Resveratrol and fenofibrate ameliorate fructose-induced nonalcoholic steatohepatitis by modulation of genes expression

AIM: To evaluate the effect of resveratrol, alone and in combination with fenofibrate, on fructose-induced metabolic genes abnormalities in ***: Giving a fructose-enriched diet (FED) to rats for 12 wk was used as a model for inducing hepatic dyslipidemia and insulin resistance. Adult male albino rats (150-200 g) were divided into a control group and a FED group which was subdivided into 4 groups, a control FED, fenofibrate (FENO) (100 mg/kg), resveratrol (RES) (70 mg/kg) and combined treatment (FENO + RES) (half the doses). All treatments were given orally from the 9th week till the end of experimental period. Body weight, oral glucose tolerance test (OGTT), liver index, glucose, insulin, insulin resistance (HOMA), serum and liver triglycerides (TGs), oxidative stress (liver MDA, GSH and SOD), serum AST, ALT, AST/ALT ratio and tumor necrosis factor-α (TNF-α) were measured. Additionally, hepatic gene expression of suppressor of cytokine signaling-3 (SOCS-3), sterol regulatory element binding protein-1c (SREBP-1c), fatty acid synthase (FAS), malonyl CoA decarboxylase (MCD), transforming growth factor-β1 (TGF-β1) and adipose tissue genes expression of leptin and adiponectin were investigated. Liver sections were taken for histopathological examination and steatosis area were ***: Rats fed FED showed damaged liver, impairment of glucose tolerance, insulin resistance, oxidative stress and dyslipidemia. As for gene expression, there was a change in favor of dyslipidemia and nonalcoholic steatohepatitis (NASH) development. All treatment regimens showed some benefit in reversing the described deviations. Fructose caused deterioration in hepatic gene expression of SOCS-3, SREBP-1c, FAS, MDA and TGF-β1 and in adipose tissue gene expression of leptin and adiponectin. AIM: To detect whether the combination of vincamine, thioctic acid and lutein will improve the retina and optic nerve functions in cases of an opaque media with an optic nerve and/or a retinal defect or ***: Totally 2000 patients(2000 eyes) of corneal opacities with defects in the optic nerve or/and the retinal functions were studied. Every patient received three types of drugs: thioctic acid with cyanocobalamine, vincamine, and lutein. The drugs were given daily for 3-12 mo according to patient’s responses. Full field flash electroretinogram(ERG) and flash visual evoked potential(VEP) were done before treatment and at 1, 3, 6, and 12 mo sequentially. Patients were followed up for 12 ***: In the 2000 eyes, 1000 eyes had both moderate optic nerve and retinal function defects;and 840 eyes out of the 1000 improved with the medical treatment. Another 500 eyes out of the 2000 eyes had extinguished retinal function with normal optic nerve function and only 125 eyes of them improved. The 290 out of the 2000 eyes had severe defects in optic nerve with normal retinal function and 130 of them improved. Another 210 eyes have mild optic nerve and retinal function defects and 194 ***: The combination of vincamine, thioctic acid with cyanocobalamine, and lutein improved both retina and optic nerve functions mainly in mild and moderate defect than in severe cases.

AIM： TO establish a cell culture system with longterm replication of hepatitis C virus （HCV） genome and expression of viral antigens in vitro. METHODS： HepG2 cell line was tested for its susceptibility to HCV by incubation with a serum from a patient with chronic hepatitis C. Cells and supernatant were harvested at various time points during the culture. Culture supernatant was tested for its ability to infect na＇ive cells. The presence of minus （antisense） RNA strand, and the detection of core and E1 antigens in cells were examined by RT-PCR and immunological techniques （flow cytometry and Western blot） respectively. RESULTS： The intracellular HCV RNA was first detected on d 3 after infection and then could be consistently detected in both cells and supernatant over a period of at least three months. The fresh cells could be infected with supernatant from cultured infected cells. Flow cytometric analysis showed surface and intracellular HCV antigen expression using in house made polyclonal antibodies （anti-core, and anti-E1）. Western blot analysis showed the expression of a cluster of immunogenic peptides at molecular weights extended between 31 and 45 kDa in an one month old culture of infected cells whereas this cluster was undetectable in uninfected HepG2 cells. CONCLUSION： HepG2 cell line is not only susceptible to HCV infection but also supports its replication in vitro. Expression of HCV structural proteins can be detected in infected HepG2 cells. These cells are also capable of shedding viral particles into culture media which in turn become infectious to uninfected cells.

