Explanation of the Damage to the Royal Family’s Cemetery in Historic Cairo and Examination of the Building Materials

The cemetery of the royal family, who ruled Egypt from 1805 to 1952, is one of the masterpieces of the UNESCO World Heritage Site of Historic Cairo. It is a complex building from massive stone masonry walls supporting hemispherical domes and is very rich with the marble decorative elements. Unfortunately, it suffers nowadays from serious cracking due to on-going structural damage. Almost all the structural elements are cracked. Besides, the continuous rise in the groundwater table affects both its structural stability and aesthetics. A detailed inspection was carried out to identify and explain all the manifested damage by the structural elements of the cemetery. The differential settlement damage was found to be very noticeable in the form of many diagonal cracks that are active and threaten the overall stability of the cemetery. The construction history was investigated and found to have a clear effect on the noticed damage. Examination of the construction materials and deterioration products was carried out by inspecting representative samples of the stone, the marble, the mortar, the plaster and the salt. They were examined using different analysis techniques including the Polarized Microscope, the Stereo Microscope, the X-Ray Diffraction (XRD), and the Scanning Electron Microscope (SEM) provided with Energy-Dispersive X-ray spectroscopy (EDX) unit. This examination helped in identifying the type of the used stone, the mortar components, the types of salts affecting the structure, and the deterioration manifested by the marble. It seems that the cemetery needs an urgent conservation project to stop the deterioration and keep it safe for the next generations.

目的 :进一步探讨免疫功能改变在艾滋病相关的卡波西肉瘤 (AIDS- KS)发病中的作用。方法 :对 12例AIDS- KS,32例无卡波西肉瘤的 HIV感染者 (HIV- NKS)和 16例正常对照进行了研究 ,用 EL ISA对 8例 AIDS- KS,2 8例 HIV- NKS和 16例正常对照血清 (或血浆 )进行了 s Fas,β 2 -微球蛋白 (β 2 - MG) ,IL - 10 ,IL- 16 ,IL- 18,IL- 6和可溶性 IL - 4受体 (s IL- 4R)测定 ,对 12例 AIDS- KS,32例 HIV- NKS外周血淋巴细胞、淋巴细胞亚群及 CD38+CD8,HLA- DR+ CD8进行了分析。结果 :β 2 - MG,s IL- 4R水平在 HIV- NKS组明显高于正常对照组 ,IL- 16水平在HIV- NKS组明显低于正常对照组 ,IL - 18水平在 HIV- NKS组和 AIDS- KS组均明显高于正常对照组 ;CD3、CD4、CD8、NK、HLA- DR+ CD8在 AIDS- KS均低于 HIV- NKS,而 AIDS- KS组 CD19、CD38+ CD8高于 HIV- NKS组 ,但各组间差异无显著性。结论 :AIDS- KS和 HIV- NKS一样存在一定程度的免疫激活 ,但 AIDS- KS和 HIV- NKS间免疫状态无明显差异 ,提示免疫状态的差异可能不是艾滋病患者并发卡波西肉瘤的主要原因。

BACKGROUND Calcitriol-induced hypercalcemia has been rarely reported in cases of lung cancer;however,it is frequently reported in cases of lymphoid malignancy and granulomatous *** present a rare case of hypercalcemia associated with squamous cell cancer of the lung with elevated calcitriol *** SUMMARY A 61-year-old Caucasian female with severe hypercalcemia of 15 mg/dL,which led to a new diagnosis of metastatic lung *** the parathyroid hormonerelated peptide(PTHrP)level was minimally elevated at 2.1 pmol/L,we believe excessive calcitriol production by tumor cells was the underlying mechanism for *** was significantly elevated at 130 pg/mL with a low 25-hydroxyvitamin D level of 25.9 ng/mL and suppressed PTH level of 8 pg/*** are generally used to treat calcitriol-induced hypercalcemia,but we successfully treated our patient with bisphosphonate,highlighting the further utility of bisphosphonates in hypercalcemia *** We believe that the underlying cause of hypercalcemia,in this case of metastatic squamous cell lung carcinoma,was elevated calcitriol,which was likely produced by the tumor *** addition to PTHrP,calcitriol levels should be included in the workup for hypercalcemia in cases of lung ***,the pathophysiology and prognostic significance of dysregulated calcitriol production in solid tumors remain unclear and warrant further *** may be used as a steroid-sparing therapy even in cases of calcitriol-induced hypercalcemia and warrants further investigation.

