The Beneficial Effect of Combination of Mycophenolate with Low-Dose Corticosteroids and Calcineurin-Inhibitor as Well as Angiotensin-Converting Enzyme Inhibitor or Angiotensin II Blocker in Induction and Maintenance of Remission in Corticosteroid- and Rituximab-Resistant Minimal Change Nephrotic Syndrome in Adults

Management of steroid-resistant minimal change disease remains elusive with international guidelines suggesting high-dose corticosteroids and/or Calcineurin inhibitors for months similar to those with refractory idiopathic FSGS. Unfortunately, with such approach, the overall remission rates were 47% - 66%. Moreover, complete remission rates were 32% - 47% and partial remission ones were 19% - 29%. Those limited options of treatment and their poor outcomes led us to conduct the present study to assess the efficacy and safety of a new combined drug-therapy at induction and subsequent maintenance of such disease. The regimen consisted of an initial induction phase of 3-month Prednisone, Calcineurin-inhibitor, Mycophenolate and ACEI/ARB. The latter was followed by a maintenance phase of minimal dose Prednisone and nearly 1/2 the induction dose of Calcineurin inhibitors to decrease their long-term side effects. The results were satisfactory with 14 of the 22 patients, had complete remission. Moreover, 5 patients manifested partial remission and only 3 did not respond. Creatinine clearance was maintained in patients with complete remission yet, was mildly reduced in the partial and non-responsive ones. The safety and efficacy of such new combined drug-therapy provide new tool and future prospective in management of such relentless disease.

Assessment of Wastewater Reuse in Kuwait and Its Impact on Amounts of Pollutants Discharged into the Sea

Kuwait has recently implemented a vigorous campaign that aims to reclaim and reuse all treated wastewater in an at- tempt to alleviate water scarcity problem and to preserve seawater quality. This paper assesses the present status of wastewater treatment, reclamation and reuse in Kuwait, and discusses the impact of wastewater reuse on the amounts of pollutants discharged into the sea. Through analysis of the historical records of the wastewater treatment plants, it has been found that reuse of reclaimed wastewater in Kuwait has greatly reduced the amounts of pollutants discharged into the sea. Results showed that more than 50% reduction in volumes of wastewater discharged into the sea had been achieved from year 2000 to year 2010. However, this study has predicted that the amounts of wastewater discharged into the sea will start increasing again by the year 2020 due to shortages of storage capacity for reclaimed wastewater and due to the limitation of wastewater reuse applications to basically agricultural and landscape irrigations. In contrary, the on-going works and future plans of the Ministry of Public Works (MPW) are expected to overcome this problem and lead to a zero discharge of wastewater into the sea.

The Deleterious Effect of Inappropriate Calcium and Vitamin D Supplementation during Pregnancy in Women Predisposed to Calcium Oxalate Urolithiasis

Background: Gestational formation of new urolithiasis is rare yet the impact of inappropriate gestational calcium and vitamin D supplementation (Ca/DS) is underestimated. Patients and Methods: we retrospectively evaluated 75 pregnant women with history of UL, yet were stable for >2 years on dietary restrictions, for new UL after Ca/DS. Results: During the past 5 years 21 (48%) of those who had received Ca/DS had developed UL and all had high Vitamin D with hypercalcemia while the remaining 31 patients, without Ca/DS, did not have UL and maintained normal vitamin D urinary calcium without need for supplementation. Overt UL was evident by 30th weeks of gestations and most were diagnosed by ultrasonography and managed by medical expulsive therapy. Conclusion: in patients with history of UL, prudent use of Ca/DS is indicated to avoid new UL.

Seasonal Influence on the Ambient Air Quality in Al Jahra City for Year 2010

Eight primary criteria air contaminants were measured continuously for the year 2010 to evaluate ambient air quality in Al Jahra, which is one of the oldest and busiest cities in the state of Kuwait. The state of the art instrumentation was used to record the pollutants concentration to ppb levels maintaining quality control and quality assurance. Hourly base data for Non-Methane Hydrocarons (NM-HC), CH4, CO, CO2, O3, SO2, NO2and Particulate Matter (PM10) were analyzed for year 2010. Meteorological parameters contributing to air pollution, such as (temperature, solar intensity, wind speed and direction) have also been considered. The effect of winter and summer seasonal changes on pollutant concentration levels were analyzed to identify the most probable sources for the application of the futuristic mitigation methods for pollution abatement. The obtained results consistently suggest that the foregoing pollutant concentration levels are higher in winter than summer due to poor dispersion and shallow inversion layer with the exception of O3, CO2and PM10. However, all of the pollutant concentrations are below the allowable standards limits except for NM-HC.

Antibody to El peptide of hepatitis C virus genotype 4 inhibits virus binding and entry to HepG2 cells in vitro

AIM： To analyze the neutralizing activity of antibodies against E1 region of hepatitis C virus （HCV）. Specific polydonal antibody was raised via immunization of New Zealand rabbits with a synthetic peptide that had been derived from the E1 region of HCV and was shown to be highly conserved among HCV published genotypes. METHODS： Hyper-immune HCV E1 antibodies were incubated over night at 4 ℃ with serum samples positive for HCV RNA, with viral loads ranging from 615 to 3.2 million IU/mL. Treated sera were incubated with HepG2 cells for 90 min. Blocking of viral binding and entry into cells by anti E1 antibody were tested by means of RTPCR and flow cytometry. RESULTS： Direct immunostaining using FITC conjugated E1 antibody followed by Flow cytometric analysis showed reduced mean fluorescence intensity in samples pre-incubated with E1 antibody compared with untreated samples. Furthermore, 13 out of 18 positive sera （72%） showed complete inhibition of infectivity as detected by RT-PCR. CONCLUSION： In house produced E1 antibody, blocks binding and entry of HCV virion infection to target cells suggesting the involvement of this epitope in virus binding and entry. Isolation of these antibodies that block virus attachment to human cells are useful as therapeutic reagents.

