = 0.004), high cardiac index (p = 0.043), ventricular dilatation (p = 0.034) and left atrial dilation (p = 0.015), as well as dilation of the inferior vena cava (p = 0.048).
