As a natural and environmentally friendly renewable material,Northeast China ash wood(NCAW)(Fraxinus mandshurica Rupr.)was cut by water-jet assisted CO_(2) laser(WACL),the surface quality was evaluated by surface roughness of cut *** surface roughness was measured by three-dimensional(3D)***,the micromorphology of machined surface was observed by scanning electronic microscopy(sEM).Carbon content changes of machined surface were measured by energy dispersive spectrometer(EDs).A relationship between surface roughness and cutting parameters was established using response surface methodology(RsM).It is concluded that the cutting speed,laser power and water pressure played an important role in surface roughness of cut *** surface roughness increased as an increase in laser *** decreased caused by increasing of cutting speed and water *** revealed that the surface quality of NCAW part was improved using the optimized combination of cutting *** established quadratic mathematical model of a good prediction is helpful for matching suitable cutting parameters to obtain expected surface quality.

High-quality wood products and valuable wood crafts receive everyone’s favor with the rapid development of the *** order to improve the cutting surface quality of wood forming parts,the cutting experiment of renewable Fraxinus mandshurica was conducted by waterjet-assisted CO_(2)laser(WACL)technology.A quadratic mathematical model for describing the relationship between surface roughness changes and cutting parameters was *** effects of cutting speed,flow pressure and laser power on the kerf surface roughness of Fraxinus mandshurica when cutting transversally were discussed by response surface *** experimental results showed that kerf surface roughness decreased under a lower laser power,higher cutting speed and higher flow *** the cutting speed was 30 mm/s,flow pressure was 1.58 MPa and laser power was 45 W,the actual surface roughness of the optimized Fraxinus mandshurica was 2.41μm,and it was in accord with the theoretically predicted surface roughness value of 2.54μm,so the model fitted the actual situation *** the analysis of 3D profile morphology and micromorphology,it was concluded that the optimized kerf surface of Fraxinus mandshurica was smoother,the cell wall was not destroyed and the tracheid was *** provides the theoretical basis for wood micromachining.

激光切割单板的理论工艺参数求解与试验验证

【目的】利用固体激光器切割单板,获取合理的切割工艺参数,降低切缝宽度以减少锯路损失,保证单板切口断面平整度。【方法】从激光脉冲能量角度建立激光切割单板数学模型,获得激光输出功率(Pb)与激光切割速度(v)之间的关系表达式。以厚度0.3 mm、含水率10%的黑胡桃单板为加工对象,选取2组激光输出功率进行试验验证,每组激光输出功率在6种不同切割速度下切割,理论切割速度在6种试验切割速度范围内,加工后多次测量直线切缝宽度取平均值,在40×10光学显微镜下观察切割后试件切缝宽度及切口断面平整度,找出最优试验结果对应的切割速度,并与计算的理论切割速度进行对比。【结果】第1组激光输出功率(脉冲电压400 V,单脉冲能量96.5 mJ,脉冲频率10 Hz)下,当切割速度为2.5 mm·s^-1时可保证单板切缝被完全切透,形成连续完整的切缝,切缝宽度最窄,平均值为0.544 mm,锯路损失最小,且切口断面较为平整;第2组激光输出功率(脉冲电压500 V,单脉冲能量198.2 mJ,脉冲频率10 Hz)下,当切割速度为5.0和5.5 mm·s^-1时可保证单板切缝被完全切透,切缝宽度最窄,平均值分别为0.69和0.62 mm,且切口断面也较为平整。对比2组试验数据得出,当切割速度与理论切割速度接近时,单板切口连续,切缝宽度最小;当切割速度低于理论切割速度时,切缝宽度逐渐增大;当切割速度高于理论切割速度时,切口容易出现不连续现象。【结论】利用固体激光器切割单板时,切割工艺参数对切缝宽度和切口断面平整度具有直接影响,合理的切割工艺参数对获得较小的切缝宽度至关重要,每个激光器的输出功率对应着最佳切割速度,在实际加工过程中,应保证切割速度与理论切割速度一致或接近,以使切割效果更好。

