BACKGROUND Nonalcoholic steatohepatitis-related cirrhosis is one of the liver complications in type 2 diabetes mellitus(t2DM)and reported to be a risk factor for developing hepatocellular carcinoma(HCC).A reliable screening biomarker of liver cirrhosis(LC)and HCC among t2DM patients is important to reduce the morbidity and mortality of this ***(miRNA)is considered a key player in HCC and t2DM,and it might be a hidden culprit in diabetes-associated HCC,making it a promising reliable prognostic *** to investigate the signature of serum miRNAs as early biomarkers for the screening of HCC among diabetic *** Expression profiles of miRNAs in serum samples of diabetic LC and diabetic HCC patients were assessed using Illumina sequencing;then,Rt-qPCR was used to validate significantly altered miRNAs between the two *** miRNAs were tested in serum samples of 200 t2DM patients,270 LC patients,200 HCC patients,and 225 healthy control ***,receiver operating characteristic(ROC)analysis,with area under the curve(AUC),was performed to assess the diagnostic performance of the screened miRNAs for discriminating HCC from LC and nonmalignant patients(LC+t2DM).RESULtS Expression of the sequenced miRNAs in serum was different in HCC vs LCpositive t2DM *** miRNAs(miR-34a,miR-221)were significantly upregulated and five miRNAs(miR-16,miR-23-3p,miR-122-5p,miR-198,miR-199a-3p)were significantly down-regulated in HCC compared to LC *** of ROC curve demonstrated that the combination of these seven miRNAs can be used as a reliable biomarker for detection of HCC in diabetic patients,as it could identify HCC with high diagnostic accuracy in diabetic LC patients(AUC=0.993)and in diabetic nonmalignant patients(AUC=0.961).CONCLUSION this study validates a panel of serum miRNAs that can be used as a reliable noninvasive screening biomarker of HCC among t2DM cirrhotic and noncirrhotic *** study recommends furt

BACKGROUND Healthy vestibular system adjusts balance during static and dynamic *** is important for normal development(standing up and walking).Vestipulopathies(central and peripheral)are common complications of diabetes in adult *** studies are scare in children with type 1 diabetes(t1D).AIM to assess saccular function of otolith organ in children with t1D and predictors for its *** Cervical vestibular evoked myogenic potential(cVEMP)was used for objective *** the study included 40 patients(boys=15;girls=25).Patients had mean age of 13.63±1.50 years,duration of diabetes of 5.62±2.80 years,frequent attacks of diabetic ketoacidosis(55%)and hypoglycemia(30%),hyperlipidemia(20%),hypertension(12.5%)and peripheral neuropathy(40%).Dizziness was found in 10%.Compared to healthy children(n=25),patients had prolonged cVEMP P1 and N1 latencies and reduced P1-N1 *** cVEMP abnormalities were found in 60%(vs 25%for unilateral abnormalities).Higher frequencies and severe vestibulopathies were found with chronic diabetes of>5 years,hemoglobin A1c values>7%,frequent diabetic ketoacidosis and hypoglycemic attacks and presence of *** analyses showed that predictors for prolonged P1 latencies and reduced P1-N1 amplitudes were only chronic diabetes(>5 years){odds ratio(OR)=2.80[95%confidence interval(CI):1.80–5.33],P=0.01;OR=3.42(95%CI:2.82–6.81)}and its severity(hemoglobin A1c>7%)[OR=3.05(95%CI:2.55–6.82),P=0.01;OR=4.20(95%CI:3.55–8.50),P=0.001].CONCLUSION Dysfunction or injury of the saccular macula and its pathways is prevalent in children with *** glycemic control is important to prevent diabetes related vestipulopathies.

the perfect combination of renewable energy and desalination technologies is the key to meeting water demands in a cost-effective,efficient and environmentally friendly *** desalination technique by humidificationdehumidification is non-conventional approach suitable for areas with low infrastructure(such as rural and decentralized regions)since it does not require permanent *** this study,this technology is implemented by using solar energy as a source of thermal power.A seawater desalination unit is considered,which consists of a chamber with two evaporators(humidifiers),a wetted porous material made of a corrugated cellulose cardboard and a condenser(dehumidifier).the evaporation system is tested with dry bulb temperature and relative air humidity *** results of numerical simulations indicate that higher inlet air velocities(from 0.75 to 3 m/s)lead to a decrease in theΔt,ΔRH,and *** the air remaining within the evaporator for 30 cm,the temperature differential increases to 5.7°C,accompanied by a 39%rise in relative humidity *** changes result in a significant enhancement in humidification efficiency,achieving a remarkable efficiency level of 78%.However,a wettability value of 630 m^(2)/m^(3)leads to a smaller reduction of these *** the pad thickness,particularly to 0.3 m,improves performance by boostingΔt,ΔRH,and effectiveness,especially for pads with a wettability of 630 m^(2)/m^(3),for which superior performances are predicted by the numerical tests.

