Contribution of Imaging in the Diagnosis and Management of Type 1 Autoimmune Pancreatitis Associated with Main Bile Duct Cholangitis: A Case Report

Introduction: Autoimmune pancreatitis type 1 (AIP) is a rare condition characterized by inflammatory and fibrosing involvement of the pancreas, associated with elevated serum IgG4 and potential involvement of other organs. It often mimics a malignant tumor with which the differential diagnosis is often difficult. Observation: We report a case of AIP type 1, in a 70-year-old patient, with a heavy history of arterial hypertension, insulin-induced type II diabetes, overweight, gastric ulcer, ischemic heart disease with triple bypass, COPD, revealed by intense abdominal pain and acute evolution. The diagnosis was evoked on imaging, in front of an aspect of “sausage pancreas”, with delayed contrast enhancement of the pancreas, loss of lobulations, and the presence of a fibrous peripancreatic shell. The biological and histological assessment confirmed the diagnosis and avoided inappropriate surgical treatment. Conclusion: AIP type 1 is a rare pathology whose diagnosis is difficult. Only a perfectly interpreted imagery can evoke it. The biological assessment, in particular, the elevation of the serum level of IgG4 and the histology make it possible to confirm the diagnosis. The challenge in its management is to avoid unnecessary surgery and/or inappropriate treatments. Imaging, in this context, plays a preponderant role, in particular, thanks to the dynamic injection sequences of gadolinium chelates on MRI which suggest the fibro-inflammatory nature of pancreatic lesions.

Umbilical Artery Resistance Index in the Surveillance of Pregnancies at Risk of Fetal-Maternal Hemodynamic Disorders in Lomé(Togo)

Objective: Evaluate the role of umbilical artery Doppler ultrasound in the surveillance of pregnancies at risk of vascular disorders in Lomé. Study Method: The study was a cross-sectional analytical study carried out in the department of radiology of Campus teaching hospital over a period of 6 months. This study was based on the measurement of the resistance index of the umbilical artery in pregnant women presenting vascular risk and other pregnant women without vascular risk. The correlation between the pathological index and the at-risk pregnancies was assessed by the Odds Ratio as well as the correlation between the resistance index and the Apgar score at birth. Results: The resistance index was measured in 209 at-risk pregnant women and in 425 pregnant women without vascular risk. The average age of pregnant women was 26.31 years for “the at-risk pregnant” versus 25.38 years for the “pregnant-witnesses”. The association between the pathological resistance index (RI) and the gestational pathologies studied, had been positive and significant with an odds ratio of 1.57 for a 95% confidence interval of [1.07 - 2.20]. A pathological RI is a risk factor for the occurrence of a pathological Apgar score at birth because this association was positive and significant for “pregnant-cases” as for “pregnant-witnesses”. Conclusion: Measuring the index of resistance is not a common practice in our communities. However, it could be an important tool in the surveillance of at-risk pregnancies for diseases such as malaria, pre-eclampsia, and maternal anemia.

Aetiological Diagnosis of Infertility at Conakry University Hospital: Role of Hysterosalpingography and Pelvic Ultrasound

Objective: The objective of this study was to investigate the etiologies of infertility and to determine the contribution of hysterosalpingography coupled with ultrasound in the exploration of female infertility at Donka University Hospital. Methodology: This was a prospective descriptive cross-sectional study carried out in the Radiology Department of Donka National Hospital over a period of ten (10) months. It involved 78 women who came to the department for hysterosalpingography and/or pelvic ultrasound examinations, as part of the exploration of infertility. Data collection involved the use of pre-established survey forms to gather information on the parameters studied. Sociodemographic parameters, ultrasound and hysterosalpingography results were studied. A correlation was made between age at marriage and infertility to determine whether early marriage has an impact on primary infertility, with a statically significant result for p value greater than 0.05. Results: The mean age of our patients was 33.7 ± 5.6 years, with extremes of 18 and 35 years. The 18-35 age group was the most represented, with a frequency of 80.7%. The 34% of our patients were married before the age of 18, with a marriage duration ranging from 6 months to 15 years. The indication for investigations was dominated by secondary infertility, with a frequency of 65%, followed by primary infertility (35%). All our women underwent ultrasound-hysterosalpingography, i.e. 100%, in search of the cause of infertility. Ultrasound was pathological in 35.8%. The most common ultrasound lesions were myomas and ovarian dystrophies, with 12.8% each. However, hysterosalpingography was pathological in 35%. Tubal obstructions affected almost a third of our women (29.5%), followed by phimosis and tubo-peritoneal adhesions. Conclusion: Diagnostic evaluation of infertility requires a multidisciplinary approach, including collaboration between infertility gynecologists, radiologists and other infertility specialist

Compliance of Magnetic Resonance Imaging Examination Requests at the Diagnostic Center of the National Social Security Fund of Conakry