Despite the efforts made by the Malian government with technical and financial partners in the fight against HIV and AIDs, there are no data on the evolution of people with HIV in medical consultation in antiretroviral treatment sites. The objective of this work is to fill this gap by generating factual data describing the evolution of the number of people with HIV in medical consultation in Mali. To achieve this goal, we conducted a cross-sectional study using data from the Coordination Unit of the sectoral Committee for AIDs Control of the Ministry of Health and Public Hygiene. sampling was comprehensive including all people living with HIV. The data collected concerned those of the years 1999 to 2014 by the EsOPE software and analyzed with stata version 12. A Total of 39,619 people were enrolled in the study. And 26,212 people living with HIV in the 17 EsOPE sites of Mali were female. Patients aged 25-34 and 35-44 years were strongly represented in consultation in the years 2007 and 2011, and those aged 15-24 were the least frequent. HIV1 was the most dominant with 97%. The majority of patients presented for initial consultation were clinical stages 2 and 3 in all years. Cases of lost sites seen in the more common in hospitals are 62.79% at the Hospital of Mopti and 57% at the University Hospital of Point G. The number of cases of people living with HIV in medical consultation has progressively augmented from 1999-2007. Then it dropped from 2008-2010. After this number has increased by 2011, it dropped again in 2012-2014. This study made it possible to note the evolution of the patients in medical consultation in EsOPE antiretroviral treatment sites according to the years and events (safe, subsidized by activities by the global fund). The activities of sensitization and communications, case research should be strengthened by involving the community.

Introduction: Goitre is an increase in the volume of the thyroid gland. Goiters may be congenital or acquired, diffuse or nodular, functional or non-functional, benign or malignant. Purpose: The objectives of this work were to study and determine the frequency of benign goitre in the general surgery department of CHU Gabriel Touré, and to study epidemiological, clinical and therapeutic aspects. Patients and Method: This was a retrospective and prospective study conducted in the general surgery department of CHU Gabriel Touré, from January 1999 to December 2019. Included in the study were all patients with benign goiter confirmed in histology, hospitalized and treated in the service. Patients with thyroid malignant tumours, strumite, thyroid tract cyst, cervical abscess and neck trauma were not retained. Socio-demographic, clinical, para-clinical, therapeutic and post-operative aspects were the study parameters. Word processing and tables were done with Microsoft Word and Excel 2016 software. short Term Outcomes of Laparoscopic versus Open Distal Gastrectomy with D2 Lymph Nodes Dissection for Gastric Cancer: A Prospective study

Background: Laparoscopic distal gastrectomy (LDG) for gastric adenocarcinoma (GA) is gaining more acceptances worldwide. Its results are still controversial. This study aimed to assess short term outcomes of LDG and compare it to the standard open distal gastrectomy. Patients and Methods: 27 patients with GA of the distal 2/3 of the stomach were included and divided into 2 groups;Group A: 15 patients submitted to open distal gastrectomy with D2 lymph node (LN) dissection, and Group B: 12 patients submitted to LDG with D2 LN dissection. Results: The median age was 54 and 54.3 years in group A and B respectively. The median operative time was 118.7 and 210.2 minutes in group A and B respectively. The median safety margin was 6.52 and 5.7 cm in group A and B respectively while the median number of excised LN was 24.2 and 21.4 in group A and B respectively. One patient in group B had intraoperative bleeding that was controlled laparoscopically. No conversion to open surgery needed in group B. The median number of narcotic doses was 5.9 and 4.25 in group A and B respectively. The median length of hospital stay was 7.2 days in group A and 7.3 days in group B. Three patients in group A and 2 patients in group B had postoperative complications and all were treated successfully conservatively. Conclusion: LDG with D2 LN dissection is oncologically safe with short-term outcomes comparable to those of the open surgery. Gaining more surgeons’ experience is necessary to improve these results.

Background:?Many ventral hernia repair methods have been described among surgeons. The traditional primary repair entails a laparotomy with suture approximation of strong fascial tissue on each side of the defect. However, recurrence rates after this procedure range from 12% to 24% during long-term follow-up. Laparoscopic ventral hernia repair (LVHR) is a well recognized minimally invasive surgical technique for repair of different types of abdominal wall ventral hernias. However, the best method of mesh fixation during LVHR is still a subject ofdebate.?Patients & Methods: In the present study, 50 patients were presented with ventral hernia between June 2012 and October 2013. Demographics of the patients were recorded. All patients were submitted to LVHR with mesh fixation by “Double Crown” of tackers. The first crown was placed on the mesh periphery with 1 cm between each 2 successive tackers and the second crown around the edges of the defect. Operative complications, VAs scale, post-operative complications, and length of hospital stay were reported. Results: The mean age was 40.08 years. Female to male ratio was 3:2. The mean BMI was 32.3. The diameter of the hernial defect was s larger than 15 cm were excluded. LVHR was successfully completed in all the patients with no conversion. Only 1 patient had intra-operative bleeding from omental vessels that was successfully controlled. The mean operative time was 79 minutes. Post-operatively, the mean VAs was 3.96, 2.12, and 0.24 at 24 hours, 2 weeks, and 4 weeks, respectively. Two patients developed post-operative ileus that was treated conservatively and 1 patient developed persistent seroma that was treated by repeated aspiration. The mean length of hospital stay was 3.08 days. Conclusion: “Double Crown” tackers mesh fixation in LVHR seems to be a safe and effective surgical technique with favorable outcome.