Titanium allergy or not? “Impurity” of titanium implant materials

For patients suffering from allergies to nickel, chrome and cobalt, titanium implants are the implants of choice. Nevertheless, titanium implant sensitivity has been reported in the form of “allergies” and an increasing number of patients are confused. This paper aims to use spectral analysis as a diagnostic tool for analyzing different titanium implant alloys in order to determine the percentage of the alloy components and additions that are known to cause allergies. Different materials, such as sponge titanium, TiAl6Nb7, Ti21SRx, TiAl6V4 [forged alloy], TiAl6V4 [cast alloy], TMZF, pure titanium [c. p. 1] and iodide titanium were analyzed for the presence of the elements that have been associated with allergic reactions using spectral analysis. All the implant material samples contained traceable amounts of Be, Cd, Co, up to a maximum of 0.001 percent by weight [wt.%], Cr up to 0.033 wt.%, Cu up to 0.007 wt.%, Hf up to 0.035 wt.%, Mn up to 0.007 wt.%, Ni up to 0.031 wt.%, and Pd up to 0.001 wt.%. This paper demonstrates that all the investigated implant material samples contained a low but consistent percentage of components that have been associated with allergies. For example, low nickel contents are related to the manufacturing pro- cess and are completely dissolved in the titanium grid. Therefore, they can virtually be classified as “impurities”. Under certain circumstances, these small amounts may be sufficient to trigger allergic reactions in patients suffering from the corresponding allergies, such as a nickel, palladium or chrome allergy.

AIM： TO establish a cell culture system with longterm replication of hepatitis C virus （HCV） genome and expression of viral antigens in vitro. METHODS： HepG2 cell line was tested for its susceptibility to HCV by incubation with a serum from a patient with chronic hepatitis C. Cells and supernatant were harvested at various time points during the culture. Culture supernatant was tested for its ability to infect na＇ive cells. The presence of minus （antisense） RNA strand, and the detection of core and E1 antigens in cells were examined by RT-PCR and immunological techniques （flow cytometry and Western blot） respectively. RESULTS： The intracellular HCV RNA was first detected on d 3 after infection and then could be consistently detected in both cells and supernatant over a period of at least three months. The fresh cells could be infected with supernatant from cultured infected cells. Flow cytometric analysis showed surface and intracellular HCV antigen expression using in house made polyclonal antibodies （anti-core, and anti-E1）. Western blot analysis showed the expression of a cluster of immunogenic peptides at molecular weights extended between 31 and 45 kDa in an one month old culture of infected cells whereas this cluster was undetectable in uninfected HepG2 cells. CONCLUSION： HepG2 cell line is not only susceptible to HCV infection but also supports its replication in vitro. Expression of HCV structural proteins can be detected in infected HepG2 cells. These cells are also capable of shedding viral particles into culture media which in turn become infectious to uninfected cells.

Modular Multilevel Inverter with Maximum Power Point Tracking for Grid Connected Photovoltaic Application

A single-phase modular multilevel inverter based photovoltaic system for grid connection is proposed. This photovoltaic system utilizes two conversion stages： a boost converter for tracking the maximum power point and a modular multilevel inverter used as an interfacing unit. The maximum power point tracking is achieved with a fuzzy logic controller, and the modular multilevel inverter regulates the DC link voltage and synchronizes the grid voltage and current in order to achieve unity power factor operation. The proposed system provides high dynamic performance and power quality injected into the grid. The validity of the proposed system is confirmed by simulations.

The demand for short term energy storage providing high power for electric and hybrid-electric vehicles is increasing drastically. Stationary FESS （flywheel energy storage systems） is established as UPS （uninterruptible power supply） and represent an emerging market. In contrast, mobile FESSs are currently only used in a few application, e.g., in motor sports. To enable a wider use in personal and public transportation the life-span of the flywheel＇s bearings needs to be increased significantly. This paper presents an alternative approach to extend the lifespan of the flywheel＇s bearings significantly by using a CREAMB （combination of rolling element and active magnetic bearings）.

Graft vs host disease impacts overall survival post allogeneic hematopoietic stem cell transplantation for acute lymphoblastic leukemia/lymphoma

AIM To examine the outcome and prognostic factors for high risk patients with acute lymphoblastic leukemia/lymphoma(ALL/LBL) who underwent allogeneic hematopoietic stem cell transplantation(HCT) at our center during the period of2010-2017 METHODS After due institutional review board approval, patients with high risk ALL/LBL post HCT were identified and included. All records were retrospectively collected. Time to event analysis was calculated from the date of HCT until event of interest or last follow up with Kaplan-Meir means. Cox regression model was used for multivariable analysis *** A total of 69 patients were enrolled and examined with a median age of 21(14-61). After a median follow up of 15 mo(2-87.3), the 2-year cumulative incidence of relapse, cumulative incidence of non-relapse mortality, progression free survival and overall survival(OS) were 34.1%, 10.9%, 54.9% and 62.8%,respectively. In a multivariable analysis for OS; acute graft vs host disease(GVHD) and chronic GVHD were significant with corresponding hazard ratio 4.9(1.99-12; P = 0.0007) and 0.29(0.1-0.67; P = 0.0044), *** Allogeneic-HCT for high risk ALL/LBL resulted in promising remissions particularly for patients with cGVHD.
