Far Lateral Disc Prolapse: Surgical Options in Limited Resources Centers

Far lateral disc prolapse accounts for 6.5% to 12% of all lumbar disc prolapses. Surgical options include open laminectomy with discectomy, microscopic and endoscopic excision of the prolapsed disc. Some of these options may not be available in limited resources centers. We will highlight the effectiveness of various surgical options. Twenty patients with foraminal and extraforaminal lumbar disc prolapse were operated upon from January, 2015 to June, 2016 in the neurosurgical departments of Cairo and Fayoum Universities in Egypt by different modalities (open laminectomy with discectomy, microscopic and endoscopic discectomy). Seventeen patients had foraminal disc prolapse and only three patients had extraforaminal disc prolapse. Twelve patients were operated by conventional laminectomy approach. Microscope was used in four patients and four patients were operated endoscopically. Excellent radicular pain improvement was achieved in 15 cases (75%) including all of the laminectomy groups. Conventional laminectomy and discectomy in far lateral disc prolapse remains an excellent option especially in limited resources centers. Although building up experience with other surgical modalities is mandatory.

AA （Alopecia areata） is the most frequent cause of inflammation-induced hair loss, affecting 0.1 to 0.2% of population worldwide. The development of organ-specific autoimmune reactions directed against anagen hair follicles seems to play a key role in the pathogenesis of alopecia areata. However, the triggering antigen（s） responsible for inducing autoimmune phenomena in these individuals remain unknown. Viral, bacterial or fungal pathogens have been implied as possible triggering factors of autoimmune reactions. The present study aims to identify the role of dematiaceous fungi in the pathogenesis of alopecia areata. 30 patients diagnosed clinically as alopecia areata and 30 normal age matched persons have undergone mycological examination. Mycology examination of the epidermal scrapings was done by DME （direct microscopic examination）, culture on SDA （sabouraud＇s dextrose agar） and imaging. There is significantly higher percentage of positive results for Alternaria species by culture on sabouraud＇s agar in patients group （20%） compared to controls （13.3%） P-value 〈 0.05. The possible role of Alternaria antigens （e.g. antigens involved in melanin synthesis） in triggering autoimmunity in alopecia areata still needs further research on a wider scale of cases.

AIMTo investigate the diagnostic ability of a non-invasive biological marker to predict liver fibrosis in hepatitis C genotype 4 patients with high accuracy. METHODSA cohort of 332 patients infected with hepatitis C genotype 4 was included in this cross-sectional study. Fasting plasma glucose, insulin, C-peptide, and angiotensin-converting enzyme serum levels were measured. Insulin resistance was mathematically calculated using the homeostasis model of insulin resistance (HOMA-IR). RESULTSFibrosis stages were distributed based on Metavir score as follows: F0 = 43, F1 = 136, F2 = 64, F3 = 45 and F4 = 44. Statistical analysis relied upon reclassification of fibrosis stages into mild fibrosis (F0-F) = 179, moderate fibrosis (F2) = 64, and advanced fibrosis (F3-F4) = 89. Univariate analysis indicated that age, log aspartate amino transaminase, log HOMA-IR and log platelet count were independent predictors of liver fibrosis stage (P CONCLUSIONOur predictive model is easily available and reproducible, and predicted liver fibrosis with acceptable accuracy.

Background: Children with Cerebral Palsy (CP) often receive physiotherapy to achieve maximum motor potential and prevent secondary conditions. Conductive Education (CE) is an education programme that combines special education and rehabilitation. Purpose: Study the effect of physiotherapy and conductive education in development of fine, gross motor skills and mental function in the children with CP. Subjects and Methods: This is a prospective case study that was carried out on 105 children with CP less than 4 years old selected from outpatient clinic in National Institute for Neuro Motor System. Cases were taken consecutively from February 2019 to September 2019. A written informed consent was taken from all participants after proper explanation of the study. They were classified into three groups (group I, group II and group III) according to their age. All children are subjected to complete history taking, clinical examination, use of developmental quotient (DQ) sheet before and after 6 months of regular physiotherapy and conductive education program. Results: There was statistically significant improvement in fine motor, cognation and gross motor in 3 groups after 6 months of regular physiotherapy and conductive education program. Indium tin oxide(ITO)is widely used in transparent conductive films(TCFs);however,several disadvan-tages,such as high cost and toxicity of indium,limit its ***,it is necessary to develop other materials that can replace *** nanowires or single walled carbon nanotubes(SWCNTs)have attracted considerable interest owing to their unique electrical,optical,and thermal stabilities,and thus,they are ideal for transparent electrodes for flexible or stretchable *** this study,we develop a novel architecture of composite TCFs on a polyethylene naphthalate(PEN)flexible ***,the silver nanowires-SWCNTs films with nested density structure were fabricated through ultrasonic spraying technology by varying the spraying *** achieving enhanced transmittance,we combined the larger irregular grids and holes with fewer nanowires stacked in the longitudinal direction,more optical chan-nels,and good carrier ***,aluminum-doped zinc oxide(AZO)was used as capping to the structure for enhancing the optical properties of the *** silver nanowires-SWCNTs/AZO(ASA)bilayer was obtained in the optimized architecture,which showed superior optoelectronic performance to that shown by commercial ITO with a high optical transmittance of 92%at the wavelength of 550 nm and low sheet resistance of 17/*** the specially structured conductive film,the significant improvement in the transmittance and uniformity of the sheet resistance was attributed to the effective nanowire junc-tion contact compared to that in ordinary structure of silver nanowires,which reduced the mean density of small clusters of *** with the silver nanowires-SWCNTs films,the ASA bilayer film exhibited excellent resistance to boiling,mechanical bending(10,000 cycles),and CO_(2)***,the sheet resistance of ASA changed slightly after the tape tests,thereby illustrating a strong adhesion to the PEN substrate after the enclosure of ***