Background: Bronchopulmonary dysplasia (BPD) continues to be an important problem in neonates especially premature infants despite improved facilities of care, monitoring and treatment. Pulmonary hypertension (PH) is a major complicating factor and key cause of mortality in this population. Altered vascular and alveolar growth particularly in canalicular and early saccular stages of lung development following mechanical ventilation and oxygen therapy result in arrest of the lung development leading to BPD with PH. Early recognition of PH in infants with these risk factors is important for optimal management. We tested the hypothesis that asymmetric dimethylarginine, would be greater in infants with bronchopulmonary dysplasia associated pulmonary hypertension than in infants with BPD alone. The Aim: The aim of the current study was to measure the Asymmetric dimethylarginine (ADMA) levels, arginine levels & the plasma arginine-to-ADMA ratio in newborn infants with broncho-pulmonary dysplasia, to evaluate echocardiographic parameters among neonates with bronchopulmonary dysplasia, to correlate between plasma ADMA & arginine-to-ADMA ratio and echocardiographic (ECHO) parameters in those patients and to compare full term & preterm neonates with bronchopulmonary dysplasia as regard to plasma ADMA level. Methods: A case-control study was carried out of ninety (90) newborns selected from those admitted to Neonatal Intensive Care Unit at Maternity & Children Hospital and Alzhraa University hospital during the period from October 2015 to March 2018. Neonates were divided into 2 groups: Patient with BPD with PH (cases group): It included 45 neonates with BPD & PH, 35 preterm neonates and 10 full term neonates. 目的:通过节细胞内网状层(GCIPL)评估中度和重度青光眼的损伤程度并比较其与视盘周围视网膜神经纤维层(PRNFL)的诊断效能,包括敏感性与特异性及ROC曲线下面积(AUC)。方法:前瞻性研究。共210眼(包括中度青光眼患者30例54眼,重度青光眼患者34例59眼和正常人50例97眼)纳入本研究。所有参与者均接受全面眼科检查,包括视野检查、3D-OCT视盘检查和3D-OCT黄斑部垂直扫描。记录并比较所有参与者的GCIPL和PRNFL的AUC、敏感性与特异性。结果:在中度和重度青光眼组中,上部、下部及整体GCIPL和PRNFL厚度明显变薄(P<0.001)。在中度青光眼组中,GCIPL的敏感度与特异性高于PRNFL(仅上半部分敏感性相同)。重度青光眼组,上部、下部及整体GCIPL的敏感度均低于PRNFL。整体GCIPL的特异性低于PRNFL。上部GCIPL的特异性高于PRNFL。下部GCIPL的特异性与PRNFL相同。结论:对于区分中度和重度青光眼,黄斑GCIPL参数的功能远高于PRNFL。两者联合在病情分析中效果最优,能够提供更准确的损伤程度评估。

目的:探究不同类型的激素类避孕剂对于泪腺功能及泪膜的影响。方法:前瞻性随机对照试验。选取60位来自马来西亚贝尼苏夫大学与法尤姆大学医学院的患者随机分为6组(每组10位患者)。第一组:口服复合避孕药;第二组:每月注射复合避孕剂(Mesocept);第三组:注射甲羟孕酮(DepoProvera);第四组:仅服用黄体酮药片;第五组:皮下埋植(Implanon);第六组:对照组。所有患者均进行全面妇科和眼科检查,包括Schirmer试验和泪膜破裂时间,随访至少3mo。结果:所有患者的平均年龄为31.03±6.97岁。年龄和孕产次各组之间差异无统计学意义。结果显示:口服复合避孕药干眼患病率最高,其次是每月注射复合避孕剂、注射甲羟孕酮、仅服用黄体酮药片和皮下埋植。对照组无干眼患者。结论:与仅含有孕激素的避孕方法相比,同时含有雌激素和孕激素的避孕方法导致干眼的可能性更大。

AIM: To study the cost benefit of external fixation vs external fixation then nailing in treatment of bone infection by segment ***: Out of 71 patients with infected nonunion tibia treated between 2003 and 2006, 50 patients fitted the inclusion criteria(26 patients were treated by external fixation only, and 24 patients were treated by external fixation early removal after segment transfer and replacement by internal fixation). Cost of inpatient treatment, total cost of inpatient and outpatient treatment till full healing, and the weeks of absence fromschool or work were calculated and compared between both ***: The cost of hospital stay and surgery in the group of external fixation only was 22.6 ± 3.3 while the cost of hospital stay and surgery in the group of early external fixation removal and replacement by intramedullary nail was 26.0 ± 3.2. The difference was statistically significant regarding the cost of hospital stay and surgery in favor of the group of external fixation only. The total cost of medical care(surgery, hospital stay, treatment outside the hospital including medications, dressing, physical therapy, outpatient laboratory work, etc.) in group of external fixation only was 63.3 ± 15.1, and total absence from work was 38.6 ± 6.6 wk. While the group of early removal of external fixation and replacement by IM nail, total cost of medical care was 38.3 ± 6.4 and total absence from work or school was 22.7 ± 4.1. The difference was statistically significant regarding the total cost and absence from work in favor of the group of early removal and replacement by IM ***: Early removal of external fixation and replacement by intramedullary nail in treatment of infected nonunion showed more cost effectiveness. Orthopaedic society needs to show the cost effectiveness of different procedures to the community, insurance, and health authorities.

Background: Minimal hepatic encephalopathy (MHE) is defined as hepatic encephalopathy (HE) without symptoms on clinical/neurological examination, but with deficits in some cognitive areas that can only be measured by neuropsychometric testing. Aim: Our aim was to study serum zinc levels in cirrhotic patients with and without MHE and correlate it with the results of neuropsychological tests and P300 event related potential. Patients and Methods: Fifty-five patients with liver cirrhosis were recruited for the present study. All patients were subjected to full history taking, clinical examination, abdominal ultrasonography, laboratory investigations including liver function tests and serum zinc, psychometric studies (line tracing test and serial dotting test) and P300 event related potential. According to the results of psychometric studies and P300 event related potential, the patients were divided into 3 groups in addition to the control group. Results: Among the 55 patients included in the study, 30 patients had no clinical evidence of HE and 25 patients had overt HE with different grades. In patients with no clinical evidence of HE, psychometric tests and P300 event related potential revealed that 13 patients had MHE, while 17 had no HE. There were significantly lower serum zinc levels in patients with MHE and in cirrhotic patients without HE compared with the healthy controls. Also, zinc levels were significantly lower in higher grades of hepatic encephalopathy. Serum zinc levels showed positive correlation with serum albumin levels and inverse correlation with serial dotting test and P300 latency. Conclusion: Reduced serum zinc level and prolonged P300 latency can serve as predictors of minimal hepatic encephalopathy in cirrhotic patients. Zinc deficiency is common in cirrhotic patients with MHE. These findings may have important prognostic and therapeutic implications in MHE and overt HE patients with zinc deficiency.
