Do Not Resuscitate status as an independent risk factor for patients undergoing surgery for hip fracture

AIm To determine morbidity and mortality in this specific patient group and also to assess for any independent associations between Do Not Resuscitate(DNR) status and increased post-operative morbidity and *** We conducted a propensity score matched retrospective analysis using de-identified data from the American College of Surgeons' National Surgical Quality Improvement Project(ACS NSQIP) for all patients undergoing hip fracture surgery over a 7 year period in hospitals across the United States enrolled in ACS NSQIP with and without Do Not Resuscitate Status. We measured patient demographics including DNR status, co-morbidities, frailty and functional baseline, surgical and anaestheticprocedure data, post-operative morbidity/complications, length of stay, discharge destination and *** Of 9218 patients meeting the inclusion criteria, 13.6% had a DNR status, 86.4% did not. mortality was higher in the DNR status compared to the non-DNR group, at 15.3% vs 8.1% and propensity score matched multivariable analysis demonstrated that DNR status was independently associated with mortality(OR = 2.04, 95%CI: 1.46-2.86, P matched cohort demonstrated that DNR status was associated with a significant, but very small increased likelihood of post-operative complications(0.53 vs 0.43 complications per episode; OR = 1.21; 95%CI: 1.04-1.41, P = 0.004). Cardiopulmonary resuscitation and unplanned reintubation were significantly less likely in patients with DNR *** Whilst DNR status patients had higher rates of postoperative complications and mortality, DNR status itself was not otherwise associated with increased morbidity. DNR status appears to increase 30-d mortality via ceilings of care in keeping with a DNR status, including withholding reintubation and cardiopulmonary resuscitation.

A Retrospective Study of Paediatric Dental Patients Treated under General Anesthesia

Dental care under general anesthesia (DGA) was found to be a safe, efficient and effective quality treatment for children. Purpose: To describe the characteristics of child dental patients treated under DGA and describe the indications of the treatments and types of treatments provided. method: The sample consisted of 40 patients treated under DGA at Department of Paediatric Dentistry of school and Hospital of Stomatology of Wuhan University between June 2011 and December 2012. Detailed information was collected from dental records. SPSS software package was used for statistical analysis. Results: The age of the patients ranged from 2.5 years to 24.1 years. males were more common than females m:F 1.5:1 .The mental retardation patients accounted for (5%) of the sample. The most common indication was inability to cooperate and accept dental treatment under local anesthesia (95%). The treatments rendered included: caries restorations (37%), root canal treatments (34%), extractions (7%), fluoride application (6%), fissure sealants (6%), indirect pulp capping (5%), stainless steel crowns (4%), pulpotomies (0.8%) and labial frenectomy (0.1%). In the follow-up visit fillings were found to be lost in 3 patients, with 2 anterior teeth and one posterior tooth. Conclusion: Caries restoration and root canal treatments were the most common treatments provided. Preventive strategies should be targeted toward children to reduce the number of healthy children receiving treatment under general anesthesia. Clinical significance: This study provides baseline information regarding the types of treatments for paediatric dental patients inWuhanand it is hoped that the data from this study will be useful for other researchers.

Vulnerability Assessment Using Raster Calculation and DRASTIC model for the Jordan Valley Subsurface Basin (AB1) Imaging maps

The vulnerability to pollution of the area between Wadi Shueib and the Dead Sea in the Jordan Valley (AB1) subsurface basis was assessed and evaluated using raster calculation and DRASTIC model for imaging maps in this research. The seven DRASTIC model parameters are: Depth to water, net Recharge, Aquifer media, Soil media, Topography, Impact of vadose zone and Hydraulic conductivity. The seven variables are evaluated by the rating and the weighting numerical indexes. The vulnerability parameters are categorized depending on a fixed interval of suburban area percentage. It showed that the ABI Subsurface was categorized by high vulnerability classes while the middle and western parts were categorized by high to extreme vulnerability classes. The southern part of the AB1 displayed low aquifer vulnerability. The vulnerability map shows the high risk suffered by the middle and western parts of the AB1 Subbasin due to the high possibility pollution of intensive fruit and vegetable cultivation.

The"resource availability hypothesis"predicts occurrence of larger rodents in more productive *** prediction was tested in a dataset of 1,301 rodent *** used adult body mass as a measure of body size and normalized difference vegetation index(NDVI)as a measure of habitat *** utilized a cross-species approach to investigate the association between these *** was done at both the order level(Rodentia)and at narrower taxonomic *** applied phylogenetic generalized least squares(PGLS)to correct for phylogenetic *** relationship between body mas and NDVI was also investigated across rodent *** controlled for spatial autocorrelation using generalized least squares(GLS)*** cross-species approach found extremely low support for the resource availability *** was reflected by a weak positive association between body mass and NDVI at the order *** find a positive association in only a minority of rodent *** best fit GLS model detected no significant association between body mass and NDVI across ***,our results do not support the view that resource availability plays a major role in explaining geographic variation in rodent body size.

Adversarial Attacks on Featureless Deep Learning malicious URLs Detection

Detecting malicious Uniform Resource Locators(URLs)is crucially important to prevent attackers from committing *** researches have investigated the role of machine learning(mL)models to detect malicious *** using mL algorithms,rst,the features of URLs are extracted,and then different mL models are *** limitation of this approach is that it requires manual feature engineering and it does not consider the sequential patterns in the ***,deep learning(DL)models are used to solve these issues since they are able to perform featureless ***,DL models give better accuracy and generalization to newly designed URLs;however,the results of our study show that these models,such as any other DL models,can be susceptible to adversarial *** this paper,we examine the robustness of these models and demonstrate the importance of considering this susceptibility before applying such detection systems in real-world *** propose and demonstrate a black-box attack based on scoring functions with greedy search for the minimum number of perturbations leading to a *** attack is examined against different types of convolutional neural networks(CNN)-based URL classiers and it causes a tangible decrease in the accuracy with more than 56%reduction in the accuracy of the best classier(among the selected classiers for this work).moreover,adversarial training shows promising results in reducing the inuence of the attack on the robustness of the model to less than 7%on average.

Ca^(2+)signaling in plant responses to abiotic stresses

Adverse variations of abiotic environmental cues that deviate from an optimal range impose stresses to *** stresses severely impede plant physiology and ***,such stresses dramatically reduce crop yield and negatively impact on ecosystem stability and *** components of abiotic stresses can be,for example,suboptimal temperature and osmotic perturbations,while representative chemical facets of abiotic stresses can be toxic ions or suboptimal nutrient *** sheer complexity of abiotic stresses causes a multitude of diverse components and mechanisms for their sensing and signal ***^(2+),as a versatile second messenger,plays multifaceted roles in almost all abiotic stress responses in that,for a certain abiotic stress,Ca^(2+)is not only reciprocally connected with its perception,but also multifunctionally ensures subsequent signal ***,we will focus on salt/osmotic stress and responses to altered nutrient availability as model cases to detail novel insights into the identity of components that link stress perception to Ca^(2+)signal formation as well as on new insights into mechanisms of Ca^(2+)signal ***,we will deduce emerging conceptual consequences of these novel insights and outline arising avenues of future research on the role of Ca^(2+)signaling in abiotic stress responses in plants.