Introduction: MRI is a rapidly growing technique with more and more indica-tions and requests in the Republic of Guinea. Its correct prescription is a guar-antee for the satisfaction of the actors, both prescribers, radiologists and pa-tients. The main objective of this study was to evaluate the compliance of MRI examination requests at the Diagnostic Center of the National Social Se-curity Fund (CNSS) in Conakry. Material and Methods: This was a descriptive cross-sectional study of MRI prescription forms sent to the MRI unit of the CNSS Diagnostic Center from February 1 to May 1, 2021. The 8 compliance criteria established by the French High Authority for Health were used to evaluate the compliance of the examination requests. Results: A total of 7003 examination forms were sent to the facility, including 7% (n = 468) of MRIs. 56.2% of MRI requests were performed by specialists. We observed an overall compliance of 10%. Administrative and clinical compliance were missing in 24% and 38%, respectively. More specifically, the purpose of the examination was not mentioned in 60%, followed by the requesting department in 48.1% and the patient’s age in 35.1%. Conclusion: This study allowed us to highlight the gaps in establishing MRI requests. It would be important to organize an awareness campaign for prescribers on the usefulness of correctly filling an MRI request and to design templates to be filled out by prescribers.

Relevance of Medical Imaging in the Pre-Therapeutic Evaluation of Acute Intestinal Obstruction at CIMED

Objective: We initiated this work with the aim of studying the contribution of imaging in the diagnosis of acute intestinal obstruction at CIMED. Patients and methods: This was a prospective, descriptive and analytical study involving 96 patients collected at the radiology and medical imaging department of CIMED, from January 2022 to January 2023. Result: The age of our patients varied between 11 and 86 years with an average age of 36 years. There was a male predominance of 64.6% compared to 35.4% for women, i.e. a sex ratio of 1.82. The notion of previous surgery was found in 61.5% of our patients. Pain was present in all patients. Radiography of the ASP was performed in 89.6% of patients. It showed hydro-aerial levels in 96.5% of patients. Abdominopelvic CT was performed in 12 patients and made it possible to make the diagnosis of occlusion in all patients. The results of the positive diagnosis were concordant with those intraoperatively in 92% of cases. 8% of our patients, compared to the treatment, spontaneously resumed their transit, 91% benefited from surgical treatment and 1% died before surgery. The outcome was favorable in 80 patients or 83.3%, poor with death in 16 patients or 16.7% of cases. Conclusion: Acute intestinal obstruction remains a serious pathology for which ASP radiography often remains the only radiological examination performed urgently. However, abdominopelvic CT seems widely indicated thanks to its contribution both for the positive diagnosis and for the serious and etiological diagnoses. However, this imaging technique is largely underused in our practice due to its high cost and lack of availability.

Post-COVID-19 Late Pulmonary Embolism in a Young Woman about a Case

COVID-19 appears to be associated with a disproportionate risk of thrombosis. The occurrence of thrombosis is a frequent complication of many medical and surgical conditions. Their prevention by LMWH is the option of choice as well as their early diagnosis in this pandemic context. In addition, several recent observations support that severe pulmonary embolism is very common in hospitalized patients with COVID-19. We report the case of late pulmonary embolism in a 26-year-old patient with a medical history of (SARS-CoV2) and newly discovered diabetes. The clinical examination found exertional dyspnea, intense chest pain, blood pressure at 145/85mmHg, heart rate at 129 bpm, respiratory rate at 25 cycles/min, blood sugar at 1.45 g/l, oxygen saturation at 92%. In front of this clinical picture, the thoracic CT angiography to ask shows a pulmonary embolism of the right segmental and sub-segmental branches less marked on the left, involving the lingular and basal branches; the management was limited to anticoagulation associated with rehabilitation sessions (respiratory physiotherapy).

Ectopic Pelvic Leiomyoma and Diagnostic Difficulty in Imaging: A Case Report from Conakry, Guinea

Introduction: Fibromyomas are benign tumours of frequent uterine location, most often found in black women, whose diagnosis of certainty is histological. The extra uterine location is rare and the physio-pathogenesis is poorly understood. This location poses diagnostic problems on imaging. Management is based on tumour resection. Observation: The authors report a clinical case of a 31-year-old woman who was seen for non-febrile hypogastric pain with a large abdominopelvic mass on physical examination. There was no particular history. The biology did not show any abnormality. Ultrasound with Doppler found a mass that was difficult to characterize, and was completed by a CT scan and a pelvic MRI. The management was surgical with an evolution marked by complications. Conclusion: Ectopic leiomyoma is a rare condition. This location poses diagnostic problems on imaging. Management depends on the type of extra uterine presentation of the myoma.

