Does Rifaximin Improve EEG and VEP in Egyptian Cirrhotic Patients with Minimal Hepatic Encephalopathy?

Background: Minimal hepatic Encephalopathy (MHE) is defined as HE without symptoms on clinical/neurological examination, but with deficits in some cognitive areas that can only be measured by neuropsychometric testing. However, numerous studies have shown that, although the neurological symptoms are slight, affected patients are markedly impaired in their quality of life and ability to work. Various treatment modalities that have been shown to reverse MHE include lactulose/lactitol, probiotics/synbiotics, L-carnitine but rifaximin has shown a general trend toward better efficacy and better tolerability in patients with overt hepatic encephalopathy (OHE). Objective: Our objective is to assess the diagnostic role of minimental state examination (MMSE), electroencephalography (EEG) and visual evoked potential (VEP) in detection of MHE and to evaluate the efficacy of rifaximin in improving EEG and VEP in patients with MHE. Patients and Methods: Sixty cirrhotic patients were enrolled in the study depending on clinical evidence of stigmata of chronic liver disease, laboratory investigations including liver function tests, ultrasonographic features of liver cirrhosis and with no evidence of overt hepatic encephalopathy. Diagnois of MHE was made depending on minimental state examination (MMSE) and neurophysiological tools including EEG and VEP. A control group of sixty healthy volunteers with age and sex matched were included. The patient group received Rifaximin 550 mg twice daily for 8 weeks then follow up EEG and VEP studies were done. Results: MHE was detected in 36.7%, 48.3%, 51.7% of our series based on MMSE, EEG and VEP respectively. Child Pouph A, B, C was found in 51.7%, 35%, 13.3% respectively. Rifaximin was well tolerated. At the end of treatment, EEG and VEP studies were done which showed signficant changes between pre and post treatment results (P value = 0.03, 0.001, Conclusion: MMSE as well as EEG and VEP were reasonable diagnostic tools for early detection o

Recently,much interest has been given tomulti-granulation rough sets (MGRS), and various types ofMGRSmodelshave been developed from different viewpoints. In this paper, we introduce two techniques for the classificationof MGRS. Firstly, we generate multi-topologies from multi-relations defined in the universe. Hence, a novelapproximation space is established by leveraging the underlying topological structure. Cooling system design for thermal management of electronic equipment,batteries and photovoltaic(PV)modules is important for increasing the efﬁciency,safety operation,and long life span the *** the present study,two different cooling systems are proposed with nano-enhanced multiple impinging jets for a conductive *** present cooling systems can be used in electronic cooling and PV *** porous object(PPO)and sinusoidal porous object(SPO)are used in the jet cooling system.2D numerical analysis usingﬁnite volume method is conducted considering different values of permeability of the objects(Darcy number(Da)between 10^(-6) and 10^(-1)).When PPO is used in the cooling system,num-ber of cylinders(between 1 and 6),and size of the cylinders(between 0.015 and 0.075)are *** the case of using SPO,amplitude(between 0.1 and 2)and wave number(be-tween 1 and 12)are ***-water nanoﬂuid with cylindrical shaped nanoparticles is used as the heat transferﬂ*** permeability is changed for PPO,the average temper-ature increases by roughly 3.89℃ for a single cylinder and drops by roughly 0.57℃ for a six-cylinder *** the size of the cylinder in the PPO case at highest permeability results in temperature drop of 5.3℃.When changing the number of cylinders,cooling rate varies by about 3.6%.Wave number of SPO is more inﬂuential on the cooling performance enhancement as compared to amplitude and permeability of the *** average surface temperature drops by 12.4℃ when the wave number is increased to *** compared to reference case of jet impingement cooling without porous object,using PPO and SPO along with the nanoﬂuid result in temperature drop of 12.3℃ and 14.4℃.

