Introduction: Unmet need for family planning (UNFP) is defined as women with unmet needs who want to stop or delay childbearing but are not using any method of contraception. The objective of this study was to analyze the factors associated with unmet needs for family planning among couples living in rural and urban areas of Guinea in 2019. Methodology: This was a prospective, analytical cross-sectional, multicenter study of a six-month period from August 1, 2018 to January 31, 2019, focusing on couples with unmet needs for family planning. Result: Among 189 couples interviewed, 567 had UNFP (33.3%), the reasons for not using modern contraceptive methods were desire for pregnancy (AOR = 2.74, 95% CI: 1.74, 4.31), husband’s refusal (AOR = 0.23, 95% CI: 0.06, 0.81), spousal attitude (AOR = 0.20, 95% CI: 0.130, 30), birth spacing (AOR = 2.10% to 95%: 1.16, 3.82), difficulty with a new pregnancy (AOR = 0.17, 95% CI: 0.04, 0.74), and spousal attitude (AOR = 0.20, 95% CI: 0.14, 0.30). Conclusion: The involvement of spouses, especially in rural communities, would help achieve family planning objectives and reduce unmet needs for family planning.

Adult Posterior Cranial Fossa Arachnoid Cyst: A Case Report

Adult posterior fossa arachnoid cysts are rare lesions that are considered to be mostly congenital in origin. We present 49-year-old man admitted with a chief complaint of tremor and balance problems for the past 2 months. He had ataxia on examination. Magnetic resonance imaging revealed a well circumscribed midline cystic lesion of the posterior fossa sharing the same signal characteristics as cerebrospinal fluid and non-communicating to the fourth ventricle. He underwent surgical treatment for his symptomatic posterior fossa arachnoid cyst with a good outcome at 2 years follow up.

Non-isthmic coarctation of the aorta is a rare congenital malformation in adults. Arterial hypertension is a frequent circumstance of discovery. We reported the case of a 22-year-old Guinean man who had been followed for 5 years for hypertension. Clinically, he presented with hypertension of the upper limbs with a systolic pressure gradient of 100 mmHg. The diagnosis was confirmed by thoracic angioscan, which showed a 65.8% coarctation of the abdominal aorta. He was on triple antihypertensive therapy combining Atenolol 100 mg, Amlodipine 10 mg and Perindopril 10 mg. He is awaiting interventional treatment. His blood pressure is stable at around 140/90 mmHg.