自中非共和国(CAR)危机爆发以来,中非国内暴力冲突日益加剧。尽管有国际维和部队的帮助,武装组织仍盘踞在各个省份。这就是为什么研究人员认为中非当局及其国际伙伴无法阻止暴力事件的增长,解决危机的有效方法遥遥无期。为什么失败,有人认为,中非政府和联合国的将解决危机的重点放在解除反对派武装,复员其成员,让他们重新融入社会;除此,维和部队无法在对反对派的管理中建立适当的平衡,以及政府没能有效应对国家分裂的紧张局势,也是重要的原因。其他一些原因可能是各种国际斡旋者之间存在分歧和竞争,导致各种解决危机的方案一再被拖延,反而不利于危机的解决。针对以上情况,本文通过研究,意在向中非当局及其国际伙伴提出可持续的有效建议。本文认为,结束中非共和国危机的重点必须对反对派施压,而不是无休无止的谈判——一方面,采取措施切断反对派的资金来源,并保持强大的军事威慑;另一方面,应该制定方案,重新获得普通百姓的信任和支持。本文分为五个部分:第一章主要介绍了中非共和国危机的背景,综述了研究文献,简介了拟采用的理论及研究方法。第二章分析了中非共和国危机的起源,导致中非共和国危机的原因之中潜伏着解决方案,因此这一章的内容与第四章有呼应关系。第三章则是在另一个层次上,分析了在国内外和平努力之下,中非共和国危机没有缓解反而加剧了的原因,包括宗教、种族冲突及普遍存在的敌意氛围等。这样安排的用意很明显,危机的起因与危机持续加剧的原因显然是有区别的。这也说明,要彻底解决危机,也必须从这两个层次上去化解矛盾。第四章分析了中非共和国政府及国际社会解决危机的种种努力,并突出了中非当局目前正在实施的方案措施。第五章提出了解决中非共和国危机的建议。作者深信,如果这些建议得以执行,中非可能会获得真正需要的和平。

Distribution of fluoride in surface water and a health risk assessment in the upper reaches of the Yongding River

The excessive exposure to high concentrations of fluoride in drinking water can lead to a serious disease called *** upstream region of the Yongding River is an ecological protection area for *** studies have reported that there is a high concentration of fluoride in the groundwater in this ***,there are few data on the distribution of fluoride in surface water and health risk assessments in this *** this study,the fluoride concentrations were determined by using the spectrophotometric method using data from 2013 to 2017 from 9 surface water quality monitoring stations in the upper reaches of the Yongding *** health risks of fluoride were assessed using the approach developed by the United States Environmental Protection Agency(US EPA).The results indicated that the fluoride content in the drinking water ranged from 0.30 to 1.50 mg L 1,with an average of 0.86 mg L"*** total,22.7%of the analyzed samples exceeded the Chinese limit of 1.0 mg L"1 for fluoride,and 5.5%of samples had less than the permissible limit of 0.5 mg L"*** fluoride concentrations and fluorosis hotspots were found to be predominately located downstream of the Yanghe River close to the Guanting Reservoir,where 71.4%of samples exceeded the limit of 1.0 mg/L'*** spatial distribution of high fluoride concentrations was found to be primarily determined by *** hazard quotient(HQ)index for children,teenagers and adults indicated that 19.6%,15.6%,and 5.1%of the samples in the upper reaches of the Yongding River,respectively,posed health hazards to the associated ***,the HQ index more than 1 for children,teenagers and adults had values of 64.3%,56.1%,and 19.4%,respectively,in samples from the downstream region of the Yanghe ***,there are potential risks of dental and skeletal fluorosis in the upper river reaches of the Yongding *** is imperative to take measures to reduce the fluoride pollution in surface water and contro

Conformity Assessment of Lead Aprons Used in Conventional Radiology: A Multi-Centre Survey in Togo

Aim: Lead aprons are used to protect against scattered radiation from the patient during interventional procedures and certain special conventional radiological examinations. Given the importance of the role lead aprons are supposed to play in radiation protection, we propose to assess their conformity in medical imaging departments in public and religious hospitals in Togo. Materials and method: A multi-centre survey conducted from 26 November to 06 December 2021 in the radiology departments of public and religious health facilities in Togo. All aprons in use were included. The evaluation criteria were physical (visual), quantitative (radiographic) and qualitative (dosimetric). Results: We had registred 43 aprons among wich 27 (62.79%) leaded aprons were labelled non-compliant and 16 (37.21%) were labelled compliant. Of the aprons judged to be non-compliant, 70.37% were more than 10 years old and 96.30% showed defects on the radiographic images. The most common defects were vampire marks (18.64%), multiple folds (16.96%), cracks (16.96%), multiple cracks (15.25%), tears (8.47%), absence of lead (5.08%), holes (3.39%) and lead corrosion (1.69%). Defective aprons (62.96%) had at least two defects. The defects were of thoraco-abdomino-pelvic (74.07%), thoracic (14.82%) and abdomino-pelvic (11.11%) topography. For indirect exposure at 50 and 70 kilovolts, all the aprons had an attenuation factor greater than 90%. After dosimetric measurement, 13.95% of aprons had attenuation factors below 90% for indirect exposure at 100 kilovolts. Conclusion: The compliance of the leaded decks is trifactorial (physical, radiographic and dosimetric). However, there is no significant difference in X-ray attenuation capacity between defective and normal decks.