Enhancing Canola Yield and Photosynthesis under Water Stress with Hydrogel Polymers

While Egypt’s canola production per unit area has recently grown,productivity remains low,necessitating increased *** are water-absorbent polymer compounds that can optimize irrigation schedules by increasing the soil’s ability to retain ***,twoﬁeld experiments were conducted to examine hydrogel application to sandy soil on canola growth,biochemical aspects,yield,yield traits,and nutritional quality of yielded seeds grown under water deﬁcit stress *** experiments were conducted by arranging a split-plot layout in a randomized complete block design(RCBD)with three times replications of each *** water stress at 75%or 50%of crop evapotranspiration(ETc)lowered chlorophyll a,chlorophyll b,caro-tenoids,and total pigments content,indole-3-acetic acid,plant development,seed yield,and oil and total carbo-hydrates of seed yield,hydrogel treatment enhanced all of the traits mentioned ***,hydrogel enhanced to gather compatible solutes(proline,amino acids,total soluble sugars),phenolics content in leaves,seed protein,and crop water productivity,which increased while the plants were under water *** results revealed that the full irrigation(100%ETc)along with hydrogel compared to water-stressed(50%ETc)led to enhanced seed yield(kg ha^(-1)),Oil(%),and Total carbohydrates(%)of rapeseed by 57.1%,11.1%and 15.7%,***,under water-stressed plots with hydrogel exhibited enhancement by 10.0%,3.2%and 5.1%in seed yield(kg ha^(-1)),oil(%),and total carbohydrates(%)of rapeseed by 57.1%,11.1%and 15.7%,respec-tively compared to *** a result,the use of hydrogel polymer will be a viable and practical solution for increasing agricultural output under water deﬁcit stress situations.

Phytochemical profiling and anticancer potential of Cymbopogon citratus extract

Objective:To evaluate the anticancer potential of Cymbopogon citratus ***:GC-MS analysis was used to identify phytocomponents in the methanolic extract of Cymbopogon citratus.A fractionation method was employed to isolate and assess the bioactivity of different fractions and their cytotoxic activities against cancer cell lines HCT116,LoVo,Caco-2,and HT-29 were investigated.A dual staining method with acridine orange and ethidium bromide was used to assess the effect of the extract on cell ***,the expression levels of Bax and TP53 were quantified using real-time PCR in Caco-2 cells treated with the ethyl acetate fraction of Cymbopogon citratus extract.A protein array was employed to profile key pro-and anti-apoptotic proteins in Caco-2 ***,molecular docking studies were conducted to investigate the interactions between key compounds of Cymbopogon citratus extract and specific apoptosis-related protein domains(PDB IDs:7wql and 4bkx).Results:A significant growth inhibition was observed in Caco-2 cells treated with Cymbopogon citratus *** the seven fractions of the plant extract,the ethyl acetate fraction showed the highest cytotoxicity against Caco-2 cells with an IC50 value of(6.16±0.01)μg/*** immunofluorescence assay showed that the ethyl acetate fraction could induce apoptosis of Caco-2 ***,the fraction upregulated the gene expressions of Bax and TP53 in a dose-dependent *** docking analysis demonstrated the interaction of five compounds isolated from the ethyl acetate fraction with key proteins in Caco-2 cells,indicating their anticancer ***:Cymbopogon citratus extract shows anticancer activity against Caco-2 cells by inducing *** may be a promising candidate for the treatment of colon cancer,which needs further investigation.

Sodium-glucose cotransporter-2 inhibitors (SGLT-2 inhibitors) have transformed diabetes management by targeting renal glucose reabsorption. Designed initially as antidiabetic agents, their ability to lower blood glucose levels independently of insulin is well-documented. Beyond glycemic control, emerging research has unveiled their profound cardiorenal benefits. By inhibiting SGLT-2 protein, these drugs enhance glucose excretion in urine, reducing blood glucose levels. This mechanism has translated into significant cardiovascular and renal protection, establishing SGLT-2 inhibitors as pivotal in managing not only diabetes but also cardiovascular and renal diseases. Recent studies have illuminated the broader therapeutic potential of SGLT-2 inhibitors beyond diabetes. Evidence indicates their efficacy in managing heart failure, chronic kidney disease (CKD), and cardiovascular complications in individuals with or without diabetes. This expanded therapeutic landscape has catalyzed a paradigm shift in SGLT-2 inhibitor use, positioning them as key agents in the cardiorenal metabolic continuum. Moreover, their role in the secondary prevention of cardiovascular events and slowing CKD progression in T2DM patients has garnered considerable attention. This consensus-based review aims to offer practical guidance in an algorithmic approach to primary care healthcare professionals to optimize SGLT-2 inhibitors utilization and maximize their benefits. The review seeks to empower clinicians to effectively manage patients who may benefit from SGLT-2 inhibitor therapy by addressing common initiation barriers and optimizing treatment strategies. Additionally, it aims to raise awareness among primary care physicians regarding the multifaceted benefits of these medications and overcome clinical inertia in their adoption into routine clinical practice.