目的:探索HIV感染者血清白介素16(IL-16)水平变化及HAART对其水平的影响。方法:77例HIV感染者为研究组,按美国疾病控制中心与世界卫生组织标准分期,对照组15例。检测血清CD4+T细胞、CD8+T细胞和IL-16,观察HIV感染组及各期与对照组、HAART治疗组与未治疗组之间的差异。结果:HIV感染者及各期的CD4+T细胞均低于对照组(P<0.01)、CD8+T细胞均高于对照组(P<0.05或P<0.01)。HAART治疗组55例血清IL-16为(266.6±174.1)ng/ml,未治疗组22例血清IL-16为(182.9±63.5)ng/ml,治疗组血清IL-16水平明显高于未治疗组(t=3.087,P<0.01)。HIV感染者及A、B期的血清IL-16均低于对照组(P<0.05)。细分期显示,HAART治疗后HIV感染者血清IL-16逐渐回升,C期治疗组明显高于未治疗组(P<0.05)。结论:HAART可提高血清IL-16水平,HIV感染者动态观察血清CD4+T细胞、IL-16,对病情监测有价值。

A Retrospective Study of Paediatric Dental Patients Treated under General Anesthesia

Dental care under general anesthesia (DGA) was found to be a safe, efficient and effective quality treatment for children. Purpose: To describe the characteristics of child dental patients treated under DGA and describe the indications of the treatments and types of treatments provided. Method: The sample consisted of 40 patients treated under DGA at Department of Paediatric Dentistry of school and Hospital of Stomatology of Wuhan University between June 2011 and December 2012. Detailed information was collected from dental records. SPSS software package was used for statistical analysis. Results: The age of the patients ranged from 2.5 years to 24.1 years. Males were more common than females M:F 1.5:1 .The mental retardation patients accounted for (5%) of the sample. The most common indication was inability to cooperate and accept dental treatment under local anesthesia (95%). The treatments rendered included: caries restorations (37%), root canal treatments (34%), extractions (7%), fluoride application (6%), fissure sealants (6%), indirect pulp capping (5%), stainless steel crowns (4%), pulpotomies (0.8%) and labial frenectomy (0.1%). In the follow-up visit fillings were found to be lost in 3 patients, with 2 anterior teeth and one posterior tooth. Conclusion: Caries restoration and root canal treatments were the most common treatments provided. Preventive strategies should be targeted toward children to reduce the number of healthy children receiving treatment under general anesthesia. Clinical significance: This study provides baseline information regarding the types of treatments for paediatric dental patients inWuhanand it is hoped that the data from this study will be useful for other researchers.

Identification and Conservation State of Painted Wall Plasters at the Funerary House in Necropolis of Tuna El-Gebel, El-Minia-Upper Egypt

During this study, the principal aim carried out was to obtain more information about technique and conservation conditions of the Egyptian wall paintings during the Roman period in the funerary house in necropolis of Tuna el-Gabal, El-Minia-Upper Egypt. It’s going back to 2nd century AD and involves different sites of Ptolemaic and Roman chapels;some are in the immaculate established style while others are a blend of Pharaonic-Greek style and both are secured with mural painting. Deterioration problems observed on the wall paintings of the funerary house are, loss of plaster layers, disintegration of plaster layers, loss of paint layers (blistering and peeling), discoloration and severely damaged owing to a lot of deterioration factors as weakness of mud brick support, deterioration of surface treatments and to the widespread presence of different salts. The materials used in the painting, preparation layers and the state of conservation of the mural painting at funerary house were investigated by integrated physio-chemical measurements, particularly micro-Raman spectroscopy (µRaman), light optical microscopy (LOM), scanning electron microscopy (SEM) coupled with an energy dispersive X-ray analysis system (EDX), X-ray powder diffraction (XRD) and Fourier transform infrared (FT-IR). In addition, the morphology of multilayer plaster from wall painting was investigated using atomic force microscopy (AFM). A wide color palette utilized as a part of the necropolis has been identified with mineral pigments and pigment mixtures. It is found that, the paints were based on an organic binder and traditional pigments (azurite, hematite, ochre, vegetable black) were used as colorants on plaster. The examination demonstrated that the preparatory layer is verging on made of pure lime while the plaster layer based mainly of lime and gypsum with variable amounts of quartz. Innovative Approach to Increase Name Plate Capacity of Oil and Gas Gathering Centre

Purpose： The oil and gas gathering and processing facility of Kuwait Oil Company is built with a nameplate capacity of X MBOPD （thousand barrels oil per day） with 50% water cut. However, the facility was operating with a water cut of 35%. This comprehensive technical study was conducted to evaluate possibility of increasing oil processing capacity of this facility in line with current lower water cut and other operational flexibilities available in the facility without utilizing its design margin. Topic： This paper shares an innovative approach to increase name plate capacity of oil and gas processing facility utilizing available operational flexibility and operational margins with minor modification. It shares a case study where facility capacity is increased by around 19% without utilizing design margins of equipment or pipeline. Method： The study includes theoretical verification and analysis of all major equipment and piping to identify available capacity and limitation, in order to utilize available additional margin and to propose debottleneck options to overcome limitations. Achievement： The study confirmed that, facility name plate capacity can be revised from X MBOPD （with 50% w.c （water cut）） to X ＋ 32 MBOPD （with： 45% w.c） minor modification in separator and utilizing margin available in feed specification ofdesalter trains.