为了降低汽车和航天设备的能耗,人们对材料提出了更高的轻量化要求,这进一步推动了轻质高强材料的发展。5083铝合金是一种不可热处理的材料,具有出色的焊接性和良好的耐腐蚀性等优点,因此在航空航天、汽车、建筑和船舶等行业得到广泛应用。然而,传统变形工艺制备的5083铝合金的强度和延展性有待提高。5083铝合金中含有较高浓度的Mg(大于3%),通过固溶强化,使其具有优异的抗拉强度。因此,通过增加合金中的Mg元素含量可以提高5083铝合金的力学性能。然而,Mg含量过高会导致晶界析出较多的β相(Al3Mg2)等粗大的第二相颗粒,这会严重阻碍晶粒的再结晶,因此Mg元素的含量也不能过多。人们期望获得具有优异强度的结构材料,而超细晶结构可以有效提高材料的强度。将金属材料的晶粒尺寸细化到几百纳米可以同时提高其强度和延展性,因此超细晶材料的制备工艺目前成为研究热点之一。Hall和Petch提出了室温下晶粒尺寸对材料屈服强度的影响。晶粒细化会导致大角度晶界占比增大,从而使位错迁移更加困难,进而提高材料的屈服强度。此外,当晶粒尺寸减小到纳米级时,材料的物理、化学和机械性能可能完全不同。目前,实现铝合金的晶粒细化主要有三种技术,包括先进的热变形工艺、加入浓度较低的合金元素(如Zr或Sc)以及大塑性变形工艺。结合以上两种或三种技术可能实现更好的强化效果。综上所述,通过调控Mg元素含量和使用大塑性变形工艺,可以提高5083铝合金的强度和延展性,满足对轻量化、高强度材料的需求。大塑性变形工艺是一种新兴的方法,可以增强铝合金的机械性能。该工艺通过对块状金属材料进行剧烈的塑性变形,实现显著的结构细化。大塑性变形工艺制备的超细晶材料非常致密,适用于结构应用,因此成为制造超细晶材料的研究热点之一。相比于粗晶材料,高塑性应变产生的合金中的小晶粒和较高的位错密度导致强度大幅提高。目前应用广泛的大塑性变形工艺包括等通道角压、高压扭转、累积叠轧、多向锻造、不对称轧制、深冷轧制和约束槽压制。钪(Sc)是一种轻金属,密度为3 g/cm。尽管Sc的价格相对较高,但随着全球发现了许多大型Sc矿床,有效的提取方法和不断创新的技术将会大幅降低Sc的价格。Sc可以提高铝合金的强度,其中沉淀硬化是提高铝合金强度的重要机制之一,而Sc相比其他稀土元素,能更有效地增强铝合金,这是由于Sc的多重强化效应。强化机制主要包括固溶强化、沉淀硬化、抑制再结晶和促进析出相的发展。除了铝锂合金,铝钪合金也是一种具有广泛应用潜力的高性能铝合金材料,可在海洋、航空航天、核能和火箭导弹等高科技和新兴技术领域得到应用。通过在5083铝合金中添加微量的Sc,可以改善合金的力学性能,使其更适用于航空航天和汽车行业。研究表明,添加Sc可以提高5083铝合金的屈服强度、极限抗拉强度和伸长率,这主要是由于Sc和铝之间形成稳定的金属间化合物Al3Sc,进一步增强了合金的强度。此外,Sc的添加还可以改善合金的微观结构,使沉淀物分布更均匀,从而实现晶粒的细化。因此,通过晶粒细化技术可以提高5083铝合金的强度。控制基体中晶粒的分布(包括粗晶粒和超细晶粒的组合)和引入颗粒增强、微量元素是实现晶粒细化的主要方法。研究表明,纳米级别的小晶粒尺寸结构可以显著提高材料的综合力学性能。大塑性变形工艺制备的超高强度合金通常与纳米结构的形成相关,并遵循霍尔-佩奇关系的强化机制。近年来,添加Sc元素并结合深冷轧制的微合金化技术在5083铝合金中引起了广泛的研究。通过这种工艺改善合金的力学性能具有巨大的潜力。通过在5083铝合金中添加Sc,可以增强合金的力学性能,并使其更适用于航空航天和汽车行业。添加Sc的主要作用是形成稳定的金属间化合物Al3Sc,进一步增强合金的强度。同时,Sc的添加还可以改善合金的微观结构,使沉淀物分布更加均匀,从而实现晶粒的细化。在铝合金中加入Sc微量元素是提高合金强度和塑性的有效方法。掺入一定量的Sc会导致产生较硬Al3Sc颗粒,可以较大程度的细化铝基体中的晶粒。Al3Sc是由四个原子(三个Al原子和一个Sc原子)组成的简单立方结构,这种结构类似于铝的FCC晶体结构。Al3Sc能够在Al基体中细化晶粒,同时控制晶粒结构的弥散体。Al-Mg-Sc是研究最多的铝钪合金。有研究证明,Mg元素的存在对Al3Sc颗粒的形成没有影响。因此,Al-MgSc合金的反应与两种二元合金Al-Mg和Al-Sc的反应相当。Mg在固溶体中对基体的强化作用,证明了其可以促进Al3Sc颗粒的析出。在确定铝钪合金的化学成分之前,必须选择合适的Sc浓度,以便在连续熔铸的情况下,大多数Sc保留在过饱和固溶体中,从而确保形成具有良好亚晶粒(多边形)结构,提高抵抗再结晶的能力。在共晶转变温度下,Sc在Al中的最大溶解度为0.4%。通常的冷却速率下的凝固温度范围内,会

Osgood Schlatter Disease: Study of 2 Observations in Conakry, Guinea

Introduction: OSD is a growth osteochondrosis affecting the apophyseal cartilage of the anterior tibial tuberosity of the knee, 20 to 30% of young growing athletes suffer from this pathology, described by Osgood and Schlatter in 1903. Objective: To report 2 observations and review the literature. Observation 1: This was a 14-year-old Guinean teenager, Halpoular, living in Conakry, a right-handed footballer who presented with a painful swelling of the right knee that looked mechanical, whose father had HLA B27 positive ankylosing spondylitis. X-ray of the right knee showed fragmentation of the anterior tibial tuberosities, and ossicle with edema of the patellar tendons at stage 4 according to Ehrenbord and Lagergren (1961). Thus, the disease of OSD is retained, He received 120 mg of diclofenac, 20 mg omeprazole for 1 month and the cessation of sports activity, the evolution was favorably marked by the regression of pain. Observation 2: A 16-year-old girl, initials M.L.T, Guinean, residing in Coyah (Republic of Guinea), a handball player, consulted for intense pain (VAS: 8/10) in her knees after a violent shock received when she hit the post. The physical examination found a painful, soft swelling at the anterior tibial tuberosity of the right knee, the rest of the examination was normal. The X-ray of the right knee shows fragmentation of the anterior tibial tuberosity, and ossicle with edema of the patellar tendon at stage 4 according to Ehrenbord and Lagergren 1961, thus the OSD disease is retained, the treatment instituted was 120 mg of diclofenac, the cessation of sports activity which gives a good evolution (EVA at 2/10). Conclusion: OSD is a growth osteochondrosis affecting the apophyseal cartilage of the anterior tibial tuberosity of the knee in young growing athletes, resolving at rest.