BACKGROUND Monomorphic epitheliotropic intestinal t-cell lymphoma(MEItL)is a rare and rapidly progressive intestinal t-cell non-Hodgkin lymphoma associated with a very poor prognosis and a median survival of 7 *** in the identification of MEItL over the last two decades have led to its recognition as a separate *** patients,predominantly male,typically present with vague and nonspecific symptoms and diagnosis is predominantly confirmed at ***,there are no standardized treatment protocols,and the optimal therapy remains *** SUMMARY We report a case of MEItL that was initially considered to be gastrointestinal stromal tumor(GISt)and Imatinib was administered for one *** 62-yearold man presented with abdominal pain,abdominal distension,and weight loss of 20 *** 2 wk,the size of the mass considerably increased on computed tomography *** patient underwent surgery followed by chemotherapy with CHOP(cyclophosphamide,doxorubicin,vincristine,and prednisone)and stem-cell transplant.A correct diagnosis of MEItL was established based on postoperative ***,the neoplastic cells fulfilled the diagnostic criteria for MEItL as they were CD3+,CD4+,CD8+,CD56+,and tIA-1+.CONCLUSION Given that MEItL has no predisposing factor and presents with vague symptoms with rapid progression,the concomitant presence of abdominal symptoms and B symptoms(weight loss,fever,and night sweats)with hypoalbuminemia,anemia,low lymphocytic count and endoscopic findings of diffuse infiltrating type lesions should alert physicians to this rare disease,especially when it comes to Asian *** laparotomy should then be carried out followed by chemotherapy and stem-cell transplant.

针对化工事故致因分析较少考虑利益相关者间复杂耦合关系的问题,首先,以2016—2021年间80个典型的化工事故为案例,提取出6个利益相关者和26个相关的风险因素,构建风险矩阵,利用Netdraw软件可视化矩阵,得到化工安全风险网络图;然后,整体层面上,运用社会网络分析(SNA)中的块模型密度矩阵和像矩阵得到核心块中的7个风险因素;个体层面上,运用节点度值和中心度确定9个风险因素;将整体和个体层面筛选出的风险因素取交集,确定网络中的4个核心风险因素;最后,从整体网络密度和聚类系数2方面检测管控后的风险网络效果。研究结果表明:核心风险因素为违章作业,化工企业不具备安全生产条件,安全评价机构预评价、现状验收评价不到位,材料设备性能、质量不足。管控核心风险因素后,新风险网络的密度降低35%,聚类系数降低21%,有效降低了因某一关键风险的引发而导致化工事故的可能性。

A Quasi Experiment to Implement Multimodal Strategy to Improve Hand Hygiene Behavior in a Healthcare Facility in Central Saudi Arabia

Background: Hand hygiene (HH) compliance of healthcare workers (HCWs) remains suboptimal despite standard multimodal promotion, and evidence for the effectiveness of novel interventions is urgently needed. Aim: Improve HCWs’ HH compliance toward minimizing healthcare associated infection (HCAI) risk in Wadi Al Dawasir Hospital (WDH), central Kingdom of Saudi Arabia (KSA). Methodology: A quasi experimental approach was adopted to achieve study aim. the HCWs’ behavior of HH during the duration between 2015 and 2016 was evaluated before and after a HH educational plan based on the World Health Organization (WHO) “Multimodal HH Improvement Strategy” (MMHHIS). the HCWs’ compliance in response to HH indications represented by the WHO’s “My 5-Moments for HH” and the type of HH action taken, whether hand washing (HW) or hand-rubbing (HR) were analyzed. Results: the number of opportunities observed of HH performance accounted 230 in 2015 (pre-education), and 237 in 2016 (post-education). the HCWs’ HH compliance rate in the pre-education phase did not vary by the 5-moment indications [χ2(df 4) = 0.01, p = 0.98]. Conversely, the compliance rate after HH education was higher than non-compliance across all 5-moment indication opportunities (ranged between 57.0% up to 88.9%) [χ2(df 1) = 18.25, p rd and 4th 5-moment indications (“after body fluid exposure” and “after patient contact,” respectively) were met with a significant HH improvement [χ2(df 1) = 8.98, p = 0.003;and χ2(df 1) = 16.3, p −4.38, p = 0.001). Only physicians and nurses showed a significant “within-profession” improvement in HH compliance after education, compared to that before education (Z = −3.51, p = 0.001, Z = −2.48, p = 0.013, respectively). Conclusions: Applying a HH education plan based on standardized multimodal HH strategy proved effective in improving the HH compliance of the hospital’s staff. Hepatocellular carcinoma(HCC)is the sixth most common cancer in the *** majority of patients with HCC present with unresectable *** patients have historically had limited treatment options secondary to HCC demonstrating chemoresistance to the currently available systemic ***, normal liver parenchyma has shown intolerance to tumoricidal radiation doses,limiting the use of external beam *** of these limitations,novel percutaneous liver-directed therapies have emerged. the targeted infusion of radioactive microspheres (radioembolization)represents one such therapy. Radioembolization is a minimally invasive transcatheter therapy through which radioactive microspheres are infused into the hepatic arteries that supply tumor. Once infused,these microspheres traverse the hepatic vascular plexus and selectively implant within the tumor *** within the arterioles, the 90Y impregnated microspheres emit high energy and low penetrating radiation doses selectively to the *** has recently shown promise for the treatment of patients with unresectable HCC. the objective of this review article is to highlight twocurrently available radioembolic devices(90Y,188Rh)and provide the reader with a recent review of the literature.