Introduction: Cranioplasty is the surgical repair of a bony defect or deformity in the skull that is caused after cranial surgery or trauma. It carries cosmetic and protective benefits. Many types of materials are allowed. The subject of this study is to compare the outcome of two different manufacturing processes in reconstruction of calvarial skull defects by using 3D custom-made cranioprostheses versus hand-made bone cement implants. Patient and Methods: This is a prospective comparative study conducted on 20 patients of calvarial skull defects of different etiologies, sites and sizes, admitted in the neurosurgical departments of Cairo and Fayoum Universities in the period from August 2017 to February 2018. Patients are divided into two study groups: (group 1) 10 patients operated upon by 3D custom-made implant;(group 2) operated upon by hand-made bone cement implant. Statistical Analysis Used: Mann-Whitney test and Chi-square test. Results: Craniotomy using 3D custom-made implants gives better results than using bone cement in the functional restoration of skull shape and cranial protection with shorter operative time and less rate of postoperative complications. There is no statistically significant difference between the two study groups regarding cosmetic outcome. Conclusion: 3D custom-made implant is recommended for large and complex skull defects. Further and large studies might be needed.

合成了9种3-(2-羟基萘基-1-偶氮)-1,2,4-三氮唑(HL1),3-(2,4-二羟苯基-1-偶氮)-1,2,4-三氮唑(HL2),3-(2-羟基-3-羧基萘基-1-偶氮)-1,2,4-三氮唑(HL3),3-(2-羟基-5-溴苯基-1-偶氮)-1,2,4-三氮唑(HL4)和3-(2-羟基-5-甲基苯基-1-偶氮)-1,2,4-1,2,4-三氮唑(HL5)的Zr(Ⅱ)配合物并用元素分析,摩尔电导,磁矩,IR,UV-Vis,1H-NMR以及热分析(TGA和DTA)对其进行了表征。结果表明HL1-HL5以二齿一元配体方式通过偶氮的氮原子和羟基基团的氧原子与Zr(Ⅱ)离子配位生成单核配合物。用4种革兰氏阴性菌,即大肠杆菌(Escherichia coli),粘质沙雷氏菌(Serratia marcescens),阴沟肠杆菌(Enterobacter cloacae)和普通变形杆菌(Proteus vulgaris),以及2种真菌,即白色念珠菌(Candida albicans)和黑曲霉菌(Aspergillus niger)对配体及其配合物的生物学活性进行了研究。最小抑菌浓度(MICs)用纸上杯碟琼脂扩散法测定,结果表明在大多数情况下,金属化的配合物的抗微生物活性与自由配体相比有所增强。

The Need for Cardiac Pacing after Isolated Coronary Artery Bypass Graft Surgery

Background: The aim of this study is to assess the necessity of temporary cardiac pacing and identify the predictors of pacing after coronary artery bypass graft (CABG). Methods: This was a retrospective observational study of 150 patients who had isolated CABG between November 2013 and December 2021. Patients were classified into two groups: group A, (n = 135) who did not need to be paced and group B, (n = 15) who needed temporary epicardial pacing. Only 10% (15/150) of patients in the study needed pacing. The primary outcome variable was assessment of the need for postoperative temporary cardiac pacing (TCP) (patients were paced during chest closure or at any time during hospital admission). The incidence of pacing during the postoperative period was observed. Univariate and multivariate predictors for postoperative pacing were determined and specicific predictors for cardiac pacing were found. Results: In both univariate and multivariate analysis, Old age > 65 years, diabetes mellitus, pacing to come off cardiopulmonary bypass (CPB), CPB time > 100 min, cardioversion to leave OR, antiarrhythmics to leave OR and new onset of atrial fibrillation (AF), were found to be significant predictors for the need to cardiac pacing. Conclusion: After coronary artery bypass surgery, a small percentage of patients require TCP. We emphasize unique predictors for postoperative pacing in this study.

Yield and Quality Response of Ryegrass, Egyptian Clover and Their Mixtures to Different Sources of Fertilizers

Two field experiments were conducted during two successive winter seasons of 2008/2009 and 2009/2010 to investigate the potentialities of mixing Egyptian clover with ryegrass under bio, organic and mineral fertilization treatments and their combination to increase forage yield and quality grown under sandy soil conditions. The experiment included the combination of five mixing ratios (Egyptian clover alone, ryegrass alone, 75% Egyptian clover: 25% ryegrass, 50% Egyptian clover: 50% ryegrass and 25% Egyptian clover: 75% ryegrass) and eight fertilizer sources, which include control, organic fertilization, bio fertilization, chemical fertilizer, organic + bio fertilizer, organic + chemical fertilizer, bio + chemical fertilizer and combination of organic and chemical and bio fertilizers. The obtained results indicate the superiority of 75% E. clover: 25% ryegrass mixture fertilizedby Bio + O + N in fresh and dry forage production. On the other hand, it reported the lowest dry weight of weeds g/m2. Chemical analysis of forage plants showed that the mixture of ?75% E. clover: 25% ryegrass surpassed that of other treatments yield for crude protein, ether extract and ash. The results also revealed that the highest record of DCP, crude fiber and TDNY was obtained by forage mixture of 75% E. clover: 25% ryegrass fertilized with Bio + O + N. Such higher yield of these characters hassecured a balanced ratio which is really needed for ruminants ration.