Introduction: Female Genital Mutilation (FGM) is a public health problem. There are 100 to 140 million girls and women who suffer every year in the world [1]. The aim of this study is to improve the medical care and reduce complications of FGM at the Regionalhospital of N’zérékoré, to determine their frequency, and to evaluate the maternal-fetalprognosis. Methods: The study was conducted at the Regional Hospital of N’zérékoré. This was a cross-sectional, descriptive and analytic study of 6 months, from 1 September 2016 to 28 February 2017, including all pregnant women admitted for childbirth who had a complication of female genital mutilation. Results: A total of 1295 women gave birth in the service, of which 1204 women were women with female genital mutilation. Given a frequency of 92.97%, of these 1204 mutilated women, 223 presented obstetrical complications during their delivery, a proportion of 17.22%. They were mostly young patients, mostly housewives who were not in school. Type II FGM was the most common (53.06%). Obstetric complications were dominated by complicated perinatal tears (54.08%), and hemorrhages (40.81%). The catch was dominated by perineorrhaphy. Conclusion: The frequency of FGM was 92.97% and that of their obstetric complications 17.22%. Most were house-wives, not in school. There was FGM type II. The abandonment of FGM would reduce maternal and perinatal morbidity and mortality.

As part of the determination of the possible impact of human activities on surface waters, case of Lake Sonfonia, six sampling sites were selected according to their solicitation by the population and their exposure to probable sources of pollution. The objective of this work is to monitor the physico-chemical quality of the waters of Lake Sonfonia during the low water level (March) and during the flood period (August) of the year 2021. Two sampling campaigns of water were carried out in dry weather and two others in cold weather. Parameters such as temperature, pH, EC (Electrical Conductivity), dissolved oxygen, TDS (Total Dissolved Solids) were measured in situ. Suspended matter, phosphate, nitrate, nitrite, sulphates, total iron, COD (Chemical Oxygen Demand) were measured in the laboratory by the colorimetric method. Stata 15 software was applied for the statistical analysis of the data and the correlation test between the parameters gave highly significant correlations. It has been noted that the situation is not very good and that this pollution comes mainly from human activities.

Introduction: the roots of Leptadenia hastata, (L hastata) are remedies from the Senegalese pharmacopoeia and are as widely used as the leaves. However, few researchers have devoted themselves to their toxicity, unlike the leaves. However, in the traditional pharmacopoeia, the indications for use are very different. The aim of our study was to study the effect of the administration of methanolic extracts of roots of L hastata on renal tissue, using an animal model. Materials and Methods: a cohort of 18 rats was studied with a random distribution of the animals in 3 groups (n = 6). The first group was the control group. The treated groups (Group II and III) received the methanolic extract of L. hastata with an administration of 500 mg / kg / day and 1000 mg / kg / day respectively, for 28 days. Results: The dose of 1000 mg / kg / day was lethal in group III, from the first week in females. Serum creatinine was significantly higher in rats given the root extract. There was kidney damage with vacuolar degeneration starting at 500 mg / kg / day. The lesions were more severe in group III with glomerular involvement characterized by retraction in the renal corpuscles. Conclusion: If the leaves of L hastata seem to be safe for consumption according to the majority of authors in the literature, the roots of this same plant could be harmful. In addition to the lethality observed at the doses tested, the lesions in the renal parenchyma would be dose-dependent.

Introduction: High blood pressure is a public health problem in Guinea as well as everywhere in Africa. The objective of our study was to assess the epidemiological, clinical and therapeutic aspects of High blood pressure in the general medicine department of the Nzérékoré regional hospital. Methodology:This was a prospective study of a descriptive type, lasting 06 months, from July 1 to December 31, 2014. Results: We recorded 156 cases of High blood pressure among a total of 570 patients, for a hospital frequency of 27%. The 56 - 65 age group was the most affected, at 29.4%, with a predominance of men (sex ratio: 1.5). Alcoholism was the most common cardiovascular risk factor with a frequency of 51.2%, smoking and physical inactivity were found in 35.8% and 30.7% respectively;psychosocial stress was found in 24.3% of patients. The clinic was dominated by headache (100%) and dizziness (57.6%). All our patients were subjected to a healthy diet. Fixed dual therapy was the most widely used treatment regimen, i.e. 91 patients (58.3%) and included a diuretic in 37.1% of cases, a calcium channel blocker in 25% of cases and an ACE inhibitor in 24.3%. Conclusion: This study revealed a high frequency of hypertension in the general medicine department of the regional hospital of N’zérékoré.

In African countries,regulations on the management of WTE(Waste from Information Technology Equipment)are non-existent or sometimes insufficient,if they *** study shows how to mention waste from information technology equipment in a country that does not have a regulatory legislative framework or organized management channels.A methodological approach based on documentary research and semi-directed interviews was adopted in this *** results showed that 54.55%of institutional managers are unaware of the existence of regulations for the management of WTE,both at the national and international levels;72.72%of these managers do not have a specific implementing text for this management;72.73%of institutions have adopted an acquisition policy based on the needs of workers and 81.82%of institutions store WTE in *** with waste collection SMEs(Small and Midsize Enterprises),especially for IT equipment,are limited for 72.73%of *** terms of improving the capacity to manage WEEE(Waste Electrical and Electronic Equipment),90.91%of institutions do not offer training or awareness ***,no significant indicators are in place to assess WEEE management in institutions,which reflects a lack of commitment and awareness among institutional *** addition,there are no financial means for ecological and sustainable *** 27.27%of managers are aware of the dangers to human health and the environment related to IT equipment,they are often unaware of the harmful consequences of abandoning it in nature.
